Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

Frame sections 1 and 2 were sub-assembled on the floor and moved into position by a forklift. Frame 1 was moved to an upright, vertical position next to a hoist's vertical support pole, and left in an unrestrained, freestanding position. Two three-inch steel channel bars were tack-welded to the base of Frame 1. The channel bars were thought to provide upright support as well as a positioning aid for Frame 2. Frame 2 was moved into position by the forklift, and the channel bars from the base of Frame 1 were positioned on the top of the I-beam base of Frame 2. The forklift was supporting the top I-beam of Frame 2, and the victim was facing the forklift and standing between the unsecured Frame 1 and the forklift supported Frame 2. The victim was directing the forklift driver to properly position Frame 2. He was also helping with minor position adjustments of Frame 1. When Frame 2 was in the proper position, the victim was going to tack-weld the channel bars from Frame 1 to Frame 2's base I-beam. During the positioning of Frame 2, the forklift driver observed Frame 1 begin to fall. The driver yelled to the victim to get out of the way. The victim could not react quickly enough, and the top I-beam of Frame 1 struck the victim, and he fell against Frame 2. Fellow workers called 911 and lifted Frame 1 from the victim. Paramedics arrived and took the victim to the hospital where he died a short time later. Recommendations: 1. Heavy objects subject to tipping should not be stored in a freestanding, unsupported position - always brace or tie off the object to support it and protect it from potential movement. 2. Employers should develop, implement, and enforce a comprehensive health and safety program that includes, but is not limited to, training in hazard recognition and avoidance and job hazard analysis. The victim and a coworker had been previously lowered from the dock level via a crane-suspended personnel platform and landed on a barge. After they had disembarked from the personnel platform, the port manager, who was operating the crane, began to hoist it back to the dock. The platform had reportedly been raised about 2 feet when the right-side boom pendant rode out of the boom tip pendant sheave, immediately introducing 200 feet of slack into the boom-hoist system. The slack subsequently caused the boom to fall. The victim apparently observed the boom falling and pushed his coworker aside and out of the way. The boom hit the edge of the dock, broke over the dockside, and struck the victim. The port manager immediately left the crane cab and went to the edge of the dock. Seeing the victim under the boom tip, he contacted another river-port company for assistance. A truck driver who had been waiting on the dock called 911. Emergency medical service personnel and fire fighters responded within 7 to 10 minutes. However, the victim died on the scene. NIOSH investigators concluded that to help prevent similar occurrences employers should: 1. ensure that examinations are conducted each day prior to placing a crane in service to detect defective operating components or other conditions affecting safe operation AB - 2. ensure that idled cranes are positioned to facilitate effective pre-use examinations AB - and, 3. ensure that workers are trained to evaluate the effects of severe weather conditions on facilities and equipment. The victim climbed on top of the purge guard and leaned over the top of the stationary platen of the HIMM in an attempt to see if the tools were left within the mold area and placed his head beneath the robot's gantry frame. His position placed him between the robot's home position and the robot's support frame on the stationary platen. While trying to look inside the mold area, the robot cycled, and the victim's head was struck from the side and crushed between the robot and the robot's support frame. Another employee noticed the victim on top of the HIMM and went to investigate. Upon seeing the victim's condition, fellow employees were called to move the victim to the floor. Emergency responders were called, and awaiting emergency responder arrival, employees began chest compressions and other first aid procedures. The victim was pronounced dead on arrival at the local hospital. Recommendations: 1. The robot and the point of operation should be safeguarded to prevent entry during automatic operation. 2. Users should conduct a risk assessment of the robot/robot system to identify equipment, installation, standards, and process hazards so adequate employee safeguards are provided. 