Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

All three incidents took place in Massachusetts warehouses. Each of the three companies had been in business over 10 years with one company in business for more than 20 years. The victims' experience with forklift trucks ranged from 4 - 20 years. Two of the three companies had written comprehensive safety programs. Investigation #1. The victim was a 22-year-old male carpet installer who also worked in the warehouse section of the carpet company. The victim worked for the company approximately four years. Driving a three-wheeled forklift truck in the parking lot, he took a sharp turn to the left while moving at 9 mph, the maximum speed. The vehicle tipped to the right and the operator fell out of the vehicle; it landed on top of him and crushed him. He was not wearing the seat belt. Investigation #2. The victim was a 49-year-old male inventory control clerk for a food storage warehouse, who worked for the company approximately 20 years. He was using a high lift order picker truck, which is a style of forklift truck that raises the operator's work area along with the truck's forks. He was using the vehicle in the raised position to apply barcode labels to upper shelves in the warehouse freezer section. To reach the next barcode location, he stepped from the operator's area onto a box of product on a shelf. The box shifted and the victim fell 12 feet to the ground. He was wearing a body harness and a lanyard but did not have the lanyard attached to the anchor point above the operator's area on the order picker truck. Investigation #3. The victim was a 40-year-old male forklift truck operator for a food production and distribution company, who worked for the company approximately seven years. The day of the incident he was performing the typical task of moving product to a different row of the warehouse using a forklift truck. During this task, the load of product became caught in the narrow aisles and the victim lowered the load back to the ground. He tilted the mast forward and stood between the mast and the cage looking over the load to see the obstacle that was preventing him from placing the product on the shelf. His foot caught the lever, which controlled the mast causing it to tilt back toward the cage. He tried to duck out of the way, but was unsuccessful; his head was crushed between the mast and cage. To prevent future forklift-related injuries employers should: Enforce operator restraint use. This includes seatbelts for forklifts and body harnesses and lanyards for high lift order pickers. Eliminate hazardous conditions such as narrow aisles, objects on the floor, and incorrectly positioned loads. Equip forklifts with backup alarms and flashing lights to warn pedestrians. Ensure a properly designed forklift personnel platform is used when lifting workers. Develop and enforce a forklift safety program that is annually reviewed and updated which includes, but is not limited to, worker training in forklift use and hazards. His co-worker was at the next manhole some distance away, and not in visible sight due to a ravine and landscape between the two positions. His co-worker stated that as the sewer cleaning was in process, he heard the nozzle approaching his end of the sewer line, indicating there were no obstructions in the line, and radioed the victim. The victim started reeling the hose back to the truck. The co-worker heard the truck idle down and knew that the hose was back at the start of the sewer line. The victim radioed the co-worker to confirm that he was shutting the unit down. The co-worker started fastening down the manhole cover following normal procedures, when he heard the truck idle back up to operating speed. He then heard boys screaming and knew something had happened. It took him some time to get through the ravine and back to the truck. Upon arriving at the truck, he found the victim lying by the manhole with massive head injuries. The nozzle had somehow come back up through the manhole and struck the victim in the head. Bystanders reported that they heard something explode from the back of the truck. They saw the hose spraying water into the air and the victim lying on the street, and immediately called 911. The city police and fire department responded, and the victim was unconscious when they arrived. Life Flight helicopter was notified and transported the victim to a local hospital. The victim remained unconscious and died two days later. Recommendations based on our investigation are as follows: 1. Employees must follow manufacturer's instructions when using equipment. 