Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The decedent asked his coworker to chock the tire and take off the air brake. The truck was on a slight incline so his coworker chocked the back of the front right tire. His coworker released the air brake but did not take the truck out of drive. The decedent was using a small sledge hammer to tap on the brake drum. When the brake popped free, the truck rolled forward, over the decedent and his coworker's leg. The truck came to rest across the street against a wall. Emergency response was called. He was declared dead at the scene. Contributing factors: MIFACE identified the following key and possibly contributing factors: 1. The truck was parked in the public works yard for an extended period of time in adverse weather conditions. 2. The truck was running, transmission in gear, and one wheel chocked in only one direction while the task was performed. 3. No standard operating procedure to minimize the likelihood of stuck/frozen brakes. Recommendations: MIFACE investigators concluded that, to help prevent similar occurrences, employers should: 1. Establish written standard operating procedures to minimize the possibility of brakes freezing. 2. Develop safe work practices and train truck drivers and other appropriate personnel to safely free stuck/frozen truck or other vehicle brakes. The area where the forklift operator was stacking the bales was next to a door leading to the company's shoe department. Clothing bales were stacked on both sides of the door. The employees who worked in this room regularly used this door to access the warehouse. The victim, who was the shoe department manager, was walking toward the door after returning from a break. The new bales were stacked six high and had only been in place for a few minutes. As she approached the stack, the forklift operator noticed that the bales had started to move. He called to her "look out!" As she turned to look toward the forklift operator, four bales fell from the top of the stack. One bale hit the lower part of her body, knocking her to the ground, and then a second bale landed on her upper body. The bales weighed from 500 to 780 pounds and measured approximately 48 inches by 60 inches by 33 inches. Recommendations: Washington State Fatality Assessment and Control Evaluation investigators concluded that to protect employees from similar hazards employers should: 1. Perform a job analysis and hazard assessment of bale storage practices. Based on the results of the job analysis and hazard assessment, employers should create a written safe materials stacking guide and train employees on the procedures. 2. Ensure that areas in facilities where bales are to be stored should be of sufficient size and adequate layout to accommodate the intended bales, forklift traffic, and pedestrian traffic without exposing employees to the hazard of bales falling from stacks. 3. Ensure that bale storage areas are designated and access limited to authorized employees. 4. Ensure that bales are in good condition and are stored in a manner that will minimize the hazard of stacks becoming unstable causing bales to fall. 5. Implement daily inspections to evaluate the conditions of bale stacks to ensure stability of the stacks and correct any deficiencies that are identified. Near the bottom of the tank was a pipe to drain the contents. The drain pipe had been turned "up" rather than "down". The workers hooked up a gasoline-fueled pump to the bottom valve to pump the molasses/water mixture from the tank. When no more of the mixture could be removed, his coworker indicated that a sufficient quantity of the molasses/water mixture had been pumped from the tank. The coworker stepped away, and several minutes later, noticed the decedent preparing to enter the tank. At the top of the tank was a 15-inch hatch opening. After some discussion, the coworker, who told the decedent not to enter the tank, agreed to "spot" the decedent while he entered the tank to rotate the drain pipe to the "down" position in order to be able to pump more of the mixture from the tank. To enter the tank, the decedent placed a lift truck in front of the tank and raised the forks to a position so the forks were straddling the 15-inch opening. Suspended from the mast and the forks were two cargo tie-down straps. The decedent donned a pair of full-length waders and a full-face respirator equipped with ammonia cartridges and rappelled to the base of the tank to reposition the drain pipe. He pushed the drain pipe into position, and then attempted to climb out of the tank by holding the straps and placing his feet on the side of the tank and pulling/walking up. The decedent twice attempted to climb out of the tank. The coworker observed the decedent become unresponsive and attempted to pull him out of the tank, but was unsuccessful. The spotter then ran to the firm's office and emergency response was summoned. Firm personnel used a circular saw to cut a 4-foot by 4-foot hole in the tank. Two employees entered the tank and found the decedent submerged in the mixture. They pulled the decedent from the tank and began CPR. Emergency response personnel arrived and the decedent was transported to a local hospital where he was declared dead. MIFACE identified the following key and possibly contributing factors: 1. The business did not have a permit-required confined space program in place, and consequently workers had no training in confined space hazards, entry, or rescue. 