Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

A small flag indicating the well's position marked the location. The marker was near a fence separating the lot from an adjacent road. Above the marker were four power lines that ran parallel to the road. After extending the truck's front outrigger, the operator began raising the drill rig mast to position it over the marker. The victim was standing near the rear of the driver's side of the truck unloading equipment when the mast contacted the high voltage power line. Two workers employed by the contractor were standing several feet from the driver's side of the truck and heard a noise. They saw the victim and the operator frozen to and then collapse away from the truck. One worker went into a nearby building to call 911 as the other worker went to check both men. A worker from the building and a passerby arrived at the site as the first worker returned. Two teams were coordinated and CPR was started on the victim and operator. Emergency medical services arrived minutes later. The victim and the operator were transported to a nearby medical center where the victim was pronounced dead. The operator survived, but was unable to recall details of the incident. Based on the findings of the investigation, to prevent similar occurrences, employers should: 1. Ensure that a hazard assessment has been completed to identify all hazardous conditions that may affect operation of equipment AB - 2. Ensure that equipment is not operated where any part is within 20 feet of electric power lines unless the lines have been de-energized and either grounded or insulation barriers have been installed AB - and, 3. Ensure that a safety checklist is included in the written standard operating procedures (SOP) and is used prior to the start of any drill activity for each work site. In addition, all companies responsible for marking drill sites should: 1. Maintain a minimum 20-foot safety zone and be knowledgeable of all applicable OSHA requirements for work near utilities and electric power supplies AB - and, 2. Communicate to all parties involved in drilling activities the location of both above and below ground utility and electric power supplies near a drill marker that are within a distance equal to the height or extension of the drill equipment plus 20 feet. To help prevent or reduce the severity of injury in emergency situations, all drill rig owners and operators should ensure that all operator's controls are in good working condition and are clearly labeled. When the well operator opened the wellhead gate valve, the bull plug, needle valve and gauge assembly blew out of the connection and struck the victim in the head and neck inflicting fatal injuries. OKFACE investigators concluded that to prevent similar occurrences: 1. The responsible employer on multi-employer worksites should monitor critical safety activities performed by contractor employees, particularly personnel whose background and skills may be unknown or uncertain. 2. Until connections are adequately tested, the responsible employer on oil well locations should place signs or barricades in areas that lie in direct line with wellhead flanges and connections that may be subject to flying debris in the event of a system failure. 3. The responsible employer on multi-employer job sites should plan and coordinate job activities and communicate planned operations and their associated hazards with representatives from each organization working at the site. 4. Employers should develop and implement a formal occupational health and safety management system that is focused on continual improvement. 5. Employers should ensure that all employees are properly trained and skilled to perform each task that they are assigned, including training in hazard recognition and control. Before the tailgate was fully lowered, the operator alerted the victim, by saying "stand clear" and the victim responded accordingly. The tailgate was then lowered by the operator using controls on the driver's side of the truck, out of sight from the victim on the opposite side of the truck. When the operator walked to the passenger side for a final check, he found the victim lying on the ground with significant head injuries. His head had been caught between the frame of the truck and the tailgate as it was closing. The operator and a bulldozer operator called 911 for assistance. The County Sheriff, his deputy, and ambulance personnel arrived at the scene within minutes. The county medical examiner was notified, responded, and pronounced the man dead at the scene. The truck was found to be in normal operating order. A discarded fishing pole was found inside the garbage compartment of the truck, and it is possible that the victim was trying to retrieve this item immediately prior to getting caught. Recommendations based on our investigation are as follows: 1. Safety work practices should be re-emphasized with all employees, and in particular, with part-time employees. 