Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The pallet-keg combination weighing approximately 470 pounds was loaded by forklift into the trailer. When the trailer was loaded, the decedent drove the semi-truck/trailer to the fueling area. The decedent opened the trailer door to place the shipping manifest in the semi-trailer. Although the injury sequence of events was unknown, the decedent most likely used the Interstate Commerce Commission (ICC) bumper to access the trailer deck. It is unknown if the decedent was on the ICC bumper or had stepped up onto the deck at the time of his fall. In an attempt to regain his balance, he grabbed the shrink-wrapped kegs and as he fell, the kegs and pallet were pulled down with him. The decedent was found by a coworker lying on his back on the ground with a wooden pallet on his legs and empty kegs strewn around him. The coworker ran to a nearby building and requested assistance. Emergency response was called, and the decedent was transported to a local hospital where he died several days later from the head injuries sustained at the time of the incident. The incident investigation found one shipping manifest on the deck of the trailer and the second manifest under the trailer. Possible contributing factors to this incident: 1. Using the ICC bumper as an access to the trailer deck. 2. Height of shipping manifest pocket on the trailer wall. Recommendations: 1. Develop a written truck yard health and safety program that includes safe work procedures for truck porters. 2. Ensure access ladders available to workers to climb onto trailer deck. 3. Employers and/or trailer manufactures should consider mounting the shipping manifest pocket at a height easily accessed from ground level. The decedent's sunglasses were found under the box. It is hypothesized that his sunglasses fell under the box and that he may have been reaching under the box to retrieve his sunglasses while it was still resting on the tilt frame end (stinger). While reaching under the box, the box came off the stinger and landed on top of him. The decedent was found by a fellow employee operating a front end loader. After getting help, the loader operator attached chains to the bucket and lifted the box from the decedent as his coworkers moved the decedent from under the container. Emergency response arrived and he was declared dead at the scene. Contributing factors to this incident include: 1. Standard operating procedures not followed (unhooked the wire rope prior to complete unloading of the container). 2. Reaching under container with no secondary support. Recommendations: 1. The company should review and update their Safety Operation Manual for Roll off Trailers to incorporate current operation information provided in the Benlee Operations Manual. 2. The company should conduct periodic unannounced workplace inspections to ensure compliance with company safe operating procedures for container loading and unloading. 3. The company should establish a health and safety committee as part of their safety program. 4. The company should ensure employees conduct thorough pre- and post-vehicle inspections and promptly notify appropriate personnel so required maintenance may be performed. 5. The manufacturer should develop and provide a safety label to be applied near the trailer operator controls to remind operators to never unhook the cable from the container until the container is completely on the ground. The victim and another worker were loading the trailer with rocks that were excavated from the orchard rows. The victim went behind the trailer, as it was moving, to pick up rocks from the ground. The tractor started to lose traction, causing the trailer to hop. The hopping motion caused the trailer coupler to unhitch from the trailer hitch ball. The trailer rolled down the sloped road, and struck and ran over the victim. Emergency medical services were contacted and arrived on the scene. The victim was transported to the hospital where he was put on life support. Approximately two days after being admitted the victim was taken off life support and passed away. Following the incident, investigators noted that the trailer hitch ball on the tractor was not the proper size for the trailer coupler, and the safety chains and other safety equipment were not properly connected or not functioning. To prevent similar incidents Washington State Fatality and Control Evaluation (FACE) recommends that: Employers: 1. Develop a system to match trailer hitch balls and trailer couplers by size and to ensure that each tractor-trailer combination has the necessary safety equipment. 2. Develop and enforce policies requiring tractor operators to properly connect, inspect, and maintain safety systems before towing a trailer or other equipment. 