3. Users should ensure that personnel who interact with the robot or robot system, such as programmers, teachers, operators, and maintenance personnel are trained on the safety issues associated with the task, robot and robot system. He was the operator at the unload station of a barrel plating operation. The hoist supporting the barrel placed the barrel containing the plated parts onto a cart that transported the barrel from the last plating station to the unload station. Approaching the unload station from the right, a tab on the barrel contacted a wire on a limit switch that stopped the barrel rotation so that two doors on the barrel were facing the operator. He removed the two doors from the barrel and pushed two palm buttons on a control panel to the right of the station, so the barrel continued, moving right to left, to a dump station. At the dump station the barrel rotated so the parts fell onto a conveyor belt through the door opening. After emptying, the barrel rotated clockwise to find its correct position for door replacement and returned to the operator's station. The tab on the barrel contacted the wire on the limit switch to stop the barrel's rotation for door replacement. Although this was an unwitnessed event, the following event sequence was based on the operator's resting position. As the barrel was returning to the operator's station after dumping the parts, the victim reached into the barrel to remove some parts that had not fallen out at the dump station before it had completely returned to the door replacement position and while it was still rotating. When he reached into the barrel, his left sweater sleeve caught on something inside the barrel. The limit switch apparatus did not function, so the barrel continued to revolve instead of stopping as it was supposed to. As the barrel continued to rotate, it pulled the victim between the barrel and the hoist support bar, crushing his head. The first person to notice something wrong saw the operator caught between the barrel hoist support bar and the barrel. Police and rescue personnel were called immediately. His body was pried out of the equipment, and he was pronounced dead at the scene. Recommendations: 1. Review the design characteristics of the holes in the barrel to ensure that parts tumble out freely when it is tilted. 2. Guard moving parts at equipment point of operation. 3. Allow time at the beginning of each shift for the operator to perform a check of equipment to ensure that all machinery is operating correctly. 4. Establish a written schedule for conducting preventive maintenance on critical parts of the system. 5. Place a warning sign on the equipment depicting the potential danger of the body or clothing becoming entrapped in moving portions of the machinery. 6. These deaths are all the more tragic because they are largely preventable. Information about when and how they occur is essential in order to target effective prevention programs. In Massachusetts, the Occupational Health Surveillance Program (OHSP) in the Massachusetts Department of Public Health (MDPH) collects information on all fatal occupational injuries in the Commonwealth as part of the national Census of Fatal Occupational Injuries (CFOI), conducted in cooperation with the Bureau of labor Statistics (BLS), U.S. Department of labor. OHSP also conducts in-depth work site investigations of targeted fatal occupational injuries as part of the national Fatality Assessment Control and Evaluation project (FACE), sponsored by the National Institute for Occupational Safety and Health (NIOSH). The purpose of the FACE project is to develop a detailed understanding of how fatal injuries occur and to identify effective countermeasures to prevent similar incidents in the future. Excerpts from selected FACE investigations are highlighted within this report. This update provides an overview of fatal injuries at work that occurred in Massachusetts during 1999. These include fatalities traditionally linked with factors in the work environment such as falls, electrocutions, and exposure to toxic substances. They also include homicides and suicides at work, as well as motor vehicle-related fatalities that occurred during travel on the job. Deaths caused by occupational illnesses and most fatal heart attacks at work are not included in this fatality census. In 1999, 77 men and 6 women in Massachusetts suffered fatal injuries at work, the highest number of fatalities since 1993. This translates into an average of 1.6 deaths per week and an annual occupational fatality rate of 2.6 per 100,000 workers. There were 8 deaths of workers from racial groups other than white and 6 workers were of Hispanic origin. The average age at death was 44 years. Forty-eight victims were 44 years of age or younger and 8 were 65 years old or older. The 83 fatalities resulted in an average of 31 years of potential life lost for each death (number of years before the victim reached age 75), for a total of 2,573 years of potential life lost. Of the 83 workers fatally injured, 70 were wage and salary workers and 12 were self-employed. One worker died while volunteering. The tractor then turned right toward the road's steeply banked shoulder. As the tractor began to overturn down the bank, the victim attempted to jump from the tractor in the same direction as the overturn. The motorist who witnessed the incident found the victim pinned under the tractor. Emergency medical persons were called and responded to the incident. The victim died at the scene. To prevent similar occurrences, the Washington State Fatality Assessment & Control Evaluation (FACE) Investigative team concluded that persons operating farm tractors should follow these recommendations: 1. All tractors should be equipped with Rollover Protective Structures (ROPS) and seat belts. ROPS should be installed as manufacturers' original equipment or retrofit per the manufacturers' guidelines. 