2. The hatch door was not locked, which provided unrestricted entry into the molasses tank. The atmosphere of the tank was not tested prior to, or monitored throughout, entry. 3. Employees were not empowered to prohibit the entry of coworkers into a confined space. After several unsuccessful attempts to inflate the tire, the two mechanics working on the tire used a jack to raise the rear of the truck to take the pressure off of the tire. After several more unsuccessful attempts to inflate the tire, the mechanics removed the outside tire. After the outside tire was removed, the mechanics again attempted to inflate the tire AB - this time it was successful (they were able to get the bead) using a TSI Cheetah bead seating tool, but they could hear air leakage from the tire. Mechanic #1 asked the decedent to release the cheetah's air brake. The tire was rotated and a piece of metal was found. Mechanic #2 left the scene to retrieve a tire plug kit. Mechanic #1 was positioned to the right of the tire. The decedent walked up to the left of Mechanic #1 (the decedent was standing directly in front of the tire) presumably to point out the metal. Mechanic #1 warned the decedent to move away while he removed the air chuck. The sidewall of the pressurized tire failed releasing the pressurized air. The force of air from the "explosion" launched the decedent backward. He landed approximately 12 feet away on his back and struck his head on the concrete floor. Emergency response was called and the decedent was transported to a local hospital where he died. MIFACE identified the following key and possibly contributing factors: 1. The decedent was not trained on truck maintenance safety, including safe practices during tire maintenance. 2. The company's program to ensure that only trained and authorized personnel were allowed entry to the maintenance area was not enforced. 3. A tire cage was not used during the attempted inflation and maintenance of a tire. On the day of the incident, a foreman instructed them to get some plywood forms to be used to repair form work. They were unclear about where to get the forms so they walked through the tunnel until they found forms from a newly poured concrete wall section at the top of a ventilation knockout. Assuming that these were the forms they were looking for, they set up a Baker scaffold below the forms at the knockout. Working from the 8-foot-high scaffold deck, the victim stripped a form from overhead. His coworker was standing below inside the knockout ready to grab the 8'x2' form. As the victim leaned over the handrail to lower the form, the scaffold began to tip over. The coworker was able to move out of the way. The scaffold hit the lower wall of the knockout and he was thrown off, landing headfirst 11 feet below on the concrete. He suffered severe head, neck, and spinal injuries resulting in impaired mobility. Investigators found that the employer did not follow procedures to ensure that new workers were prepared to work, and that they did not receive proper direction, instruction, and supervision. The Baker scaffold was not appropriate for the assigned task, and it had not been assembled correctly according to the manufacturer's specifications, which require outriggers to be used when a 39" deck extension is added. The employer had established that boom lifts were the most stable and safest equipment to use when workers were accessing forms that were located at elevation. The employer had boom lifts available to perform this task, though the workers were not advised of this. Perform a hazard assessment prior to beginning work to be aware of the hazards in the immediate work area. 2. Require drivers to have a spotter when working around electrical lines. 3. The victim, who had never done any tree work before, was hired by the employer to assist ground work at three residential job sites. All three jobs involved cutting down trees and chipping the cuttings with the wood chipper. At the frst site, the employer demonstrated to the victim how to feed the branches into the chipper and how to turn the machine off. He also showed the victim how the feed control bar and the "last chance cables" worked. The employer instructed the victim not to feed the chipper when the feed wheels were engaged. However, he also told the victim that he could feed the chipper when the experienced workers were nearby or next to the machine. The work scene at the second job site was captured by a surveillance video camera on the property. The images showed that the victim was feeding the chipper when the feed wheels were engaged, leaning down and reaching into the chipper, and pushing small debris into the chipper with his hands. The incident occurred at the third job site. No one saw the instant