Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

After the car in tow was unloaded, the truck's flatbed was raised to unload the second car. There were some problems unloading the second car, so the tow truck operator jerked the truck back and forth to release it. In doing so, he backed over the victim who was working behind the truck. He then ran over him a second time as he moved forward, with the truck coming to rest with the victim pinned under the rear wheel. Scrapyard employees tried to rescue the victim by lifting the truck with a grapple crane, but the grapple slipped, and the truck fell back on him. The victim was freed on the next attempt but died despite efforts by police and EMS to resuscitate him. NJ FACE investigators concluded that, to prevent similar incidents in the future, these safety guidelines should be followed: 1. Tow truck operators must use caution while operating and backing their vehicles. 2. Employers should become familiar with available resources on safety standards and safe work practices. He cut away a tree section, pushed down on it, and it fell onto the victim's head. NJ FACE staff concluded that, to prevent similar incidents, the following safety guidelines should be followed: 1. The employer should conduct a job briefing at the start of each job. 2. The employer should establish a written safety and health policy and written company operational policies that include ANSI Z133.1. 3. The victim was working outside of the tower at the 40-foot level where he was bolting together two tower sections. The tower design allowed the workers to reach through access ports from the outside, place the bolt through the section flange and tighten the nut to specifications. There were several bolts that could not be reached from the outside of the tower. The co-worker climbed down to the ground and climbed up the inside of the tower. The co-worker then could slide the bolt through the flange and the victim could tighten the nut to specifications. At the time of the incident the victim was wearing a saddle style-positioning belt. Attached to his positioning D-rings was a spreader bar, with a large hook. Apparently, the victim unhooked from the tower and was moving to the next bolt location when he fell. The victim was taken by ambulance to a local hospital and then life-flighted to a trauma center where he died the morning of November 27, 1999. The MO FACE investigator concluded that in order to prevent similar occurrences all employers should: 1. provide employees with a 100% fall protection system compatible with the work being performed, instruct employees in the proper use of the system and equipment, and ensure their use AB - 2. employers should ensure that proper personal protective equipment is available and instruct workers in the proper use and limitations of the system, and ensure its use AB - 3. develop, implement, and enforce a comprehensive written safety program which includes a commitment to 100% tie off and written procedures to implement 100% fall protection. The victim either jumped or was thrown from the forklift operator's seat. When the forklift overturned, the falling object protective structure (FOPS) of the forklift landed on his chest. A call was placed for emergency assistance as the owner, manager, and a witness assisted the victim. Emergency assistance arrived within minutes and transported the victim to a hospital where he died early the next morning from his injuries. The Massachusetts FACE program concluded that to prevent similar occurrences in the future, employers should: 1. Comply with federal and state child labor laws which prohibit youth less than 18 years old from operating forklifts. 2. Train all forklift operators in safe operating procedures. 3. Provide adequate supervision for young workers, new employees and any inexperienced workers. 4. Develop, implement and enforce a written comprehensive health and safety program In addition, agencies that have contact with small businesses, such as government and insurance agencies should: 5. Risk factors resulting from these incidents have been identified through surveillance findings and results of investigations conducted by CDC's National Institute for Occupational Safety and Health (NIOSH) Fatality Assessment and Control Evaluation (FACE) program and the Bureau of Labor Statistics Census of Fatal Occupational Injuries (CFOI), a nationwide multisource reporting system for occupational deaths. This report describes the results of two baler and compactor-related investigations conducted during 1992-2000, summarizes surveillance data from 1992 through 1998, which indicated that some employers and workers may have been unaware of the hazards of operating or working near compacting and baling equipment, and suggests safety recommendations for preventing future incidents. On the day of the incident he was working as a member of a road resurfacing crew. His employer had been contracted to apply a surface seal coat to 16 miles of paved road. The seal coat consisted of liquid asphalt sprayed on the