Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The handyman enlisted the help of his 15-year-old neighbor and brought him to the worksite. Neither the handyman nor laborer had received training in fall protection methods and no means of fall protection had been provided by the employer. They worked on the roof for approximately 6 hours, patching cracks with tar and gravel, and were nearly done with repairs, when the victim fell through an unguarded skylight. The handyman did not see the victim fall. Immediately following the incident, a worker inside the warehouse reported the incident to office personnel who immediately called 911. Personnel from the sheriff's office and emergency medical services (EMS) responded within 5 minutes. EMS personnel administered cardiopulmonary resuscitation (CPR) and transported the victim via ambulance to a local hospital emergency room where he was pronounced dead upon arrival. A state road crew was working that morning to fill potholes in the right-hand lane of the highway. This mobile operation would drive up to a pothole, fix it, and continue on to the next hole. The work zone was marked by trucks carrying warning signs and arrow boards that drove behind the work crew. Two large dump trucks equipped with impact attenuators drove directly behind the work zone to protect the crew. At about 11:20 a.m., the victim approached the road crew and struck the impact attenuator on a dump truck. The collision caused the victim's truck to flip over on its side and slide down the highway until it came to rest with the truck cab upside down, killing the driver. No workers on the road crew were injured. A second driver involved in the ensuing motor vehicle accident was treated at the local hospital for neck injuries and released that evening. NJ FACE investigators concluded that, to prevent similar incidents in the future, these safety guidelines should be followed: 1. State highway authorities should consider reducing speed limits in construction work zones on high-traffic highways. 2. State, county, and local authorities should consider stationing law enforcement officers in patrol cars and using radar surveillance for traffic speed control at highway work zones. A 32-year-old laborer, who was standing behind the backhoe, was killed when he was pinned and crushed between the backhoe and a dumpster. One coworker remained outside with the victim and cleaned up around the landscaping on the rest stop grounds. At the time of the incident, the victim had begun to mow along a bank bordering one of the rest stop's parking lots. A 15-foot-high bank with a 35-degree slope was present on the side of the parking lot. The victim had been instructed by the crew leader to mow across the bank and approximately 1/3 of the way up the bank due to the steep slope. The victim was instructed to use a weed eater to finish cutting the grass the rest of the way up the bank. Work proceeded all morning, and shortly after lunch, as the victim began to cut the bank, the coworker working outside with him notified the victim that he was going inside to get a drink and to get out of the sun for a while. The victim told the coworker that he would continue mowing. When the coworker returned ½ hour later, he found the mower overturned, with the victim pinned underneath it. The coworker summoned the rest of the crew to help him lift the mower off the victim. A crew member called the Emergency Medical Service (EMS) from a phone in the rest stop. EMS personnel contacted the county coroner who pronounced the victim dead at the scene. Fatality Assessment and Control Evaluation (FACE) Program AB - A 14-year-old male laborer (the victim) died of crushing injuries after a damaged portion of hoisting apparatus (illusion unit) at an automobile repossession yard fell on him. The youth had been hired to pull weeds and perform general clean-up around the perimeter of the yard and the buildings located within the yard. At the time of the incident, the youth was pulling weeds in the vicinity of a 74½-inch-high, 300-pound portion of hoisting apparatus that was positioned but unsecured against one of the buildings in the yard. The incident was unwitnessed, but the presence of several large weeds behind the unit indicates that the victim might have been trying to move the illusion unit to gain access to the weeds when the unit fell on him. The victim was last seen at lunch. When his father, who also worked at the repossession yard, went to look for him an hour later he found the victim underneath the fallen unit. The father lifted the unit off of the victim and called the Emergency Medical Service (EMS) from the company office. Due to the extent of the victim's injuries, no first aid was attempted. When the EMS arrived they summoned the coroner, who pronounced the victim dead at the scene. NIOSH investigators concluded that, to help prevent similar incidents, employers should: 1. isolate, stabilize, dismantle or restrain equipment that has been out of service in such a manner that would prevent employee exposure AB - 2. comply with child labor laws which prohibit youth 14 and 15 years of age from working in occupations that involve warehousing and storage AB - 3. ensure that constant supervision is provided when youths are present in a working environment