Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

His brother, who heard the crashing sound of the tractor rolling down the embankment, discovered the victim soon after and had someone call the emergency squad, which responded within minutes. The county coroner was called and pronounced the victim dead at the scene. New York State Fatality Assessment and Control Evaluation (FACE) investigators concluded that to prevent similar incidents from occurring in the future, the following recommendations should be followed: 1. Equip all tractors with rollover protective structures (ROPS) and a seatbelt AB - 2. When the worker investigated, he found the victim on the floor of the plant next to his ladder. The victim's ladder was supported on the crossbeam of a movable hoist and not on the building I-beam that was nearer to the area where he was working. There was no evidence that the ladder had been originally leaned against the I-beam and fallen to the hoist crossbeam. When pulled backwards from the hoist crossbeam, the ladder contacted the back of the I-beam. The victim was lying perpendicular to the ladder on his right side with his back toward the ladder, facing away from the ladder. Emergency personnel indicated that he appeared to have fallen heavily on his right side, because he had multiple rib fractures on the right side as well as head injuries. It is possible that he was standing on his ladder with his back to it, and that when he fell, he twisted to his right and fell onto his right side. Recommendations: 1. Employers should develop a written accident prevention plan that identifies and describes hazards that could be encountered in the worksite and how to recognize and avoid them. 2. He lost control of his vehicle and spun out, striking a tree alongside the road. Police and EMS units arrived and transported him to the local hospital, where he died later that evening after undergoing surgery. NJ FACE investigators recommend following these safety guidelines to prevent future incidents: 1. Minors under the age of 18 should not be permitted to drive a motor vehicle for work as per Federal child labor laws. 2. Car owners should ensure that their vehicles are in proper operating condition. 3. Employers should become familiar with available resources on safety standards and safe work practices. The 48-year-old worker was on his second day on the job with his employer, a painting contractor. The job involved painting the exterior of a multi-story, multi-unit condominium. He was hired on for the duration of the job. Prior to the incident, a supervisor had given the worker a fall protection harness. He was not given any training or instruction in using the lift, he had never used such a lift before. The supervisor then left the worker in order to attend to another task. The worker then got into the bucket of the manlift and raised it to about 35 feet where it contacted an energized 26,000 volt powerline. The worker then somehow fell from the lift and struck the ground. He was taken to a hospital where he was pronounced dead. On the day of the incident the victim had been working with his brother at his brother's farm place. Around 3:00 p.m., the victim left to return to his farm place to do various chores and the evening milking of his cows. The next day, the victim's brother became concerned when his brother did not arrive by late morning to work on various tasks. He drove to his brother's farm and found him lying in a muddy lot near a barn. He notified emergency personnel and after the victim was examined they prepared to remove him from the lot. About the same time, the bull became aggressive toward the rescue personnel. The victim's brother tried to herd the bull from the scene but the bull charged him and pinned him against a fence. A law enforcement officer at the scene responded and used a gun to kill the bull. After the bull was killed and the victim removed, the victim's brother concluded that the dairy cows had not been recently milked. He also found a cow inside an addition built onto the barn. A newborn calf was also in the area and the cows hind legs were loosely tied. It appeared that the victim may have assisted in "pulling" the calf. In doing so, his hands and clothing may have obtained the scent of the birth of the calf which may have provoked an attack. An autopsy revealed that the victim had numerous broken ribs and several internal injuries that injuries indicated the victim had been attacked. Because of the aggressive nature of the bull toward rescue personnel, the most likely scenario was that the bull attacked the victim. The tractor was equipped with a side mount sickle-bar cutting attachment. With his left rear wheel on the downhill side, he was traversing a slope exceeding a 50% grade (27 degrees). Speed of travel was unknown but his right wheels ran over a woodchuck mound which increased the grade to approximately 60% (31 degrees). This shift placed the tractor's center of gravity (C.G.) outside of the tractor's stability baseline, causing it to roll downhill to the left and land on the victim. The tractor did not have a rollover protective structure (ROPS) or a seat belt. The victim's son-in-law came out to the field to check on the victim and found him lying on the ground shortly after being struck-by the tractor. He called EMS which arrived within minutes and transported the victim to a medical center were he died approximately 3 hours later. The cause of death was listed as traumatic arrest as a result of thoracic aortic disruption. The WV FACE investigator concluded that to reduce the likelihood of similar occurrences, the following guidelines should be observed by tractor owners: 1. Equip all tractors with rollover protective structures and a seat belt. 2. Consider permanently omitting steep areas from mowing. On the afternoon of the incident, the decedent drove the tractor, pulling a wagonload of tobacco, from the farm where it had been harvested to the tobacco barn one mile away. The road was hilly and had a sharp curve to the right at the bottom of the hill. As the decedent drove the tractor pulling the wagon down the hill, the wagon jackknifed and broke loose from the tractor. The tractor slid sideways, flipped over, slid around the curve, and came to rest with the decedent underneath, but not pinned. The wagonload of tobacco also flipped, skidded, and came to rest next to the tractor. The decedent's brother, along with the other migrant workers who had followed the tractor in a van, retrieved the decedent from underneath the tractor and drove him to the area hospital where he was declared dead from blunt force injuries. Recommendations 1. All tractors should be equipped with Roll Over Protective Structures (ROPS) and seatbelts. 2. Tractors should be operated in a safe manner on hills, including gearing and downshifting for the appropriate task and ground conditions. 3. Farm laborers should be provided training in their own language on tractor operation for all terrains, i.e. flat fields, hillsides, paved and gravel roads. 