Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

Due to layout of the farm's driveways, the semi could not back straight into the chute area but had to back in on an angle, which made it more difficult for the trailer to be in alignment with the chute. The animal delivery occurred in the early morning. The decedent, after conversing with the semi driver, walked back to the chute and at some point, climbed onto the chute floor. As the semi was backing to the southeast, it appears the decedent placed his head between the backing trailer and the chute wall, most likely to determine proper alignment of the backing trailer. During the backing of the semi, the driver stated he could see the decedent's hand waving him back. The decedent's head and neck were crushed by the trailer against the loading chute. The decedent was declared dead at the scene. They hope to make sure all workers can get down from a crane safely, regardless of physical ability. Construction equipment operators, riggers and maintenance personnel are the most at risk of a fall. While not necessarily fatal, injuries to backs and knees may be lifelong and deteriorate quality of life and ability to work. In this case the victim had difficulty dismounting from the crane turntable deck, but problems could equally arise with any areas where workers may be expected to perform work. Washington State Fatality Assessment and Control Evaluation, (FACE), came up with ways to prevent similar occurrences in the future. It has put together the following guidelines for all users of mobile cranes and other mobile equipment. 1. Training/hazard identification Decreasing the risk is about identifying the most risky activity and letting employees know. This usually means creating a written company policy with a crane mounting and dismounting procedure, such as: A. Maintain three points of contact - two hands and one foot, or one hand and two feet - with the equipment or ground at all times.B. Face toward the equipment, both when mounting and dismounting, for improved contact and balance. C. Never jump, as this could lead to a slip or an ankle/knee injury. D. Prevent slips by wearing clean, slip-resistant footwear and looking for mud, sand or ice. E. Mount and dismount equipment only where steps, ladders, and handrails/handholds are provided. F. Use a hand line and bag/bucket to raise or lower equipment so hands are free to climb. G. Make a procedure not only for the cab but also for any other part where work is carried out. The next step is putting it in action by training and supervising employees. 2. Buy or refurb cranes with rungs and steps: Whether or not you have a training program, you should at least have a crane with the right steps and handholds, low enough for people to access, because it is more effective to provide the right equipment than to enforce wide scale changes in employee behavior. The best mobile cranes for safety are the ones that have easy access by steps, ladders, handrails, handholds, platforms and guardrails that allow workers to enter and exit the cab and other crane parts. Equipment access systems should be designed for the ease and safety of the user. If these systems are absent or poorly designed, then there is an increased risk of a fall. Crane owners can look at existing access equipment to make sure it is low enough for current employees to reach. Employees should able to easily reach steps, handrails, and handholds, both during entry and exit or mounting and dismounting. Employers can also get employee input on access when buying new equipment or retrofitting older equipment. Large employers can review injury data to see whether injuries are occurring within their employee base. When purchasing or retrofitting older equipment be sure that the access systems are designed with sound safety engineering principles. Ask the equipment's manufacturer or distributor about safe access systems. This may be available from distributors or directly from the manufacturer, so long as it is compatible with the crane. "After market access systems are available, but must fit the equipment, suit the user's needs, and incorporate elements of safe access design," says Jalonen. FACE says "Many falls from mobile equipment and vehicles can be attributed to nonconformance to established design standards and guidelines." When checking whether your manufacturer's crane to see if it has the best access equipment, you may want to check it against a list from 2007 which supplies a list of basic access requirements. The guidelines from American Society of Mechanical Engineers' consensus standard ASME B30.5-2007 Mobile and Locomotive Cranes are not comprehensive description of all criteria to be considered when designing and implementing safe access systems, but is meant to give an idea of what to have. Some minimum criteria for mobile equipment and vehicle access systems are: A. It should be easy to use them without special training. B. They should minimize protrusions that could cause a user to trip. C. All surfaces should be non-slip. D. The shortest expected user should be able to easily reach the first step from the ground. E. Components of the access system should be placed to allow and encourage the user to follow the three-points of contact method and face toward the equipment while ascending/descending or mounting/dismounting. F. Two handholds or handrails (handrails are preferable) should be accessible to the shortest expected user from the ground and while ascending or descending. G. Handrails/handholds should be placed along the access system to provide continuous support so as to allow the user to maintain balance. H. Steps should be coordinated with properly positioned handrails and handholds so that three points of contact can be maintained. I. Handrails/handholds should have smooth surfaces. J. Step design should provide the user with natural foot placement or should be clearly visible to the user while descending. K. Steps or rungs should allow the midpoint of the shoe, not just the toe, to rest solidly on the step or rung. L. Steps should be wide enough to stand on with both feet. M. Steps should be uniform in size and shape. N. Steps should be designed to allow minimal accumulation of mud and debris. O. The height and depth between steps should be uniform. 