Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The victim was standing in front of the hatch checking for leaks as another employee (standing to the side of the vessel) increased the pressure. The fatal injury occurred when the vessel was overpressurized and the front hatch exploded outward, striking the victim. NJ FACE investigators recommend following these safety guidelines to prevent similar incidents: 1.) Employers should ensure that pressure vessels are not pressurized above allowable limits, all pressure relief valves are properly functioning, and that any repairs to this type or similar pressure units be conducted by a certified worker. 2.) Employers should ensure that workers are properly trained on the safe operation of pressure vessels and are knowledgeable about the associated hazards. 3.) A safety and health plan based on a job hazard analysis should be developed and followed at the worksite. Another contractor had placed a metal roof curb measuring 72 3/4-inches long by 40 3/4-inches wide by 14inches high over the make-up air opening, but did not secure it to the roof. There was no marking either on the roof for the roof curb indicating the open shaft below. Because of an approaching storm, the company owner determined that the work area should be cleaned up and the plywood sheets located a few feet south of the make-up air opening secured. The decedent's two coworkers were walking toward the roof access stairs with tools and other construction materials and did not witness the decedent's work activity which led to his fall. It is postulated that the decedent was in the process of securing the nearby plywood sheets. It appears he was moving the make-up air roof curb so he could place it onto the plywood. The decedent lifted the north end of the curb but apparently did not check under the curb prior to pushing it forward toward the plywood sheets. Due to the size of the roof curb, as he pushed it forward he could not see the make-up air opening. He walked into the opening and fell to the basement's concrete floor. He was wearing a fall harness and lanyard, but was not tied off. Emergency response was summoned to the scene. The decedent was transported to a local hospital and was declared dead. Recommendations: 1.) Employers should ensure that employees on walking/working surfaces are protected from falling through holes more than 6 feet above lower levels by personal fall arrest systems, covers, or guardrail systems erected around such holes. Covers should be labeled and meet the requirements specified in the MIOSHA Fall Protection Standard. 2.) Construction employers should conduct a daily hazard assessment to identify and mitigate hazards, such as roof openings, to ensure employee safety. 3.) Construction employers should review MIOSHA Construction Safety and Health Standard, Part 45, Fall Protection to ensure understanding of the standard's requirements to effectively implement the standard at the work site and to provide appropriate employee training. 4.) The employer should develop and implement a written safety program, which should include the formation of a health and safety committee. 5.) Employers should require that all job site hazards are communicated on an ongoing basis to all workers required to be in the area. 6.) Employers on multi-employer sites should utilize contract language that clearly defines the safety responsibilities of each contractor prior to the initiation of work. The victim was standing in the track behind the clipper by the clip bag when a people mover (bus) that was being operated by a co-worker struck the victim crushing him against the clipper. Co-workers radioed the dispatcher who then placed a call for Emergency Medical Services (EMS). EMS responded to the incident site within minutes along with the transit, municipal, and state police. The victim was transported to a local hospital where he was pronounced dead. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, railroad transportation companies/agencies should: 1.) Ensure that routinely accessed items, such as clip bags, are not located where roadway workers have to repeatedly foul tracks (position themselves in between or near) to access them AB - 2.) Strictly enforce the required ten foot clearance between equipment as well as other regulations established to protect workers AB - 3.) Inspect and evaluate all purchased or acquired equipment, especially equipment altered by another entity, to ensure it is safe to operate prior to placing it in operation AB - and 4.) Conduct a routine job safety analysis (JSA) for each piece of equipment and the tasks the equipment will be involved in to ensure worker safety. The driver, who had entered the roll storage area through the doorway, was traveling in a forward direction (not trailing the load). He intended to place the roll in its appropriate place on 2- by 4-inch pieces of wood. The clamp truck/paper roll struck the decedent. The driver looked to his right on the ground and could see the decedent's body to the right of the truck's right tire. He immediately backed up and called for emergency assistance. Emergency response arrived. The decedent was declared dead at the scene. Recommendations: 1.) The employer should revise its forklift safety rule regarding driving the forklift in the direction that affords the "best visibility" to clarify and require the operator to trail a load when a driver's forward vision is obscured. 2.) Employers should continually stress the importance of adherence to established safe work procedures. 3.) The company should review the locations of the designated pedestrian aisleways to determine if they are appropriately located for pedestrian travel. 