2. Employers should ensure that a job safety analysis has been performed on all work-related tasks. According to the coroner's report, just prior to the incident, the victim had taken a break from breaking up asphalt and concrete and was standing to the side watching the excavator operator remove debris. As he watched, the operator swung the excavator around and the excavator bucket struck an air line attached to an air compressor, pulling it loose. The air line tangled around the bucket. The victim, a coworker, and a bystander, who was a friend of the excavator operator, entered the area between the excavator and the air compressor to see what had happened. The bucket suddenly became disentangled and started to swing to the right. The operator moved the controls and the bucket moved back to the left toward the air compressor, pinning the victim and the bystander between the bucket and the compressor. Workers notified the general contractor, who was working inside the restaurant. He called 911 at 2:55 p.m. According to the police report, police and emergency medical services (EMS) personnel arrived at 2:59 p.m. EMS personnel immediately attended to the bystander, who had a scalp laceration, and then attended to the victim. EMS transported both individuals to a local hospital. The victim was admitted and emergency surgery was performed. Internal bleeding caused by compression injuries sustained in the incident could not be stopped and he died the following morning. The bystander was treated in the emergency room for a scalp laceration and released. NIOSH investigators concluded that, to help prevent similar occurrences, employers should: 1) provide workers with training in the recognition and avoidance of unsafe conditions and the required safe work practices that apply to their work environments AB - 2) ensure that equipment operators have been trained to use the equipment they are assigned to operate at the job site. Additional notes/recommendations AB - 3) Rental companies should ensure that earth-moving equipment is functioning properly and that a manufacturer's operator's manual is supplied to customers at the time of rental. He climbed up a ladder on the rear of the tanker to look into the rear tank to ensure it was empty. The weight of the molasses in the front tank caused the tanker, which was not connected to a truck but was resting on its support leg, to tip forward. The rear of the tanker tipped up and crushed the victim between the tanker and an overhead steel beam. The victim was declared dead at the scene. The Nebraska Department of Labor Investigator concluded that to prevent future similar occurrences employers should: 1. ensure manufacturer's load limits are not exceeded when using equipment. 2. develop, implement and enforce a comprehensive safety program that includes, but is not limited to, training in all hazard identification, avoidance, and abatement. The youth had been operating the forklift to transport an empty pallet from a building supply center warehouse to an outside storage yard. As he turned from an asphalt paved alley onto the gravel-surfaced storage yard, the forklift tipped to the outside of the turn. The youth, who was not wearing the forklift's seat belt, either jumped or was thrown from the operator's seat. The forklift tipped over on its side and he was pinned to the ground by the overhead guard. A short time later, one of the building supply center's co-managers saw the forklift lying on its side in the supply yard and went to check on the victim. He found the victim under the machine and went to another store located across the alley for help. One of the workers in the store, who was an emergency medical technician (EMT), immediately went to the victim to render assistance while a second worker called 911. Emergency rescue personnel arrived within 8 minutes of notification. The victim was transported to a local hospital where he was pronounced dead. NIOSH investigators concluded that, to help prevent similar occurrences, employers should: 1) ensure that forklifts are operated only by authorized employees who have been specifically trained to use safe operating procedures and to recognize the hazards of improper operation AB - 2) ensure that operators of sit-down forklifts use the seatbelts provided as part of the operator protection system AB - 3) comply with child labor laws prohibiting youth less than 18 years of age from operating power-driven hoisting apparatus such as forklifts. Both the foreman and the operator were standing at the west side of a paving machine that was positioned in the median at the inside edge of the inside southbound lane. The 17-year-old driver of a light pickup truck traveling southbound in the inside lane at a high rate of speed allegedly looked down to adjust the radio and veered across the edgeline, striking and killing both workers. Oklahoma Fatality Assessment and Control Evaluation (OKFACE) investigators concluded that to prevent similar occurrences: 1. Employers should consider all applicable elements of a traffic control management program in accordance with the degree of risk to personnel in a work zone. 2. Employers should consider affecting temporary lane closures when high-risk operations must be performed, particularly during periods of high traffic volume. 3. Employers should consider deploying flaggers and/or traffic monitors when high-risk operations must be engaged, particularly for short periods of time when the use of barriers is not practical. 4. Employers should use a particularly high-visibility device at the beginning of road channelizing demarcation to increase the visibility of the impending hazard. 5. Employers should develop and implement a formal occupational health and safety management system that is focused on continual improvement. 