3. Repair and replace dysfunctional access equipment Steps ladders and handrails get dirty or break, and employers should include this in any checks. Checks to surfaces may include ensuring that walking and working surfaces of the equipment have anti-slip surfaces, that these work, and that they are clean. Employees should be encouraged to take action at the earliest stage when they observe a breakdown, whether by notifying a maintenance person or the employer. They should be advised to remove accumulations of rainwater, ice, and snow. 4. Consider older workers' safety The advancing age of employees in the industry often changes employees' ability to keep up with physical tasks. "Older workers are at higher risk of fatal falls, and workers 55 and older have the highest rate of fatal falls. The average height of fatal falls for older workers is about a third shorter than the average height at which younger workers die from falls," says Jalonen. Many employers nonetheless wish to maintain such employees as they bring their valuable experience and expertise to the workplace. Employers can take measures to ensure that their employees as they age will still be able to perform their jobs. Again, keeping adequate steps and handholds, enough of them and within reach, is essential to making access safer and easier for older workers. Consideration must be taken not only for the condition of decreased physical strength but for vision, low flexibility, instability of gait, and other health conditions. 5. Manufacturers should increase access offerings FACE has concluded that the availability of mobile equipment access systems is a key factor in determining whether users are able to perform their work activities safely. Poorly designed systems can be a major contributor to falls, and so they should be designed to eliminate or minimize fall hazards. "Equipment access steps, ladders, handrails, and handholds should be designed and placed in accordance with sound design principles taking into account the safety needs of all potential users," says Jalonen. "The design and placement of access system components determine how users will interact with them and whether their interactions result in easy and safe access to and from equipment or whether they result in unsafe actions or falls. Improvements in design are more effective than relying on changing worker behavior. Manufacturers, too, need to take note of the study. FACE found it concerning that not all manufacturers consider ergonomics when designing equipment. FACE says, "If key components of an access system are missing, cannot be safely reached, involve an awkward posture, provide inadequate footing or handhold, or require use of a part of the equipment not designed as part of the access system (such as stepping on a vehicle tire), then risk of potential injury to users is increased." Workers are not to blame for jumping and causing injury to themselves or others, even fatal, when there is no step for getting down in the first place, FACE says. "Risk taking behaviour by workers, such as jumping down from equipment, may be the result of lack of or inadequate or poorly placed steps, handholds, and handrails." Designers and manufactures of mobile equipment, as well as their clients, may look to available guidance as to design of access equipment, FACE says. Guidance includes the National Institute for Safety and Health's "Prevention through Design" initiative and the American Society of Safety Engineers' Prevention through Design guidelines when creating access systems. These measures are intended to encourage business leaders, designers, and manufacturers to "design out" or minimize hazards so as to prevent occupational injuries and fatalities, says FACE. FACE strongly suggests that manufacturers not already doing so provide safe access systems as a standard part of their mobile cranes. 6. Safety regulations need more thinking out Researchers have noted that occupational health and safety regulations and design standards are 'incomplete, inadequate, vague, or non-specific, providing only general specifications and guidelines'. That means that these days it is up to users to evaluate their access systems to accommodate their workforce's needs. FACE proposes that more specific future government regulations and industry standards should be developed. Research could be into personal characteristics, anthropometry, demographics, and injury experience of the user population that is leading to the falls. "The results of this research should be used to inform equipment designers and manufacturers, industry associations, standards boards, and occupational health and safety regulators," says FACE. As he crossed the roof to the weather vane, he tripped and fell through the opening 15 feet to the breezeway floor below. There were no safeguards in place to prevent the fall through the opening. The work crew included the company owner, the owner's son, and the victim. They were on and off the roof several times over the past four weeks on the job. Key contributing factors identified in this investigation include unguarded roof openings, lack of employee training, and lack of a job hazard assessment to