Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

He was the crew leader of a three-person ramp crew. The tug was traveling westerly at below five miles per hour toward an airport taxiway. The tug had a front seat designed to accommodate two individuals. The decedent was positioned on the back of the tug in an area behind the front seat that was designed for equipment storage. He was in a squat position with his right side against the seat frame. He held on to the seat frame to maintain his position. The decedent was wearing a baseball cap. It is postulated that a gust of wind began to blow the cap off of his head. In his attempt to keep his cap, the decedent released his hands from the seat frame, stood up and reached for it as it blew off. He lost his balance and fell to the tarmac on the driver's side of the tug. The tug stopped immediately and radioed for assistance. Emergency response personnel were on scene within one minute. They initiated treatment, and then the decedent was transported by helicopter to a nearby hospital. He died eight days later from complications of the head injury sustained at the time of the fall. Recommendations: 1.) Air museum staff should update the Ramp Crew Safety Manual to: 2.) Adopt a "No Seat - No Rider" policy, 3.) Comply with requirements of MIOSHA General Industry Safety and Health Standard, Powered Industrial Trucks, Part 21 and, Include industry best practices. 4.) Volunteer organizations, such as this air museum, should develop and implement a safety and health program that includes a safety policy, safe work practices and procedures, hazard assessment, volunteer training, and checklists to assist all members in the safe performance of their duties. The guard on the side of the chute did not fully enclose the chute to prevent entry. The machine was set on automatic mode with the electronic sensors blocked which meant that the machine constantly cycled. The company had a lockout/tagout program but it was not implemented by the victim. The company had a daily inspection log for the machine that was filled out by the operator. The CA/FACE investigator determined that in order to prevent future incidents, employers who use recycling machinery should ensure that: 1. Guards around chutes are fully enclosed to prevent entry. 2. Electronic sensors are not disabled or bypassed. 3. The lockout/tagout procedure is enforced when servicing or maintaining the baling machine. 4. Inspection of the baling machine is performed periodically by the supervisor in order to verify the machine's condition. At the time of the incident, the student was operating a jointer and was injured when the student's gloved left hand came in contact with the machine's cutting head. Co-workers attended to the injured student, and one co-worker transported the student to a local hospital. The student was then transported to a larger hospital in a neighboring state where the student's left hand ring finger was amputated at the middle knuckle. The Massachusetts Department of Public Health concluded that to prevent similar occurrences in the future, employers and schools with mill/carpentry shops should: 1.) Ensure that gloves are never worn while operating woodworking machinery AB - 2.) Develop, implement, and enforce standard operating procedures for operating machinery AB - 3.) Provide adequate supervision for young workers, new employees and other inexperienced workers AB - and 4.) Develop procedures, including calling 911, to be followed during emergency medical situations to ensure appropriate and prompt treatment. Employers and Job Placement Coordinators should: 5.) Ensure that Cooperative Education Agreements always include the knowledge and skills to be acquired by the students while at their cooperative work placements. School Cooperative Education or Job Placement Coordinators should: 6.) Routinely visit companies and work sites to which students have been assigned AB - and 7.) The crew was erecting a line of 40-foot tall columns equipped with crane rail sections and upper girders. At the time of the incident the victim was working from the basket of a raised boom lift waiting for a crane operator to set a steel roof truss so that he could weld it in place. Two of the columns and the attached crane rails and girders tipped over, hitting the victim's boom lift, knocking it to the concrete floor. The victim sustained head injuries and died at the scene. An investigation determined that no bracing or guying was used to support the collapsed column and crane rail structure. The victim was employed by an excavating and snow removal company. At the time of the incident, he and a co-worker were getting their loaders ready to remove snow from a shopping mall parking lot. Soon after he started his loader, the victim noticed that his left front tire mounted on a multi-piece wheel was "soft"(under-inflated). The multi-piece rim wheel referring to the tire assembly consisted of six components: the rim base, valve assembly, tire, side ring, O-ring and lock ring. The function of the lock ring was to hold the tire in place by locking and securing the other wheel components when the tire was inflated. The victim asked the co-worker, whose loader had an air compressor, to put air in the tire. The initial tire pressure