road and crushed rock spread over the liquid asphalt. Equipment involved in this incident included a rock spreader and a pneumatic roller. The spreader had a computer rate control (CRC) unit that controlled the rate of application of crushed rock. The roller was a nine-wheel pneumatic roller equipped with hydraulic brakes that were activated via a foot pedal. The unit was also equipped with a hand operated emergency brake. The workers began by applying a layer of seal coat to a "test area" of the road. A liquid asphalt tankers was used to spray liquid asphalt to the test area. A dump truck filled with chipped rock was backed up to the rock spreader. The spreader operator drove the spreader forward pushing the gravel truck across the test area. After crossing the test area, the workers realized that the layer of crushed rock coming from the spreader was not uniform. The spreader operator disengaged the unit and backed it off the test area. After the operator parked the spreader, he dismounted and walked to the front of the unit to a computer control panel. While he attempted to determine what was wrong with the CRC unit, an operator of a pneumatic roller packed the rocks that had been spread over the test area. The operator drove his roller onto the test area with the unit facing away from the parked spreader. Due to the incline of the road the roller traveled up a slight incline and was backed down the incline as the operator packed the seal coat in the test area. As the operator approached the end of the test area near the parked spreader, the roller's manual transmission slipped out of gear and the roller began to roll toward the spreader. The operator stepped on the hydraulic brake foot pedal but was unable to stop the roller. He attempted to pull the emergency brake while he also yelled to workers to clear the area. Other workers ran from the area however the victim apparently did not hear the call to clear the area. The victim was crushed when the roller struck the front of the spreader. The roller operator immediately shifted it into gear and drove the roller forward. The victim fell to the ground and other workers immediately placed a call to emergency personnel. Emergency personnel arrived at the scene shortly after being notified. They transported the victim to a local hospital where he died shortly after he arrived. MN FACE investigators concluded that, in order to reduce the likelihood of similar occurrences, the following guidelines should be followed: 1. employers should designate an on-site worker who is responsible for warning pedestrian workers in the event of an emergency involving nearby mobile equipment AB - 2. employers should ensure that equipment is always maintained in the proper working condition AB - and 3. employers should design, develop, and implement a comprehensive safety program. Although the incident was unwitnessed, police concluded that the victim either lost his balance while feeding material into the chipper or was leaning across the feed table pushing trimmings into the feed chute when his gloves were caught by the feed rollers, and he was pulled through the chipper blades. The CA/FACE investigator determined that, in order to prevent future occurrences, employers, as part of their Injury and Illness Prevention Program (IIPP) should: 1. Ensure that all employees use a long limb, branch, or a push stick to feed small trimmings into the brush chippers. 2. Ensure that all employees stand to the side of the feed table when feeding trimmings into the brush chipper. 3. The victim was part of a five-person crew that was contracted to pave a private residential driveway which was located on a wooded lot on hilly terrain. One witness to the incident reported that he saw the victim position the compactor close to the edge of the driveway. It appeared that the smaller front roller of the compactor became caught on the pavement edge. With part of the front roller hanging over the pavement edge, the witness reported that the victim started to reverse the compactor when the larger back roller slipped sideways over the edge of the pavement. When the compactor started to slip, the victim apparently tried to jump clear, but his leg become entangled and he went over the embankment with the compactor. Coworkers immediately ran over the hill to aid the victim while the property owner called 911. While waiting for emergency medical services (EMS) to arrive at the site, coworkers removed the victim from underneath the compactor and attempted to administer first aid. EMS and fire department crews arrived at the site and transported the victim to the hospital, where he was pronounced dead. NIOSH investigators concluded that, to help prevent similar incidents, employers should: 1) ensure that all equipment used in a location where there is a danger of overturn be equipped with a Rollover Protective Structure (ROPS) and that operators are trained to wear seat belts when operating machinery equipped with ROPS. AN INSPECTOR KILLED BY A BACKING ASPHALT DUMP TRUCK: A 36yearold construction inspector, Michael C.