AB - and, 4. develop, implement, and enforce a comprehensive safety program. The victim was part of a two-man crew clearing earth away from the foundation footer of the residence to install a French drain around the perimeter of the house. The French drain was to rest on the 6 to 8 inches of footer protruding from underneath the cement blocks of the residence's foundation. The backhoe operator began digging an approximately 2-foot-wide by 2-foot-deep excavation around the perimeter of the foundation while the victim was using a hand shovel to remove any extra earth from the foundation footer after the backhoe passed through. Earth had been removed to a point approximately 9½ feet from the right of the steps when the victim realized that the amount of footer protruding was decreasing and would not be sufficient for the French drain to rest on. He motioned for the backhoe operator to look at the amount of footer that was protruding so they could determine the amount of extra earth to be removed. The operator lowered the backhoe's bucket to rest on a pile of earth approximately 8 feet from the victim, placed both the machine's transmission controls in neutral, set the parking brake, and dismounted the backhoe using the steps on the right side of the machine. After the correct amount of earth to be removed had been determined, the operator returned to the machine and climbed the left rear tire to access the cab. The operator adjusted the seat toward him and stepped over the top of the tire to sit down. While doing so, he inadvertently contacted the machine control that activated the boom swing toward the victim standing in the ditch. The boom struck the victim, pinning him against the house. The operator swung the boom away from the victim, dismounted the vehicle, and yelled to a nearby resident to call 911. He then removed the victim from the ditch and initiated cardiopulmonary resuscitation (CPR). When the emergency medical service (EMS) arrived, the operator moved the backhoe away from the residence to give the EMS personnel more room. EMS summoned personnel from the Medical Examiner's office who pronounced the victim dead at the scene. On this day, the victim's son had been unloading the bin using the bottom-unloading auger, and had just filled their semi-truck with 900 bushels (31.5 cubic meters) of corn. The son then drove the grain truck to a grain terminal, and returned 1.5 hours later. He could not find his father, and was suspicious that he might be in the bin because of the behavior of farm dogs. The grandson began to search under the surface of the corn, and eventually found his grandfather completely buried in the corn. Emergency response crews were contacted and upon their arrival, the main bin door was opened. It took an additional two hours to remove enough corn to gain access to the victim, using a grain auger and a grain vacuum. Prior to the unloading of the bin, the victim's son was not aware that his father was in or around the bins during unloading, and there was no reason for anyone to be in the bin at that time. Recommendations based on our investigation are as follows: 1. No one should be in grain bins during unloading. 2. A safety harness and safety line should be worn when entering bins. 3. When entering grain bins, a second person is needed outside the bin, with an adequate communication method. 4. Bin unloading equipment should be locked-out prior to an individual entering a bin. 5. A safety-response plan should be developed prior to entering a grain bin. He had recently rented the field and was working it the first time, mowing corn stalks and preparing the field for spring planting. While mowing, he drove into what appeared to be a washout from a tile standpipe. The force of the jolt knocked the seat and the farmer off the tractor. The man fell into the washout area, and the mower ran over him, causing fatal head injuries. He was found dead the next day. This tractor model had a sliding seat attachment for adjusting the seat position with a spring-loaded pin locking the seat in place. This pin was damaged, and was not able to keep the seat firmly in place. There are metal tabs at the end of the tracks keeping the seat from sliding off the tracks. Apparently these stops were not adequate, and the seat slid back, and separated from its base. Recommendations based on our investigation are as follows: 1. Owners/operators of tractors should ensure that the tractor seat is in good condition and firmly attached to the base. 2. Tractors should be equipped with ROPS and seatbelt, which should be used when mowing. 3. Equipment manufacturers should design the seat support structure to withstand use in demanding conditions for the lifetime of the tractor. 4. Farmers should be alert for washouts and other hazardous conditions in fields and repair them, or clearly mark them, to avoid injury. The man's hands were next to the control levers for the machine, which has led family members to suspect he may have lost consciousness prior to being pinned by the machine. There was nothing obviously in the vicinity that the man could have been reaching for. The work surface was flat concrete, and he was moving snow, as he had done many times before with the same machine. He had removed the protective cage on the machine years ago to provide access to an old-style hog house, so there was no protection between the operator and the hydraulic lift arms. He had owned this skid-steer loader for many years, and was very accustomed to using it without the cage. Recommendations based on our investigation are as follows: 1. Owners and operators of skid-steer loaders should not modify factory-installed protective structures, nor alter other built-in safety devices. 