when the victim was pulled into the chipper. The employer stated that he saw the victim right before the incident. The victim was standing by the chipper and "there was nothing to pick up and put in the chipper". Less than a minute later, he heard the chipper making a "weird noise". He turned and saw the victim's feet sticking out of the chipper. The employer rushed to the chipper and turned it off before calling 911. The EMS and police arrived within minutes and the victim was pronounced dead at the scene. The wood chipper was examined by the manufacturer 12 days after the incident. The examination did not fnd any defciency or malfunctioning parts on the chipper that might have contributed to the incident. CONTRIBUTING FACTORS - Key contributing factors identifed in this investigation include: 1. Inexperienced and untrained workers were assigned to feed chipper AB - 2. Worker training on chipper hazards and safe feeding techniques were not provided AB - 3. Safe feeding techniques were not followed AB - 4. Workers were not required to wear proper clothing and PPE AB - and 5. Adequate supervision was not provided. RECOMMENDATIONS - NY FACE investigators concluded that, to help prevent similar occurrences, employers should: 1. Ensure that workers follow safe feeding techniques recommended by chipper manufacturers. 2. Ensure that all employees receive proper and adequate training before assigning them to feed wood chippers. 3. Ensure that workers wear proper clothing and personal protective equipment when operating/feeding wood chippers. 4. Strictly follow manufacturer's requirements and instructions when operating and feeding wood chippers. 5. Always keep the user's manual on the chipper and make it accessible to workers. 6. Chipper manufacturers should develop fully automatic feed control devices to prevent fatal caught-in incidents. 7. More research and resources should be dedicated and allocated to help self-employed and small businesses achieve better worker protection. The construction site had a high water table. The trench was part of a 175-foot long trench. The trench in the incident area was approximately 7 feet wide at its top, 6½ feet deep, and 4 feet wide at its base. An 18-inch bench was cut approximately 18 inches below the top of the trench and 5 feet from the base. The trench had a 1½-foot seam of wet sand at its base, and then another 5 feet of wet clay. A sand/clay ramp located on the east side of the trench provided access. The excavator operator noticed moisture in the trench and asked the decedent if he would like to utilize the trench box on site AB - the decedent declined. The decedent and his coworker were preparing to install drain tile pipe when the competent person who had just arrived at the excavation after performing other site work, noticed the impending wall collapse. He yelled to the two workers to get out of the trench. Both ran toward the ramp. One worker was able to exit safely. The decedent was struck by the falling earth. The clay forced a shovel to strike his right side and pinned him into the opposite side of the excavation burying him to his waist. His coworkers entered the trench and manually dug him out. They carried him out of the trench and called for emergency response. Emergency responders transported him to a local hospital where he was declared dead in the emergency room. CONTRIBUTING FACTORS: Key contributing factors identified in this investigation include: 1. Inadequate tench support systems (shoring, benching, sloping). 2. A qualified person did not inspect the trench prior to entry. 3. Inadequate employee training in the recognition and avoidance of unsafe conditions and required safe work practices (insufficient understanding of hazards). RECOMMENDATIONS: MIFACE investigators concluded that, to help prevent similar occurrences, employers should: 1. Ensure that employees working in excavations are protected from cave-in by an appropriate protective system, such as trench boxes, shields, benching and/or appropriate sloping of trench sides designed in accordance with MIOSHA Construction Safety Standard, Part 9, Excavation, Trenching, and Shoring. To ensure this is done correctly, a qualified person must inspect and approve the excavation, adjacent areas, and supporting systems on an ongoing basis. 2. Ensure that all employees are trained to recognize and avoid hazardous work conditions. Employers should also ensure that the training in recognizing and avoiding hazards is coupled with employer assessment that workers are competent in the recognition of hazards and safe work practices. In addition, a job safety analysis (JSA) should be completed prior to working so that the hazards could be recognized. 