2. Safety issues should be re-emphasized periodically during staff meetings. Light rain was falling at the time of the incident, and the trailer was wet. The trailer was equipped with two metal and wood ramps that led from the ground to a wooden "beavertail" platform. After one unsuccessful attempt to drive the bulldozer onto the platform, the victim drove up the ramps and partway onto the platform. The tracks of the machine began to slide sideways towards the ditch, and off the trailer. He tried to jump out of the cab, but was pinned beneath the cab structure when it landed on top of him. The victim's wife had accompanied him to the site on the weekend morning when the incident occurred, and witnessed the incident from the passenger seat of the truck cab. She flagged down a passing car to call for help. EMS responders arrived, and elevated the bulldozer with air bags and blocks to remove the victim's body. The victim was pronounced dead at the scene by the medical examiner. The FACE investigator concluded that, to prevent similar occurrences, employers and bulldozer operators should: 1. Always use an operator restraint system while operating equipment equipped with rollover protective structures (ROPS). 2. Evaluate the terrain prior to loading track equipment onto a trailer, and plan safe strategies for addressing the hazards. 3. When the picker was nearly full of cotton, the picker operator stopped and told the victim to walk over to the cotton packing machine and get ready to pack the load while he finished picking. Approximately 20 minutes later, the picker operator drove the fully loaded 240-horsepower picker over to the packing machine. When he arrived at the cotton packing machine he noticed that the tractor that used to power it was not running and its lights were not illuminated. He did not see the victim but, thinking the victim had walked to his truck to get some tobacco, drove the picker along the side of the cotton packing machine and dumped the load into it. After he dumped the load, he and the farm owner, who had returned to the packing area after cutting stalks, became concerned when the victim did not return to the cotton packing machine. They began to search for him and when they could not locate him, decided to use the tractor to pull the cotton packing machine forward in order to expose the stack of cotton inside. They found the victim buried in loose cotton within the cotton stack and pulled him out. They found that he had no pulse and was not conscious or breathing. At approximately 10:30 p.m., the farm owner ran to a work truck parked nearby and used a mobile phone to call 911. The co-worker initiated cardiopulmonary resuscitation (CPR). Law-enforcement officers arrived within approximately 10 minutes and took over CPR efforts. EMS arrived within approximately 20 minutes of the 911 call and transported the victim to a local hospital where he was pronounced dead, approximately 2 ours after the incident. NIOSH investigators concluded that, to help prevent similar occurrences, employers should: Develop, implement and enforce a policy that prohibits workers from entering the box of cotton packing machines and that requires a visual inspection or the box and the use of an agreed-upon signaling system prior to dumping cotton into the box AB - and develop, implement, and enforce a comprehensive safety program for all workers which includes but is not limited to, training in hazard identification, avoidances, and abatement. Emergency personnel were called and coworkers performed CPR while waiting for them to arrive. Emergency personnel continued resuscitation efforts upon their arrival, but they were unsuccessful. The Nebraska Department of Labor Investigator concluded that to prevent future similar occurrences employers should: 1. identify grain storage bins as confined spaces and workers should follow confined space entry procedures when entering bins. 2. ensure personnel are wearing a safety harness and a lifeline attached to a fixed external anchor to keep them at a level at or above the level of stored grain products and that a coworker is stationed outside a bin whenever a worker enters a bin. 3. ensure all equipment which presents a danger to employees is locked-out, blocked-off, or otherwise prevented from operating. 4. ensure that visual and/or audible communications be maintained between a worker entering a bin and the observer stationed outside the bin. 5. provide and have in place equipment for rescue operations which is specifically suited for the task being conducted. 6. develop, implement and enforce a comprehensive safety program that includes, but is not limited to, training in all hazard identification, avoidance, and abatement. The building sat on a concrete slab. The victim was working from an aluminum extension ladder putting the finishing touches around the roof beams near the front of the building. The contractor and the ladder fell backwards to the concrete floor where he suffered massive head trauma. The local emergency medical rescue unit was summoned and responded to the incident scene. The victim was airlifted to a local hospital emergency room. He died two days later from the injuries sustained in the incident. To help prevent similar occurrences, the Washington Fatality Assessment & Control Evaluation (FACE) Investigative team concluded that persons working from ladders should follow these guidelines: Use the type and length of ladder that provides the safest application for the work being done. Set up non-self-supporting ladders (e.g. a straight