4.) The company should consider using a highly visible hard hat color (such as yellow or orange) and when job appropriate, the use of reflective vests for pedestrians. Occupational and Environmental Medicine AB - On the winter of 2008, a 32-year-old male journeyman mason died as a result of falling from an unguarded working surface of a Hydro Mobile 2 scaffold raised to 35 feet. The scaffold had recently been lowered and moved to its present location. The competent persons erecting the scaffold did not re-install the guardrails at the ends of the working platform nor install the proper planking prior to raising the scaffold. The scaffold was repositioned in a north-south direction and then raised to 35 feet above the ground. The decedent was installing backing rod and his coworker was caulking. The two workers worked in a south to north direction, moving backwards on the work platform. The decedent fell from the unguarded edge to the concrete surface below. Emergency response was called, and the decedent was taken to a local hospital. He died in the emergency room. RECOMMENDATIONS: 1. Employers should ensure that all required components of a scaffold are properly installed prior to employee use. 2. Employers should routinely conduct scheduled and unscheduled workplace safety inspections. 3. Employers should periodically evaluate their organizational commitment and leadership and employee understanding regarding their safety program. 4. Employers should institute a Health and Safety (H&S) Committee as part of their health and safety program. The tractor slid through the grass beside the roadway, struck a concrete culvert, sideswiped a small tree and then hit a large tree head-on. The tractor and trailers then skidded back down the embankment toward the highway where the entire unit came to rest. The tractor and lead trailer overturned onto the driver's side. The second (rear) trailer uncoupled from the first trailer but remained upright and attached by safety chains. Emergency medical services were contacted. Upon arrival, EMS found the entire unit engulfed in flames. The coroner was contacted, and upon his arrival, declared the driver dead at the scene. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Commercial tractor-trailer drivers should be trained to recognize signs of fatigue and when to seek appropriate rest areas. Recommendation No. 2: Companies should provide new and refresher commercial driver safety training for company drivers addressing driver distraction and including defensive driving techniques. Recommendation No. 3: Vehicle stabilizer and sensory systems should be mandatory equipment on all commercial vehicles. Recommendation No. 4: Log books and other important driver records should be stored in a fireproof container in the cab. The driver of the motor vehicle entered a pedestrian walkway after failing to negotiate a turn in the hospital's driveway. The vehicle then crashed through two sets of glass doors and entered the radiation therapy unit's lobby. The car came to rest after striking the lobby reception desk. A total of four workers were struck in the collision. The two victims were pinned on the floor by the vehicle and pieces of debris. The two injured co-workers were pinned against the wall behind the lobby desk. Calls were placed to emergency medical services (EMS) and fire and police were dispatched to the scene within minutes. Hospital medical staff and EMS were on-scene immediately after the crash. The two victims and the two injured co-workers were extracted from the debris and treated in the hospital's emergency room. The physician was pronounced dead an hour after the incident in the hospital's emergency room. The receptionist was transported to a regional level one trauma center where she was pronounced dead later that same day. The two injured co-workers were treated at the hospital where the incident took place and then released. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers and property managers should: 1.) Evaluate the layout of parking lots and driveways to determine locations susceptible to damage from vehicle intrusion AB - 2.) Consider installing exterior perimeter security barriers to protect employees and minimize building and structural damage from vehicle intrusion AB - and 3.) The data was compiled by the Washington State Fatality Assessment and Control Evaluation (FACE) Program. The data presented in this report were collected according to the North American Industrial Classification System (NAICS). NAICS six digit codes beginning with 23XXXX representing the construction industries were used for this report. This report exclusively includes incidents involving construction machinery and not vehicles that can go on the street. Construction machinery in this report does not include construction vehicles, such as dump trucks or other motor vehicle. Fatalities associated with construction machinery that were determined to be caused by electrocution were also excluded from this report. The data in this report may vary from data generated by other sources due to counting criteria. According to the National Institute for Occupational Safety and Health (NIOSH), the construction industry had the highest number of fatalities of any sector in 2006. This was an increase of 3% from 2005. Construction and extraction occupations and transportation and material moving occupations together accounted for nearly half (48%) of all fatal work injuries in 2006. The events most frequently leading to work-related fatalities in construction from 1995 through 1999 were falls (33%), transportation incidents (20%), contact with objects or equipment (19%), and exposure to harmful substances and environments (17%). Each event type accounted for a similar proportion of construction occupational fatalities on 2006. Within the construction industry, NIOSH identified machinery related traumatic occupational incidents as the fourth leading cause of fatalities. Cranes, excavating machinery, and tractors were the machines most frequently involved. The most common incident types were: struck by a mobile machine; overturn; and struck by a boom (Pratt, Kisner et al. 1997). In Washington, the construction industry sectors where machinery fatalities happened most often were: 1. Highway, bridge, and street construction. 