was not checked. The co-worker connected one end of an air hose to the quick-connect fitting on the air compressor and the clip-on chuck at the other end to the valve stem of the tire. The co-worker then went to his loader's cab and used the throttle control to maintain the airflow. The air hose did not have an in-line pressure gauge. The victim checked the air pressure at the tire valve with a hand-held pressure gauge periodically as the tire was being inflated. The maximum cold tire inflation pressure specified by the tire manufacturer was 35 pounds per square inch (psi). After a few minutes, the victim informed the co-worker that the air pressure in the tire was about 15 to 20 psi. This was well below the 80% of the manufacturer recommended pressure. Under such tire pressure, the Occupational Safety and Health Administration requires the tire to be completely deflated, and the tire assembly to be dismounted from the vehicle, disassembled, inspected and reassembled prior to inflation. The co-worker told the victim not to let the pressure exceed 30 psi. As the victim continued inflating the tire, the co-worker placed a phone call from his cab to the owner of the company. As soon as he hung up the phone, the co-worker heard a loud noise. He ran to where the victim was and saw the victim lying on the ground with head injuries. The victim apparently was struck in the head by the lock ring that was propelled from the tire assembly. The co-worker immediately notified a mall security staff who called 911. Emergency medical services arrived within minutes. The victim was airlifted to a trauma center where he died two days later. New York State Fatality Assessment and Control Evaluation (NY FACE) investigators concluded that to help prevent similar incidents from occurring in the future, employers should: 1.) develop, implement and enforce a standard operating procedure (SOP) for inflating multi-piece rim wheels that requires that only trained workers service or inflate multi-piece rim wheels AB - 2.) ensure that all equipment operators receive training and demonstrate competency on the SOP as well as the safety hazards and precautions associated with inflating a multi-piece rim wheel AB - and 3.) provide an air hose that has an in-line pressure gauge and sufficient hose between the in-line pressure gauge and the clip-on chuck to allow workers to stay outside the trajectory of wheel parts when inflating a tire. The three granite installers worked for a subcontractor hired to install granite vanity tops and sinks in a newly constructed hotel. Work had commenced at 7:00 AM that morning. They were installing the second set of ten vanities for the day. All ten vanities, with back and side splash pieces, had been loaded onto a homemade three-sided construction box and lifted via a forklift to a second story window. The loaded construction box was not secured to the forklift. While the father unloaded the granite pieces from the construction box into the second story room, Son 1 was on the second floor working in another room, and Son 2 was on the third floor taking measurements. To reach the last vanity top, the granite installer climbed out the window and onto the construction box which then fell off the forklift onto the ground. Son 1 heard the noise, went to the room where his father was working and found his father on the ground outside of the hotel. He yelled for Son 2, and the sons went outside and found their father unresponsive. Emergency medical services were called and performed cardio-pulmonary resuscitation, and transported the father to the hospital where he was declared dead. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Slab carts should be used to transport finished granite slabs. Recommendation No. 2: Subcontractors should have their onsite competent person and their onsite safety person conduct a hazard assessment of the job site each day before work commences. Recommendation No. 3: Employers/ general contractors should ensure that all subcontractors have a written worker safety program. Recommendation No. 4: Forklift operators should be trained by a competent person to operate forklifts correctly and safely. Recommendation No. 5: Work should only be performed when the general contractor has a competent person on the job site. The victim used a hand-held CB radio to communicate with the pilot car operator, who was making runs along the road bringing the motoring public past the work zone area. The victim's job was to ensure that traffic flow did not impact the movement of trucks carrying asphalt from the staging area, from the asphalt paver, or departing from the work zone. The driver of a dump truck pulled out from a line of parked trucks, honked the horn twice and began reversing. It was noted that the truck's back-up alarm was working. The victim walked out from behind a parked trailer into the open lane of the road and was attempting to make radio contact with the pilot car operator. The victim was facing away from the backing truck when they were struck and run over. The employees lubricating the guide rails had to be within the moving parts of the spreader in order to reach certain areas of the guide rails. At the time of the incident, there was no written standard operating procedure or employee training for the spreader guide-rail lubrication procedure. NJ FACE investigators recommend following these safety guidelines to prevent similar incidents: 1. Lubrication procedures for intermodal cargo-container spreaders should be designed so that employees can perform this task without reaching into moving parts of the spreader. These procedures may require both specific tools and well-designed work practices. 