The CDC’s National Center for Chronic Disease Prevention and Health Promotion’s (NCCDPHP or “Center”) mission is to help people and communities prevent chronic diseases and promote health and wellness for all. Chronic diseases are defined broadly as conditions that last one year or more and require ongoing medical attention or limit activities of daily living or both. Presently, six in ten Americans live with at least one chronic disease such as heart disease and stroke, cancer, and diabetes, among others. These, and other chronic diseases, are the leading causes of death and disability in America as well as a leading driver of health care costs.