2. All personnel must wear a seat belt when operating a tractor equipped with a ROPS. 3. All persons who drive tractors need to be trained, educated and knowledgeable in all aspects of the safe operation of tractors, including the towing of implements, and be made aware of hazards related to their operation. Drivers should also be familiar with the tractor's operation manual. 4. Extreme care should be used when operating tractors on or near sloped terrain, ditches, or embankments. The Oklahoma State Department of Health Injury Prevention Service began a statewide, population-based occupational injury fatality surveillance program in July 1997. The Oklahoma Fatality and Control Evaluation (OKFACE) program is funded by the National Institute for Occupational Safety and Health (NIOSH). The goals of the OKFACE program are to monitor occupational deaths statewide, determine the epidemiology of fatal work-related injuries, and formulate and disseminate prevention strategies to those who can intervene in workplaces. In Oklahoma, a total of 309 work-related deaths were identified from January 1, 1998 through December 31, 2000 (average annual rate 6.5 per 100,000 workers). During the 3-year period, farming-related deaths were the leading cause of occupational deaths in Oklahoma, accounting for nearly one-quarter of all deaths. This report describes the epidemiology of these injuries. Department of Health AB - A 41-year-old male farmer (victim) died after he became entangled in an unguarded power-take-off (PTO) shaft. The victim and another worker (a truck driver) used a power-take-off driven auger to transfer animal feed pellets from a semi-truck to a farm grain wagon. They were standing in a restricted area between the PTO shaft that powered the auger and the left side of the semi-truck box. The victim apparently stepped slightly back toward the PTO shaft to provide additional room for the truck driver when the truck driver closed the truck's unloading door. When the victim stepped back, the end of the jacket sleeve on his right arm became entangled in the rotating PTO shaft. The truck driver initially tried to grab the victim's legs to free him. He then got on the tractor, disengaged the tractor's PTO shaft and placed a call to emergency medical personnel. They arrived shortly after being notified and freed the victim from the PTO shaft. They transported him by air ambulance to a nearby major medical facility where he died later on the day of the incident. Department of Labor A2 - Nebraska Workforce Development AB - A 57-year-old male equipment operator was killed when he was pinned between two trucks. The victim was power washing the windshield of a snowplow equipped truck, when another truck backed out of a nearby shed and pinned him against the vehicle's front fender. He was immediately provided first aid until rescue personnel arrived, then transported to a local hospital where he was pronounced dead. The Nebraska Workforce Development Department of Labor Investigator concluded that to prevent future similar occurrences: 1. Employers should develop, implement and review their policies and training programs on the particular type of equipment the employees will be utilizing to include safe backing practices. 2. Employers should ensure that "close proximity devices" and other such devices are evaluated to see if they would be beneficial for this type of operation. 3. Employers should develop, implement and enforce a policy that requires each bay entrance to be clear of all obstacles to allow for a clear path of travel and adequate clearance for the trucks and plows. 4. Employees should ensure that their path of travel is clear prior to putting the vehicle in motion. 5. Employees should read, understand and obey all written safety requirements for the type of equipment they are operating. The town police and fire departments arrived at the incident site within minutes. The victim was med-flighted to a hospital where he was in critical condition for seven days before he died from the injuries sustained in the explosion. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future employers should: 1. Develop, implement and enforce standard operating procedures (SOPs) for removing lids from large, sealed metal containers. 2. Routinely undertake hazard assessments of the work site. 3. Provide workers with training in the recognition and avoidance of hazards and safe work practices that pertain to their work environment. 4. Maintain their chemical inventory at a few months supply, whenever possible, to reduce the number of large metal containers used. A crew of three workers was installing the last few lights in the rear of the building, located about 25 feet from the edge of a bay. Due to a large awning that covered an outdoor patio, the crew could not use their cherry-picker lift truck and instead used a battery-powered, hydraulic scissor lift to reach the soffit. Using a large sheet of plywood as a base on the ground, the crew positioned the lift near the side of the building. The supervisor raised the lift to test it before turning it over to the victim with his job instructions. At about 4:30 p.m. the victim's co-worker was sawing wood on the ground below the lift when he heard the victim cry out and saw the lift fall over. The lift's work platform crashed to the ground at the edge of the bay, throwing the victim headfirst into the shallow water. To prevent similar incidents in the future, NJ FACE recommends following these safety guidelines: 1. Employers should ensure that personnel lifts are correctly used and positioned per manufacturer's directions. 