3. Train tractor operators and workers on foot on how to: A. Identify hazards associated with working behind vehicles, trailers, and other equipment being towed on slopes. B. Properly use safety systems and to follow safety policies and procedures. On the day of the incident he was working with a field crew and serving as a side tender. He was preparing the trucks as they approached and departed the harvesting operation in the field. The hop vines were trellised on vertical ropes suspended from horizontal wires that were secured to the tops of 18-foot poles. The harvesting procedure involved a bottom-cutter, (a tractor with a cutting implement) which cut the bottom of the vines, followed by a top-cutter harvester machine that cut the top of the vines. As the vines were cut, they fell into a truck that was in front of the top-cutter. The victim's job, among other duties, was to vertically insert 3-foot pieces of PVC pipe into pegs mounted on either side of the front of the truck, so that as the truck moved forward the vines were guided inward into the truck bed before being cut. When the truck was full, he would remove the pieces of pipe and place them on the next truck. He was walking next to the top-cutter. The top-cutter operator had warned the victim to not walk beside the machine as it was moving. The employer required field crew members working the harvest to wear hard hats. The victim was not wearing a hard hat. The top-cutter operator noticed that a pole was falling and yelled "pole!" The victim was hit on the head by the falling pole. It is not known whether the top-cutter struck the pole. The victim was taken to a hospital where he died four days later. The host employer's superintendent did not see the victim fall, but heard a crash and turned around to find the victim on the floor behind him. He then placed a call for emergency medical services (EMS). EMS, the fire department and the local police arrived at the incident location within minutes. The victim was transported to a local hospital where he died of his injuries two days later. Contributing factors identified in this investigation included roof access was only by a fixed ladder with no mechanism to get materials up to the roof, lack of training on ladders, and lack of a comprehensive contract between the staffing agency and the host employer. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Ensure that lifting mechanisms are available to get materials to a higher level when access is only by ladder AB - and 2. Ensure that employees are trained in the proper use of ladders, including maintaining three point contact with the ladder. To help prevent injuries and fatalities to temporary workers, staffing agencies and host employers must work together to inform workers about job hazards and provide training. Staffing agencies and host employers should: 3. Develop a comprehensive contract that clearly states the tasks that will be performed and outlines the training and safety responsibilities for which each the staffing agency and the host employer are responsible. In addition, commercial builders and architects should: 4. Adopt and implement the concept of Prevention through Design (PtD) to ensure the safety and health of the end users of their buildings, such as designing roof access with stairways instead of fix ladders. This is the 13th annual Michigan Fatality Assessment and Control Evaluation (MIFACE) report on acute traumatic work-related (WR) deaths in Michigan. There were 134 work-related deaths in 2013, representing 131 employers and 131 separate incidents. A narrative summary of each work-related fatality is in Appendix I. MIFACE educational material, including on-site investigation reports, summaries of MIOSHA investigations, and hazard alerts are located on the MIFACE webpage on the Michigan State University Department of Occupational & Environmental Medicine (MSU OEM) website. Key findings for 2013: The number of work-related deaths (134) and the fatal injury rate (3.1 deaths/100,000 workers) were down compared to 2012 (135 work-related fatalities, 3.2/100,000 workers, respectively). Transportation & Warehousing (26, 19.4%) had both the largest number of work-related deaths and the highest risk of death (24.6 deaths/100,000). Construction was second in both number and risk (25, 18.7%; 18.9 deaths/100,000), and Agriculture was third in number and risk (13, 9.7%; 16.0 deaths/100,000) of a work-related death. Manufacturing, although 4th in the number of deaths (10, 7.4%) had one of the lowest risks of death (1.8 deaths/100,000). Arts, Entertainment & Recreation had 6 (4.5%) deaths, but were fourth in the risk of death (12.7 deaths/100,000). The most common cause of death was from a motor vehicle (27, 20.5%), followed by struck by (24, 18.2%) and then suicide (22, 16.7%). Falls were the fourth leading cause (19, 14.4%) and then homicide (16, 12.1%). The cause of death could not be determined for two individuals. The number of work-related suicides in 2013 nearly doubled compared to 2012 (22 suicides in 2013 compared to 12 suicides in 2012). Individuals who died were most likely to be men (90.3%) and White (78.4%). The average age was 46.4 years old and ranged