Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The decedent responded to the call and parked his truck approximately 8-10 feet in front of the school bus. The decedent was lying on the ground, looking under the bus trying to decide what equipment he would need to hook the bus to move it. An SUV traveling in the left lane abruptly veered right, swerving across the right and exit lanes, driving onto the shoulder. As the SUV attempted to reenter the freeway it struck the driver's side rear of the school bus, pushing it forward eight to ten feet. The bus pinned the decedent against the boom, the tire ran over and crushed him, and the bus spring shackle bolts struck him in the chest. He was declared dead at the scene. CONTRIBUTING FACTORS - Key contributing factors identified in this investigation include: 1. SUV driver allegedly impaired by alcohol and drugs lost vehicle control due to an abrupt lane change. 2. Empty school bus parked on shoulder of exit ramp on busy expressway. RECOMMENDATIONS - MIFACE investigators concluded that, to help prevent similar occurrences, employers should: 1. School districts and firms employing school bus drivers should develop a policy regarding stopping on an active roadway. 2. Tow truck operators should work in conjunction with law enforcement/emergency service providers to secure the work area prior to loading and securing a vehicle. CONTRIBUTING FACTORS - Key contributing factors identified in this investigation include: 1. Personal flotation device (PFD) and non-slip shoes not worn. 2. Emergency escape ladders or life rings not installed escape. 3. Cluttered, narrow boat slip walkways. 4. Medical condition may have caused him to lose balance. 5. Working alone. RECOMMENDATIONS - MIFACE investigators concluded that, to help prevent similar occurrences, employers should: 1. Wear a PFD and non-slip shoes while working on docks and boats. 2. The incident happened in the parking lot of a distribution center under construction. A construction contractor had rented the lift and found that it was not working due to a leak in its hydraulic line. They contacted the rental company, which then contacted a hydraulic and industrial hose repair service. The repair service sent two experienced service technicians to repair the lift. After they repaired the hydraulic line, they decided to test that the repair had been successful. The technicians were focused on their task and did not notice the high voltage 69 kV power transmission line 36 feet above the ground overhead. The day was overcast with intermittent showers, which may have affected their ability to see the power line. From the ground, one technician operated the lift's controls to elevate the boom. The other stood behind the lift to check for a hydraulic fluid leak. As the technician raised the boom, it came near to the power line, but did not touch it. Electrical current arced from the line to the lift basket's wet metal railing and traveled through the lift's frame into the parking lot asphalt where the technicians were standing. The shirt of the technician standing behind the lift caught fire and he received electrical burn wounds to his upper body and left foot. The other technician received electrical burns to his fingers. Both were hospitalized; one worker returned to work after several days, the other after several weeks. That afternoon while crabbing, the vessel's external lights failed and the Captain decided to return to port. While returning across the bar, a wave washed over the stern, capsizing the vessel. It rolled and broke apart on the rock jetty. A rescue locator beacon (Emergency Position Indicating Radio Beacon or EPIRB) on the vessel activated, and was noted by the local Coast Guard station. Search and rescue operations began shortly after. The Captain was thrown from the capsized vessel, swam clear of the debris, and was pushed by waves onto the jetty. He found someone to take him to the Coast Guard station where he reported the incident. He refused medical treatment and the required drug and alcohol testing, and left the station. One crew member was recovered later that evening, deceased. A second crew member was found approximately three weeks later, also deceased. The third missing crew member was not found but was presumed deceased. CONTRIBUTING FACTORS - Key contributing factors identified in this investigation include: 1. Crossing the bar to fish during especially adverse weather conditions. 2. Generator failure resulting in the Captain's decision to return to port. 3. Failure to request U. S. Coast Guard escort for the return bar crossing. 4. None of the crew members were wearing Personal Floatation Devices (PFD). 5. Failure to secure the cabin door in heavy seas. 6. Inadequate training (drills) for possible "abandon ship" and "man overboard" scenarios. RECOMMENDATIONS - Oregon Fatality Assessment and Control Evaluation (OR-FACE) investigators concluded that to help prevent similar occurrences, employers should: 1. Confirm major vessel repairs are adequate prior to sailing, especially prior to sailing in rough seas and inclement weather. 