2. Site preparation. 3. Water and sewer line construction. The most common fatal incidents involved: 1. Earth moving, and scraping machinery. 2. Excavating machinery. 3. Lifting machines and appliances. Between 1998 and 2008, fatalities were almost equal for machinery operators and workers on foot. Operators were killed most frequently in backhoe/trackhoe incidents while workers on foot were killed most frequently in excavating machinery incidents. Furthermore, machinery operators were predominately involved in fatal roll over incidents while workers on foot were either mainly struck by machinery or other objects. The majority of machine operators killed were not wearing seat belts or occupant restraints. Employers and employees should follow the detailed prevention measures outlined by NIOSH for site set up, machinery operators, and other site workers (page 14). Areas to focus prevention efforts are the use of seat belts and occupant restraints among machinery operators and reducing struck by incidents among workers on foot. Seventy percent of machinery operators involved in roll-over fatalities did not use seat belts or occupant restraints. Nearly 90% of workers on foot were struck and killed by machinery or other objects. He was the operator of a tower crane working on building a multi-level condominium complex. The victim reported to his supervisor that he would be coming down from the crane's cab. The crane's mast was placed inside the concrete elevator shaft of the building. The mast had a ladder and a series of landings inside the structure allowing the operator access to the crane. He climbed down from the cab and apparently attempted to access the building's 6th floor, where there was a temporary 2x4 railing guarding the shaft. The victim attempted to either jump or step across the gap from the crane's mast to the 6th floor, but instead fell unobserved about 70 feet down the elevator shaft. The broken railing was found at the bottom of the shaft. He was taken to a hospital where he died of his injuries. Occupational and Environmental Medicine AB - On January 11, 2008, a 39-year-old male steel industry supervisor died when he was pulled from a catwalk, struck against a conveyor system support structure and fell to the floor below. The decedent was pulled from a catwalk located approximately 11 feet above the floor when the air hose to the 10-foot long metal pipe (stinger) he was using to blow debris from a nearby non-operational conveyor table and rollers located parallel to the catwalk caught between a belt conveyor and an idle roller. He struck the framework at the mounting point of the belt conveyor tension roller and fell to the floor below. The unguarded belt conveyor was located 72 inches above the catwalk and then turned downward from above the catwalk 25 inches away from the 42-inch standard rail of the catwalk. Facing the non-operational conveyor, with the stinger/air line placed over his shoulder, he passed under the belt conveyor and its idle roller, which was located behind him. It appeared that the stinger/air hose became entangled in the belt conveyor's idle roller, drawing the decedent over the catwalk handrail where he struck against the framework supporting the belt conveyor and its tension roller. The decedent then fell to the floor. The air hose and stinger, his hard hat and his respirator were found jammed between the framework and the tension roller. One of the crewmembers standing in another area of the building looked in and saw him lying on the floor. Another crewmember used the decedent's radio to call for help. Because of the excessive noise in the building, it was necessary for this coworker to exit the building to summon help. Another crewmember ran for help. The company's emergency response personnel were the first to arrive and began emergency treatment. The city fire department rescue squad was the next to arrive. After the fire department called in the decedent's condition to a local hospital, he was declared dead at the scene. Recommendations: 1. Employers should provide guards on conveyor belts at all in-running nip points where employees may be at risk of contacting moving/rotating parts. Company safety management should develop a risk assessment protocol for prioritizing work orders that concern safety issues. 2. Employers should ensure all workers are trained to recognize and avoid hazards in the workplace. 3. Employers should determine if there are feasible alternative methods to using compressed air to clean conveyor systems. 4. Employers and workers should work towards developing a "positive" safety culture for their workplace. 5. Employers should ensure that personal protection program policies are followed. The decedent gathered his personal belongings (lunch container, newspaper, etc.) and exited his truck. As the decedent walked behind his truck to switch trucks, he dropped his newspaper. The second driver entered the decedent's dump truck and looked for him using the dump truck mirrors. He saw the decedent in the passenger side rear view mirror. The decedent, noting he dropped his newspaper, walked back behind the truck and bent down to retrieve it. Simultaneously, the second driver began to back the decedent's truck to the excavation site. The excavation crew noticing the decedent was in the path of the backing dump truck ran toward the vehicle and yelled warnings to him and the driver. The decedent stood up and was struck by the backing truck. Emergency response was summoned and the decedent was transported to a local hospital. He died approximately two weeks later in the hospital from complications of the injuries sustained at the time of the incident. Recommendations: 1.) Employers should ensure that written backing protocols are in place and that designated individuals are assigned as spotters to direct backing construction vehicles on construction sites. 