ladder or extension ladder) using the "4 to 1" rule to obtain the most stable working position. Use safe working techniques to ensure that the ladder remains stable. Secure all non-self-supporting ladders at the top and stabilize them at the bottom. The company had been at the site for 2 weeks to repair the beacon light at the top of the tower, paint the tower, and install rest platforms on the tower. On the day of the incident, the owner was going to work on the beacon light at the top of the tower while the two other workers were going to continue painting the tower. A 3,000-foot length of 3/4 inch nylon rope and a 1,000-pound -capacity portable electric capstan hoist was used to assist them in riding the hoist line, were tied into the hoist line approximately 6 feet apart. The stepson was first on the line, then the 19-year-old, then the company owner. Using a length of woven rope, the male workers had attached one of the rest platforms to the end of the nylon rope 62 inches below the last loop. The company owner's wife was operating the capstan hoist using a foot pedal located on the ground. As the wife was operating the hoist and hoisting the men up the side of the tower, the hoist line began to lip around the capstan. The wife tried to hold the rope, but could not and the men fell to the ground. The wife went to the tower's service building and called the radio station that owned the tower, who in turn told her to call 911. The owner of the company that maintained the county equipment attached to the tower was in his truck and hear via radio that the county emergency medical service had been dispatched to the scene. He proceeded to the scene and was the first responder to arrive. The county fire rescue squad arrived next, then the county emergency medical service. Due to the extent of the victims' injuries, no first aid was initiated. After the county sheriff's personnel secured the scene, the victims were taken to the local hospital, where they were officially pronounced dead. NIOSH investigators concluded that to help prevent similar incidents, employers should: Ensure the hoisting equipment used to lift personnel is designed to prevent uncontrolled descent and is properly rated for the intended use AB - comply with OSHA Compliance Directive CPL 2 -1.29 "Interim Inspection Procedures During Communication Tower Construction Activities" during maintenance and construction activities on tower AB - ensure that worker inspect equipment on a daily basis to identify and damage or deficiencies AB - ensure that required personal protective equipment is available and properly used AB - know and comply with child labor laws which include prohibitions against work by youth less than 18 years of age in occupations which are declared by the Secretary of Labor to be particularly hazardous (Hazardous Orders). The decedent was moving bins of lemons during harvest. He was moving the bins from the orchard to an area where they were later retrieved by hydraulic trailer. The decedent decided to move four bins of lemons down a hill using the straddle fork. This was not an area in which the straddle fork was intended to be operated. As he was traveling down the hill, the straddle fork picked up speed and went off the road into a ravine. The straddle fork had no seat belt, and the decedent was thrown from the operator's seat. The employer had no operator's manual for the straddle fork. Training documentation was available, but it did not specifically address emergency actions necessary to regain braking control. The employer had a written Injury and Illness Prevention Program (IIPP). The CA/FACE investigator determined that, in order to prevent future occurrences, employers should as part of their Injury and Illness Program: 1. Ensure straddle forks are equipped with seatbelts for the use of the operator. 2. Ensure straddle forks are equipped with a rollover protective structure (ROPS). 3. Ensure employees are formally trained in all aspects of straddle fork use including emergency procedures. 4. Ensure employees operate machinery only in their designated areas. 5. He apparently had completed this task, and was standing on ice-covered soil near the rotating driveline. Then, he either slipped and fell onto the driveline, or his clothing was caught and pulled by protruding parts of the rotating shaft. He was spun around the driveshaft, and portions of his clothing were entangled on the driveshaft and torn from his body. His wife approached the site of the incident when her husband had not returned to the farmhouse as expected, and found him entangled on the driveline. The tractor engine was not running. She called to the victim's brother who was working in the barn, and he freed the victim by cutting the tightly tangled clothes. The brother summoned EMS, while the victim's wife began CPR. EMS responded within several minutes. The coroner's office was contacted, and pronounced the victim dead at the scene. The FACE investigator concluded that, to prevent similar occurrences, farm machine/equipment operators should: 1. Observe and follow all applicable safety precautions when operating machinery driven by tractor power take-off equipment, including disengaging the PTO and stopping the tractor engine before approaching the machinery to make repairs, adjustments or perform maintenance. 