Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

After the delivery, he realized a tie-down strap was inside the warehouse. He pulled the truck a short distance away from the loading dock and returned to get the strap. Standing between the semi-trailer and loading dock, he banged on the roll-up door. The receiving company employee opened it, handed him the strap, noticed the semi-trailer moving, and yelled a warning, but the driver was pinned. The truck was pulled forward off of the victim by the warehouse employee. The ramp had a 2 degree slope. The wheels of the trailer were not chocked, and the warehouse employee reported that the victim left the truck running and in neutral with none of the brakes set. However, the Fire Department reported that "a trailer brake" was set, but not the tractor parking brake. The Fire Department also moved the truck, chocking the wheels to establish a safe work area. There were no mechanical problems found on the tractor or trailer during the post incident inspection. The brake systems were working properly. Multiple factors may have allowed the truck/trailer movement. Based on interviews with eyewitnesses and with trucking experts, it is likely that the parking brakes were not set, allowing the truck and trailer to move and crush the victim. The slider axle of the trailer was also unlocked, which could have allowed the trailer to move on the rail over the axle as the truck rolled backwards. RECOMMENDATIONS: 1. Fully engage tractor and trailer parking brakes before leaving the cab. 2. Use wheel chocks to secure trailers and tractors against inadvertent movement, especially when parked on a slope. 3. Confirm that the sliding axle assembly is locked prior to working in or around a trailer (trailer had potential of 8 feet of movement on this model). 4. Do not work or stand in pinch points where vehicles could move or roll. 5. Employers should ensure that workers are trained to operate the controls of rental equipment prior to use, especially key components such as locking mechanisms and brakes. 6. Employers should periodically monitor worker performance to ensure appropriate and adequate safe operating procedures are being used. 7. While current options for automatic axle locking and braking mechanisms are limited, employers should seek out built in protective features, such as automatic locks and brakes, when renting or purchasing equipment. They were standing on the scaffold and holding a sheetrock panel against the wall when the scaffold moved backward on the victim's side. He fell between the scaffold and the wall and hit his head on the concrete floor, and died two days later. The lead worker had locked the wheels on his side, but the younger worker had not. The victim was new on the job and had not had general safety or scaffolding training. So here are some ways we can prevent something like this from happening where we work. Lock all four wheels on mobile scaffolds and check stability before you climb. Look out for younger workers by watching them work and teaching them proper safety steps for preventing falls. Guardrails and toe guards are required on scaffolds 10 feet or higher above the ground, but we can use them at lower heights for added fall protection. ASK: "Does anyone have more ideas or comments to share?" Pause for discussion. Then see if there are ways to take action. END WITH ACTION PLAN (ideas for what to ask or say). The victim started down the ladder, but the plywood landed on him in two sections. Heavy equipment was used to remove the plywood while another person called 911. The victim was conscious just after the incident, he said that he thought his back was broken and could not move his hands or feet. The victim later died at the hospital. So here are some ways we can prevent something like this from happening where we work. Stack plywood and/or other supplies level and flat on a cricket or table, rather than angled to the pitch of the roof. Make sure that ladders are positioned to allow safe access and never set up a ladder under an unstable load of material. In pre-planning, we should consider the use of access equipment other than ladders. ASK: "Does anyone have more ideas or comments to share?" Pause for discussion. Install guardrails, covers, or safety nets to block openings or edges that are over 6 feet above the ground, like unfinished stairwells. Wear a fall arrest system if working near a fall hazard 6 feet off the ground. Know your location so you can give an address or directions to emergency responders if needed. Occupational Health Surveillance Program AB - Even in today's tough economy with fewer teens working than in the past, on average, over 20% of Massachusetts 15- to 17-year-olds are employed at any given point in time.(1) While work can provide benefits to teens, it can also pose health and safety risks: nationally, young workers have about twice the rate of nonfatal work-related injuries per hour worked than older workers.