Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

It was the victim's second day on the job with little training on operating the roller/compactor. The victim had just finished compacting one plot and was moving to the next. The plots were side-by-side, consisting of compacted dirt bounded by un-compacted dirt. Flush with the side of one of the plots was a steep incline of un-compacted dirt. The rollover was not witnessed, but evidence suggests the victim was not wearing his seatbelt and was ejected from the operator's cab. A coworker noticed the overturned roller/compactor and sought help from the foreman. Emergency Medical Service (EMS) was called. The decedent and a coworker had previously installed two C10 channels to sandwich a hollow cinder block wall separating a new building addition from the existing building. After the hollow wall was partially demolished for access between the addition and the existing building, the decedent installed a continual steel plate at the base of the sandwiched wall to complete the header for the access way. On the day of the incident the decedent and the same coworker arrived at the jobsite to install steel channels on each side of the access way opening from the floor to the header. The crew unexpectedly found screwed, epoxied 2- x 8-inch lumber on the north and south door walls. The coworker pried off the north board without incident. The decedent removed the screws on the south board, twisted the board, and then used a hammer to hit the base of the board, releasing it from the wall. As the decedent turned, the header beam came down, striking him in the head and landing on his legs. His coworker ran to another person on site and asked him to call 911. The coworker returned to the decedent and attempted to move the beam with a pole. Unable to move the beam, the coworker applied pressure to the decedent's head to control the bleeding. After calling 911, the other person on site retrieved a jack from his vehicle to raise the beam. This jack could not raise the beam, and a second jack was retrieved. Using two jacks, the beam was raised from the decedent. He was taken by emergency response to a local hospital where he died four days later. Recommendations: 1. Professional expertise should be obtained in writing prior to contractors making structural support changes necessitated by jobsite conditions. 2. Jobsite protocol should include employees reporting unexpected or new work site conditions to the home office or to the general (controlling) contractor to ensure enough information is gathered to proceed with an alternate work plan. 3. General (controlling) contractors should report alterations and changes in work site conditions to appropriate subcontractors who could affect the safety of subcontractor personnel. 4. General contractors should ensure that all required documentation is available on site. 5. Temporary shoring (support) systems should be approved by a qualified professional engineer and identified by the contractor who installed it. Steel erectors should insist that the controlling contractor provide them with the required written notifications regardless of job size. This provided clearance for the farmer to access the hydraulic cylinder and lift linkage area under the middle of the dump trailer box. The man was working alone in his machine shed at the time of the incident. He had disconnected the hydraulic cylinder from the underside of the trailer box. The skid steer forks reaching in from outside the doorway held the front of the box in an elevated position similar to when a load is being dumped from the wagon box. As he leaned across the trailer frame under the box to remove the hydraulic cylinder, the trailer moved rearward. The front edge of the wagon slipped off the forks of the skid steer and the trailer box fell suddenly. The man was crushed over the lift linkage and frame by the underside of the trailer box. A few moments after the incident a relative discovered the farmer trapped under the wagon's box. Emergency crews were summoned. They used jacks and struts to secure both the wagon and the raised lift arms of the skid steer. Resuscitative efforts were not successful and the farmer was pronounced dead at the scene. RECOMMENDATIONS: (1.) Dump trailers should be equipped with rigid mechanical means to secure the trailer box in a raised position and they must be put into place without entering the area under the raised wagon box before making inspections, adjustments, or repairs under a raised box. (2.) Persons planning repairs underneath a dump trailer should securely block (chock) the rear axle wheels to prevent unexpected, inadvertent movement of the trailer. (3) Persons inspecting or working on dump trailer lift systems should, when possible, choose to perform the task with the trailer box in its lowest position. (4.) Dump trailers should be designed to accommodate, when practicable, service and replacement of the lift system while the trailer box is in its lowest position. The CA/FACE investigator determined that, in order to prevent future occurrences, employers, as part of their Injury and Illness Prevention Program (IIPP), should: 1. Ensure vats and tanks are safeguarded by grates or covers whenever work occurs above them. 2. Ensure that a general hazard assessment is conducted prior to beginning any job or work task. 3. Ensure that employees use a safe method to gain access to elevated work areas. 