Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The CA/FACE investigator determined that, in order to prevent similar future incidents, tree trimming companies and palm tree trimmers should: 1. Ensure that a proper job hazard assessment and briefings are performed prior to starting work by a qualified tree worker who can assess the hazards, including electrical hazards. 2. Ensure that a qualified line clearance tree worker is used when tree trimming is within10 feet of energized high voltage power lines. In addition, homeowners and property management companies who need palm trees trimmed or removed should: 1. Hire only tree workers who are trained or certified by organizations such as the Tree Care Industry Association (TCIA) or the International Society of Arboriculture (ISA). He worked in the hay processing plant where he operated a hay press that compressed field baled and loose hay into high density bales for shipping. On the day of the incident, the victim had been operating the hay press as usual. Near the end of his shift, the plant supervisor asked the victim and two other employees to get the machine ready for maintenance. They started cleaning up by manually placing a few bales onto the conveyor, which had been stopped, and pushing them along toward the scale. The victim was responsible for locking out the equipment, but he did not do this. During normal operation, a conveyor moved hay into the covered area of the scale. When the hay reached the correct weight, a guillotine blade would slice off the section of hay before it was moved along by conveyor to be compressed. The victim crawled into the covered area where the scale was located and started to push a bale through to clear the area. His supervisor and coworkers did not notice him get into the machine. As he was lying on the scale, the steel guillotine blade activated and struck him, resulting in near decapitation. Incident investigators found out through interviews with employees that the victim had on a number of occasions exposed himself and others to unsafe conditions by not following proper procedures for lock out/tag out of the hay press. The six crew members were working on the boat in different areas that were covered with plastic tarps in order to prevent paint chips and dust from entering the water. At the start of the day, the gasoline-powered pressure washer was positioned on the dock. Later, employees of another contractor moved the pressure washer onto the boat to clear the way for supply deliveries. Shortly before the end of the work day, the victim moved the pressure washer onto a side deck passageway that was fully enclosed by plastic tarps. Working alone, he used the pressure washer for about 20 minutes. A co-worker went to look for the victim and found him unconscious on top of the pressure washer. Emergency responders arrived and determined that the victim was deceased. The medical examiner reported the cause of death as "carbon monoxide intoxication due to inhalation of engine exhaust." RECOMMENDATIONS: Washington State Fatality Assessment and Control Evaluation (FACE) investigators concluded that to protect employees from the hazard of carbon monoxide (CO) poisoning from small gasoline-powered engines and tools, employers should: 1. Ensure that gasoline-powered pressure washers or other fuel-powered tools are not used in enclosed or partially enclosed areas where CO can build up. 2. Train employees on the hazards, sources, symptoms, and control of CO exposure from fuel-powered equipment and tools. 3. Ensure that personal CO detectors equipped with audible alarms are used by employees when working with small gasoline-powered engines in locations where CO may build up. 4. Coordinate work activities at job sites where there are multiple contractors and their employees to ensure safe work. 5.when no longer readable. 6. Safety and Health Assessment and Research for Prevention AB - In July 2012, a 64-year-old maintenance mechanic at an aluminum foil processing facility died after being exposed to an approximately 180-degree Fahrenheit solution of boric acid and water while trying to remove a recirculation pump from its housing. The pump was part of a boric acid evaporation system. Workers believed that the system had been completely drained. As the victim and other workers used two 1.5-ton "come-along" puller devices to free the pump from its housing, a small amount of hot water/boric acid solution fanned out from around the seal. Seconds later, the force of the solution pushed the pump out of the housing, and hundreds of gallons of hot liquid flowed out. The victim fell to the floor and was covered with the solution. He was wearing no personal protective equipment other than safety glasses, and received severe burns to over 80% of his body. Some of the workers were able to get him to an emergency shower and began removing his soaked clothing, while others called 911. Emergency responders arrived within fifteen minutes and the victim was airlifted to a burn trauma unit. He died two days later due to his injuries. RECOMMENDATIONS - Washington State Fatality Assessment and Control Evaluation investigators concluded that to protect employees from similar exposure hazards, employers should: 1. Design new equipment for manufacturing processes using the concept of Prevention through Design (PtD) to control risks by incorporating prevention methods in the final product. 