2. Standard Operating Procedures (SOPs) and training specific to the maintenance and hazards of spreaders should be developed and provided to all employees who handle the loading and off-loading of intermodal cargo containers. These SOPs and training should include information on hazards and safe work practices. 3. Ground crews working with intermodal container-cargo spreaders should have the ability to operate the spreader, have panic buttons and interrupt switches available for use in the event of an emergency, and at least one ground crew member should act as a spotter. 4. Communication among all workers must be effective and include properly operating voice communication systems. Communications must be real-time and allow for employees to communicate from the location where they are working. 5. Employers should investigate equipping the mobile equipment, such as the cargo-container or the crane, with additional visual or sensing devices to enhance the operator's ability to detect the presence of workers on foot near the spreader. This includes technologies such as cameras, radar, and/or sonar to alert the operator to the presence of workers in "blind areas," as well as tag-based warning systems, which can detect workers wearing tags. 6. Employers should conduct a job hazard analysis of all work activities with the participation of the workers. Department of Public Health AB - On May 5, 2009 a 49-year-old male municipal laborer (victim) was fatally injured while performing the task of collecting refuse from private residences. The victim and a co-worker were riding on the rear step of a refuse collection truck while traveling from one residence to the next. The truck drove over a depression in the roadway causing the victim to fall from the back of the truck. The victim landed on the asphalt roadway with the back of his head striking the ground. The truck driver noticed the victim and stopped the truck. The co-worker climbed off the truck's rear step and ran to the closest house to call for emergency medical services (EMS). The truck driver stopped a passing motorist to help him attend to the victim. Within minutes EMS and the local police arrived at the incident location and the victim was transported to a local hospital where he died two days later from his injuries. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1.) Ensure that employees are never positioned on refuse collection trucks' rear riding steps when the trucks are traveling faster than 10 miles per hour or traveling more than 0.2 mile AB - 2.) Ensure that alternative transportation options are available for employees when there is not enough legal seating within a refuse collection truck's cab for all work crew members and the truck will be traveling faster than 10 miles per hour or further than 0.2 mile AB - 3.) Consider adequate legal seating for the entire work crew inside a truck's cab when building and/or purchasing refuse collection trucks AB - 4.) Help ensure employee safety on the road by developing a traffic safety program and promoting traffic safety principles AB - 5.) Routinely provide employees with refresher trainings that are updated as needed AB - 6.) Ensure that employees have access to a communication device that can be used during emergency situations AB - and 7.) Consider the feasibility of implementing automated processes for refuse collection. In 2007, the logging industry employed 101,000 workers, and accounted for 88 deaths. This resulted in a fatality rate of 87.1 deaths per 100,000 workers that year. This rate is over 23 times higher than the overall fatality rate in the US in 2007 (3.7 deaths per 100,000). This excessive risk for fatal work injuries points to a need for prioritizing research and intervention programs to make this industry less hazardous. Data were compiled from Washington State Fatality Assessment and Control Evaluation (FACE) surveillance activities for the years 1998-2008. Fatalities were determined to be logging related based on the North American Industry Classification System (NAICS), the Standard Occupational Classification (SOC) system, and other details that indicate relationship of fatalities to logging. These data are unique to the FACE program and methods of collection and classification may differ from other data sources. There were 67 total logging related fatalities in WA State from 1998-2008. The majority of logging related fatalities occurred to workers in the logging industry. Workers in the truck transportation industry made up 20% of logging related fatalities. Nearly all of the 67 total fatalities can be categorized into three types of incidents. These were struck by object, motor vehicle, and machinery incidents. Washington State logging related fatalities peaked in 1999 with 14 and declined dramatically in 2000. From 2000, the number of fatalities increased and held relatively