from 19 to 85 years of age. Foreign-born workers accounted for 9.0% of all work-related deaths in Michigan in 2013. Illegal drugs, alcohol or side effects of prescribed medication was a potential factor in approximately 21% of the deaths. The Transportation & Material Moving occupation which had the largest number of work-related deaths (31) followed by Construction & Extraction and Management (20 each). Forty-three (43, 51.8%) of Michigan's 83 counties had a fatal work-related incident. Wayne County had the highest number of work-related fatal incidents (30, 22.7%), followed by Oakland County (14, 10.6%). The county of incident was unknown for two fatal incidents. Of the 134 work-related fatalities, 27 (20.1%) were MIOSHA program-related and were investigated by a MIOSHA compliance officer. Inexperience and lack of health and safety training are also factors. Information about where and how teens are injured at work is essential to prevent future injuries and promote safer work opportunities for youth. For over 20 years, the Teens at Work: Injury Surveillance and Prevention Project at the Massachusetts Department of Public Health has tracked work-related injuries to teens under age 18. Over the years, the Teens at Work Project has collaborated with partners on a variety of initiatives to prevent injuries to working teens. Continued efforts are needed to ensure jobs in which teens are employed are safe. And as we engage teens in the workplace- whether as employers, schools, jobs programs, or parents with family businesses-we need to provide them with basic health and safety skills that will help protect them now and in the future. Ensure that equipment is inspected prior to each use to confirm that safeguards and interlocks are functioning properly. 2. Ensure that lift arm support devices are installed for maintenance tasks that require the lift arm to be in the raised position. (This may require two people.) 3. Ensure that safeguards and interlocks are used and never bypassed. 4. Ensure that employees never access the controls of a loader from outside the loader's cab. The tarp became caught on the scissor lift's joystick control causing the lift to rise up further unexpectedly and resulting in the compression injuries. Workers from another company found the victim when they went to check on him. A call was placed for emergency medical services (EMS). The local police and EMS arrived at the incident location within minutes. The victim was transported to a local hospital where he was pronounced dead. Contributing factors identified in this investigation included the tarpaulin being draped across the raised lift's guardrail, working alone from an elevating work platform, and the bar guard design not preventing the tarp from becoming entangled and engaging the joystick. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Develop standard operating procedures (SOPs) for installing tarpaulins from scissor lifts that includes never draping tarpaulins over or across the lift's guardrail AB - and 2. Develop, implement and enforce a policy that prevents employees from working alone when using elevating work platforms. In addition, equipment manufacturers should: 3. The incident occurred on the top of a 6,000-gallon, above-ground storage tank which was located outside of the plant. On the day of the incident, the victim was using a wooden measuring stick to determine the remaining volume of toluene in the tank. After he opened the port on the top of the tank, he lowered the measuring stick into the tank. He reached down into the port to obtain a measurement (the stick was shorter than the depth of the tank) and was overcome with vapor. Coworkers found the victim shortly afterwards laying on the top of the tank and 9-1-1 was called. Emergency responders arrived at the site and transported the victim to the hospital, where he died shortly afterwards. The cause of death was determined to be toluene intoxication. Contributing Factors: 1. Measuring stick was shorter than depth of tank. 2. High daytime temperature (approximately 95oF). 3. Entry of permit required confined space NJ FACE investigators recommend that these safety guidelines be followed to prevent similar incidents: 1. A method to determine volume of product remaining in tank should be developed that does not put the worker at risk of exposure to volatile chemicals or confined space entry. 2. Extra care should be taken when working with any volatile chemicals under high-temperature conditions. 3. A safety and health plan based on a job hazard analysis should be developed by the employer and followed where workers are assigned tasks. He fell 11-12 feet from the loft onto hard packed dirt. The decedent was working alone up in the loft. His spouse was feeding the horses below. She heard a "thud" but thought it was a bale landing on the ground. She went to investigate a few minutes later. She saw her husband lying on the ground. She ran to their home to call emergency response. Emergency response arrived and transported him to a local hospital where he died the next day. Key contributing factors identified in this investigation include: 1. Old, frayed clothing. 