2. Identify machinery/equipment components that are PTO driven, and ensure that appropriate guards, recommended by the manufacturer or dealer, are installed. 3. Avoid wearing clothing that is loose-fitting, or has loose ends that could be caught by moving machine parts and lead to entanglement. The pressure vessel he was on was lying lengthwise cradled in devices called positioners. This pressure vessel suddenly tipped up. He was thrown off and crushed between the tipping pressure vessel and an adjacent pressure vessel. The pressure vessel that tipped had one of the two heads (a type of end cap) welded into place. It was heavy on that end and the positioner was placed so that any additional weight on that end would easily cause it to tip. The decedent was not safeguarded by fall protection. The CA/FACE investigator determined that, in order to prevent future occurrences, employers should as part of their Injury and Illness Prevention Program (IIPP): 1. ensure employees place the positioners so that pressure vessels will not tip under minor, additional loads. 2. ensure employees use fall protection or an alternate means of access to work when working at heights. 3. develop a method for welding on shorter pressure vessels that reduces the possibility of tipping. The worker was asphyxiated under the large piles of materials. FACE investigators concluded that in order to prevent similar incidents, the following safety guidelines should be followed: 1. Emergency shut off devices should be functioning and easily accessible to workers on the conveyor belt and to bystanders. 2. The production process should be modified to decrease the amount of loose material on and around the conveyors. 3. The victim was using a 26 horsepower, three cylinder diesel, four wheel drive tractor which was owned and usually operated by the property's owner. The tractor was equipped with a front end loader and a rear mounted brush hog. The operator (victim) was inexperienced in tractor operation and set-up. He was instructed by the owner to perform other assigned tasks and wait for his presence and instruction before moving the tractor out of the storage shed, setting it up, and mowing. Usual set-up included installing the tractor's rollover protective structures (ROPS) which had to be taken off in order to store the piece of equipment. The field had been graded two years prior to the incident and had never been mowed. The grading had left a fairly level area adjacent to the access road. Above this level area was a 12 foot wide ridge which ran along the woods line and created a 3 foot ledge leading down to the level area. Just prior to the fatal incident, the victim decided to access, set-up and operate the tractor without the owner being present and before receiving instruction. He began mowing the field at the highest point which was along the woods line upon the ridge. According to the owner, this ridge was not included in his mowing plans, as it was to be cut with a string trimmer. The victim was operating the tractor with its left side parallel to and within 2 feet of the ledge. He had completed 30 feet of mowing when the left front wheel and then the left rear wheel dropped off the edge causing the tractor to rollover pinning him to the ground. The tractor did not have a ROPS due to incomplete set-up. Approximately one hour later, the tractor's owner drove up to the mowing site and spotted the upside down piece of equipment. The victim was lying face up with the upside down tractor across his chest. The owner got back into his car and drove to a nearby towing business and called 911. Two of the towing business employees checked for vital signs. The EMS arrived within minutes. The tractor was removed from the victim and he was pronounced dead. The coroner estimated that the victim had died shortly after tractor impact. The WV FACE investigator concluded that to reduce the likelihood of similar occurrences, the following guidelines should be followed by tractor owners: 1. Ensure that unauthorized tractor operation is prevented by having exclusive control of the ignition key. Additionally, employees should: 2. Follow the employer's instructions concerning the scheduling and implementation of assigned tasks and pre-task training. 3. Ensure that all tractors equipped with removable or folding (ROPS) and a seat belt only be operated with the system fully installed. The tractor was equipped with a rollover protection structure (ROPS) and seatbelt, but apparently the victim was not using the seatbelt at the time of the incident. The farm owner discovered the injured victim in the cab of the overturned tractor when he went to the field to deliver sandwiches for an afternoon break. The farm owner went to a nearby house to call for emergency services, which arrived within five minutes. The victim was pronounced dead at the scene by the medical examiner. To prevent future fatalities of this type, the FACE investigator recommends farm tractor owners and operators should: 1. Always use an operator restraint system while operating a tractor equipped with rollover protective structures. 