2. Before deciding to fish and venture to sea, Captains must consider weather conditions and vessel suitability. 3. If the decision is made to cross the bar in especially rough seas, the Captain should request and obtain a Coast Guard escort for both departing and returning crossings. 4. Vessel Owners/Employers and/or Captains should require crew members to don personal flotation devices (PFDs) when on deck, especially in rough seas and/or when crossing the bar. 5. Secure all potential water infiltration points in rough seas, including doors, windows, and hatch covers. 6. Captains should routinely assess job hazards and provide regular, monthly safety training and drills, including for man overboard scenarios. 7. Captains should encourage crew members to seek out available regional safety training. Department of Labor and Industries AB - A 28-year-old worker died when the orchard tractor he was operating with an attached sprayer went down an embankment and rolled over. He had worked as a tractor operator for three months at the apple orchard and had completed the employer's tractor training program. On the day of the incident, he was applying insecticide to trees at the orchard. The tractor was a low profile, narrow chassis orchard/vineyard type tractor. Its wheel track was four feet wide. The sprayer contained an estimated 350 gallons (approximately 2800lbs.) of insecticide solution. He started his shift at 12 midnight. He and other members of the spray crew met with their supervisor who explained their duties for that shift. He and another tractor operator were working as a team spraying trees in a block he had sprayed multiple times over the past several months. The trees in this block were mature with a full canopy. Because of this, the area was exempt from the Washington State rollover protective structure (ROPS) requirement, as use of ROPS would have interfered with normal operations. Both operators had their tractor ROPS in the down position and their headlights on. At about five a.m., the operator's coworker went ahead down a sloping orchard row and then turned onto a dirt service road to go to another area. Shortly after, the coworker looked behind and did not see the lights of the operator's tractor so he turned around to look for him. He found the operator's tractor at the bottom of a 6-foot embankment. It had left the service road a few yards from the orchard row and rolled down the embankment. The operator was lying next to the tractor. He died from a skull fracture due to blunt force trauma. Investigators concluded that the operator had most likely misjudged the turning distance of the tractor and steered too wide as he came out of the row onto the road. The weight of the tractor and sprayer caused the edge of the road to crumble, leading it to roll down the embankment. In addition, the lighting near daybreak may have affected his visibility of the embankment at the edge of the road. On the day of the incident, they were working to remove three layers of roofing materials to check for rotten spots. Warning lines were set up near the roof's edges and a safety monitor was used. Workers were not required to use personal fall protection while inside the warning lines. Outside of the warning lines, they were required to use a personal fall arrest system consisting of a full body harness with ropes tied-off to anchor points. Most of the visible rotten roof was in the area outside of the warning lines. The roofer was inside the warning lines near the roof ridge using a shovel to scrape off shingles and insulation. As he stepped backward, a patch of rotten roof gave way and he fell through, landing 20 feet below on wood flooring. He was severely injured and suffered numerous fractures and internal injuries. Investigators found that a worker had previously placed an orange cone to mark a rotten spot near where the roofer broke through the roof. The spot he fell through was three feet away from the cone and under three layers of roofing material so he was not able to recognize it was rotten. Workers had also been walking across the roof in the area for several weeks. At the time of the incident, the safety monitor was on the other side of the roof ridge throwing debris into a truck below. After the incident, the employer required workers to use a personal fall arrest system at all times. The pickup truck had a flat tire. The pickup driver stopped near an expressway ramp intersection lit by overhead lights. The decedent arrived and activated the tow truck's flashing lights. When exiting the truck, he did not don his reflective vest. He was unsuccessful in lowering the spare tire from the under-the-truck mounting mechanism. He repositioned the tow truck to the front of the disabled pickup truck. He attached the front hooks to the vehicle and winched the pickup partway up the flatbed. He returned to the rear of the pickup truck and began to place the rear securement chains when a driver, returning home from a party did not stop and who had an elevated blood alcohol concentration (BAC) and marijuana level, struck the decedent, one of the pickup truck passengers, and the rear of the tow truck. The decedent was pinned between