Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

They used a ladder to access the top plate of the unfinished single-story structure where they intended to place a rim joist. They had previously been working from ladders and, according to the laborer, intended to return to working from the ladders once they placed the rim joist. Fall-protection gear was available on-site, but they did not use it. As they were moving the rim joist into place, the laborer was standing on the top plate and the victim was standing on 6½-inch-wide beam, when the victim fell 12 feet 7 inches to a concrete surface. The laborer called emergency services, who transported the victim to a hospital, where he died of head injuries two weeks later. This is the 12th annual Michigan Fatality Assessment and Control Evaluation (MIFACE) report on acute traumatic work-related (WR) deaths in Michigan. There were 135 WR deaths in 2012, representing 131 employers and 133 separate incidents. Key findings for 2012: The number of WR deaths (135) and the fatal injury rate (3.2 deaths/100,000 workers) were down compared to 2011 (141 WR fatalities, 3.3/100,000 workers, respectively). The largest number of WR deaths occurred in Construction (20, 14.8%), then Transportation & Warehousing (19, 14.1%), then Agriculture (18, 13.3%) and then Retail Trade (12, 8.8%). Manufacturing, Administrative & Support & Waste Management & Remediation Services and Other Services each had 11 WR deaths (8.1%). Agriculture had the highest risk of death (20.7 deaths/100,000 workers), followed by Transportation & Warehousing (18.6 deaths/100,000 workers), and Construction (15.7 deaths/100,000 workers). The most common cause of death was from a motor vehicle (36, 26.7%), followed by homicide (28, 20.9%), then falls (16, 11.9%), and then non-vehicular machines (14, 10.5%). The cause of death could not be determined for one individual. Individuals who died were most likely to be men (92.6%), white (79.2%), married (59.7%) and have at least a high school education (70.0%). The average age was 48.3 years old and ranged from 18 to 84 years of age. Thirty percent of work-related fatalities occurred to college-educated individuals, the highest percentage since this was first tracked in 2001. Illegal drugs, alcohol or side effects of prescribed medication was a potential factor in approximately 10% of the deaths. The largest number of work-related traumatic events occurred on a Wednesday (26) followed by Monday (23). March and June were the most common months (16), and 12:00p.m. to 3:59p.m. was the most common time (33). Forty (41, 49.4%) of Michigan's 83 counties had a fatal WR incident. Wayne County had the highest number of WR fatal incidents (38, 28.4%), followed by Oakland County (10, 7.5%). The county of incident was unknown for one fatal incident. Of the 135 WR fatalities, 27 (20.6%) were MIOSHA program-related and were investigated by a MIOSHA compliance officer. The number of WR homicides in 2012 nearly doubled compared to 2011 (28 homicides in 2012 compared to 15 homicides in 2011). A narrative summary of each WR fatality is in Appendix I. While using a high-pressure water hose to flush residual compost, the 16-year-old was overcome by hydrogen sulfide and fell to the bottom of a 10-foot shaft. The 22-year-old collapsed at the bottom of the shaft after attempting to rescue his brother. Contributing factors identified in this investigation were the high concentration of hydrogen sulfide in the shaft, failure to implement a confined space and hazard communication program, and the age of the youngest victim. The CA/FACE investigator determined that, in order to prevent exposures to hydrogen sulfide while cleaning out drainage systems, employers should: 1. The employees were not given any training in trench safety and shoring procedures. Factors that may have contributed to this death were untrained workers in an unshored trench more than five feet deep with unstable soil conditions. The CA/FACE investigator determined that, in order to prevent future incidents: Homeowners who need trenching / excavation work should: 1. Hire contractors who are licensed to perform trenching and excavation work. Contractors licensed to perform trenching should: 1. Ensure that the trench is properly permitted and inspected daily by a competent person to determine all necessary safety precautions. 2. Ensure that trenches five feet deep or greater are properly shored, shielded, or sloped. In addition, employers who hire day laborers to work in a trench /excavation should: 1. The victim's supervisor opened the drum door, and then the victim began to retrieve laundry by leaning inside the drum. Video shows the supervisor then switched the machine on and the drum suddenly rotated upward, pulling the victim off his feet and pinning his torso between the drum and frame of the washing machine. The supervisor switched off the machine and then attempted to free the victim. Other employees in the shop also came to assist. The door safety interlock was disconnected at the time of the incident. Factors identified in this investigation which may have contributed to the victim's death included the disconnected safety interlock switch and the work practice of one worker reaching into the washer while a second worker was near the washer controls. The CA/FACE investigator determined that, in order to prevent washing machine operators from being pulled into machines, employers should ensure that: 1) Comprehensive Injury and Illness Prevention Programs (IIPP) are developed, implemented, and enforced which include worker training in washing machine operations and safety with instructions that are available in the primary language of the user. 2) The electrical, mechanical, and safety features of industrial washing machines are maintained or repaired in operating condition, according to manufacturer's specifications. The incident