2. Evaluate the terrain prior to beginning an operation with a tractor, and plan safe strategies for addressing the hazards. In addition, farmers and other employers with workers assigned to work in isolated situations should: 3. Provide personal communication devices to workers assigned to remote worksites. The victim's job at this site was to compact the dirt ramp in preparation for hard surfacing. Two days prior to the incident, the company had delivered to the site a new compactor equipped with a rollover protective structure (ROPS), but no seatbelts. The victim was operating the compactor, with the enclosed cab door open, back and forth over a built-up roadbed when she backed up near the edge and the earth under the rear tires gave way. When the compactor tipped over the embankment, the unrestrained victim was partially thrown from the open door of the cab and pinned under the ROPS. Emergency rescue personnel were immediately called and arrived at the site within 15 minutes. The victim was pronounced dead at the site. In order to pull out the rig, the drivers attempted to attach a cable on the front of the transport vehicle to a hook on the back of the rig. The co-worker positioned the transport vehicle behind the rig AB - the vehicles were aligned with the left front of the transport vehicle slightly to the outside of the left rear corner of the rig. Due to a slight grade in the road and slackness of the tracks, the transport vehicle tended to slide back when it was stopped. The victim stood between the vehicles to fasten the cable onto the hook. Two attempts were made to attach the cable AB - each time the victim jumped from between the vehicles as the transport vehicle moved close to the rig. On the third attempt, the victim was caught between the vehicles. His co-worker moved the transport vehicle away from the rig and flagged a co-worker. They pulled the victim from between the vehicles and checked his injuries. The victim briefly regained consciousness, spoke to his two co-workers, and then collapsed again. Upon the arrival of more co-workers, CPR was initiated, and one person was sent to a truck with a two-way radio to notify Emergency Medical Services (EMS). The victim was placed on a sled behind a snowmachine and towed to the truck. Before EMS technicians arrived, the victim was transferred to the truck and driven to a nearby medical facility. He was pronounced dead on arrival. Based on the findings of the investigation, to prevent similar occurrences, employers should: 1. Ensure that workers are able to recognize and avoid hazardous situations and develop and implement a driver training program that includes, but is not limited to, snow travel and techniques for assisting stuck vehicles AB - 2. Ensure that a written and comprehensive safety program covers all work practices performed by employees AB - 3. Ensure that workers follow all prescribed work practices for transporting equipment and supplies and enforce a comprehensive safety program. He had been using the tractor with a front bucket loader to grade the driveway on his property. The victim was backing the tractor into the driveway when the rear wheel went into the ditch and the tractor overturned, pinning him under the tractor seat. The tractor was not equipped with a rollover protection structure (ROPS). His daughter and her husband witnessed the event, but it happened too quickly for their warnings to prevent the overturn. The daughter ran to the house and called 911, while her husband went to a neighbor's farm to get a tractor. The son-in-law returned to the scene within ten minutes, at about the same time as the emergency services vehicles. The EMS used air bags to raise the tractor enough to remove the victim from under the tractor. The ambulance transported him to the local hospital, then he was flown to a regional medical facility where he died two weeks later. To prevent future fatalities of this type, the FACE investigator recommends farm tractor owners and operators should: 1. Avoid using tractors that are not fully equipped with rollover protective structures (ROPS). 2. Fearing that the catering truck would leave, the victim went to the roof edge and yelled to the driver to wait. He stepped off the steel roof onto a fiberglass panel that broke under his weigh, causing him to fall 42 feet to the concrete floor below. NJDHSS FACE investigators concluded that, to prevent similar incidents in the future, these safety guidelines should be followed: 1. Employers must require fall protection when employees are constructing or working near leading edges. 2. Employers should develop, implement, and enforce a comprehensive employee safety program. 