determine potential risk factors employees may encounter. The CA/FACE investigator determined that, in order to prevent falls through roof openings, employers of house painters should develop, implement, and enforce a comprehensive written Injury and Illness Prevention Program (IIPP) that includes hazard recognition, worker training in the language(s) and at the literacy level(s) of workers, and procedures on how to protect against unsafe conditions. Bureau of Occupational Health and Injury Prevention AB - In October 2007, a 44 year-old self-employed master stone mason (the victim) died after falling approximately 30 feet to the ground from the upper tier of a handmade, wooden, two-tiered work platform. The victim was a subcontractor on the renovation project of a 19th century mansion that included the construction of a second story addition. At the time of the incident, the victim had been applying a stucco finish to the addition and painting metal trim at the edge of the roof from the upper tier of the platform. He was preparing to exit the platform onto the roof. The work platform, used for moving both materials and employees, had been fabricated by the victim and an employee of the general contractor (GC) two weeks prior to the incident. The platform had been fastened to the forks of a powered industrial truck (PIT) known as a telescopic handler. The front guardrails on the upper tier of the platform were not in place at the time the incident occurred. The victim was not wearing any type of fall protection as he worked near the unprotected front side of the upper tier. Safety procedures were not in place requiring entry/exit three-point contact (two hands and a foot or two feet and a hand) and 100% tie-off. One other worker was on site on the day of the incident. He was an employee of the GC and the same employee who helped the victim build the two-tiered platform. At approximately 3:15PM, the victim asked the employee to lower the platform slightly. The employee stated that he went down to ground level and climbed into the cab of the telescopic handler, waiting for the victim to exit the platform and tell him when he should move the platform. He stated that he could not see what the victim was doing, since the boom of the machine was elevated and the platform blocked his view. He saw a paintbrush fall and then saw the victim fall, landing on the ground at the base of the addition. The victim was transported to a local hospital where he was pronounced dead from severe head and upper body injuries. New York State Fatality Assessment and Control Evaluation (NY FACE) investigators concluded that to help prevent similar incidents from occurring in the future, employers and contractors (general and independent) should: 1. Strictly follow the manufacturers' instructions on proper usage of the powered industrial trucks (PITs) such as telescopic handlers, and never use any PITs as man-lifts or equip them with any form of personnel work platform, if they are not so designed. 2. Require that powered industrial trucks, such as telescopic handlers, are not modified or authorized to be modified in any way. 3. Perform a hazard assessment before starting any work project that involves material movement and the lifting of workers in order that the correct equipment is selected for use. 4. Develop and implement a fall protection plan that includes training and equipment requirements to protect all employees, subcontractors and site workers who may work at heights six feet or more from the ground or lower story. 5. Require that all workers who operate powered industrial trucks (PITs) have received the required training and are certified to operate the specific machinery in use at a work site. 6. Establish and clearly communicate an enforcement policy for all employees, subcontractors and site workers that states that work practices are expected to be performed in a manner consistent with applicable ANSI regulations, OSHA standards and company health and safety policies and consequences will be applied if there is a failure to do so. Accountability for ensuring that compliance is maintained is the responsibility of the employers and contractors, including general contractors on a multiemployer work site. Additionally, 1. Local town governing bodies and codes enforcement officers should consider modifying building permit applications to require building permit applicants to certify that they will follow written fall protection plans in accordance with applicable standards and regulations, for any projects that involve working at heights above 6 feet, before the building permits can be approved. 2. Local town governing bodies and codes enforcement officers should receive additional training to upgrade their knowledge and awareness of high hazard work, including work at elevations. The decedent used a neighbor's 20-foot aluminum extension ladder rather than the company-issued 28-foot fiberglass ladder located on the work truck. Two lines from a nearby utility pole entered the home at this location. One of the two lines was a cable TV line and the second line was thought to be a phone line but this could not be confirmed. He placed the ladder against the outside of the home under the entering lines to reach a second story window located 19 feet 7 inches above the ground. The ladder was extended to its working height of 17 feet and positioned 4 feet 6 inches from the wall. The ladder was rated for a 200 pound individual AB - the decedent weighed 231 pounds. The lines were located between the decedent and the ladder. The decedent had an unwitnessed fall. The decedent lived directly next door to the jobsite and was scheduled to have computer work done at his home on the day of the incident. When