tree was the last tree of the clump to be felled. A coworker (Coworker 1) was nearby operating a skidder. The terrain was snow covered and the tops from other trees felled previously by mechanical means were present. The decedent's coworker (Coworker 2) cut and completed the notch on the east side of the tree. Coworker 2 noted the locations of the skidder operator and the decedent before beginning the back cut. He noted that the decedent was standing behind him on the north side of the tree. He completed the back cut, intending for the tree to fall to the east. As the tree began to fall it twisted on the stump and began to fall to the south. Coworker 2 looked up and observed the decedent to the south and in the direction of the falling tree. Coworker 2 yelled to the decedent to "look out", and the decedent began to run to get out of the way, but ran toward and into the path of the falling tree. Coworker 1 attempted to drive the skidder under the falling tree to keep it from striking the decedent. The decedent was located approximately 50 feet from the base of the tree when he was struck on his back and head by one of the branched sections as he dove toward a nearby tree top to get out of the way. The decedent was not wearing head protection. Coworker 2 used the decedent's chainsaw to cut the tree away from him while the skidder operator used his cell phone to call for emergency response and notify the crew leader. Coworkers immediately began first aid while emergency response was en-route. The decedent was declared dead at the scene. Contributing Factors: 1. Working within two tree lengths of tree during felling operations. 2. Improper tree assessment and felling techniques. 3. Lack of supervisory authority to enforce safety policies. 4. Failure to establish, clear, and maintain an escape route prior to felling trees. RECOMMENDATIONS: 1. Employers should instruct each employee and enforce the safe work practice that no workers, with the exception of the tree feller, should be closer than twice the height of the tallest tree being cut. 2. Fellers should assess a tree's condition prior to felling the tree and use proper felling techniques. 3. Workers on a logging site should wear appropriate personal protective equipment. 4. Employers should develop a written company safety program which includes, but is not limited to, development of safe work procedures and worker training in hazard identification, avoidance and abatement. The employer should ensure the program is consistently implemented and enforced including methods for dealing with worker noncompliance. Both the decedent and the farm owner were on the incident site but working on independent tasks on opposite sides of the 13-acre blueberry/potted plant farm operation. The water tank was located on top of a storage platform built by the decedent approximately three years previously. The storage platform was constructed of construction common-grade lumber. Four 4"x 6" corner posts were 52' tall and flush with the ground. For both the upper platform to support the water tank and the bottom platform to provide off-ground storage, the decedent used 2x6 wood planks. A polyethylene plastic material was installed to cover three sides of the structure to create and protect the storage area under the upper platform/tank. The nails used for the structure's assembly were not galvanized nails. The event was not witnessed. The sheriff's office determined that the wooden platform collapsed on the decedent and pinned him under the water tank, perhaps while he was retrieving tools. The owner heard the sound of the crash and came to the area where he found the decedent under the tank. The owner called a neighbor who then called for emergency response. Emergency response arrived and the decedent was transported to a local hospital where he died several days later from the injuries sustained at the time of the incident. A beer can was found at the structure's open side. Contributing Factors 1. Water tank weighing approximately 5,000 pounds fell onto decedent. 2. Improper construction of support structure. RECOMMENDATIONS: 1. Outdoor rigid frame structures/platforms constructed with outdoor-rated, pressure-treated lumber should have the appropriate lumber strength, bracing, and fasteners to withstand at least two times the weight of the supporting mass. 2. Farm-built, outdoor, pressure-treated lumber wood support structures should be inspected at least annually for evidence of wood decay and structural deficiencies. 3. Employers should consider measures that contribute to a drug-free work environment, including the development and implementation of an alcohol-and drug-free workplace program, particularly for jobs related to machine and motor vehicle operation. After dropping off two children at their residences, the van was traveling 46-50 mph in the left hand lane headed east in a 35 mph zone on a non-divided four-lane state road. As the van approached a side street on the right, the right rear tire blew out and the van veered sharply to the left, crossing over two westbound lanes and striking a tree head-on. Witnesses called emergency services and began extracting the children, operator, and monitor from the van. All occupants were transported to local hospitals for treatment of injuries. The monitor died at the hospital AB - the operator and all other passengers survived. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Daycare facilities that utilize 15-passenger vans should ensure operators are properly trained in safe vehicle operations. Recommendation No. 2: Daycare facility owners should ensure company vehicles have routine maintenance and inspections performed. Recommendation No. 3: Daycare employees involved in the transportation of children must follow all the transportation rules and laws that apply. Recommendation No. 4: Kentucky daycare facilities should use alternatives to 15-passenger vans to transport children for the safety of the children and the daycare employees. Recommendation No. 5: Kentucky should authorize one agency to oversee daycare transportation operations. Recommendation No. 6: Manufacturers should improve the safety of 15-passenger vans, especially since they are often used to transport children. Sixty eight of the 119 (57%) of the fatal falls were the types of falls highlighted in the OSHA Fall Prevention Initiative: 33 falls from a roof edge or through a roof opening, 24 falls from a ladder, and 11 falls from a scaffold. The most common heights of all construction falls were 10-19 feet (28 deaths), 21 deaths were from less than 10 feet. Specialty trade contractors were most likely to have a fatal fall; roofing (31), structural steel (11), framing and painting (16) and masonry (7). Individuals who died from a fall from a height of greater than 6 feet were either not equipped with or were not properly using fall protection equipment (i.e., a harness was worn but the lanyard was not attached to an anchor point). Upon hooking up the cargo tank hoses to the facility's fuel unloading bulkhead and starting to dispense fuel, he discovered that the transfer pump was leaking externally. This condition would prevent safe delivery of the fuel. The driver used his cell phone to contact his supervisor through his dispatcher at 6:21 p.m. to alert him of the situation. The supervisor told the driver he would need to take the transport to another city for repairs the following day and that he would make arrangements and then call him back with details. The supervisor attempted to call the driver numerous times, but could not make contact with him. At 7:44 a.m. the next morning a representative of the bulk plant facility arrived at the yard where he found the driver lying deceased underneath the semi-tractor's right rear wheel. He contacted emergency services and a deputy sheriff and fire and rescue personnel arrived at the scene. Upon examination of the incident scene by the first responders they noted that 1) the vehicle's engine was running and headlights on, 2) the brake for the cargo tank transport semitrailer was set, 3' the tractor's brake was not engaged, and 4) no wheel chocks were visible. Apparently, the driver had attempted to drive away, but was unable to because the trailer's brakes were still engaged by the air brake safety interlock system. When a metal hinged gate located on the trailer's undercarriage in front of the fuel loading and unloading connections is opened, an air valve is activated which engages the trailer's brakes. In order for the vehicle to be driven this gate must be closed. (This system is designed to prevent a driver from driving away while hoses are connected to the fuel loading/unloading bulkhead.) To close the brake interlock gate the driver exited the cab and walked around to the trailer's right side. He did not engage the parking brake (handbrake) in the cab. When he closed the safety interlock gate on the trailer, the brakes which had been holding the vehicle were released, thereby allowing it to roll backward down the dirt driveway. He ran alongside the trailer to open the gate again, so as to stop the truck. He succeeded in opening the gate, which engaged the brakes, but as he did so he either slipped or was struck by a structural member of the semitrailer and fell down and was run over by a wheel on the semi-tractor. Recommendations: To prevent similar occurrences in the future, the Washington State Fatality Assessment and Control Evaluation (FACE) investigation team recommends employers of propane transport truck drivers follow these guidelines during fuel loading and unloading operations at bulk plants: A. Create, implement, and enforce written policies to ensure that propane transports will not be allowed to roll away when the driver is outside the cab. These policies should include procedures requiring drivers to 1) set the vehicle's parking brake before leaving the cab AB - 2) use wheel chocks AB - 3) never attempt to stop a rolling vehicle from outside the cab AB - and 4) perform a vehicle pre-departure inspection. B. Train drivers to recognize the hazard of vehicle rollaway due to improper vehicle securement when parked and to use safe work practices to prevent unintended vehicle movement. Additionally, vehicle designers and manufacturers should consider creating safety systems to either alert or prevent the vehicle operator from releasing the trailer parking brake at the brake interlock gate without first manually setting the parking brake from the cab. Alternatively, an alarm could be created to sound-off when the driver's door is opened and the parking brake has not been manually engaged. The decedent drove the tractor to the tree's location and stopped it with the front wheels at the top of the embankment slope. It is postulated that the decedent parked the tractor with the bucket raised slightly and resting against the tree to help direct the tree's fall. The tractor was running so as to keep the bucket raised and against the tree. The decedent got out of the tractor and cut the tree. When the tree began to fall, the tractor moved forward and down the embankment. The decedent was unable to move to safety and was struck by and pinned under the front end loader's driver's side front tire. His wife found him and called for emergency response. The decedent was declared dead at the scene. Key factors in this incident: 1. Misuse of the tractor's loader bucket. 2. Working on the ground in the travel path of the tractor. 3. Improper tree felling techniques. Recommendations: 1. Use tractors, including tractors equipped with attachments, according to manufacturer's operator's manual. 2. Before initiating a task, conduct a site assessment to determine hazards and appropriate work practices to minimize identified hazards, including topography challenges, equipment location and working on the ground in the travel path of equipment. 