2. Enforce a comprehensive written hazardous energy control program. Train all potentially exposed workers in the hazardous energy control procedures for the system, and document training. 3. Develop written standard procedures for regular maintenance operations. These should include information about hazards, required PPE, necessary safety checks, lockout/tagout procedures, and coordination with other workgroups to be done prior to beginning the task. Ensure that all workers performing maintenance tasks are trained on the procedures. 4. Ensure that workers who may be exposed to hot liquids are provided with and use the proper personal protective equipment (PPE) for the job, including appropriate eye/face, hand, and body protection. CONTRIBUTING FACTORS: 1. Incomplete draining of the evaporation system before pump removal. 2. No method to identify the potential for solution to remain in the system after standard draining procedures were conducted. 3. No way to adequately determine the level and temperature of solution left in system. 4. No method to estimate the time need for cooling of the system to a safe level. 5. No mechanism to lock out the pump from other stored energy, such as solution under pressure. 6. The pump was located directly behind the heat exchange support column, leading to limited access and egress for workers during pump maintenance. 7. Not wearing appropriate personal protective equipment. On Monday afternoon at approximately 2:45 pm, a dump truck driver crossed into Kentucky from Ohio to pick up a load of #55 limestone gravel and deliver it to his assigned destination AB - this type of rock is very popular for use on driveways and walkways. The company stated the victim had driven this winding road many times over his 15-year tenure. As he traveled north, he approached a left angle curve. Tire marks suggested the truck gradually drifted to the right, went off the road and into a small ditch area. The driver attempted to drive out of the ditch 224.10 feet from the final resting spot of the truck. The deputy sheriff stated the truck driver was traveling over the recommended speed limit of 35 mph. While the speed limit along the state road was posted at 55 mph, the curve was posted at 35 mph with an advisory speed sign prior to the curve. As the victim entered the curve, neighbors who lived in the vicinity stated they heard the driver trying to apply the truck's compression release engine brake - commonly referred to as a Jake brake - in an attempt to slow the truck. The deputy stated the Jake brake failed to slow the truck to a safe speed, causing the loaded truck's center of gravity to shift toward the passenger side, and the driver to lose control of the vehicle. The truck left the roadway on the passenger side and flipped onto its right side into the culvert eight feet below the highway. As the truck flipped, the fuel tank struck a piece of rebar that was protruding from the concrete retaining wall leading to a water drainage tunnel under the highway. The fuel tank ruptured, causing a small fire in the engine that rapidly began to spread as fuel leaked from the tank. Two neighbors, a husband and wife who heard the crash and saw the smoke from their residence, ran to assist the driver. When they arrived and noticed the fire, the husband desperately tried to pull the unconscious driver out of the burning truck through the front windshield that had become dislodged, while his wife called for emergency assistance and the fire department. The couple stated the driver was thrown and trapped along the passenger side of the truck where the flames were hottest, making it impossible to pull his unconscious body from the burning truck. When the flames became too dangerous, the neighbors retreated to safety. Within four minutes, the cab became engulfed in flames. The local fire department arrived at the scene within five minutes of receiving the call and put out the flames. The truck driver was pronounced dead at the scene by the coroner. The decedent was facing south, finishing the concrete driveway and the power lines were at his back (north) when the incident occurred. A line of brush was approximately 8 feet away from him under the power lines, which may have required him to elevate the bull float handle rather than pull it in a more horizontal position. The decedent was wearing non-electrical rated rubber boots over his leather work boots, vinyl work gloves, hard hat and safety glasses. When the bull float handle contacted the overhead line, the decedent fell forward, letting go of the pole. The utility company measured the height of the two overhead lines after the incident. The bottom neutral line was 17 feet high and the 7,200-volt primary line was 23-feet high vertically and 24 feet from the edge of the concrete forms. There were two visible impact burn points on the wire. One was a 1.5-2-inch width of the bull float pole and the second contact was approximately 2.3-3.0 feet long where the pole may have slid along the wire. Emergency response was called and the decedent was transported to a local hospital where he died. MIFACE identified the following key and possibly contributing factors: 1. Unfamiliarity with residential jobsites and associated hazards relating to nearby, above ground power lines. 