2.) Employers should ensure that workers who are on foot stay out of the work area where heavy equipment is operating and in clear view of operators. 3.) Employers should utilize the National Institute of Occupational Safety and Health (NIOSH) Safety and Health topic Highway Work Zone Safety topic page to provide employee training concerning blind spots for construction equipment. 4.) Safety department/personnel should be provided high visibility and the power to implement changes and evaluate compliance with safety plans and programs. 5.) Heavy equipment owners should consider equipping vehicles with devices to detect the presence of individuals or objects behind the vehicle. After the tire failed, the trailer slipped off the right side of the road, overturned, and went over an embankment. The trailer remained attached to the tractor as both went over the embankment, flipping the entire unit upside down. A witness called emergency medical services who arrived and called the county coroner. The coroner arrived and declared the driver dead at the scene. It was determined by Kentucky State Police that the driver was not wearing a seat belt at the time of the incident. Speed at the time of the crash was not listed in the investigation report. Toxicology tests detected tramadol, oxycodone, and hydrocodone in the driver's system at the time of the crash. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Employers should require drivers to perform pre-trip and post-trip inspections on the tractor and trailer prior to operation. Recommendation No. 2: Employers should require the use of seat belts while operating a commercial vehicle. Recommendation No. 3: Companies should provide professional training for company truck drivers. Recommendation No. 4: Stabilizer and sensory systems should be mandatory equipment on all commercial vehicles. Recommendation No. 5: Comprehensive expanded new-hire prescreening and after-hire random drug/alcohol testing policies should be implemented and enforced for substance abuse. While attempting to clear the jam, the truck driver climbed to the top of the trailer, stepped onto the woodchip pile, and then fell through the woodchips to the bottom of the pile. He was engulfed by the woodchips and suffocated. Recommendations for Employers: Employees operating open top trailers to transport loose material should: 1. Ensure that trailers are identified and labeled as confined spaces. 2. Develop, implement, and enforce a permit space program for permitrequired confined spaces. (See box on reverse side for more information on confined spaces.) 3. Provide confined space training for employees that address all potential hazards inside open top trailers, such as engulfment by woodchips. In addition, employers using open top trailers with movable floor unloading systems to transport loose material should: 1. Never fill trailers above cross stability bars. This will minimize jamming during offloading. 2. Develop and strictly enforce procedures on how to clear jammed material where employees do not have to enter the trailers. 3. Train employees about the potential formation of voids within loose material inside trailers that can engulf them. These unnoticeable voids can develop within the material, especially during material jams when offloading. 4. Install automatic sweeping tarps as part of movable floor unloading systems, available from most system manufacturers, to reduce the need for employees to enter trailers. During the unloading process, it appears that the victim climbed to the top of the open trailer because the woodchip load had become jammed and he was going to clear the jam. Although the incident was un-witnessed, it appears that the victim stepped from the top of the trailer onto the woodchip pile. The woodchip pile gave way as he stepped onto it and he was engulfed and suffocated by the woodchips. A co-worker realized that the victim had not been seen in approximately 20 minutes and walked over to the victim's trailer. The co-worker found the victim in the discharged pile of woodchips at the rear of the trailer. A call was placed for emergency medical services (EMS) and to the local police department. EMS and the local police department arrived within minutes. The victim was pronounced dead at the incident location. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1.) Minimize product jamming by ensuring that trailers equipped with both cross stability bars and movable floor unloading systems are not loaded up to or above the cross stability bar. 2.) Ensure that trailers with movable floor unloading systems are either equipped with automatic sweeping tarps or that manual sweeping does not begin until after the entire trailer has been offloaded. 3.) Conduct routine hazard assessments of equipment and how tasks are completed to identify potential hazards to which workers are exposed. 4.) Identify all confined spaces and develop, implement, and enforce a permit space program for permit-required confined spaces, such as trailers loaded with woodchips. 5.) Provide employees frequent training on loading, unloading, and clearing jammed product from tractor trailers. Manufacturers of movable floor unloading systems should: 6.) Consider providing safety decal(s) that address the hazards of engulfment, clearing jams, and overloading trailers.