*(* Not the victim's real name), died when an asphalt dump truck backed over him. The truck was bringing asphalt to a road being paved. Michael was wearing an orange reflective vest and hardhat. The truck traveled about 770 feet in reverse before backing over Michael, who had his back to the vehicle. The truck driver said he was traveling about 5 to 10 miles per hour when the incident occurred. The driver never saw Michael. A CONSTRUCTION FLAG PERSON DIES AFTER BEING BACKED OVER BY A TRUCK: A 26yearold construction flag person, Jaime H.*(* Not the victim's real name), died when a dump truck backed over him. The dump truck was bringing dirt to the center of a sixlane street and had just entered the lane closest to the center. This lane was separated from the rest of the lanes with tubular markers. The dump truck was backing into position to dump a load of dirt onto the center. Jaime had just let the dump truck into the separated lane. He was wearing an orange reflective vest and hardhat. The dump truck's backup alarm was working properly. The dump truck traveled only a few feet in reverse before backing over Jaime. The driver looked into both of his tractor's side rear view mirrors and did not see Jaime, who had his back to the vehicle. RECOMMENDATIONS: Require drivers to get out and check behind their trucks before backing. Use a second person as a spotter when backing heavy equipment. Additional safety devices can help warn workers of a backing heavy vehicle. Other devices can warn drivers when someone is behind them. Occupational Injury Prevention Program AB - A 58-year-old truck driver (the victim) was fatally injured as a result of falling into a hopper at an ore storage facility. The victim was driving a tractor with two trailers loaded with ore concentrate. After arriving at the storage facility and while waiting his turn to unload, he had released the tiedown straps holding the tarps onto the trailers. Strong winds blew the tarp and its rollbar off the top the first trailer, leaving them hanging over the side. The victim was unable to lift and secure the tarp and rollbar over his load. After he was signaled to enter the building housing the dumping platform, he drove the tractor through the entrance AB - the trailers remained outside the building. He then climbed onto the catwalk at the front of the first trailer. The facility operator tried to assist him but was unable to lift the rollbar high enough for the victim to grasp. She handed one of the front tiedown straps to him, and he was able to pull up the front end of the bar. Once the end of the bar was placed in a holding bracket, he exited the catwalk. When asked if he needed further assistance, the victim stated that he did not and would roll the tarp after the trailers were inside the building. The victim went back to the cab and pulled the trailers forward so that the operator could close the entry door. The operator then secured and rigged the trailers for dumping and did not see the victim's activities. After the trailers were secured and rigged, the operator looked for the victim. Not locating him inside the building, the operator went to the control room and radioed a waiting driver who stated that he had not seen the victim exit the building. The operator searched the area again and went back to the control room. She observed the victim's feet in a video monitor that was used to view the ore transferring through a shoot to the last conveyor AB - his body was trapped in the shoot. The operator radioed for help and stopped the conveyor. Emergency medical personnel were requested. The victim was removed from the conveyor system and transport to a clinic where he was pronounced dead. Based on the findings of the investigation, to prevent similar occurrences, employers should: 1. Require tarps to remain secured to trailers until the trailer is ready to be unloaded AB - 2. Ensure that workers are able to recognize and avoid hazardous situations, such as climbing on loads or anywhere outside a trailer's catwalk AB - 3. With the assistance of workers involved in the activity, develop procedures to remedy problems AB - 4. Consider adding communication skills in safety classes during new hire orientation. The decedent was using the machine to center punch holes into metal plates when the incident occurred. The machine guarding was in place but was not properly adjusted. According to the owner of the company who witnessed the incident, the machine made a strange noise during the punch cycle just prior to the incident. The owner said he yelled at the decedent to stop when he heard the noise. The decedent looked at owner, bent over to look at the machine, then stepped on the machine pedal. This caused the punch to punch through a steel plate then engage the edge of the hole in the misaligned die. The hydraulic pressure on the punch finally forced the punch into the hole in the die. A half-moon shaped piece from the edge of the punch sheared off and shot out from underneath the steel plate striking the decedent in the abdomen. The machine manufacturer recommends documented scheduled maintenance on the machine, which the employer could not verify. The employer did not have a written Injury and Illness Prevention Program, or a documented training program for machine operators. The CA/FACE investigator determined that, in order to prevent future occurrences, employers, as part of their Injury and Illness Prevention Program (IIPP) should: 1. Provide a program of documented scheduled maintenance on the machine in accordance with the manufacturer's recommendations. 