6. Authorities with local jurisdiction for traffic control should consider deploying photo radar and red light camera enforcement programs in, or random assignment of traffic control officers to, construction projects with high traffic volume areas involving high-risk operations. Concrete was being hoisted from street level with a crawler-mounted mobile crane and landed under the direction of a roof-top spotter. As the crane operator hoisted a bucket load of concrete, swung it over the roof, and boomed out toward the empty forms, the crane lost stability, tipping toward the victim. When the crane operator realized what was occurring, he radioed a warning to the spotter who relayed the warning to roof-top workers. The victim had started to move when the uncontrolled concrete bucket swung toward him, striking his head and shoulder. Workers notified emergency personnel who responded within 6 minutes. The victim was pronounced dead at the scene. NIOSH investigators concluded that, to help prevent similar occurrences: 1) employers, crane owners, and operators should ensure that cranes are operated within their safe lifting capacities as recommended by the crane manufacturer's load chart AB - 2) crane owners should ensure that monitoring instruments used for guidance during hoisting operations are accurately calibrated and operating correctly AB - 3) employers should develop and implement safe work procedures to ensure that workers in or near the landing area of hoisted loads are notified when loads are in the air AB - 4) employers should develop and implement safe work procedures to ensure that workers and crane operators have a clear and complete understanding of the landing locations before loads are hoisted. Department of Public Health AB - On October 29, 1999, a 49-year-old male inventory control clerk (the victim) was fatally injured when he fell approximately 12 feet from a high lift order picker truck platform while applying barcode labels in a warehouse freezer section. The victim fell as he stepped from the raised order picker truck platform onto a stacked box of product in an attempt to reach the next higher shelf. The victim was wearing a body harness and lanyard but had not secured the lanyard to the order picker truck anchor point. The victim was transported to a local hospital then to a hospital in a neighboring state where he was removed from life support four days later and died. The MA FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Adopt a mandatory tie-off/no unhook policy for employees using order picker trucks. 2. Ensure that aisle ways are maintained free from obstructions that would interfere with lift truck access to storage shelves. 3. Strictly enforce the safety precautions outlined in comprehensive safety programs and constantly review and update the programs and training. In addition, high lift order picker truck manufacturers should: 4. Fatality Assessment and Control Evaluation (FACE) Program AB - A 24-year-old male baler operator (the victim) died after he fell into a baling machine and was crushed when the machine automatically cycled. The baling machine was used to compact and bale waste cardboard for recycling. Although the fall was unwitnessed, it is believed that the victim may have climbed up the outside of the conveyor to the top of the baler's loading chute to dislodge a cardboard jam. He may have lost his balance and fell 14 feet to the bottom of the baling chamber. Upon hitting the bottom, the victim's body broke the beam of a sensor light which sent a signal to the baler's computer that the hopper was full, triggering the machine to automatically cycle, crushing the victim. At approximately 9:30 a.m. the next day, the victim was discovered by the first-shift baler operator, after he had processed a few bales of cardboard. After noticing what looked like a piece of company uniform sticking out from one of the bales of cardboard, he looked closer and discovered what he thought was a body. The day shift operator then went to the production supervisor's office to report his findings. After calling maintenance, they returned to the baler and after observing what looked like a body, the production supervisor called the police and the Emergency Medical Services (EMS). After the police determined there was no foul play involved, they allowed EMS and representatives from the Medical Examiner's office to remove the body from the bale. NIOSH investigators concluded that, to help prevent similar incidents, employers should: 1) ensure that workers are protected from the unexpected movement of machine parts by implementing and enforcing the use of lockout/tagout procedures AB - 2) establish written operating procedures for machinery that include training in safe operating practices and a safe method for clearing jams AB - 3) provide a safe means of access to eliminate the need to climb onto the equipment in the event of jamming AB - 4) establish a systematic communication procedure by which employees are accounted for during their shift. In addition, manufacturers should evaluate the designs of baling and compaction equipment to eliminate or reduce the likelihood of jammed materials. Fatality Assessment and Control Evaluation (FACE) Program AB - 17-year-old male laborer (the victim) died and a coworker was injured after one of the unprotected walls (i.e., no shoring, shielding or adequate sloping) of the trench they were working in collapsed, striking and partially burying them with soil. The day before the incident the excavator operator had removed the trench shield used during the sewer installation project to facilitate the removal of broken sewer pipes. When work was resumed the next day, the trench shield was not replaced, and the victim and a coworker (pipe setter) went into the unprotected trench to replace two sections of pipe and to