the computer repair person arrived and did not find the deceased at home, he called his cell phone. The repairman heard the decedent's cell phone ringing. He followed the sound to the incident location and found the decedent on the driveway. He called for emergency response. The ladder was propped against the home and an unidentified line had been disconnected from the home. The decedent was declared dead at the scene. Known Contributing Factors: 1. The victim ran from the vehicle with the bucket of fuel, leaving a trail of spilled gasoline which ignited. The ignited trail of fuel caused the bucket of fuel to also ignite, burning the victim. Contributing factors identified in this investigation were the positioning of the drop light, the type of drop light used, the use of a bucket to store and pour gasoline, and the filling of an incompletely assembled fuel system. The CA/FACE investigator determined that, in order to prevent fires within an automotive repair shop, owners should ensure that: 1. Sources of ignition are eliminated when work involves flammable liquids. 2. When refueling vehicles with a portable container, the container should be equipped with an automatic closing cap and flame arrester. 3. Gas tanks are properly installed before refueling. The grinder then deposited the asphalt in the back of a dump truck. When the dump truck was full the grinder would back up so a sweeper could clean up material. An empty dump truck would then back up to the grinder to continue the process. The highway was a four-lane road divided by Jersey barriers with two lanes in each direction. Prior to the incident the victim was part of a night crew working in the closed right lane of the highway. The victim was working as a groundman by walking alongside the pavement grinder and guiding the operator. At 10:36 p.m., a reversing dump truck ran over the victim as the crew was repositioning the grinder. The incident was unobserved and it is uncertain what the victim was doing at the time. An investigation found that the dump truck's lights, mirrors, and back up alarm were in working order. He started work at 10:30 a.m. and last spoke with his foreman about 2 p.m. The foreman told him to finish spreading the bait in the area he was working, then continue in a block of cherry trees until his 3:30 p.m. quitting time. The cherry trees were 10 feet from each other. The rows were 15 feet apart on a slope varying from 8 to16 degrees. When the victim did not return home his wife contacted the foreman, who sent workers to look for him. At 10:15 p.m., the victim was found deceased beneath the overturned tractor. An investigation determined the tractor had no seatbelt, but did have a manufacturer-installed two-point foldable rollover protective structure (ROPS) that was not deployed and was in the "down" position. The area of the orchard where the victim was working did not have overhead clearance issues and therefore the ROPS should have been used. At the time of the rollover, the tractor was traveling downhill and was equipped with front tires that were light truck tires, not the original tractor tires. The victim was using the lathe to shape an eccentric shaft. When he reached over the work piece to smooth out a radiused transition with emery cloth and a file in hand his loose clothing (left arm sleeve) became entangled around the rotating work piece near the lathe tailstock. He was pulled into the part and suffered multiple fractures to his left arm and injuries to the neck and chest as he was struck repeatedly by the eccentric shaft. A co-worker rushed to his aid and instinctively pushed the red button on the lathe to stop the machine. However, the red button was not an emergency stop and the machine continued. The victim was lifeflighted and died enroute to the hospital. The co-worker suffered non-life threatening injuries. RECOMMENDATIONS: 1. Employers should establish, communicate and enforce a clothing policy specific to the work environment and particularly for machine operators. 2. Employers should establish formal training on recognized hazards, injury prevention and emergency procedures, and to communicate expectations to adhere to safe practices and policies. 3. Supervision should monitor, reinforce safe behaviors, and immediately correct unsafe behaviors or conditions. 4. Job hazard analyses should be conducted by operators and supervisors and should include a review of the manufacturer's equipment/operator's manual, machine labeling, color coding, etc., especially for legacy machines. 5. Employers should implement preventive maintenance and inspection processes for hazardous machines. Where necessary, employers should install, adjust, label and/or repair appropriate controls (e.g., machine guards, emergency stops). OHSP also conducts in-depth investigations of select fatal occupational injuries as part of the Fatality Assessment and Control Evaluation (FACE) project, supported by the National Institute for Occupational Safety and Health (NIOSH). The purpose of the FACE project is to develop a detailed understanding of how fatal injuries occur and to develop recommendations to prevent similar incidents in the future. These recommendations are disseminated to industry, labor, equipment manufacturers and others in positions to take action to prevent work-related deaths. This update provides an overview of fatal injuries at work that occurred in Massachusetts during 2010. These include deaths traditionally linked to the work environment such as falls, electrocutions, and exposure to toxic chemicals. What happened in Massachusetts?: In 2004, a 43-year-old contractor was hired to refinish a bathtub. While stripping the bathtub with an MC-based stripper, he was found unconscious in the