2. Provide a program of documented training and safe work practices. At each paper box stop, the victim would step out of the minivan to place an advertising newspaper in the paper boxes mounted on posts on the edge of the road. He usually did not use a seat restraint or lock the door after returning to the vehicle and riding to the next stop. On the afternoon of the incident, the victim had placed newspapers in the paper boxes of an unknown number of residences in a suburban community. He was in the vehicle and had the next paper in hand after placing a paper in a box, when the driver told him to close the door. The vehicle began to move forward, when the door swung open and the victim fell out. The rear wheel of the van ran over him before the driver could stop. Emergency services were summoned and were at the scene within four minutes. The victim was transported to a hospital, where he died the following day. The FACE investigator concluded that, to prevent similar occurrences, employers should: 1. Develop and enforce safety policies that require workers who are in motor vehicles to keep all doors closed and locked whenever the vehicle is in motion. 2. Know and comply with child labor laws which exclude employment of youths in occupations which are deemed detrimental to their health or well-being by the Secretary of Labor or state legislatures. Additionally, agencies responsible for setting standards for highway safety should: 3. Develop and enforce requirements for all occupants of moving vehicles to use occupant restraint systems whenever the vehicle is in motion. After he arrived at the farm he entered the cab of a four-wheel drive tractor equipped with dual wheels on both the front axle and the rear axle. He turned the ignition key but the tractor's starter failed to operate. On several occasions prior to the incident the starter failed to operate due to improper functioning of a safety switch in the transmission's shift lever linkage. The victim used a two-way radio to contact one of his sons and told him the tractor would not start. The victim's son drove to the location of the tractor to help start it. They decided to activate the starter by contacting the shaft of screwdriver to the terminals of the starter. The victim and his son walked between the front and rear wheels on the left side of tractor. The victim's son stood near the side of the tractor while his father stood in front of the rear wheels and held a flashlight. The victim's son touched the shaft of the screwdriver to the starter terminals and the engine started. The tractor moved forward and knocked both men to the ground. The victim's son rolled to the area directly beneath the tractor. The victim fell to the ground in front of the left rear wheels and was run over by them. The victim's son stood up after the tractor moved past him and he entered the cab of the tractor and stopped it. He used a two-way radio to notify family members of the incident. They placed a call to emergency personnel who arrived at the scene shortly after being notified of the incident. The victim was transported to a local hospital where he was pronounced dead approximately one hour later. MN FACE investigators concluded that to reduce the likelihood of similar occurrences, the following guidelines should be followed: 1. machinery should only be started when a qualified operator is seated in the operator's seat AB - and 2. machine components and safety equipment should be maintained in proper working order. Bureau of Occupational Health AB - A 17-year-old male newspaper delivery assistant (the victim) was a passenger in an automobile that burst into fire after it was struck from behind by another vehicle. The victim was seated in the front passenger seat of the auto driven by an newspaper carrier (co-worker) who employed the youth as a casual worker to assist with the driver's daily newspaper delivery route. The incident occurred shortly after sunrise while the co-worker was pulling the vehicle off a two-lane county road. She was traveling at about 5 miles per hour, and steered the auto partially onto the shoulder of the southbound lane. Neither the victim nor the co-worker were wearing seatbelts at the time of the collision. An automobile was traveling at about 55 miles per hour in the southbound lane and struck the back of the newspaper delivery workers' auto. The impact pushed the workers' auto into the ditch and caused it to overturn onto the driver's side and burst into flames. The victim was tossed into