check the grade of the sewer line with a grade pole. While they were placing the grade pole inside the terminal end of the sewer line, a section of the trench wall caved in, striking and burying the victim to his mid-chest and his coworker to his knees. The foreman, who had been working above the trench at ground level, called 911 at 9:39 a.m. and requested emergency medical services (EMS). EMS, law enforcement, and the fire department personnel arrived within 5 minutes of the 911 call. The Fire Marshall assumed responsibility for rescue operations and site safety and, in his role as Incident Commander (IC), assigned Operations Officer duties to one of the fire fighters. The Operations Officer, realizing that the trench walls remained unstable and hazardous, directed coworkers, who had jumped into the trench to render assistance, to leave the trench until fire fighters could obtain the necessary protective systems to ensure safe rescue operations. Coworkers refused to leave the trench and continued digging to free the trapped workers. The Operations Officer asked law enforcement personnel to remove the foreman from the incident site when he demanded that EMS and fire fighters go into the unprotected trench. The foreman was removed from the incident site. While fire fighters were waiting for the arrival of protective systems, they set ground pads (sheets of plywood) along the side opposite the collapse to provide paramedics with a more stable point from which to lower an oxygen tank and oxygen administration equipment. Coworkers placed an oxygen mask on the victim. By this time he had become ashen in color and was semi-conscious, with a weak pulse and labored respirations. Both the victim and his injured coworker were freed by their coworkers, placed on backboards, and carried to the shallow east end of the trench. With assistance from fire fighters working above at ground level, coworkers lifted the victim and his injured coworker to fire fighters. They in turn carried the injured workers to an area where paramedics were standing by. Both workers were treated by paramedics and transported by ambulance to a local hospital where the injured coworker was admitted in serious condition and subsequently released. The victim underwent emergency surgery, experienced a cardiac arrest, and died at 2:03 p.m., approximately 5 hours following the incident. NIOSH investigators concluded that, to help prevent similar occurrences, employers should: 1) ensure that workers are protected at all times from potential cave-ins by an adequate protective system AB - 2) know and comply with child labor laws which include prohibitions against work by youths less than 18 years of age in occupations which have been declared by the Secretary of Labor to be particularly hazardous (Hazardous Orders) AB - 3) ensure that whenever Professional Employer Organizations (PEOs) serve as co-employers, they highlight the age of minors and note the need for compliance with child labor laws AB - 4) identify all workers on the worksite who are under 18 years old and make their presence known to all other employees on the worksite. Inform all employees of the work assignments that are appropriate for these youthful workers and the limits on what they can do AB - 5) ensure that a competent person conducts daily inspections of excavations, adjacent areas and protective systems and takes appropriate measures necessary to protect workers AB - 6) provide workers with training in the recognition and avoidance of unsafe conditions and the required safe work practices that apply to their work environments. Additionally, NIOSH recommends AB - 7) all persons on an incident site should follow the directions of qualified rescue personnel who have assumed responsibility for rescue operations and site safety. Prior to the incident, the victim (Flagger 1) was on the south side of the highway facing west with her flag, preparing to stop all eastbound traffic on the highway. Another flagger (Flagger 2) was on the north side of the highway, approximately 50 feet east of the victim, and was stopping westbound traffic. Flagger 2 signaled the westbound produce truck to stop and the truck had almost come to a complete stop when a westbound tractor trailer approached at approximately 55 miles per hour and struck the produce truck in the right rear. The produce truck was struck with such force that it was momentarily airborne and the driver could not control his vehicle AB - the produce truck was propelled across the eastbound lane directly into the path of the victim, who was still facing west with her back to the oncoming truck. Flagger 2, on the north side of the road, was able to jump clear of the impacting trucks. He was unable to see or warn the victim. Fire rescue personnel arrived within minutes, followed by the arrival of an aeromedical helicopter. The victim was pronounced dead at the scene. The driver of the tractor trailer was also injured and transported by helicopter to a trauma center. The driver of the produce truck was uninjured. Investigators concluded that, to help prevent similar occurrences, employers/highway construction contractors should: 1) consider the use of additional warning signs and traffic control devices to supplement the minimum signs recommended by the Manual on Uniform Traffic Control Devices (MUTCD) AB - 2) provide and require use of hand-held or other portable radio communications equipment by flaggers at all times. Additionally, NIOSH recommends that AB - 3) state highway authorities should consider the use