2. No fall protection while working near open loft door. 3. Work practices - throwing the hay bale instead of pushing the bale. 4. Hay on floor contributing to slip hazard. 5. Possible fatigue from throwing hay bales. RECOMMENDATIONS: 1. When possible, do not throw but rather push a hay bale out of hay loft to the ground. 2. Install removable guardrail(s) across the loft door. 3. Wear gloves to protect hands from cuts and abrasions while handling twine-baled hay. 4. Use a bale cart to move bales in the hay loft. 5. Wear a fall protection harness with appropriate anchor points to safely access each loft door. 6. Do not wear frayed/torn clothing. 7. Locate a broom in loft to sweep to avoid slips and falls. The victim worked for a trucking company that had been contracted to haul logs by the company logging the site where the incident occurred. The loader operator worked for the logging company. Both were experienced in their fields, but had not worked together before beginning work at this site approximately one week prior to the incident. All of the loads previously hauled from the site had been long logs. The logs to be loaded the morning of the incident were short logs. The previous night, the victim and his employer had extended the truck's trailer and installed a rack with an extra set of bunks, also called a "turkey rack," in order to haul the short logs. Bunks are lateral fixtures secured to a trailer that have vertical stakes at either end, and are used in pairs to contain logs during transport. The height of the bunk stakes on the victim's trailer could be increased using metal extensions that must be manually lifted and secured into place. That night, the victim had left the bunk stake extensions lowered. The morning of the incident, the loader operator arrived first to the jobsite, at approximately 5:00 a.m., and started to warm up the loader. It was still before dawn and darkness would have limited visibility. The victim arrived a short time later. While communicating by radio with the operator, the victim situated the trailer next to the loader, with the loader positioned on the passenger side of the truck near the rear of the trailer. The victim remained in the cab of the truck as the loader operator placed the first load of logs in the front set of bunks. The operator then turned the boom arm away from the truck and positioned the grapple to pick up another load of logs to place in the second set of bunks. As the loader operator swung the second load toward the rear bunks, he caught a glimpse of the victim's orange high-visibility safety vest and realized that the victim was standing on the back of the trailer. The operator immediately reversed the direction of the load when he saw the victim, and two logs fell from the grapple. The loader operator stopped the equipment and went to the victim, who had been struck by the logs, and found him lying unconscious over the rear crossbar of the trailer. The loader operator could not lift the victim off of the trailer to begin CPR. He called 911 and tried to maintain the victim's airway until emergency responders arrived. The victim had sustained crushing injuries and was pronounced dead at the scene. Emergency responders reported that the victim was wearing a long sleeved shirt over his safety vest. It is unknown why the victim left the cab, but investigators believe that he climbed onto the trailer to put up the stake extensions on the bunks that he had left lowered the night before. RECOMMENDATIONS: To prevent similar incidents, Washington State Fatality Assessment and Control Evaluation (FACE) recommends that: Communication Practices - 1. Employers should train log truck drivers to always establish and confirm visual or radio communication with the loader operator before leaving the cab of the truck during loading procedures. 2. Employers should train loader operators to verify by sight or radio contact that the driver is in the clear before loading. 3. Employers should ensure that when employees are working with a new company or crew member, or when conditions or procedures change, the intended loading process is reviewed before loading begins. Personal Protective Equipment - 1. Employers should provide log truck drivers with appropriate high-visibility safety vests and train them to ensure that they are worn in an effective manner. The incident occurred as the service technician operating the crane was preparing to lift a tank. The two victims were employed by a large national retail and wholesale supplier of propane. They worked at their employer's propane service center and regional office location. This plant location provided a variety of propane services including bulk fuel delivery, tank and cylinder delivery and installation, service location fuel sales, and appliance sales, service, and