the rear of the flatbed and the front of the vehicle that struck him. The decedent struck the windshield on the driver's side and the pickup truck's hitch struck the windshield on the passenger side. Emergency response was called and the decedent was declared dead at the scene and the injured pickup truck passenger was transported to the hospital. CONTRIBUTING FACTORS - Key contributing factors identified in this investigation include: 1. Design and lack of maintenance of spare tire hoist system did not permit decedent to lower spare tire. 2. Driver operating while intoxicated. RECOMMENDATIONS - MIFACE investigators concluded that, to help prevent similar occurrences, employers should: 1. Vehicle dealerships/servicing centers servicing vehicles with spare tire safety retaining latch/hoist systems should inspect this system at least annually for functionality. One of the coworkers saw the tractor, which was still running and in neutral positioned at an angle in the tall grass against a tree on the west side of the two-track path AB - the trailer was positioned on the two-track path. The coworker looked and called for the decedent. Not seeing him or hearing a response, the coworker got on the tractor and moved it backward onto the two-track. The coworker felt the rear tire of the tractor rise and then the decedent's body came into view. Emergency response and the owner were summoned. The decedent was pronounced dead at the scene. CONTRIBUTING FACTORS - Key contributing factors identified in this investigation include: 1. Tractor was difficult to start that morning and decedent left tractor running and in neutral on a downhill slope when he dismounted. 2. Tractor's emergency brake was non-functional. 3. Unsecured orchard ladders and fruit lugs at front of trailer. RECOMMENDATIONS - NIOSH investigators concluded that, to help prevent similar occurrences, employers should: 1. Ensure tractor operators utilize proper tractor shut-off procedures prior to exiting the tractor seat. 2. Inspect and maintain equipment to ensure it is in good operating condition. 3. He had previously bypassed the skid steer's safety lap bar. He stopped the skid steer on the concrete pad in front of the wood burner and exited the cab without turning off the machine. The incident was unwitnessed. When the decedent's spouse arrived home from work, she noticed the decedent "dangling" and called out to him. She ran to the skid steer, and seeing his position, immediately called for emergency response. When responders arrived, they turned off the machine to assess his condition. Finding him deceased, they restarted the skid steer and tilted the bucket to lower him to the ground, and then lowered the bucket to the ground. CONTRIBUTING FACTORS - Key contributing factors identified in this investigation include: 1. Rewired to bypass skid steer safety interlock mechanisms. 2. Safe operating procedures not followed when exiting loader. 3. Cold and snowy weather conditions. Workers were using leaf blowers and brooms to sweep debris, which they then shoveled into plastic bags for removal. The flat roof measured 200 feet by 320 feet; it had 97 unprotected skylights. The supervisor, who was also the safety monitor, was standing about 20 feet from the victim when he saw him stepping backward toward a skylight. The supervisor yelled "skylight!" to warn him. The victim, who had been talking with a coworker, caught his foot on the skylight frame and stumbled backward. The coworker reached out to grab him, but was unable to. The victim fell onto the unprotected plastic dome skylight cover, which broke, and he fell through the skylight. He landed 30 feet below on a concrete floor. Investigators found: 1) Inappropriate fall protection was used. Safety monitor and warning line systems should not be used when unprotected skylights are present. Though warning lines must be used around unprotected sides and edges of a low pitch roof work area. 2) The employer did not protect skylights. 3) The building owner did not protect skylights. On September 5, 2019, the Kentucky FACE investigator conducted a site visit at which time photographs and measurements of the scene were taken. CONTRIBUTING FACTORS - Key contributing factors identified in this investigation include: 1. Failure to utilize fall protection when working at heights. 2. Working at heights with a medical condition which poses a direct safety threat. 3. Failure to recognize job hazards. 4. No safety training on fall protection. RECOMMENDATIONS - Kentucky FACE investigator concluded that, to help prevent similar occurrences, employers should: 1. Train employees on and enforce the use of fall protection when working at heights above 6 feet. 2. Implement a job hazard analysis process. 3. Employers who learn of physical disabilities which pose a direct threat to an employee's safety should perform threat assessments to evaluate his/her ability to safely perform assigned job duties. The CA/FACE investigator determined that, in order to prevent similar future incidents, grocery markets should: 1. Establish and enforce a safety training program that includes the hazards of climbing ladders. If ladders are used in wet conditions, safety precautions should be taken. 