constant at 6-7 per year until 2006 when they started to decline. The decline continued until 2007. The trends in logging related fatalities were comparable to trends in logging dependent industries in WA State and the region during 1998-2008. Construction and wood products industries were operating at high levels of production in 1999 and suffered great declines in 2000. There was some economic recovery from 2000-2005 and more decline from 2005-2008. Stacked on top of the 12-inch stop log were four 8-inch wide stop logs, also turned on their edge. The five stop logs were held in place by a channel in the concrete piers. At the time of the incident, each section had its full complement of stop logs. A large amount of precipitation and the area's soil composition caused water to run off to the field's drainage ditches and into the county drain resulting in high upstream water levels in the drain and ditches and field flooding. The decedent arrived at the water level control structure, parked his vehicle, removed his work boots, and donned chest waders. He stood downstream in the southernmost section and used a crowbar to loosen the top stop log. He lifted the top stop log up and out of the channel. After removing the stop log from the channel, he either slipped and lost his balance or was knocked over by the force of the water rushing over the remaining stop logs. A nearby landowner saw the decedent's unattended vehicle, and when he couldn't find him, called for emergency response. The police arrived, and called for the dive team. The decedent, who was not wearing a life jacket, was found 40 feet downstream, caught by low hanging tree branches. Recommendations: 1.) The drain commissioner should ensure that a standard operating procedure to add/remove stop logs for each of the county's water control structures is developed and implemented. This procedure should prohibit individuals from entering the drain to remove the stop logs. 2.) The drain commissioner, when a possibility of drowning exists, should require volunteers to wear an approved life jacket/buoyant work vest and have a ring buoy available. 3.) The drain commissioner should have an annual safety meeting in early Spring with the volunteer cooperative members to review safety and operation procedures. 4.) The drain commissioner should review all water level control structures walkway handrails to ensure compliance with MIOSHA General Industry Safety Standard, Part 2 - Floor and Wall Openings, Stairways, and Skylights. The decedent's left sleeve became caught on the bolts as the shaft rotated. The corn elevator and the gravity box he was unloading were empty. His son, who came out to help him, found him lying on the ground with his head resting on the motor and his shirt tightly wrapped around his upper torso and neck. His son ran to the house and called for his mother to come outside. While emergency response was en-route, the decedent's son found a knife and attempted to cut the decedent's clothing away from him. Police and emergency response arrived and declared the decedent dead. The police and medical examiner freed the decedent from his entangled clothing. Recommendations: 1.) Farmers should inspect all farm equipment to ensure that all exposed rotating or moving parts are guarded/shielded. If guards/shields are missing, appropriate guarding should be installed. 2.) Farmers should ensure farm-modified equipment is constructed using appropriate parts. 3.) Farmers should consider adding a shut off/bypass valve on the hydraulic motor for trash removal near the motor unit. 4.) Machine operators should not wear loose-fitting or frayed clothing. Operators should be aware that jackets and sweatshirts with draw-strings can also be caught in unguarded rotating/moving parts. Ensure that employees never position themselves in the trajectory (in front of or over) of inflated tires mounted on multi-piece rims. 3.) Perform routine inspections of all multi-piece rim wheels to identify damaged components. 4.) Ensure multi-piece rim wheels with mounted inflated tires are never struck with hammers or other objects. 5.) Develop standard operating procedures (SOPs) for servicing multi-piece rims and provide training on these procedures. Fatality Assessment and Control Evaluation (FACE) Program AB - On February 18, 2007, a 65-year-old male jockey (the victim) died after being thrown from his mount in the starting gate during the start of a race. The victim and his mount were being led into the eighth stall of an eight-stall mobile starting gate. As this was being done, the horse in the fifth stall of the starting gate reared up and struck the door of the starting gate. All eight stall gates opened simultaneously. A review of a videotape of the start of the race indicated that the victim had not completely settled onto his mount when the horse in the fifth stall broke for the start of the race. The victim was thrown backward and seemed to hit his head on the back door of the gate stall. Since the horses would have to pass the location of the starting gate to complete the race, the starting gate had been hooked to a tractor for quick removal from the track. When the tractor operator saw the horses break from the starting gate, he began to pull the starting gate from the track. When