of law enforcement officers in cruisers at each end of large highway construction work zones and the use of radar surveillance for traffic speed control AB - 4) state highway authorities reduce speed limits in construction work zones on high-traffic-density highways to a maximum of 45 mph. The victim was operating a Ford 4000 farm tractor with a front bucket attachment to knock down and remove trees, with a co-worker standing by for assistance. Another co-worker was cutting a tree nearby, while the fourth worker was moving a truck to avoid having it be struck by falling trees. The victim elevated the bucket till it was about ten feet high, then struck the trunk of a decayed tree with the bucket. The tree did not fall, so he struck it again with the bucket. This time, the top half of the tree cracked off and fell toward the victim sitting in the tractor seat. He leaned to the right to avoid the falling tree, but was knocked from the seat and fell in front of the right rear tractor tire. The tractor came to rest with the tire on his chest. The standby co-worker yelled for help, and the other two co-workers ran immediately to the scene. They pulled a large log in front of the tractor tire where the victim was pinned, then one co-worker drove the tractor forward onto the log while the other two pulled the victim out from behind the tire. One co-worker drove to a cabin to phone for help, while the others initiated CPR. EMS responders were at the scene in eleven minutes, and the victim was transported to a hospital. Advanced cardiac life support protocols were initiated in the ambulance and continued at the emergency room, where the victim expired. The FACE investigator concluded that, to prevent similar occurrences, farm tractor operators should: 1. Follow tractor and bucket loader manufacturers' recommendations when using a bucket. 2. Use only tractors that are equipped with falling object protective structures (FOPS) when working in a situation where objects might fall on the operator. Co-workers called 911 and attempted to administer CPR. Emergency medical services arrived within five minutes of the incident, but the injuries sustained by the laborer in the fall were sufficiently severe to cause death within approximately two hours of the incident. FACE investigators concluded that, to help prevent similar occurrences, employers should: 1. develop fall protection systems and work practices that provide protection from falling through holes (including skylight openings) for all employees who work at heights of 6 feet or more above lower levels and provide training for all workers in the use of such practices and systems AB - and 2. consider installing skylight fixtures that are sufficiently strong to support at least twice the weight of employees, equipment, and materials that may be imposed on the fixture at any one time. The laborer was positioned on a stepladder near the heat exchanger coil manifold to close an isolation valve on the pressure line connected to the manifold flange when the gasket failed, releasing a high pressure water jet in the direction of the worker. The water struck the assemblyman in the chest knocking him approximately 20 feet off the end of a loading dock onto the concrete drive below. Another employee nearby immediately contacted 911 by mobile phone and the victim was evacuated by helicopter to a local hospital where he died from injuries sustained in the incident. The exact circumstances of the fall are unclear since the activities of the supervisor immediately preceding the fall were not witnessed. The co-worker heard a sound, looked over, and saw the victim and the scaffold hit the floor. He immediately went to obtain assistance. A physician working nearby responded within two minutes. Emergency room staff transported the victim to the emergency room within ten minutes. The worker was hospitalized and died the following day from his injuries. A gust of wind caught the vinyl sign as it was unfolded and threw it back onto the crew. The crew members were wearing harnesses with lanyards, but only one crew member was tied off. He had tied off to the billboard structural members. The victim was not tied off and the vinyl sign knocked him off balance. He fell from the catwalk into the billboard metal structure and onto the ground below. He partially raised the box of his truck to dump out a portion of wet sand at a work location, then leaned under the box to manually trip the hydraulic linkage to bring the box down. The hydraulic linkage was located at the hydraulic pump, behind the cab between the main frames of the truck. This action was apparently a habit to save some time from walking around the truck to push the dump lever inside the truck cab. However, this time, because of the heavy load of sand, and because the box was only partially raised, the truck box came down immediately pinning the man between the box and the frame of the dump truck. The man suffered massive head injuries and was dead at the scene. The deceased truck driver had several years of experience with machinery and was the lead man for the small construction company. Recommendations based on our investigation are as follows: 1. Workers should use proper machine controls rather than actuating machine functions at hazardous locations. 2. Employers should train and supervise workers adequately to ensure that safe procedures are followed. 3. Manufacturers of dump trucks should consider options to discourage unsafe use of dump box hydraulics, such as placement or shielding of controls.