3. Develop a trench emergency action plan that describes rescue and medical duties and ensure that all employees are knowledgeable of those procedures. Strapped to a carrying mechanism on the rear of the tractor was a tank containing herbicide. The decedent had been in his rented field conducting weed-killing hand spraying activities at the field/woods edge. As he was returning home in the rain, he came across a washout of the ravine edge on the passenger side of the tractor. The tractor rolled to the side down the ravine wall, pinning him under the tractor. When a family member could not reach him on his cell phone, the family member called another family member to look for him. The family member and the farmer who rented the land were walking to where the decedent was last seen and found the overturned tractor. The family member called another family member who called for emergency response. The decedent was declared dead at the scene. CONTRIBUTING FACTORS - Key contributing factors identified in this investigation include: 1. He got off the tractor and walked back to his house to pick up a screwdriver and bushman's saw (similar to a hack saw). It is unknown if the tractor was left running when he went back to the house. Returning to the tractor, he went under the tractor with his body near the left rear tractor wheel. The tractor was in gear and PTO running. Using the bushman's saw he sawed the log/tree that had contributed to the tractor to becoming stuck. After the tree was sawn through, the tractor fell to the ground. Based on both clothing damage and injuries, he may have come into contact with the rotating power take off (PTO). He was found by a family member under the left rear tractor wheel. Emergency response was called and he was declared dead at the scene. CONTRIBUTING FACTORS - Key contributing factors identified in this investigation include: 1. The tractor was running, in gear, with PTO rotating when he went under the tractor. 2. Front tractor tires not chocked or blocked. 3. The elevated tractor was not supported when decedent worked under the tractor. 4. He was working alone. Always utilize proper tractor shut-off procedures prior to exiting the tractor seat. 2. Always provide secondary means of support when working under an elevated piece of equipment. 3. The decedent was declared dead at the scene. CONTRIBUTING FACTORS: Key contributing factors identified in this investigation include: 1. Decedent was starting the tractor from the ground rather than the seat. 2. Improper tractor shut down procedures. 3. Tractor in poor condition. 4. He fell through a gap between the scissor lift he was working from and the building when the deck extension platform of the scissor lift moved. The incident took place at job site where a new apartment building was being constructed. The victim had been working for the company for two years. The day before the incident, an employee reported to a foreman that the rental scissor lift had a broken deck extension locking pin. This spring-loaded pin was used to lock the lift's deck extension in place to keep it from moving while in use. The foreman instructed the lift operator to contact the rental company for a replacement pin and continue to use the lift until a new pin arrived. On the day of the incident, the replacement pin had not arrived. The victim and another laborer removed a portion of the lift's platform railing so that they could unload insulation bundles from the lift onto the building's sixth floor. They extended the platform to allow access to the floor. The other laborer positioned himself toward the rear of the platform. As he was using his foot to brace the extension to keep it from moving, he passed a bundle toward the victim who was standing on the extension. The victim received the bundle from the laborer and began rolling it along the platform. As he was doing this, the extension rolled back into the retracted position, causing him to lose his balance and fall through the gap between the building and the lift. He landed 60 feet below. He suffered numerous severe injuries. After more than eight months off work he was ready to return to light duty work. The worker was part of a crew installing a sewer pipe at a residential property. The trench was 10 feet deep and approximately 3 feet wide in the area where the collapse occurred. The collapse occurred in an unprotected area of the trench, between two pieces of shoring that were spaced 15 feet apart. When the trench collapsed, co-workers called 911 and attempted unsuccessfully to locate and dig the worker out. When emergency responders arrived, their first task before commencing rescue and recovery efforts was to shore up the trench to prevent risk of additional collapse and injuries. Emergency responders spent several hours recovering the worker's body. Key factors identified in this investigation included an inadequate trench protective system AB - no ladder available for entry or egress AB - and a spoils pile being placed too close to the edge of the trench. Underlying contributing factors were the lack of oversight by a competent person AB - and insufficient employee training on recognizing and reporting hazards. RECOMMENDATIONS: 1. Employers who have employees working in trenches that are 5 feet deep or deeper must select and install appropriate protection systems for the conditions present to protect workers from cave-ins. For trenches less than 5 feet deep, examination by a competent person for potential cave-in indicators is required. 