(2) This is partly explained by the fact that teens tend to be hired into industries that pose high risk for workers of all ages, such as restaurants and health care settings. Inexperience and lack of health and safety training are also factors. More information about where and how teens are injured at work is essential to promote safer work opportunities for youth. Since 1993, the Teens at Work: Injury Surveillance and Prevention Project (TAW) at the Massachusetts Department of Public Health has tracked work-related injuries to teens under age 18 and collaborated with partners on a variety of initiatives to reduce workplace hazards and prevent injuries to working teens. Efforts by TAW appear to be making a difference. Since the start of TAW, Massachusetts has seen a decline in the rate of work-related injuries among teens - faster than the decline in the injury rate for adult workers (2011 TAW Update). As we approach 20 years of the Teens at Work Project, it is a time to both reflect on our successes and remind ourselves of the work still to be done in ensuring that teen jobs are safe and that teens have the basic health and safety skills to protect them now and in the future. While climbing onto the truck, he slipped off the truck, fell to the ground and then was struck by the driver's side rear wheels. When the victim fell, the two co-workers, who were standing on the truck's rear riding steps, yelled stop multiple times and the truck driver stopped the truck. Multiple co-workers called for emergency medical services (EMS). Personnel from the local and state police departments, fire department and EMS all arrived within minutes of the calls. The victim was pronounced dead at the incident location. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Ensure that employees never climb onto a refuse collection truck, including the rear loading sill and riding steps, while the truck is backing AB - 2. The Deputy Sergeant (victim), Sheriff, and a Chief Deputy travelled to a rural property to conduct a welfare check on the resident who had exhibited signs of mental instability in the days prior to the shooting. The rural resident, who was known to local officers and grew up in the community, had a history of bipolar disorder. Five weeks prior to the shooting, the resident's family members initiated involuntary commitment proceedings but the petition was denied. The resident was discharged from the hospital days later. No family members or local health care providers were apprised of the individual's diagnosis or of any prescribed medications following his evaluation. On the morning of the incident, law enforcement agents spoke with the rural resident by telephone to let him know they were planning to stop at his home to check on his welfare. The three officers arrived at the farm and went to the porch to announce their arrival, but the resident did not respond. Not knowing the resident's location, the officers used a public address system to announce their arrival and then saw the resident through the window, holding a gun. Officers took cover in their vehicles and the resident opened fire from inside the house, disabling both vehicles driven to the site. Officers then took cover behind one of their vehicles, radioed for help, and returned fire. In the course of the ensuing gunfire, the resident came out of his house, firing a shotgun at the officers who had taken cover behind a vehicle. One of the shots passed through a deputy's vehicle and struck the deputy in the head, killing him instantly. When the resident turned toward the house, the two remaining law enforcement agents at the scene made a decision to retreat down the 0.2-mile lane while under fire. During the time that had passed, law enforcement personnel from multiple agencies in the surrounding area were arriving at the roadway. Over the three-hour period that followed, tactical police and a family member attempted to negotiate by phone with the resident who refused to cooperate. Tactical teams deployed to the farmstead shot and killed the resident when he exited his house armed and preparing to fire. The following recommendations are made to prevent similar scenarios in the future: 1. Provide law enforcement with training on dealing with mentally ill persons. Training should cover topics such as recognizing signs of a citizen with mental illness, effective methods to communicate/interact with those who have mental illness, and information regarding potential support resources in the community. 2. Develop a system that provides quick access to law enforcement agencies regarding the most recent status and potential conditions of citizens with serious mental illness, so law enforcement personnel have concise clear information about behaviors that may be encountered when responding to a situation involving a mentally ill citizen. 