2. Employers should conduct a job hazard analysis of all work tasks with the participation of the workers. 3. Employers should ensure that scissor lift operators are properly trained. The skid-steer loader was being used to clear snow from a parking lot. On the night of the incident, there was an accumulation of packed snow and ice that completely surrounded the foot pedals. The foot pedals controlling the operation of the loader arms and bucket are located on the floor near the front frame of the loader. The left foot pedal raises and lowers the skid-steer loader arms and the right foot pedal controls bucket tilt. It appears that the operator was attempting to clean the snow from around these pedals. Snow and ice had been cleared from in front of and behind the right foot pedal controlling bucket tilt. The front "toe" portion of the left foot pedal controlling the loader arm had been partially cleared of ice and snow, but snow was still present behind the "toe" area as well as surrounding the remainder of the foot pedal. With the lift arm/bucket in the raised position, the deceased was apparently cleaning the left foot pedal. While clearing the snow from around the foot pedal, the deceased evidently pressed the "toe" of the foot pedal, which lowered the lift arm/bucket. The deceased was found standing on the ground facing the loader, pinned between the hydraulic tilt cylinder and the skid-steer loader frame. The seat bar was in the down position, and the ignition key, which was bent, was in the cab seat. The Fire Department used the Jaws of Life to raise the lift arms to provide access to the deceased. Operators of skid-steer loaders should be trained in and follow the manufacturer's recommended procedures to safely operate, service, maintain, and exit the skid-steer loader. 2. Employers should prohibit skid-steer loader operators from working underneath raised lift arms if an approved lift arm support is not available. 3. Operators who are under the influence of drugs and/or alcohol should not operate machinery since their ability to recognize and respond appropriately to hazardous situations can be impaired. 4. MIFACE suggests that manufacturers review the design characteristics of skid-steer loaders to ensure that operators may not routinely exit or perform service activities when the lift arms are raised without a properly installed approved lift arm support in place. This shaft was connected to the mill power intake shaft by a U-joint on the side of the mill. A protective master guard over the U-joint had evidently been removed in the past, leaving this joint exposed. There were no witnesses to the event, and from the circumstances, it appeared possible that the man slipped and/or fell onto the rotating shaft. There was no obvious reason for him to be in that position near the roller mill. There was an oilcan on the floor, which could have been used at the time for oiling chains, but the chains were on the other side of the mill. The controls for the incoming and outgoing corn were on the other side of the mill as well. The were 9 sacks of feed next to the door, close to the mill, and there were several objects on the floor and around the space next to the mill where the victim would have been standing. It is possible that he stepped on or was caught by something, which made him loose his balance. The farmer died from cervical fractures. Recommendations based on our investigation are as follows: 1. Moving machinery parts such as belts, pulleys, chains, gears, and shafts, must be guarded to protect workers. 2. Good housekeeping practices should be followed to maintain work areas in safe condition. The pen was a fenced area in front of the barn, and to access the barn door the farmers had to walk through the pen. There was no escape route or protected areas from animal attack in the pen. The bull was kept in the barn at night and in the pen or pasture during daytime. The farmers handled the animals daily and had no problems with the bull until one of the brothers was attacked and five days later when the two other brothers were fatally injured by the bull. The bull's sire had been on the farm earlier, but was not kept due to aggressive behavior. There were no obvious indications that the bull was aggravated by handling or specific unusual events. Recommendations based on our investigation are as follows: 1. Aggressive animals should not be used as herd sires. 2. Handling facilities for bulls should include escape routes and protected areas for handlers. 