2. Failure to identify and assess power lines as hazards and make appropriate recommendations to employees regarding work practices and equipment. 3. Use of conductive bull float handles. Recommendations: 1. Conduct a jobsite survey and hazard assessment to identify all potential hazards, including those associated with overhead power lines. 2. Institute a hazard awareness program in which employees are educated about potential hazards as well as any specific hazards noted during the jobsite survey. 3. When working near overhead power lines, ensure that a minimum of 10 feet of space is maintained between the equipment and the power line. 4. Contact the power company to alert them about the work being performed near power lines. 5. Use bull float handles that are non-conductive and/or insulated. The victim was an experienced truck driver and a trucking supervisor who had been with his employer, a heavy civil construction company, for 27 years. On a rainy day shortly before 7 a.m., he was assisting a company truck driver who was unable to raise the landing legs of his belly dump trailer, also known as a bottom dump trailer. The driver had driven trucks for 34 years, but this was his first day pulling a belly dump trailer and he was unfamiliar with its operational features. The landing legs supported the weight of the trailer on the pavement and needed to be raised for the truck to be able to couple with the trailer properly. The driver unsuccessfully attempted to pull the pin locking the legs in the down position. He had backed his truck toward the trailer and heard a sound that he believed indicated that the truck's 5th wheel's jaws had closed and locked onto the belly dump trailer's kingpin, but he did not visually confirm the coupling had occurred. The victim determined that the truck's back was not elevated high enough to relieve the weight of the trailer so that the safety pins could be removed and the landing legs raised. He placed boards behind the truck's rear tires with the intention of having the truck back onto the boards to raise it. The victim was standing towards the rear of the truck in a narrow space between two trailers where the driver could not clearly see him. He told the driver to unlock the 5th wheel slide, which he did from the cab. He then told him to move the truck forward to allow the truck some movement before backing onto the boards. As the truck moved forward, the kingpin, which was not properly secured, came loose from the 5th wheel jaws and slid across the 5th wheel plate, causing the trailer to shift sideways, crushing the victim between the fenders of the two belly dump trailers. Methods: This longitudinal study, conducted among a cohort of United States (U.S.) adults aged 50 years and above, enabled: i) investigation of the associations between work-related physical factors and injuries; ii) comparison of the injured and uninjured workers for any new functional limitations, and reduced working hours post injury; iii) analyses of associations between psychosocial work factors and injuries; iv) comparison of two common approaches for analyzing longitudinal data with injuries as an outcome. The cohort to conduct these analyses was obtained from the Health and Retirement Study (HRS), a biennial nationally representative panel study of U.S. adults. The HRS respondents who were working for pay in the year 2004 (n = 7,212) formed the cohort for analyses investigating the associations between physical work requirements and injuries, and the consequences of such injuries. The cohort to investigate the association between psychosocial work factors and injuries included 3,305 working adults who responded to the HRS's psychosocial and lifestyle questionnaire in the years 2006 and 2010. Each of these cohorts were followed until 2014. Crude and adjusted incident rate ratios, and hazard ratios were obtained from generalized estimating equations (GEEs) and marginal structural models (MSMs), and Cox Hazard models. Variables included as confounders were demographic, health-related variables, and other work characteristics. Results: Those whose jobs required excessive physical effort, lifting heavy loads, and stooping/kneeling/crouching all/almost all the time, compared to none/almost none of the time, had over twice as high the risk of experiencing injuries. The MSMs (IRR: 2.62, 95% CI: 2.14, 3.20) provided an estimate higher than the GEEs (2.09, 1.67, 2.62). Injured, compared to uninjured adults, had a higher risk for having a functional limitation and reduced working hours post injury. Finally, important gender-based differences were identified when the modeled exposures included combinations of two psychosocial factors. Conclusions: Several physical and psychosocial factors elevated the injury risks among the aging workforce. It is therefore important to holistically investigate all exposures that may affect the risks for injuries in this population. The trailer tipper in this case had a movable platform that could hold, raise, and tilt a trailer to over 60 degrees and discharge the refuse through its rear doors (Photo 1). To get on the tipper, a tractor trailer had to back onto the trailer tipper deck. The communication between a tipper operator and