2. Owners and operators of skid-steer loaders should be made aware of the multiple dangers of using these machines. The EMS and police responded. The victim was transported by ambulance to the hospital, where he was pronounced dead. The FACE investigator concluded that, to prevent similar occurrences, employers that use snowmobiles in their business should: 1. Consult with snowmobile manufacturers to determine safe and effective in-shop methods to test snowmobile operations. 2. Develop, implement and enforce a policy for snowmobile operators to run at least one test lap on a trail at a low speed before testing the machine at a higher speed. 3. Develop, implement and enforce a policy to observe speed limits for operating snowmobiles during test drives. 4. On the day of the incident, the victim and his 25-year-old brother were cutting and stacking lumber for roof trusses on the second floor of the structure. To cut the lumber for trusses, the brothers were placing it on wood sawhorses. They had set the sawhorses up in the vicinity of a 19-foot, 7-inch by 25-foot, 2-inch unguarded elevator shaft opening in the floor. The brothers had been working cutting the lumber since 8 a.m. in the same area. At approximately 2 p.m., the older brother told the victim to pick up the scrap lumber around the sawhorses and clean up the work area. The older brother then walked around one sawhorse to pick up the circular saw while the victim walked around the other sawhorse toward the unguarded elevator shaft. The older brother heard boards rattling and looked up and didn't see the victim. He went to the edge of the unguarded shaft opening and saw the victim lying on the dirt floor at the bottom of the elevator shaft. An employee of the prime contractor working at ground level heard a noise and looked over to see the victim lying on the ground. He ran to the victim, then to the company truck to call 911. The victim had a faint pulse and shallow breathing. When the Emergency Medical Service (EMS) responded, they attended to the victim then transported him to the hospital where he was pronounced dead. The victim and another 15-year-old-male, who was sitting on the tractor's left fender, were scraping cattle manure across a concrete-surfaced barnyard toward the mouth of the manure pit. As the tractor approached the open mouth of the manure pit, the victim applied the brakes, and the tractor skidded on the concrete surface wet from the manure. The left front wheel slid over the concrete edge of the manure pit, causing the tractor to overturn into the manure pit. The tractor came to rest upside down, pinning the victim underneath. The victim and the tractor were completely submerged in the manure pit. The passenger was able to reach the dirt side wall of the pit, climb out, and run for help. Other farm workers immediately called 911 and went to assist the victim. The local Fire Department and rescue personnel arrived within minutes and started to search for the victim. Approximately 35 minutes after the incident, the victim was extracted from the manure pit. Rescue personnel-initiated CPR immediately and transported the victim to a local hospital where he was pronounced dead on arrival. On the day of the incident, he was driving a farm tractor equipped with a front-end loader on a public road. The tractor was not equipped with a rollover protective structure or a seat belt. The loader was equipped with a small general purpose bucket that was empty at the time of the incident. Although the tractor and loader were maintained in operating condition, photographs indicated general maintenance and repair was less than optimal. The speed that the tractor was being driven at the time of the incident could not be determined. Shortly before the incident occurred, the victim stopped at a stop sign at a T-intersection of a gravel road and a blacktop road. He briefly spoke with a local resident who had turned onto the gravel road. The victim and the local resident visited briefly as the victim indicated he was in a hurry. The victim made a left hand turn onto the blacktop road and proceeded south. After travelling a short distance on the blacktop road, the tractor suddenly entered the ditch on the west side of the road. For some reason, the tractor rotated 180 degrees during the incident and came to rest with it's front end pointing to the north. It also rolled three-fourths of a turn to it's left and came to rest on it's right side. The local resident who had briefly talked to the victim happened to look in his rear view mirror and saw the tractor disappear from the road. He stopped and drove to the scene where he found the victim lying next to the tractor. He placed a call to emergency personnel who arrived at the scene shortly after being notified and pronounced the victim dead. Department of Health AB - A 43-year-old city employee (victim) died