3. The laborer/rigger (the victim) and a coworker had completed their assignment of testing new foundation anchors, located below a set of three transmission lines energized to 69 kV. They had just finished moving and attaching rigging cables to two steel beams. The ground crew used a radio in a truck on a nearby access road to request the aerial lift. The transport staging area was 15-20 feet east of the north-south running transmission lines. A helicopter with an attached non-retractable 111-foot cable arrived and hovered. The wind direction was from the east, placing the helicopter upwind from the lines. As the helicopter maintained a heading nearly parallel to the lines, it descended to place the hook and 5-7 feet of the cable on the ground approximately 10 feet south of the load. The co-worker picked up the hook and brought it underneath the helicopter to the beams where the victim was holding the attached rigging cables. Once attached, the co-worker turned to move away from the load and the helicopter. He heard a crack after taking a few steps. He turned and saw the victim stagger away from the beams and collapse. Simultaneously, the helicopter crew heard a crack and saw a flash. The pilot moved the helicopter away from the lines, lifting the load and dropping it nearby. The co-worker then went to assist the victim. Unable to get any response, he ran to a truck to radio for help. Another co-worker standing near the truck went back to the victim and started CPR. After landing the helicopter a short distance away, the pilot exited the aircraft to check the victim's condition. The co-pilot took command of the helicopter and went to retrieve an emergency medical technician (EMT) employed by the company. Several minutes later, the helicopter returned with the EMT. The co-pilot then flew to a refueling area to bring a helicopter with a transport litter to the incident site. The victim was transported to a local medical facility where he was pronounced dead. Based on the findings of the investigation, to prevent similar occurrences, the employer should: 1. Ensure that all aerial lift staging areas are located outside a 20-foot safety zone around overhead power lines and are clearly marked AB - 2. Ensure that an extended safety zone is used when aerial lifts cannot be done downwind of energized overhead power lines AB - 3. Ensure that workers are provided with two-way radios for communicating with the helicopter pilot during aerial lifts AB - 4. Ensure that a spotter is present to provide directions to the helicopter pilot during aerial lift operations and that they (the spotter and pilot) are familiar with both verbal commands and hand signals AB - 5. Ensure that workers use insulated footwear and gloves while working near power lines AB - 6. Ensure first aid plans minimize response times during emergencies. In addition, helicopter operators and the helicopter aviation industry should: 1. Consider using non-conductive cables when airlifting loads near overhead power lines AB - 2. Observe and follow all applicable safety precautions when operating machinery driven by tractor power take-off equipment, including disengaging the PTO and stopping the tractor engine before approaching the machinery to make repairs, adjustments or perform maintenance. The victim and a ride operator had been assigned to work a ride known as the Tornado, which operated in a darkened enclosure. While the ride was operating a full speed, the victim attempted to board one of the passenger seats. As he jumped on the seat step, he lost his balance and fell. He was caught by the ride and dragged across the concrete floor. The ride operator observed the victim fall and immediately activated the emergency stop control and applied the emergency brake. She was able to stop the ride quickly, but not before the victim had been dragged about half-way around the ride's operating circle. Immediately after the ride came to a stop, the ride operator called the park's emergency number, reported the incident, turned on the lights, and went to the victim. Park emergency personnel arrived at the ride in less than a minute. Park patrons who had been on the ride were escorted out of the ride enclosure by coworkers who had been on the ride at the time of the incident. Park emergency personnel notified the local police, the emergency medical service (EMS), and the fire department. These agencies responded within 4 minutes. The victim was recovered by firefighters and treated by the EMS. He was removed from the enclosure and airlifted to a local emergency room. He died the next day as a result of his injuries. NIOSH investigators concluded that, in order to help prevent similar occurrences, amusement park owners and operators should: Ensure that their facilities are fully compatible with monitoring procedures for detecting and correcting unsafe work habits or conditions AB - develop safety policies and implement training programs that define direct links between unsafe behavior and the potential for injury AB - emphasize to workers the importance of reporting unsafe conditions or practices to management AB - and consider the use of engineering controls to reduce or eliminate worker exposure to moving machine parts.