a delivery driver was through a designated CB radio channel. The incident occurred on a Saturday at around 11 AM when the victim was operating the tipper and the last tractor trailer was getting ready to back onto the tipper. The trailer was hauled by a military surplus tractor ("army truck"). The "army truck" did not have a backup alarm and its radio was broken so there was no radio communication between the victim and the truck driver. At the time of the incident, the victim exited the operator's cab of the tipper and walked to the other side of the deck where the hydraulic controls for the tipper outrigger cylinders were located. He was wearing a high visibility vest. A landfill bulldozer operator stated that he saw the victim standing on the deck leaning over the outrigger controls. Meanwhile the "army truck" was backing onto the trailer deck. The bulldozer operator yelled into his CB radio to stop the delivery driver from backing onto the tipper AB - the "army truck" driver heard nothing since the truck radio was broken. The bulldozer operator could not warn the victim since the CB radio was inside the operator's cab and the victim could not hear the radio once he was outside the cab. The "army truck" driver did not see the victim and he continued backing onto the tipper deck. The victim was run over by the rear wheels of the trailer on the passenger side suffering fatal crushing injuries. The bulldozer operator got to the front of the tipper to signal the driver who stopped the truck. The bulldozer operator immediately called 911. EMTs responded and arrived at the site within minutes, but the victim was pronounced dead at the scene. CONTRIBUTING FACTORS: 1. The tipper operator and the "army truck" driver could not communicate with each other because the truck's radio was broken. 2. The tipper operator left the operator's cab and was on the deck while the "army truck" was backing onto the tipper. 3. The delivery truck driver backed the tractor trailer onto the deck without knowing/seeing that the tipper operator was on the deck. 4. The bulldozer operator could not warn the tipper operator because he could not communicate with the tipper operator once he was out of the operator's cab. 5. The "army truck" did not follow the standard operating procedure at the landfill. KEY RECOMMENDATIONS: 1. Landfill employers should implement measures to prevent tractor trailers from backing onto the tipper deck when a tipper operator is on the deck. 2. Landfill employers should provide additional communication methods for the equipment operators and other pedestrian workers who work around heavy mobile equipment. 3. Landfill employers should require that all visiting vehicles have the important safety features such as properly working CB radios and backup alarms. 4. Landfill employers should develop and implement a standard operating procedure (SOP) to ensure the safety of the tipper operators. 5. Landfill employers should develop and implement a comprehensive landfill waste disposal policy to ensure the safety of landfill employees and customers. 6. Landfill employers should provide training to ensure that workers understand and follow the SOP at the working face and the landfill waste disposal policy. 7. Solid waste trucking companies should follow the landfill safety policy and ensure that the vehicles meet the landfill's safety requirements and their employees follow the landfill safety policy. The objectives of this study were to (1) identify and characterize transportation incident management (TIM)-related occupational fatalities; (2) assess concordance of surveillance data sources in identifying TIM occupations, driver vs. pedestrian status, and occupational fatality incident location; and (3) determine and compare U.S. occupational fatality rates for TIM industries. Methods: The Kentucky Fatality Assessment and Control Evaluation (FACE) program analyzed 2005-2016 TIM occupational fatality data using multiple data sources: death certificate data, Collision Report Analysis for Safer Highways (CRASH) data, and media reports, among others. Literal text analysis was performed on FACE data, and a multiple linear regression model and SAS proc sgpanel were used to estimate and visualize the U.S. TIM occupational mortality trend lines and confidence bounds. Results: There were 29 TIM fatalities from 2005 to 2015 in Kentucky; 41% of decedents were in the police protection occupation, and 21% each were in the fire protection and motor vehicle towing industries. Over one half of the TIM decedents were performing work activities as pedestrians when they died. Media reports identified the majority of the occupational fatalities as TIM related (28 of 29 TIM-related deaths); the use of death certificates as the sole surveillance data source only identified 17 of the 29 deaths as TIM related, and the use of CRASH data only identified 4 of the 29 deaths as TIM related. Injury scenario text analysis showed that law enforcement vehicle pursuit, towing and recovery vehicle loading, and disabled vehicle response were particular high-risk activities that led to TIM deaths. Using U.S. data, the motor vehicle towing industry had a significantly higher risk for occupational mortality compared to the fire protection and police protection industries. Conclusions: Multiple data