3. Use proper directional tree felling techniques (notch and back cut) with chainsaws and wear appropriate personal protective equipment to prevent injury. 4. When working alone, establish a check-in procedure with another individual to help assure prompt emergency assistance. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Employers should provide training to drivers on the importance of being attentive and not engaging in distracting tasks or behaviors while driving. Recommendation No. 2: Semi tractor-trailer drivers should be trained to recognize signs of fatigue and drowsiness and when to seek appropriate rest areas. Recommendation No. 3: Places for truck drivers to stop and rest should be located at regular intervals along interstates that are heavily traveled by trucks. A dump truck driver arrived with a load of dirt and he got down from the bulldozer he was operating and walked over to the driver to give him instructions as to where to place the dirt. He left the bulldozer's engine running and did not set its parking brake. As he attempted to return to the operator's seat he was standing on the top of the bulldozer's tread when his elbow hit the transmission shifter lever out of neutral into reverse. As the bulldozer began to move backward his left foot was caught between the moving track and the underside of the fender. His leg was pulled in and crushed while the bulldozer carried him several yards before ejecting him onto the ground. Emergency responders took him to a hospital where he died of his injuries 15 days later. On the day of the incident, a semi-truck with an attached flatbed trailer was traveling up a 6 to 7% grade gravel orchard road to a staging area where it was to be loaded with apple bins. The truck lost traction, so orchard workers decided to use a grader to tow the truck to the staging area. The towing process involved backing the grader up to the truck and attaching a chain between the two. The victim and grader operator had both used this towing method many times. As the grader operator started reversing downhill, about 40 feet from the truck, he noticed that the grader's brakes were not working. He pumped the brakes and tried to change gears, which slowed the grader but did not stop it. The operator saw the victim and truck driver hooking up the chain to the truck and yelled to them, but they were unable to hear over the noise of the grader. The grader struck the victim, crushing him against the truck. The truck driver's right leg was pinned between the grader and truck. An investigation found the grader's brakes functioned properly and there were no defects that would have prevented braking. Witnesses stated they saw the grader stop briefly a short distance from the victim and then jolt backwards, striking him. After the incident the operator stated that if the grader was not used regularly (it had not been used in over a week) that its brakes needed to be pumped several times before they worked properly. The victim and a crane operator were working from a crane barge in support of a crew working at a job site to replace pilings underneath a pier. The crane operator was picking waste pilings from the water and placing them on an adjacent barge and then lifting new pilings to workers in the water. The mechanic was driving at highway speed during daylight hours when he failed to stop in time and struck the semi, which was moving slowly in traffic that had backed up due to a car fire two miles downstream. There was severe damage to the mechanic's truck, with underride of the cab and intrusion up to the right rear tires of the trailer ahead. The force of the collision caused a chain reaction, pushing the struck tractor-trailer forward into a second tractor-trailer that had stopped in backed up traffic. Fire and rescue responders arrived from the nearby town located four miles away. The mechanic, who was wearing a safety belt, suffered blunt force trauma as a result of the collision and was pronounced deceased at the scene. He was extricated by mechanical means from the truck cab and was found holding a cell phone. Law enforcement officers' examination of the phone activity revealed the mechanic had exchanged several voice/text messages prior to the crash and was initiating a voice/text message application at the time of the collision. The driver's failure to notice the tractor trailer ahead and stop in time was attributed to texting. Drivers of the other two vehicles were not severely injured. To prevent similar fatalities, Iowa FACE recommends: 1. Drivers should use cell phones and wireless communications devices only after pulling off the roadway and parking their vehicle. 2. Drivers should maintain a safe following distance, adjusting for weather, traffic, road conditions, and visibility. 3. Employers should implement policies banning use of cell phones and in-vehicle technologies while driving. 4. States should adopt and enforce laws prohibiting the use of cell phones and wireless devices while driving. Personnel from the local and state police departments, fire department and EMS all arrived within minutes of the call. The victim was pronounced dead at the incident location. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Ensure that all multi-piece rim wheel tires are deflated prior to removing them from a vehicle's axle AB - 2. Ensure that when tires mounted on demountable rims are being removed from a vehicle, all of the rim's wedge clamps are loose before entirely removing any of the lug nuts AB - 3. Ensure that employees never position themselves in the trajectory of (in front of or over) inflated tires mounted on multi-piece rims while servicing any wheel or tire component AB - and 4. Develop standard operating procedures (SOP) for servicing multi-piece rims and provide training on these procedures.