2. Excavation work requires a designated competent person on site who has both the knowledge and authority to identify and promptly correct hazards AB - this includes daily inspections prior to the start of work and as needed throughout the work shift, and any time site conditions change. 3. To select appropriate trench protective systems for a given jobsite, the competent person should visually and manually test the soils, and also consult the shoring or shielding manufacturer's tabulated data or OR-OSHA's tables and charts designed for this purpose. 4. Employers must provide sufficient means of safe access and egress for workers in any trench excavation of 4 feet deep or deeper, which may include ladders, ramps, or stairs. 5. Keep excavated soil (spoils) and other materials and tools at least 2 feet from the edge of any trench. 6. Before working in and around trenches, employees should receive training on trenching safe practices, and on recognizing and reporting hazards. 7. Employers should develop and maintain a safety culture where employees are encouraged to voice concerns about unsafe work conditions. The officer was en-route to her primary patrol region after escorting a suspect to the city jail when she responded to a shots fired call from municipal dispatch. Multiple university and municipal police officers were working in the same vicinity and simultaneously responded to the shots fired call. As the municipal police officer was traveling westbound, running lights and sirens, she entered an intersection against the traffic control device. At the same time, a university police officer traveling northbound, also running lights and siren, entered the same intersection. The northbound university patrol vehicle crashed into the westbound municipal patrol car. Both officers were taken to a local medical center by ambulance. The municipal police officer died from injuries sustained in the crash. The university police officer received serious injuries. At the time of the incident, the equipment operator was mowing grass at a municipality-owned pump station. As the operator turned the mower to the left, it struck a low-lying rock covered by vegetation. The mower overturned, pinning the operator against the ground. A motorist passing by noticed the overturned mower, stopped, and placed a call for emergency medical services (EMS). Key contributing factors identified in this investigation include: 1. Mower was operated across a slope greater than 15 degrees AB - 2. Mower was operated while rollover protective structures (ROPS) were in the folded down position AB - 3. Lack of a comprehensive safety and health program AB - and 4. Working alone. Massachusetts FACE Program concluded that, to help prevent similar occurrences, employers should: 1. Ensure ride-on mower operators use ROPS and seatbelts when the mower is equipped with them. 2. Routinely perform job safety analyses before mowing begins to outline the proper equipment and practices are used to perform the task safely. 3. Ensure that ride-on mowers are not operated on slopes where the angle is greater than the maximum slope specified by the manufacturer. 4. Routinely provide employees with training on the equipment used to complete tasks. 5. Develop and implement a comprehensive safety and health program that addresses hazard recognition and avoidance of unsafe conditions. 6. Consider developing policies that prevent employees from working alone when performing certain tasks. The 80 to 90-foot, fire-damaged oak tree was located below a county road that ran along the steep bank of a creek ravine. The crew had been directed not to fell the tree into the ravine because of environmental and cultural concerns. The crew had been directed to take the tree down in small sections. However, after the supervisor left, the victim decided that they should attempt to bring most of the tree down in two large sections. The crew used a rigging line, pulled by a mini-excavator, to attempt to direct the cut sections to fall uphill towards the road. Neither section fell in the direction intended AB - the second section fell uphill against the bucket truck, fatally crushing the victim. The California Fatality Assessment and Control Evaluation Program (CA/FACE) concluded that organizations involved in tree-felling operations should take the following steps to prevent similar incidents: 1. Tree-felling contractors should ensure that each crew includes, and is led by, a person with substantial experience in tree felling. 2. Tree-felling contractors should ensure that all supervisors are certified in tree-felling safety by an organization such as the Tree Care Industry Association (TCIA) or the International Society of Arboriculture (ISA). In addition, government agencies, utilities,and other project customers should include bid language requiring that all supervisors be certified in tree-felling safety by one of these organizations. 3. Tree-felling crews should always be able to readily communicate with their supervisor and medical emergency services. If cell phone reception is not reliable, the employer should issue the crew a satellite phone.