3. Mental health care providers and caregivers should carefully assess the home environment of patients, especially with respect to weapons accessibility, or other means by which mentally ill patients may harm themselves or others. In some cases, efforts should be made to communicate with community stakeholders, such as local mental health providers and law enforcement regarding potential hazards present in a home environment. The semi-truck, which was at the orchard to pick up apple bins, was unable to maintain traction and got stuck while attempting to travel up a gravel orchard road with a 6-7% grade. The worker and the semi-truck driver were attempting to tow the stuck semi-truck with the assistance of a motor grader operator. The operator was reversing the motor grader downhill into position so the worker could attach the chain from the front of the semi-truck to the rear of the motor grader. According to the operator of the motor grader, he attempted to use the brakes about 40 feet uphill from the semi-truck but they failed, causing the motor grader to collide with the front of the semi-truck. According to witnesses, the motor grader momentarily stopped about seven feet from the semi-truck, and then suddenly moved backwards crushing the worker and semi-truck driver between the two vehicles. Emergency Medical Services (EMS) was contacted and the worker and truck driver were transported to the hospital. Approximately 1 hour after the incident the worker died. To prevent similar incidences the Washington State Fatality and Control Evaluation Team (FACE) recommends: 1. Employers should implement and enforce the use of job hazard analysis (JHA) tools and techniques to identify and eliminate hazards. 2. Employers should create and enforce comprehensive standard operating procedures (SPOs) for towing vehicles. 3. Employers should ensure that all heavy mobile machinery is equipped with functioning safety systems and alarms. 4. All operators of heavy machinery should perform a safety inspection before operating machinery. Inspections should include functional testing of safety devices. Designers and manufacturers of motor graders should: 5. Design and manufacture motor graders with safety features that notify the operator that there are workers near or in the path of the motor grader. Overall, the annual number of fatalities was relatively stable compared to the past two years. In 2008, there were 61 worker deaths and 68 worker deaths in 2007. Transportation was the most common event and occupation for fatalities among workers in Oregon. The second most frequent cause of death was contact with objects or equipment. Agricultural occupations within agriculture (farming and ranching) had the second highest occupational death count. There were three delayed deaths in 2006, compared to five in 2008. Cases of delayed death (over 2 days following the date of injury) spanned from 10 days to 111 days. Compared to 2007 and 2008 data, a higher proportion of incidents involved women in 2009 (8%, compared to 4% and 5%, respectively). Violence accounted for three of five female fatalities in 2009. Tarp maintenance would have been a less expensive and less painful solution. Tips to live by - DRIVERS: Check tarps monthly for wear. Even small holes can quickly become a hazard. Report worn tarps to your supervisor. Use softeners on sharp corners to prevent tears. EMPLOYERS: Train workers to inspect tarps monthly. Keep tarping area well lit to allow tarps to be thoroughly inspected. Have a system for reporting wear and removing tarps from service until they are patched or replaced. Include in the contract that forklifts lift tarps onto the load. Request plastic wrapped loads to minimize the need for tarps. Provide plenty of good quality bungees. Kentucky Injury Prevention and Research Center AB - One summer's night, A 49 year-old semi-truck driver (D1), pulling a flatbed loaded with salvage cars for recycling traveled north on a three-lane state highway. D1 topped a hill, drove through a right curve when the unit rolled onto the driver's side, slide across the center line and entered a straight stretch approximately half way to the bottom of a hill. As D1's unit was sliding down the hill in the south bound lanes, a 52 year-old semi-truck driver (D2) hauling bottled beverages was traveling south in the straight stretch up the hill. As D1 slide down the hill, D2 tried to avoid being struck head-on and struck the guardrail. D2 was struck by D1 AB - both vehicles caught fire and both drivers died at the scene. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Transportation companies should educate commercial drivers on proper load securement. Recommendation No. 2: Transportation companies should require drivers to utilize restraints while operating commercial vehicles. Recommendation No. 3: Electronic stability systems should be mandatory equipment on all commercial vehicles. Recommendation No. 4: Companies should provide new and refresher commercial driver safety training for company drivers addressing driver distraction and including defensive driving techniques. He was an employee of a contractor who was replacing rotten plywood sheathing and joists on a warehouse roof. The victim, a foreman, and another laborer were working on the building's flat roof tearing out 4'x8' plywood sections and nailing new ones in place. The victim was wearing a full-body harness with a lanyard and using two lifelines hooked together with one of the lines looped around a skylight frame, which served as an anchor