sources are needed to comprehensively identify TIM fatalities and to examine the circumstances surrounding TIM fatalities, because no one data source in itself was adequate and undercounted the total number of TIM fatalities. The motor vehicle towing industry, in particular, is at elevated risk for occupational mortality, and targeted mandatory TIM training for the motor vehicle towing industry should be considered. In addition, enhanced law enforcement roadside safety training during vehicle pursuit and apprehension of suspects is recommended. Department of Public Health AB - On June 28, 2016, a 52-year-old male foreman (victim), employed by a roadway painting and safety company, was fatally injured when he was struck by a car. The victim was part of a work crew painting roadway lines within a highway work zone at night. While standing in front of his work truck, a passing motor vehicle intruded into the work zone. The motor vehicle struck the victim and continued forward, striking a cone truck that was backing. A call was immediately placed for emergency medical services (EMS). EMS and state police arrived within minutes. The victim was transported to a local hospital where he was pronounced dead on arrival. CONTRIBUTING FACTORS/RECOMMENDATIONS - Contributing factors identified in this investigation included the victim being on foot and in proximity to the edge of the work zone and the operator of the motor vehicle that struck the victim was allegedly under the influence of alcohol. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Employers should ensure shadow vehicle operators remain inside the shadow vehicle's cab at all times while in an active work zone. 2. Employers should provide routine start of shift reminders of potential hazards. 3. Transportation departments should consider using blue LED light trailers at work zones where a police detail is not required and the roadway speed limit is greater than 45 mph. 4. Motorists should never drink and drive and always plan ahead to ensure they have a ride with a sober driver, so remember: Ask, Designate, Hail. The company used driverless forklifts, known as laser guided vehicles (LGVs), in the warehouse to move pallets of water bottles. LGVs automatically navigated using a system of vehicle mounted lasers and reflectors positioned throughout the warehouse. Each LGV had safety sensors Photo of LGV involved in the incident. designed to detect objects or workers in the vehicle's path. When a sensor detected an obstacle, the LGV would stop moving and an alarm would sound until a worker removed the obstacle. The manufacturer's manual required workers to initiate an emergency stop before removing an obstacle detected by the LGV. An LGV would then have to be manually reset before restarting its task. Without initiating an emergency stop, the LGVs would resume automatic function immediately after an obstruction was removed. The victim was working in the warehouse when an LGV alarm was activated. Shortly after, another worker heard noises over the radio and looked into the warehouse to investigate. He saw the victim slumped over one of the LGVs. The LGV then started moving again, and the other worker hurried to it and initiated an emergency stop. He could see that the victim had severe injuries to his chest and jaw. He called for help and called 911. Emergency responders arrived within minutes and transported the victim to a local hospital where he was pronounced dead. Investigators believe that the victim attempted to remove a piece of plastic from under the elevated forks of the LGV without first initiating an emergency stop. Long strips of plastic wrap often tore off of pallets during loading and unloading and stuck to the forks of the LGVs. If the plastic entered the safety detection field of the LGV, it would be recognized as an obstacle. Other employees said that these plastic "stringers" frequently triggered LGV alarms. The victim was likely bending or kneeling under the forks outside of the safety sensor field to reach the plastic stringer. Because the LGV was not in emergency stop mode, the system reset when the obstacle was removed, bringing the elevated forks down, crushing him against the wheel cover of the vehicle. There were warning signs affixed to the vehicle indicating that workers should avoid standing beneath the elevated forks. On the day of the incident, he was assigned to remove frost buildup from the pipes on the ceiling of the freezer unit of the plant's cold storage warehouse. He used a scissor lift to access the ceiling and a squeegee to remove the frost. He had done this task many times. The lift's entry swing gate was damaged and had been removed for repair. A metal chain was put in its place. The chain was positioned twenty-five inches above the platform, at around the height of the mid-rail of the platform's guardrail. As the victim scraped the frost, it fell onto the lift's platform, which became slippery. After about 45 minutes, another laborer removing ice from the floor heard a noise and turned around to see the victim on the floor. Emergency responders declared him dead at the scene. Investigators found that the victim had fallen through the gap where the gate had been removed and landed on the floor 20 feet below. The lift did not have lanyard anchorage points.