repair. On the day of the incident, the service technician and material handler were making an annual inventory of previously used propane bulk storage tanks. These tanks were located in the tank storage yard portion of the propane service center plant. Some of the tanks were stacked two-high and placed in rows. The information that they needed for the inventory was located on the top of the tanks. In order to access the tanks on the bottom they were using a telescopic boom truck crane to lift the top tanks. The material handler rigged the tanks with a metal chain so that they could be lifted by the crane. When the service technician who was operating the crane lifted the tank, the material handler was then able to obtain and write down the necessary inventory information on sheets that he was carrying on a clip board. The material handler rigged a tank to be lifted. This tank was located at the end of a row of 320-gallon capacity tanks adjacent to a fence marking the yard's boundary. Just beyond the fence, a 3-phase 7,200 volt overhead power distribution line, crossed over into the yard at a diagonal by a few feet at this location. The service technician proceeded to position the crane boom for the lift. As he was doing so, the tip of the fully extended boom contacted one of the power line wires. Electrical current traveled down the boom to the truck and the metal grating of the crane operator's station where the service technician was standing. He died instantly when he stepped from the station to the ground, with one foot on the station and one foot on the ground, thereby creating a pathway for the current to move from the crane through his body to the ground. The boom remained in contact with the power line, creating a constant flow of voltage through the crane to the ground. The intensity of the electrical current caused the truck's tire's, nearby tanks, and adjacent ground to catch fire. The area where the material handler stood between two rows of tanks was about 28 feet from the truck. This area was not energized. He then went toward his coworker, presumably to provide aid, and entered the electrical field where he too was electrocuted. RECOMMENDATIONS: To prevent similar occurrences in the future, Washington State Fatality Assessment and Control Evaluation (FACE) investigators concluded that to protect employees from the hazard of electrocution from overhead power lines propane gas suppliers and other employers using cranes should follow these guidelines: 1. Conduct a site survey to identify the locations of overhead power lines and determine whether work tasks could expose crane operators or ground workers performing material handling activities to the hazard of electrocution through boom, rigging, hoist line, or load contact with overhead power lines. 2. Determine measures to be taken so that the hazard of power line electrocution may be avoided, eliminated, reduced, or controlled. Follow existing OSHA regulations and safe work practices. 3. Develop, implement, and enforce a comprehensive written safety program for all workers. The safety program should include training for all crane operators and ground workers in overhead power line electrocution hazard recognition and safe work procedures. Training should also include crane operator power line visual limitations and perception issues, as well as emergency procedures should power line contact occur. 4. Consider purchasing cranes with safety features intended to prevent or minimize the risk of electrocution. Alternatively, consider installing after-market safety accessories. At 4:48 p.m. the shop security camera shows the victim's fellow mechanic return to the shop after he finished repairing a truck he was working on. He returned to the shop where the victim was working, to get ready to leave for the day. He yelled for the victim but received no answer. The mechanic observed the victim's legs and called out his name again, with no response. The mechanic walked around the truck to the passenger side and observed the victim's left hand on the handle of the jack. The mechanic removed the victim's hand from the jack and shook him without getting a response. He yelled for other employees to dial 911 for assistance. The victim's head was pointed toward the jack which was on the front side of the tag axle. The victim was lying on his right side with 1/3 of his body on the creeper. The mechanic started jacking the jack but it would not move. He then turned the handle and the jack started elevating. Once the axle was lifted off the victim, another employee pulled the victim out from under the truck on the creeper. Employees trained in first aid and CPR started CPR and put an AED device on the victim and placed a call to EMS at 4:51pm. EMS and the Sheriff arrived six minutes after the call was placed, and the victim was transported via ambulance to the hospital. After being crushed by the dump truck, the victim succumbed to his injuries due to compression asphyxia on September 21, 2013 at 1:50 am. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Employers shall ensure that employees using jacks must crib, block, or otherwise secure the loads immediately after they have been raised. The hammer did not reach the 2x4 so he leaned out of his forklift to try to knock it out again. His tool belt hit the lift control and the lift assembly lowered on him. His head was crushed between the forklift assembly and the frame of the machine. The seat and safety bar hand an interlock function but may not have been functioning or had been deactivated. So here are some ways we can prevent something like this from happening where we work. Stay seated inside the protective cage when operating skid-steer loaders Turn equipment off before exiting. Never operate equipment if safety systems or features have been modified, or are not working properly. Conduct a check on safety features before use. Avoid loose clothing and remove items, like tool belts, that can interfere with safe operation of equipment. Management or I must make sure that only trained operators are allowed to operate. equipment. ASK: "Does anyone have more ideas or comments to share?" Pause for discussion. Then see if there are ways to take action. "Do we have any equipment where the safety feature has been deactivated?" The victim turned around to prepare the lower deck when the forklift operator released the car before it was properly secured to the deck. The car immediately rolled backward, and fell on the victim. He died immediately. So here are some ways we can prevent something like this from happening where we work. Always secure loads to the forklift, and make sure that the load is completely secured in the transporter before removing the forklift. Always make sure that all workers are clear before releasing a load, especially if you cannot see past your load. If necessary get a spotter to assist. Make sure to communicate and work with your loading partners. Management or I need to make sure that everyone using equipment are properly trained and retrained as needed. ASK: "Does anyone have more ideas or comments to share?" Pause for discussion. Then see if there are ways to take action. END WITH ACTION PLAN (ideas for what to ask or say). These source documents are primarily collected by the Fatality Assessment and Control Evaluation (FACE) project, which conducts in-depth investigations of select fatal occupational injuries with support from the National Institute for Occupational Safety and Health (NIOSH). The purpose of the FACE project is to develop a detailed understanding of how fatal injuries occur and to develop recommendations to prevent similar incidents in the future. These recommendations are disseminated to industry, labor, equipment manufacturers and others in positions to take action to prevent work-related deaths. This update provides an overview of fatal injuries at work that occurred in Massachusetts during 2013 and includes details collected by both the FACE and CFOI projects. Included are deaths traditionally linked to the work environment such as falls, electrocutions, and exposure to toxic chemicals. They also include workplace homicides and suicides as well as motor vehicle-related fatalities that occurred during travel on the job. The incident happened at a hospital parking lot where his employer was a subcontractor hired to install a storm water drainage system. His employer was a site preparation contractor. The victim was an experienced laborer and a member of the Laborers' International Union of North America. His job duties for this project included digging trenches and laying and connecting storm drain pipe. On the day of the incident, the victim and two other employees were digging trenches and installing storm drains. At the location they were working there was a buried duct bank which was in the way and conflicted with the plans for installing the storm drain. This duct bank contained three lines of PVC electrical conduit piping encased in concrete. Each line of conduit piping contained four power line cables carrying 7,200 volts each. In order to install the storm drain pipes to the necessary grade, the employees were using an excavator, breaker bar, and a rivet-buster-type jackhammer to chip away at the duct bank concrete. The victim was in the trench chipping the duct bank's concrete when his rivet buster punctured the conduit and contacted the power line cable. He was electrocuted and died at the scene. The local police and EMS arrived at the incident location within minutes. The victim was transported to a local hospital where he died of his injuries three days later. Contributing factors identified in this investigation included accessing the space underneath a raised dozer supported only by a bottle jack and working alone while manipulating heavy equipment. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Ensure that employees never place any portion of their body under a load only supported by a jack AB - 2.