point. The other two employees were not using fall protection. The two laborers were taking nails out of a sheet of plywood they had removed when their foreman asked them to move out of the way so that he could place a new sheet of plywood over the roof opening they had exposed. They were then to nail the plywood in place. When the victim moved aside, he stepped onto two pieces of rotten plywood that were not nailed down, and the plywood pieces and unattached joist supporting them gave way. An investigation determined that his rope grab was attached near the end of his lifeline which left too much slack in the line allowing him to fall 30 feet to a concrete floor. To avoid striking the cars and ladder, the semi driver swerved to the right, over-corrected to the left, drove across the northbound lanes, over a cable barrier, then into the southbound travel lanes. Another semi-truck driver (D2) traveling in the right southbound lane pulling a 53' trailer struck the cab of the northbound semi. D2 died at the scene, and D1 was ejected and found alive under the right steer tire. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Loads in open pickup truck beds should be firmly secured before transport. Recommendation No. 2: Ladders should be labeled by manufacturers to inform users on the importance of securing ladders during transport. Recommendation No. 3: Retail outlets should locate signage in the ladder area to instruct customers on the importance of load securement. Recommendation No. 4: Law enforcement should be alert for load securement issues in open bed trucks and issue citations for non-secure transport of ladders. The victim stepped into the roadway crosswalk and was then struck by a pickup truck. The pickup truck traveled a few feet after striking the victim before coming to a complete stop. Multiple people placed calls for emergency medical services (EMS). EMS and local police arrived within minutes and transported the victim to a local hospital where she died later that same day. Contributing factors identified in this investigation were that crosswalk markings were in poor condition and of the least visible type of marking and there were no traffic control devices present at the crosswalk location to warn motorists of the crosswalk location. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, municipalities should: 1. Develop a school route plan that meets the standards set forth in the national Manual on Uniform Traffic Control Devices (MUTCD) AB - 2. Consider additional measures, including installing signs and flashing lights, to ensure crosswalks are clearly visible to motorists AB - 3. Ensure that the design of equipment supplied to workers, such as stop paddles, does not prevent employees from using the equipment AB - 4. Provide and ensure that crossing guards are wearing the appropriate personal protective equipment, including American National Standard Institute (ANSI) compliant high-visibility safety apparel, when working along roadways AB - 5. Occupational and Environmental Medicine AB - In the summer of 2010, a Hispanic male farm laborer in his 20s was electrocuted in a potato field when he touched the energized cross-brace of an operational self-propelled irrigation system while pulling free growing corn. He was standing in pooled water. One of his two coworkers heard the decedent yell. The coworker looked back and saw the decedent leaning with his left arm on the tower cross brace between the tires. The decedent then fell to the ground. The coworkers called their supervisor at the farm. The farm office called for emergency response and turned the irrigation system off. The decedent was declared dead at the scene. After the incident, the irrigation unit and wiring were examined by multiple firms. The system could not be restarted for inspection. The investigation found that the irrigation system was properly grounded and that underground wires supplying power were "burned off" in four places, thought to be consistent with a lightning strike. A definitive determination could not be made as to how the electrocution occurred. Contributing factors: 1) Age of underground wiring AB - 2) Direct burial of wiring without the protection of conduit to avoid damage to wiring insulation from stones and frost heaving AB - 3) Working in field while irrigation equipment operational AB - 4) Weather, including undetected electrical storm damage AB - and 5) Electrical inspection not performed. RECOMMENDATIONS: 1) Ensure irrigation system wiring and grounding is continuous and in accordance with the American Society of Agricultural Engineers Standard S362 and Article 675 of the National Electric Code. 2) Farmers should implement an electrical inspection program for irrigation equipment at the beginning of each season that includes periodic inspections of all system components throughout the growing season. This inspection should include checking the integrity of the underground wiring associated with the system. The manhole was located behind the fire house. The Fire Chief and a firefighter were called in by the DPW general foreman (GF) to unlock