the back seat by the collision, and the co-worker was ejected from the passenger side window. A resident of a house situated near the collision site ran outside when she heard the crash. She saw the co-worker lying in the road in front of the burning car, and pulled her away from the flames. A driver in another vehicle saw the burning vehicles from his rear-view mirror and summoned emergency services with his cell phone. The sheriff arrived at the scene within four minutes, followed by the fire department and EMS. After the flames were extinguished, the victim was extricated from the vehicle. He was pronounced dead at the scene. The drivers of both vehicles survived the collision, and were taken to the hospital. The FACE investigator concluded that, to prevent similar occurrences, employers should: 1. Equip vehicles that must travel slowly and/or stop frequently with high-visibility flashing lights that can be seen from all directions. Confirm if such equipment is allowed under state law in the jurisdiction where the employer is operating. 2. Ensure all travelers are trained in the proper use of vehicle safety features and drivers are trained in safe driving practices. Additionally, agencies responsible for setting standards for highway safety should: 3. Develop and enforce requirements for all occupants of moving vehicles to use occupant restraint systems whenever the vehicle is in motion. On the night of the incident, he was the only person assigned to sleep in the building that housed the health services. The wood frame building had an furnace room which housed an LP gas fueled furnace and LP gas fueled water heater. The water heater was used continuously throughout the summer, while the furnace was only used infrequently and for brief time periods. There were no records to indicate when the furnace and water heater had been inspected or repaired. Smoke detectors were located on two opposite ends of the building, which held sleeping quarters for four people. There were no carbon monoxide detectors in the building. The victim went to the building about 8:30 p.m. the eve of the incident, and turned on the furnace before he went to bed. He kept the windows and doors closed. When the victim didn't show up for breakfast the next morning, a supervisor went to the building and found him motionless in bed. Emergency services were summoned, and EMS arrived at the scene within 25 minutes. The first responders found no signs of life in the victim, then exited the building until the sheriff arrived. The sheriff viewed the scene and determined there were no signs of foul play. The coroner was called to the scene, and pronounced the victim dead. The FACE investigator concluded that, to prevent similar occurrences, employers should: 1. Ensure that gas fueled appliances are installed and maintained to prevent production and buildup of carbon monoxide. 2. Ensure gas fueled appliances are inspected on a schedule recommended by the manufacturers by knowledgeable technicians with authority to make changes. 3. Install carbon monoxide detectors in buildings with gas fueled appliances. Additionally, emergency workers who respond to the scene of an unexplained death should: 4. Be trained in the hazards of an asphyxiating environment and methods to protect themselves during rescue and recovery efforts. Located in the feed bay was an overhead conveyor used to load finished product into trucks. Also located in the feed bay was an I-beam that was fastened horizontally to the side walls. The victim and a co-worker were employees of a cleaning and painting company. Their employer had rented a four-wheel drive manlift for cleaning and painting the facility. The operator's platform of the manlift was attached to the end of a telescopic boom and was surrounded by safety railings. An operator's control panel was fastened to the top of the safety railing. While facing the control panel, the operator was able to drive the manlift forward and backward and change the position of the work platform. The victim drove the manlift across the platform scale but he was not facing the direction it was moving. The platform was raised to a height where the top of the safety railing was a few inches below the bottom of the I-beam. As he drove the manlift, the safety railings of the manlift passed beneath the I-beam and the victim was pinned between the beam and the control panel. He was found by his co-worker who placed a call to emergency personnel. Rescue personnel arrived at the scene shortly after being notified. They removed the victim and pronounced him dead at the scene. MN FACE investigators concluded that, in order to reduce the likelihood of similar occurrences, the following guidelines should be followed: 1. employers should ensure that operators of mobile equipment face the direction of travel or have a coworker assist them while the equipment is moving AB - 2. employers should conduct scheduled and unscheduled workplace safety inspections AB - and 3. employers should design, develop, and implement a comprehensive safety program.