bathroom slumped over the bathtub. The bathroom did not have windows or exhaust ventilation. The victim was wearing a disposable respirator while stripping the bathtub. What can be done? To prevent similar incidents and risk of severe injury and death: Do not use MC-based strippers: MC-based strippers most likely cannot be used safely in small and medium enclosed bathrooms. Use alternative stripping methods, such as sanding and using non-MC-based strippers. WARNING: Alternate methods may have their own hazards of which to be aware. The California Fatality Assessment and Control Evaluation (CA/FACE) program tracks and investigates cases of fatal injuries at work, and makes prevention recommendations for employers and workers. The CA/FACE program is investigating the preventable death of a 24-year-old worker who was using a paint stripper inside a baptismal font (tub) of a church. What happened? The victim was removing a waterproof paint coating from the baptismal font (tub) of a church. The font is located in a narrow, enclosed room (41 feet long by 4 feet wide by 12 feet high) and bordered by two sets of steps. There are two doors on each end of the room and a large window facing the congregation. The victim applied one gallon of a paint stripper containing 60 - 85% methylene chloride to the steps and font. The paint stripper is a widely available consumer product and was purchased by church staff at a local home improvement store. There was a small portable fan close to the work area and the victim did not use respiratory protection. The victim probably did not know how quickly he could be overcome by the methylene chloride vapors in a closed room. The church pastor saw the victim working in the late afternoon. At that time the doors and window were open and the paint stripper had been applied to the font surface and steps. Later that night, however, another church staff person found the victim motionless on the floor of the font with the doors and window closed. He was pronounced dead by emergency medical personnel. What was the cause? The victim died from inhaling toxic amounts of methylene chloride in a small, poorly ventilated space. Methylene chloride vapor can build up in air quickly and can be fatal if too much is inhaled. Between 2000 and 2011, methylene chloride-based paint strippers have been linked to 13 deaths of workers refinishing bathtubs. Because methylene chloride-based paint strippers are widely available at local home improvement stores, employers and consumers can easily purchase this potentially toxic product. What should be done to prevent this from happening again? Employers and employees should recognize that methylene chloride can quickly build to toxic levels in poorly ventilated spaces. Warning labels are often not adequate to guide safe work practices in these spaces. Precautions should include an air-supplied respirator, special protective gloves, and adequate mechanical ventilation. Because of the serious harm that can occur from inhaling methylene chloride vapors, employers, workers, and consumers should always consider safer alternatives to strip paint from surfaces. Abrasive sanding with local exhaust ventilation to capture dust, or benzyl alcohol based strippers (without n-methyl pyrollidone or NMP) can be used with good results. These safer strippers can be found at select local stores. As the steel worker stepped over the lifeline, he did not attach the lanyard and fell approximately 28 feet onto the railroad tracks below. He was taken to a nearby hospital where he died in surgery approximately two hours later. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: A safety analysis should be performed by a Competent person at each job site before work commences. Recommendation No. 2: Employers should ensure all employees use personal fall arrest systems correctly. Recommendation No. 3: Employers should implement and enforce a written safety policy which states the consequences of not following personal fall arrest policy. Recommendation No.4: General contractors should ensure through contract language that all subcontractors implement and enforce appropriate safety and health programs and training specific to the work to be performed. The victim was hired by a homeowner to trim the branches of a tree growing into utility lines in his front yard. The victim was wearing a full-body harness for fall protection, and had a climbing rope, flip line, drop line, and climbing spurs on his feet. He used a chain saw to cut away the branches of the tree. A branch that was cut did not fully detach from the tree and struck the energized electric power lines. The electric current flowed through the cut branch and into the victim who was attached to the tree with his climbing spurs. Contributing factors identified in this investigation were improper work practices in line-clearance tree trimming, and working in close proximity to energized utility lines. The CA/FACE investigator determined that in order to prevent future incidents, tree trimmers who perform line-clearance tree trimming should ensure that: 1. The electric utility company is notified whenever tree trimming is closer than ten feet to energized high-voltage lines AB - and, 2. They have received training and certification in line-clearance tree trimming operations by qualified instructors. In addition, homeowners who need trees trimmed or removed that are in close proximity to high voltage lines should: 1. Contact the electric utility company and request they trim the trees AB - or, 2. Hire only qualified line-clearance tree trimmers or contractors who are trained and certified by organizations such as the ISA (International Society of Arboriculture) or the TCIA (Tree Care Industry Association).