2. Ensure that older workers who work on ladders are assessed by a healthcare provider for increased fall risk. This is the 17th annual Michigan Fatality Assessment and Control Evaluation (MIFACE) report on acute traumatic work-related deaths in Michigan. There were 153 work-related deaths in 2017, a decrease of 9 deaths compared to 2016. There were 151 separate incidents (two deaths occurred in each of two separate incidents). A narrative summary of each work-related fatality is in Appendix I. MIFACE educational material, including on-site Investigation Reports, Summaries of MIOSHA Investigations, and Hazard Alerts are located on the MIFACE webpage on the Michigan State University Division of Occupational & Environmental Medicine (MSU OEM) website. Key findings for 2017: The number of work-related deaths (153) and the fatal injury rate (3.3 deaths/100,000 workers) were down compared to 2016 (162 work-related fatalities, 3.5/100,000 workers). The overall rate of work-related deaths in Michigan is lower than the rate in the United States. This is also true for the manufacturing sector but not the agriculture, construction or public administration sectors. Construction (29 deaths, 19.0% of all fatalities) had the largest number of work-related deaths and the third highest risk of death (17.9 deaths/100,000 workers). Agriculture, Forestry, Fishing, and Hunting the second highest number of deaths (19, 12.4%) and the highest risk of death (22.1 deaths/100,000 workers). Arts, Entertainment, and Recreation had the second highest risk (20.8 deaths/100,000 workers). Motor vehicle crashes were the leading cause of a work-related death (28, 18.3%). Struck by incidents were the second leading causes of death (27, 17.6%) followed by falls (26, 17.0%) and homicides (25, 16.3%). Individuals who died were most likely to be men (90.8%) and Caucasian (79.1%). The average age was 47.6 years old and ranged from 17 to 90 years of age. Illegal drugs, alcohol or side effects of prescribed and over-the-counter medication were potential factors in 21.1% of the non-suicide and non-drug abuse deaths. By occupational group, Management had the largest number of work-related deaths (28) followed by Construction & Extraction (26) and then Transportation & Material Moving (23). Forty-three of Michigan's 83 (51.8%) counties had a work-related fatality. Wayne County had the highest number (36, 23.5%), followed by Oakland and Genesee Counties (10 each, 6.5%). Of the 153 work-related fatalities, 41 (26.8%) were MIOSHA program-related and were investigated by a MIOSHA compliance officer. The decedent applied adhesive to the back of a 2-foot by 4-foot expanded foam board and took the prepared board to the firm owner, who was working in a boom lift for installation. After handing the board to the owner, the decedent walked back to the prep area to prepare another board. The firm owner heard a "crushing" sound, and when he looked over towards the sound, he saw the decedent sitting on the skylight cover. The skylight cover then "gave out". The decedent attempted to grab the sides of the skylight to keep from falling through but was unsuccessful. He fell to the concrete floor 30-feet 8-inches below. CONTRIBUTING FACTORS - Key contributing factors identified in this investigation include: 1. No guarding for skylight. 2. Fall protection not utilized near unguarded skylight. 3. Lack of employer and employee experience working with, identifying, and training for skylight hazards. Occupational and Environmental Medicine AB - In Summer 2017, a farm owner in his 50s died when the John Deere 2320 tractor he was using to mow the edge of a hay field rented to another farmer overturned to the side, pinning him in water at the base of a 10-foot deep ditch. The tractor was equipped with front forks and a PTO-driven brush hog mower attached to the rear of the tractor. There was approximately a two-foot space between the hayfield and ditch edge. He was working alone. When the decedent's spouse returned home from work, he did not greet her at the house as was his usual practice. She checked the home and surrounding outbuildings and could not find him. His spouse called for help to find him. Family members began searching the property without success. As it was getting dark, they returned to a family member's home. One family member continued to look. This family member noticed something "green" in the ditch and found the decedent's overturned tractor with the decedent pinned under the tractor. Emergency response was summoned. The decedent was declared dead at the scene. When the tractor was raised from the base of the ditch, the tractor had a flat driver's side rear tire. Post-mortem toxicology results found the decedent to have a 0.204% blood alcohol concentration (BAC) (legal limit to drive a motor vehicle is below 0.08 % and there is increased penalty for BAC of 0.17 % or greater). CONTRIBUTING FACTORS: Rollover protection structure removed and not replaced with an equivalent means of protection. Field crop planted two- to three-feet from ditch wall edge. Alcohol