other track workers saw the horse without its rider, they signaled for the tractor operator to stop. The starting gate, which had been pulled up onto the trunk of the jockey, was pushed backward. Rescue personnel stationed in an ambulance at the track responded immediately. The victim was transported to the hospital where he died in surgery approximately four hours after the incident. It could not be determined whether the starting gate had opened due to the horse bolting against the gate in the fifth stall, or due to being opened by the starting gate operator. NIOSH investigators concluded that, to help prevent similar occurrences, track operators should: 1.) Designate sufficient spotters located at the front and rear areas of both sides of the starting gate to ensure that all horses and their riders have cleared the starting gate area prior to moving the starting gate in any direction. 2.) Establish a preventive maintenance program for the track's starting gate that would require periodic inspection, pre-race testing, and maintenance of the gate's hydraulic system to ensure proper function. 3.) Consider evaluating equipment such as starting gates to ensure that jockeys and horses are provided a safe work environment. 4.) Ensure that jockeys wear Personal Protective Equipment (PPE) such as helmets and vests that are properly fitted and in good condition. On the incident day, a wrecking yard customer was using a manually operated portable gantry crane, built by a local metal fabrication shop, to remove an engine and transmission from a salvaged vehicle. A gantry crane is a type of crane which lifts objects by a hoist. This gantry crane had four automobile wheels with pneumatic tires installed on the bottom of the horizontal supports allowing the whole crane to traverse. The customer was having difficulty moving the loaded crane and was aided by the victim. As the customer and the victim were manually moving the gantry crane, it began to roll freely. The crane rolled into a shallow depression, became unbalanced and fell, striking the victim in the head as it toppled over. Witnesses called 911 within moments of the incident. Salvage yard persons attended to the victim until emergency help arrived, but the victim died at the scene. Guidelines: To prevent similar occurrences in the future, the Washington State Fatality Assessment & Control Evaluation (FACE) investigative team concluded that any business engaged in salvage / wrecking yard or similar operations should follow these guidelines: 1.) Employers must select and make available the most appropriate crane or lift equipment manufactured and certified for the job. 2.) All crane operation / lift equipment must be used according to manufacturer's guidelines. If manufacturer's guidelines are not available, use established best practice for all crane lift equipment used. 3.) All employers and crane operators need to clearly understand and control site conditions and limitations for all crane lifts and crane travel at the site. 4.) A portable or mobile crane must only be used on a surface that can safely support its lift capacity and travel capability. 5.) Employers must conduct training for all crane operators, users and support personnel to ensure that all workers understand the hazards associated with portable and mobile cranes and crane lifts. 6.) Employers need to diligently supervise and coach young workers beyond the level provided for experienced adult workers in order to help young workers recognize and avoid the hazards of performing tasks at the organization's site. The victim was providing labor support inside the temporary traffic control activity area where a new roadway surface was being constructed next to an existing state highway to widen the roadway. There were three construction vehicles operating in the work zone, including a grader, compaction machine, and water truck. The driver of the 3-axle, 3500 gallon water truck was tasked with wetting down sections of roadbed material. The driver of the truck checked his mirrors and began reversing with the backup alarm sounding. It is unknown what the victim was doing at this time or why he entered this area of the work zone, but it appears that he was walking away from the backing truck, which caught up with him and struck him. The victim was in the driver's blind spot. The victim was pronounced dead at the scene. The truck had been backed up to the loading dock, the parking brake set, and the transmission placed in neutral. However, the truck wheels were not blocked against motion. The salesman initially operated the forklift to release tension on the winch line as the truck driver removed the binding chains. As the truck driver went to store the binding chains, the salesman backed the forklift off the bed of the truck. The truck bed was 9 inches below the loading dock, and the dock plate connecting the truck to the dock was set at an incline. The drive wheels were on the front of the forklift (to the rear in this instance), and as the salesman accelerated to go up the incline, the drive wheels on the bed of the truck pushed the truck away from the dock. The dock plate slipped off the truck bed and the forklift fell 4 feet to the ground. The victim was crushed between the forklift and the loading dock.