after he was pinned between the hopper and the suction hose of a street sweeper. The sweeper design, known as a cross wind design uses a loop of air which results in less dust being returned to the environment. A large flexible suction hose connects one end of the sweeper's pickup head to the collection hopper. All of the sweeper's operating controls are located in the cab of the sweeper except for the controls to empty the collection hopper. The hopper controls consist of two levers that are located on the right hand side of the unit. They are recessed beneath the collection hopper and are directly under a steel plate designed to prevent the levers from being inadvertently activated. A safety/warning sign on the top of the plate reads "NO STEP". When the lever to raise the hopper is pulled up, the hopper raises and debris is dumped from the hopper. When the lever is pushed down, the hopper lowers to it's down position. The sweeper was equipped with a hopper support (safety) bar to prevent the hopper from lowering during maintenance and repair. The safety bar can be put in use when the hopper is raised by pulling the head of a small pin from a rubber grommet to release the bar. The lower end of the bar then rests in a steel holder from which it must be manually lifted before the hopper can be lowered. At the time of the incident, the safety bar was not in use. The victim and other city workers worked on the east side of the city until 10:20 a.m. when they stopped for a break. After the break, the victim drove the sweeper to a landfill to empty the collection hopper. He backed the sweeper into a dumping area at the landfill, got out of the cab and emptied the hopper. After dumping the hopper, the victim apparently used a steel bar to dislodge items from the unit's suction hose. Apparently, after tapping on the hose, he climbed up to look down into it. While looking into the suction hose, he may have placed his right foot on a safety plate located above the hopper control levers. While in this position, his foot apparently slipped and struck the lever that raised and lowered the hopper. The hopper lowered and the victim became pinned between the top of the suction hose and the side of the hopper. At approximately 12:15 p.m., another worker arrived at the landfill with a dump truck loaded with debris. He noticed the sweeper in the same position that he had previously seen it earlier that day. He drove his truck over to the sweeper to determine if there was a problem and found the victim. He used a two-way radio to call for help. Two other city employees who were at the landfill were also notified by the truck driver and rushed to the scene. They raised the hopper and freed the victim. A coroner and other emergency personnel arrived at the scene shortly after the victim was freed. The coroner examined the victim and pronounced him dead at the scene. The sweeper was examined several days after the incident. It was found that all of its mechanical and hydraulic systems were in working condition and that the incident was not the result of any type of equipment or system failure. MN FACE investigators concluded that to reduce the likelihood of similar occurrences, the following guidelines should be followed: 1. workers should always use all safety devices that machines are equipped with to prevent injury and exposure to hazards, 2. equipment manufacturers should incorporate passive safety devices and systems in the design of machines to protect workers from unexpected movement of components AB - and 3. when considering human factors, manufacturers should locate operating controls so as to provide the maximum possible protection against inadvertent and unexpected movement of machine components. The front of the combine was equipped with a quick-connect coupling mechanism that enabled an operator to easily change from one header to another. It was also equipped with locking mechanisms which could be manually set to lock the header in place. Tire tracks near a storage building indicated that the victim drove the combine from the building to an outside location where the header was setting. He drove the combine up to the header and activated the hydraulic cylinders to attach and raise the header. He then drove the combine back inside the storage building to complete the attachment of the header. Before leaving the cab of the combine, he apparently left the header in a raised position. After dismounting from the combine cab he began to work on something that required him to crawl underneath the raised header. After he crawled under the header, it fell from the combine and he was pinned beneath it. A truck driver arrived at the victim's farm with a load of feed. He stopped the truck near the storage building and started an auger to unloaded the feed. While the feed was being unloaded, the truck driver entered the storage building and found the victim under the header. He ran to the nearby farm house and notified the victim's son. The son placed a call to emergency personnel and then went to the storage building with the truck driver. They used a tractor that was equipped with a front-end loader to lift one end of the header. As they began to lift the header, rescue personnel arrived at the scene. After the victim was freed, rescue personnel checked