4. Develop, implement, and enforce a fall protection program in conjunction with the IIPP. After completion of the pilot bore, the exit pit worker was alone in the exit pit 600 feet (183 m) from the drill rig operator. The worker removed the drill bit to attach a back reamer to the end of the drill rod. A pulling head was attached to the pipe and a swivel between the pulling head and the back reamer completed the connection between the HDD machine and the product line pipe. The drill rig operator radioed the exit pit worker asking if he was ready for the reamer to be rotated and for the pullback or back-reaming operation to begin. The exit pit worker responded affirmatively. Soon after engaging power, the operator radioed to learn how the reamer was progressing but there was no response. The operator halted power to the drill line and radioed for help to check on the exit pit worker. The supervisor discovered the victim's body wrapped around the drill line in the area of the pre-cutter just ahead of the back reamer. RECOMMENDATIONS: 1. Personnel must be positioned a safe distance from the drill line and reamer before the drill line is energized and rotated, and they must never step over rotating drill pipe. 2. When available, horizontal directional drilling units should be equipped with remote lockout. 3. Two-way radio communication between the entry and exit areas must be maintained at all times and the drill line must not be rotated until all personnel have been notified and acknowledgement has been made by all personnel. 4. Personnel should be trained to safely operate horizontal directional drilling equipment and supervised to ensure that they are following proper safety procedures. Department of Public Health AB - Every year, men and women in a wide variety of jobs and industries throughout Massachusetts are fatally injured at work. These deaths are all the more tragic because they are largely preventable. Information about where and how they occur is essential in order to develop effective prevention programs. In Massachusetts, the Occupational Health Surveillance Program (OHSP) in the Massachusetts Department of Public Health (MDPH) collects information on fatal occupational injuries as part of the national Census of Fatal Occupational Injuries (CFOI), conducted in cooperation with the Bureau of Labor Statistics (BLS), US Department of Labor. OHSP also conducts in-depth investigations of fatal occupational injuries as part of the national Fatality Assessment and Control Evaluation (FACE) project, sponsored by the National Institute for Occupational Safety and Health (NIOSH). The purpose of the FACE project is to develop a detailed understanding of how fatal injuries occur and to identify effective countermeasures to prevent similar incidents in the future. Excerpts from selected FACE investigations are highlighted in this report. This update provides an overview of fatal injuries at work that occurred in Massachusetts during 2006. These include fatalities traditionally linked to the work environment such as falls, electrocutions, and exposure to toxic chemicals. They also include workplace homicides and suicides as well as motor vehicle-related fatalities that occurred during travel on the job. Deaths resulting from occupational illnesses and heart attacks at work are excluded from this fatality update. The victim and other crew members completed roofing one condominium unit and moved on to work on the roof of another unit. The victim returned to retrieve tools from the first unit's roof. He accessed the roof by climbing up a 32-foot extension ladder. Before descending the ladder, the victim used his cell phone to ask a coworker to hold the ladder for him. The coworker arrived and held the ladder. The roof line where the ladder was positioned was 25 feet above ground level. The ladder was not extended, so the victim before he began to climb down, pulled the ladder up higher, causing it to extend a bit so that about one rung of the ladder was above the roof line. The victim, holding a drill case in one hand, began to descend the ladder, when at the same moment the ladder telescoped down slightly (the rung locks where not fully engaged) causing the victim to fall about 25 feet to an asphalt surface below. The victim was taken to a hospital, where he died the next day of blunt head and chest trauma. The 51-year-old victim, a painter, had 10 years experience in the painting industry and was employed by a painting contractor. The victim and another worker were assigned by their employer to a job site where they were to pressure wash and clean the brick exterior of a two-story bank building. The two workers arrived at the job site at about 1:00 pm and used pressure washing equipment and a scissor lift elevating work platform to wash the building exterior. About two hours after cleaning the north side of the building, they moved the equipment to the building's east side. The scissor lift was positioned on the surface of an asphalt parking lot adjacent to and sloping away from the building. The victim was working from the platform of the scissor lift which was extended to about 17 feet above the ground. The scissor lift tipped-over, ejecting the victim who suffered fatal injuries after hitting the pavement. As the incident was unobserved, it is unclear what caused the tip-over. What is clear from the incident investigation is the scissor lift operator was not trained in the use of this equipment; the lift was positioned parallel to the building on the sloping parking lot surface; and the lift's damaged outriggers were not being used. The generator was running, providing energy for a light. When the victim did not return home after work and did not answer his cell phone, his wife and her two brothers went to the storage container and found the victim. He had been overcome by CO and had collapsed inside the storage container. A call was placed for emergency medical services (EMS). One of the victim's wife's brothers turned off the generator and pulled the victim out of the storage container. They administered cardiopulmonary resuscitation (CPR). Once EMS arrived the victim was transported to a local hospital where he was pronounced dead. The Massachusetts Fatality Assessment and Control Evaluation (FACE) Program concluded that to prevent similar occurrences in the future, employers should: 1. Always leave fuel-burning generators outside of buildings and storage containers when operating AB - and 2. Ensure carbon monoxide detectors are used when fuel-burning generators are running and employees are located at indoor and/or partially enclosed work sites. Home builders / general contractors of large construction projects should: 1. Provide electricity at long term tool and equipment storage locations being supplied to subcontractors AB - and 2. Ensure that the maps of construction sites provided to local emergency response personnel include all storage locations for materials and equipment. The decedent was one of two laborers working in a nearly vertical, "L"-shaped excavation that was 50 feet long, 9 feet deep, 7 feet wide on top and 6 feet 6 inches wide at the bottom. The work consisted of laying schedule 40, 4-inch pipe from the water/sewer main near the street to the home. The decedent was assigned to dig under the footing near the basement while his coworker was laying pipe near the street. There were no trench boxes. The decedent's coworker heard the south corner of the wall collapse and ran to the decedent's location. He found him under a large chunk of dirt that collapsed from the south corner. The heavy equipment operator told the coworker to call the company. He did so, and the company representative instructed him to call 911. He called 911. Prior to emergency response arrival, a nearby landscape worker heard a call for help and entered the trench with the decedent's coworker to move the dirt chunk from the decedent. The excavator was close enough that the heavy equipment operator used the bucket to dig around the decedent. Emergency response arrived. While attempting rescue, the south wall collapsed again. The decedent was declared dead at the scene. Recommendations: 1. Employers should ensure that employees working in excavations are protected from cave-in by an adequate protection system such as shoring, sloping or a trench box designed in accordance with MIOSHA Construction Safety Standard, Part 9, Excavation, Trenching, and Shoring, R408.40941. 2. Employers should ensure that a qualified person inspects the excavation, adjacent areas, and supporting systems on an ongoing basis and that the qualified person ensures the appropriate measures necessary to protect workers are followed. 3. Employers should provide workers with training in the recognition and avoidance of unsafe conditions and the required safe work practices that apply to their work environments. 4. Employers should develop a trench emergency action plan that describes rescue and medical duties and ensure that all employees are knowledgeable of those procedures. 5. Employers should periodically evaluate its organizational commitment and leadership in regards to its safety program. 6. Employers should ensure that employee performance reviews include adherence to measurable safety and health criteria. 7. Employers should institute a Health and Safety (H&S) committee as part of their health and safety program. 8. Employers should take advantage of on-site consultation services provided by MIOSHA Consultation Education and Training Division and trade organizations to improve worksite safety and health. 9. Emergency medical services and fire-rescue personnel should be knowledgeable about proper rescue techniques involving excavation sites and ensure that adequate shoring equipment is on hand at all times. 10. Emergency medical service companies and fire-rescue departments providing first response should consider providing technical rescue operation training, such as trench rescue training to their personnel. 11. The State of Michigan's Office of Fire Fighter Training should partner with existing organizations that provide trench rescue training to fire fighters as a part of their "Rescue" curriculum. In an adjacent room the supervisor, who did not believe anyone was in or above the auger but was not able to see it, removed his lockout and turned on the main circuit breaker. The auger's on/off electric switch was mounted to the wall near the auger. The switch was in the "on" position so when the main circuit breaker was turned on the auger started immediately. A co-worker, who was also tasked with picking up metal debris from the work area, did not know how to turn the auger off so he ran outside the room calling for help. Another employee came into the rendering room where the auger was running and turned off the switch for the auger. The victim's legs were caught in the auger and he was face down, head down the incline, with his feet up the inclined auger. Rescue personnel arrived 10 minutes after the incident. Despite resuscitative efforts, the victim was pronounced dead at the scene 35 minutes later. RECOMMENDATIONS: 1. Equipment lockout/tagout procedures must be fully implemented, including checking the work area to ensure that all employees have been safely positioned or removed before removing lockout and notifying employees that lockout devices have been removed from energy sources. 2. Training should be provided to employees to ensure that the purpose and function of hazardous energy control of machines is understood and that they have the knowledge and skills required for safe application, usage, and removal of hazardous energy controls. 3. All employees should strive for clear communication with each other, and be aware that in hazardous situations language differences may result in misunderstanding of instructions. Notable trends in 2005 included the following. Four events with mobile machinery involved young workers, aged 21 and under. The decedent was driving the fire engine with its lights and sirens activated enroute to a structure fire. The fire station lieutenant was seated in the front passenger seat. Two additional fire fighters were seated in the back seats behind the fire engine's front compartment. As the fire engine proceeded through an intersection, a speeding SUV struck the passenger side of the fire engine near the windshield. The decedent was ejected along with the lieutenant through the passenger front windshield. The lieutenant landed in the street in front of the SUV approximately ten yards away from the fire engine. After the decedent was ejected, he rolled underneath the fire engine. The fire engine came to rest on top of the decedent's chest. The decedent expired as the fire engine was removed from his chest. Although the other two fire fighters were not ejected, they sustained serious injuries. These two fire fighters and the lieutenant were taken by ambulance to a local hospital. The driver of the SUV was pronounced dead at the scene. Recommendations: 1. The use of seat belts should be mandatory when driving or riding in a fire engine or any emergency vehicle that is in motion. MIOSHA General Industry Safety Standard Part 74, Fire Fighting requires seat belt use when the apparatus is in motion. 