consumption prior to operating the tractor. Possible terrain conditions at field/ditch edge (soft from heavy rain four days prior). RECOMMENDATIONS: MIFACE investigators concluded that, to help prevent similar occurrences, tractor operators should: Tractor owners/operators should ensure their tractors have a manufacturer approved, certified, tractor specific rollover protection structure (ROPS) and seatbelts installed. Conduct a field survey to identify hazards, including hidden obstructions and establishing crop-planting limits/boundaries to maintain farm machine access. Equipment operators should not consume alcohol before operating equipment. He arrived at the site mid-morning, mowed for a while, ate lunch with a property owner adjacent to the property being mowed, and then resumed the task. The incident occurred near a tree line while he was mowing the perimeter of the property. A tree had broken near the base of the trunk and had fallen from the tree line into the field in the mowing path. The tree had several branches that formed a "Y". The incident occurred while the decedent, seated in the tractor seat, attempted to use the loader as a bulldozer to move/push the tree to the side and out of the field. Per the police report, it appeared the decedent had made several attempts to move the tree AB - there were marks on the hydraulic arms of the loader as well as paint transfer to the tree consistent with the hydraulic arm dimensions. On the decedent's last attempt, it appeared the tree "rolled" up and over the bucket and a branch struck and pinned him in the tractor seat. At the time of the incident, the operator was using the front-end loader to consolidate a ferrous scrap pile. At 9:17 a.m., while the loader was backing away from the pile with the bucket in the raised position, some of the scrap slid down the pile. A piece of scrap became wedged between the bucket lift cylinder and the right front wheel (from the driver's point of view). The operator exited the loader without lowering the bucket. The manufacturer specifes that a lift arm lock must be installed to prevent the bucket from falling before a worker enters the "danger zone" under a raised bucket to troubleshoot or do maintenance work. The lift arm lock was not installed. The operator positioned himself underneath the raised bucket between the two front wheels, and he wiggled the jammed piece several times, trying to free it. The movement of the jammed metal pulled the pressurized hydraulic cylinder hose from its ftting. The bucket, which weighed over 2,000 pounds, fell to the ground immediately due to a sudden and complete loss of hydraulic pressure, crushing the operator. The yard staf called 911, and fre department paramedics arrived within minutes. A grapple (a material handler) was used to lift the bucket and free the operator, who was pronounced dead at the scene. CONTRIBUTING FACTORS: Key contributing factors identifed in this investigation include: 1. Procedures for controlling hazardous energies, i.e. lockout/tagout or "LOTO were not established for front-end loader operations. 2. Operators were not trained on installation of the lift arm lock. 3. The loader's front wheel fenders were removed without consulting the manufacturer. 4. Yard management was not available at the time of the incident. 5. No training on hazard identifcation was provided. 6. No job hazard analysis was conducted. RECOMMENDATIONS: NY FACE investigators concluded that, to help prevent similar occurrences, employers should: 1. Develop and implement a LOTO program for front-end loader operation to control hazardous energies and prevent crushing injuries. 2. Ensure that all equipment operators receive proper and adequate training on LOTO procedures. 3. Ensure that all equipment operates without missing parts, and that all equipment modifcations are approved by manufacturers. 4. Ensure that hydraulic hoses and threaded couplings on front-end loaders are in safe and working condition. 5. Develop and implement an equipment inspection program to ensure that front-end loaders are inspected daily and needed repairs and maintenance are done timely. 6. Conduct a Job Hazard Analysis (JHA) to identify high risk jobs and to determine appropriate employee training on recognized hazards and safe work procedures. The previous day, they had installed and blocked three trusses. The crew arrived onsite at 8:00 a.m. They set up their equipment to begin installing trusses. When the site foreman called a break a half hour later, the framer did not join them. The two workers went to look for the framer and found him lying unconscious on the ground below where they were going to install trusses. He died of spinal injuries 15 days later. State investigators found that the framer fell 20 feet from the house's six-inch-wide top plate. He was not using fall protection. Investigators also found that the employer did not have a written fall protection work plan; workers did not understand fall protection requirements; the accident prevention program did not address job-specific safety; a safety walk-around was not done; and the framer did not receive a safety orientation.