him for vital signs but did not find any. A county coroner arrived a short time later and pronounced the victim dead. On the afternoon of the incident, the window washer seated himself in the boatswains' chair and positioned himself over the edge of the roof's parapet. He then "bounced" in the boatswain's chair to make sure it was set to go. Because the carriage was not tied down and did not have counterweights attached, the carriage was pulled over the rooftop's parapet. Both workers had their fall arrest harnesses secured to the carriage. The window washer fell straight down while the helper was pulled from the roof by the carriage and struck the ground head-first. The local emergency medical unit was summoned immediately. The window washer's helper died from his injuries at the scene and the window washer suffered multiple severe injuries. To prevent similar occurrences, the Washington Fatality Assessment and Control Evaluation (FACE) Investigative team concluded that window washers should follow these recommendations: 1. Anyone working on, or from a roof with a fall exposure should be tied off with a safety line. The safety line should be attached to a specifically engineered independent anchorage point. 2. The window washer's "portable support device" (carriage) should be set up with appropriate counterweights and tie-backs. The tie-backs need to be attached to approved, specifically engineered anchorage points. 3. All persons who work at heights, should be trained, educated, and knowledgeable in all aspects of the safe use of their tools and equipment and be made aware of all the hazards related to their job. 4. Work safety and fall protection plans should be developed and implemented at all work sites. 5. Employers need to effectively supervise and coach employees who have little or no experience in performing high-risk jobs, such as working at heights in the window washing industry. 6. Employers should have a clear understanding of and abide by child labor laws that prohibit persons less than 18 years old from working in occupations that are declared to be hazardous. 7. Building owners and contractors should conduct pre-job inspections to review job requirements and safe work plans. 8. Safer methods to conduct high-risk jobs should be sought. He was pressurizing the contents of the tank with air from a compressed airline located just inside the plant to speed the evacuation of the waste oil and water into the waste storage area. The tank was not approved for use as a pressurized vessel. Fittings on the tank had been adapted for the purpose of connecting them to the compressed airline. The force of the explosion propelled the tank 500 feet in the air over the plant fence and a nearby bank parking lot onto a busy east/west road. This was a maintenance task that had been conducted in this manner for over 6 years. No one witnessed the actual event. Statements from nearby workers and maintenance staff were relied upon to reconstruct what was presumed to have occurred. The deceased was found lying near the place where he had been last seen beginning to drain the tank. Emergency personnel confirmed that he was dead when they arrived at the site. Recommendations: 1. Conduct job safety analyses and establish standard operating procedures for routine maintenance tasks and train the maintenance personnel in these procedures. 2. Do not pressurize a container not approved as a pressure vessel. 3. Train maintenance personnel to anticipate conditions that could jeopardize their safety or the safety of others. 4. Inform employees that no equipment is to be altered or retrofitted. Establish a procedure for a qualified person(s) to review proposed equipment changes. The driver, victim and two backseat passengers were all using lap and shoulder restraints. The driver pulled his vehicle out of the northbound lane to pass a large truck, started to slide on the icy road, and was struck broadside on the passenger's side by an oncoming car in the southbound lane. A driver in another vehicle summoned emergency services with his cell phone. EMS arrived at the scene within four minutes, and extricated the victim from the vehicle. He was pronounced dead at the scene. The other recruits and the driver of the oncoming vehicle survived the collision, and were taken to the hospital. The FACE investigator concluded that, to prevent similar occurrences, employers should: 1. Establish a policy that provides for postponement of nonessential travel during hazardous conditions. Additionally, employers should establish a method for notifying workers of schedule changes due to hazardous travel conditions. 2. Contact the U.S. Department of Labor, Employment Standards Administration Wage and Hour Division and the state agency responsible for child labor for guidance in complying with child labor laws which prohibit certain types of work by workers less than 18-years-old. 3. Ensure all travelers are trained in the proper use of vehicle safety features and drivers are trained in safe driving practices. Additionally, 4. The US Department of Labor (DOL) should consider amending child labor regulations to incorporate the provisions of the Drive for Teen Employment Act into the Hazardous Orders 2 regulation, and provide guidance on "incidental and occasional driving."