the firehouse and move the fire truck so it would not be blocked by the DPW utility truck while working at the manhole. Victim 2 also arrived to offer assistance. The manhole was five feet in diameter and 18 feet deep with an opening 24 inches in diameter. Victim 1 started climbing down the metal rungs on the manhole wall wearing a Tyvek suit and work boots in an attempt to clear a sewer blockage. The DPW GF, the firefighter and Victim 2 walked over to observe. They saw Victim 1 lying on the manhole floor motionless. They speculated that he had slipped and fallen off the rungs and injured himself. The Fire Chief immediately called for an ambulance. Meanwhile, Victim 2 entered the manhole to rescue Victim 1 without wearing any respiratory protection. The firefighter saw that Victim 2 fell off the rungs backwards while he was half way down and informed the Fire Chief. The Fire Chief immediately called for a second ambulance and summoned the village FD to respond. The FD responders arrived within minutes. The Assistant Fire Chief (AFC) donned a self-contained breathing apparatus. He could not go through the manhole opening with the air cylinder on his back. The cylinder was tied to a rope that was held by the assisting firefighters at the ground level. The AFC entered the manhole with the cylinder suspended above his head. He did not wear a lifeline although there was a tripod retrieval system. He secured Victim 2 with a rope that was attached to the tripod. Victim 2 was successfully lifted out of the manhole. The AFC exited the manhole before a second rescuer entered the manhole and extricated Victim 1 in the same manner. Both victims were transported to an emergency medical center where they were pronounced dead an hour later. The cause of death for both victims was asphyxia due to low oxygen and exposure to sewer gases. CONTRIBUTING FACTORS: Contributors to the DPW worker's death: 1. DPW no-entry policy for permit-required confined spaces was not enforced. 2. DPW permit-required confined space program was not implemented. 3. Employees were not trained on confined space hazards, and proper entry and rescue procedures. Contributors to the firefighter's death: 1. Firefighters were not trained in confined space rescue procedure. 2. FD confined space rescue protocol was not followed. 3. After doing those repairs he went to a nearby building, perhaps to check on the work he had done on the building's roof the previous week. He used a 20 foot aluminum extension ladder to access the building's roof which was 16 feet 8 inches above the ground. The roof was flat with an 18 inch parapet. A passerby discovered the roofer lying unresponsive on the pavement next to the ladder which was still leaning against the the building's wall. He was taken to a hospital where he was pronounced dead. The medical examiner determined that he died from head injuries due to a fall from either the ladder or the building roof. Approximately six to eight months prior to the incident, the farm initiated a change in propane suppliers. The original propane supplier disconnected the tanks, including the incident tank, but did not pick them up from the property. The cylinder had been moved from its original location behind the home to the driveway's turnaround area by the new propane supplier (Supplier 2) to provide easy access for Supplier 1 to haul away. The day and night prior to the incident, approximately 14 inches of snow fell. The decedent was either unaware of or forgot that the cylinder was on the driveway turnaround. It was dark when the incident occurred and the drifted snow may have hid the tank. The decedent made several passes with the loader. He struck the propane cylinder resulting in an 8-inch tall by 1-1 1/2-inch wide gash opening (Figure 1). It appears that releasing propane created a cloud which was ignited by either the heat of the front-end loader motor or a spark created by the metal on metal contact when the cylinder was struck by the bucket. The front-end loader was engulfed in flames from the resulting explosion. Emergency response was called and the decedent was transported to a nearby hospital where he died the next day from complications of the burn injuries sustained at the time of the incident. Factors which could be involved in this incident include: 1) Propane tank not picked up by propane supplier AB - 2) Snow hiding the propane tank location and footprint AB - and 3) Insufficient lighting to identify the tank location in the snow. RECOMMENDATIONS: 1) When moving propane tanks to a different location, set the tank in a non-traffic area away from vehicle movement. 2) Install protection (i.e., fencing, barricades or posts) to protect aboveground propane tanks from possible damage from passing vehicles and identify the propane tank's location, including the tank's footprint using stakes, flags, or poles. 3) Propane suppliers should promptly retrieve disconnected tanks when service is terminated and, if prompt retrieval is not possible, the propane supplier should properly remove any remaining gas in the tank prior to the tank placement while awaiting retrieval.