3. Handling bulls requires extra precautions. Department of Health AB - A 19-year-old male farm worker (victim) who worked as a truck driver died after he was engulfed in wheat in a truck box. The truck was a belly-dump semi-truck. The victim and coworker each drove semi-trucks that were full of recently harvested wheat to a farm site. The victim stopped his truck at a bin site and together the two workers prepared to unload the truck. At the bin site were several grain augers that were used to transfer the wheat from the trucks to storage bins. The intake end of an auger known as a swing auger was mounted on small rollers and was rolled into position under the unloading door of the rear compartment of the truck box. The workers then removed a canvas cover from the top of the truck. Then they started the unloading augers and partially opened the unloading door of the rear compartment. After the wheat began to flow from the truck they climbed a ladder mounted to the back of the truck and sat on the edge of the truck box. While they sat on the edge of the box and watched the wheat flow from the truck, the victim entered the box and walked in the flowing wheat. When the truck box was about one-third emptied the victim realized he was being pulled into the wheat. The coworker grabbed the victim's hand but was unable to pull him from the box. He climbed down the ladder on the back of the truck, closed the unloading door and stopped the auger. He climbed the ladder to help the victim but found that he was completely submerged in the wheat. The coworker yelled to others at the farm site and a call was made to emergency personnel who arrived at the scene shortly after being notified. They administered oxygen to the victim while they freed him from the grain. He was transported to a local hospital where he was pronounced dead on arrival. MN FACE investigators concluded that, in order to reduce the likelihood of similar occurrences, the following guidelines should be followed: 1. workers should never enter grain hauling equipment or a grain storage structure when the equipment or structure is being emptied AB - 2. all grain handling equipment should be stopped and the power source locked out before workers enter grain hauling equipment or grain storage structures AB - 3. the grain box of grain hauling equipment should be identified as a confined space and posted with hazard warning signs AB - and 4. inexperienced workers should be provided comprehensive safety training for all work tasks and associated hazards. The victim had been working in the family-owned store after school hours. On the day of the incident, a customer asked him to get some ground beef. There was none available, and no one working in the meat room. The youth decided to prepare the ground beef himself and went to the meat room to process the beef through the meat grinder. While he was operating the grinder, some of the meat became stuck in the meat grinder bowl. The youth removed the feed pan, reached with his right hand into the bowl and pushed the meat down into the grinder's worm.1 When meat fed from the bowl, the worm caught his hand and fed it into the grinder's barrel, amputating his hand and part of his lower right arm. Coworkers called 911 and emergency medical personnel responded within 5 to 10 minutes. NIOSH investigators concluded that, to help prevent similar occurrences, employers should: 1) ensure that workers are trained to recognize the hazard of accessing the internal components of operating or energized machinery AB - 2) prohibit workers under the age of 18 from operating power-driven meat-processing machines by implementing youth-appropriate hazardous-energy-control procedures and additionally, manufacturers of power-driven meat-processing equipment should AB - 3) consider design modifications or additions to provide redundant protection against the inadvertent release of hazardous energy through contact with operating machine components. Both operators parked their forklifts on concrete surfaces with small inclines. The operators did not set their parking brakes before getting off of the forklifts. THE UNAWARE TRUCK DRIVER: A 39yearold truck driver, Andreas P.* (* Not the victim's real name), died when he was crushed between a forklift and a flatbed trailer. Andreas had parked a forklift on a sidewalk slightly higher than his truck. He went to straighten some pallets on the trailer. The forklift rolled forward and pinned Andreas between the edge of the trailer and the mast of the forklift. Although Andreas was trained in forklift operation his employer did not make sure he followed forklift rules and regulations. THE UNTRAINED OPERATOR: A 21yearold warehouseman, George L.* (* Not the victim's real name), died when he was crushed between a forklift and the edge of the floor of a railroad boxcar. George drove the forklift to the loading area of a loading dock. After he parked it, he tried to open a boxcar door. The forklift rolled backwards toward the boxcar, and fell partially over the edge. This pinned George between the edge of the floor of the boxcar and the rear of the forklift. George was a new employee who was not trained in forklift operation. RECOMMENDATIONS: Employers should make sure that: Forklift operators follow the rules when they get off a forklift. Forklifts are regularly inspected. No one stands between parked forklifts and large, unmoving objects. New employees are not allowed to do new tasks until they are properly trained and tested in forklift operation. Manufacturers should: Make forklifts with automatic parking brakes so that the brakes engage whenever no one is in the seat.