2. Fire engines and other fire apparatus should be equipped with automatic seat belt systems or warning signals that require seat belt use for operation of the vehicles. 3. In accordance with the National Fire Protection Association (NFPA) 1451, Standard for a Fire Service Vehicle Operations Training Program, driver training should be provided as often as necessary, but not less than twice a year. 4. In accordance with the NFPA 1451, formally written Standard Operating Procedures (SOP) should be developed and implemented for safe driving, defensive driving techniques, and riding within and operating fire department vehicles during an emergency and non-emergency response. 5. The SOP should have a procedure to evaluate the effectiveness of this training every three years in accordance with NFPA 1451. 6. All fire apparatus driver/operators shall meet the requirements as outlined in NFPA 1002, Standard for Fire Apparatus Driver/Operator Professional Qualifications. 7. In accordance with the NFPA 1451, all fire department vehicle drivers/operators should possess a valid vehicle operator's licensed as required by the State. 8. The SOPs should be communicated, applied and enforced by all fire fighters to ensure "Everyone Goes Home" as promoted by the National Fallen Fire Fighter Foundation. 9. All fire fighters should be encouraged to sign the "National Fire Service Seat Belt Pledge" to re-enforce the importance of wearing seat belts. EMS transported the victim from the incident location to a local hospital where he was pronounced dead. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Ensure that existing and newly purchased machining centers with cabinet doors are equipped with interlocks AB - 2. Develop, implement, and enforce a comprehensive hazardous energy control program including a lockout/tagout procedure and training AB - 3. Provide employees training, in the employees' primary language(s) and at appropriate literacy levels, on machines they will use, worksite hazards and controls for these hazards AB - 4. Occupational Health Branch AB - A 35-year-old Hispanic female material handler died when a backing forklift crushed her against a podium that was anchored against a steel beam. The victim had exited a trailer parked at the warehouse dock and walked to the adjacent podium to enter loaded items onto the manifest. A forklift on the dock was moving pallets and was backing up and turning to the left when it crushed the victim against the podium. The forklift operator was not looking where he was backing at the time of the incident. The location of the podiums required the material handlers to have their backs to the activity on the dock. The CA/FACE investigator determined that, in order to prevent future occurrences, employers, as part of their Injury and Illness Prevention Program (IIPP), should: 1. Ensure forklift operators constantly look in the direction of travel. 2. Engineer a podium that would have material handlers facing the activity on the dock. In addition: 1. Employer and manufacturers should consider additional warning systems for forklifts. 2. Employers should consider changing the type of forklift being used on docks to those where the operator stands sideways. Company A had two individuals (Co-owner #1 and Co-owner #2) on site at the time of the incident. Company A had subcontracted the trim work to a local contractor. This local contractor had not completed the trim installation on the west wall of the addition. One of the co-owners of Company A (Co-owner #1) noted the decedent cutting and staining several pieces of 10-foot-long wood trim in the saw area adjacent to the area where trim still needed to be installed. Co-owner #1 was called away from the saw area by Co-owner #2. Co-owners #1 and #2, who were both related to the decedent, were unaware that the decedent had placed a 28-foot extension ladder against the wall to be trimmed. The decedent ascended to the top of the ladder and using a nail gun, installed two sections of trim, beginning the trim installation at the wall/ceiling junction. The fall event was unwitnessed. Co-owner #1 found the decedent on the floor between the ladder and the west wall. The ladder was found leaning against a light fixture approximately 10 feet away. Emergency response was called. The decedent was transported to a local hospital where he subsequently died. Recommendations: 1. Employers should review ladder inspection, placement, and safe use procedures with all employees, including experienced employees. 2. Construction employers should develop and implement an accident prevention program that complies with MIOSHA requirements. The decedent was transported to a local hospital (hospital #1) and then transferred to another hospital (hospital #2), where she underwent surgery for fractures of her spine. After a period of recovery in the hospital, she was transferred to a local long-term care center. After several weeks, the individual's health began to deteriorate, and she was transferred to hospital #2, where she died. Recommendations: 1. When vacuuming stairways, housekeepers should assess whether the vacuum may be safely placed at the top of the stairs or whether the vacuum should be placed at the base of the stairs. 2. Housekeepers should attempt to keep one hand on the stair rail at all times while ascending and descending a stairway. 3. Home cleaning services/self-employed housekeepers should consider using backpack/hip-style vacuum cleaners.