4. Emergency medical personnel should strive to relay accurate emergency information. A portion of the ditch was not directly accessible with a backhoe. To access the ditch, the backhoe needed to be driven through a garden, and turned toward the ditch at a 90-degree angle. The decedent had placed the pipe in the ditch, then turned the machine around and tried to exit the area. He had the bucket and outriggers up in the air instead of close to the ground. Upon exiting the area, the right tire ran up on to a tree stump causing the backhoe to tip over on its left side. The operator fell out of the backhoe and was pinned underneath the left rear cab standard. Death was due to crushing injuries. Recommendations: 1. Heavy equipment operators should use safety equipment provided by the manufacturer such as a seatbelt. The seatbelt was already part of the equipment. 2. Before starting, work sites should be inventoried for hazards and ways to avoid hazards should be communicated to employees. 3. Machinery should be operated in a safe manner. As the logger finished making the final cut on the oak tree he was felling, he began to walk away from the area. As the oak tree was falling, it hit another tree that horizontally crossed another standing tree. As the falling tree hit this crossed tree, the butt of the falling tree was thrown up into the air and backwards away from the stump. The logger was knocked down by the falling tree, and was subsequently crushed underneath the weight of the tree. When the logger did not return home, his wife became concerned and called a co-worker. The co-worker was able to find the logger after some searching, and called for Emergency Medical Services (EMS) with his cell phone. EMS responded and transported the victim to the hospital where he was pronounced dead by the county coroner. The wheel was remount pressurized, checked, and stored overnight inside a safety cage. The victim retrieved the wheel from the safety cage and began to install it on a dump truck. After the victim hand-tightened all six lug nuts on the spokes, he placed an impact wrench on one of the lug nuts. A co-worker walked over to the victim's location and was standing behind and to the victim's side. As the lug nut was tightened, the side ring of the rim released and struck the victim. Based on the findings of the investigation, to prevent similar occurrences, employers should: 1. Establish an inspection program for all (used and new) multi-piece rims to ensure proper matching of parts and serviceability AB - 2. Ensure workers have been trained on proper servicing of multi-piece rims, including demounting, inspection, and mounting procedures AB - 3. Ensure demounting/mounting procedures and rim matching charts are posted in the immediate service area per CFR 1910.177 AB - 4. Ensure a competent person inspects all multi-piece rim parts, used and new, before placing in-service, permanently removes all parts deemed unfit for use, and inspects final rim wheel assembly before installation. In addition, Employers, distributors, and manufacturers should consider placing highly visible labels on all rim parts to prompt inspection and matching of identification stamping prior to use. Fatality Assessment and Control Evaluation (FACE) Program AB - On June 24, 2002, a 21-year-old Hispanic dump-truck driver (the victim) died after being caught between the frame and dump body of an off-road dump truck while performing routine lubrication. The victim was working for an excavation contractor at a landfill expansion site on the day of the incident. The victim's foreman drove by the area where the company service truck was set up and stopped to investigate when he heard the air compressor running but not the usual clicking sounds made when workers are greasing their trucks. He found the victim caught between the frame and dump body of the truck. The foreman called out for help and then called 911 from his cell phone. An excavator operator working nearby responded to the foreman's call for help and climbed into the cab of the truck and raised the bed. Emergency medical services (EMS) and law enforcement personnel responded within 10 minutes. EMS personnel transported the victim by ambulance to a local hospital where he was pronounced dead. Victim 1 was the only person in the area who had a radio, and while he left it on the shore, none of the co-workers on shore knew how to use it. One of the co-workers ran to the equipment shed where another employee with a telephone notified emergency services. The EMS providers arrived within several minutes after receiving the call. Due to the worker's fear and the inability of workers and responders to communicate effectively, divers began searching the south side of the pond. The co-workers all spoke Spanish and only a few spoke English, so when a co-worker was located who could communicate with the deputy, the sheriff department learned that the victims had not gone into the water on the south side, but rather on the north side. The deputy and two other divers were then sent to the north side. Both victims were soon located, resuscitation efforts were started and both victims were transported to a nearby hospital. Both were pronounced dead upon arrival at the hospital. The FACE investigators concluded that, to prevent similar occurrences, employers should: 1. Ensure U.S. Coast Guard-approved personal flotation devices are available and used when an employee works near, on or over water where the danger of drowning exists. 2. Develop, implement and enforce a comprehensive written safety program for all workers that includes training in hazard recognition and the avoidance of unsafe conditions when using small water craft. 3. Train employees on how to use two-way radios for emergency communication. 4. Train non-English speaking workers about emergency situations and the role of law enforcement and/or rescue services in order to reduce anxiety when emergency contact with official agencies occurs. 5. Develop and implement a program that would remind all employers who must call for emergency services to request an interpreter for a specific language when a substantial immigrant population is employed or present at a worksite. A small backhoe was transported to the site on a trailer being towed by a dump truck. The backhoe was secured to the trailer with chains, which apparently tightened as the vehicle was moved. To remove the front chains, a worker started the backhoe and moved it forward a few inches to release the tension on the forward chains. A second employee (the victim) stood in front of the backhoe to remove the front chains. As the backhoe driver started to get off the machine to help the victim with the chains, the backhoe lunged forward, pinning the worker between the backhoe and the back of the dump truck that was towing it. NJ FACE investigators recommend following these safety guidelines to prevent similar incidents: 1. Employers should ensure that all employees are trained to stay clear of any operating equipment and vehicles. 2. Employers and employees should conduct a job hazard analysis of all work activities. 3. Employers should become familiar with available resources on safety standards and safe work practices. The loader transmission was in neutral, and the parking brake was set. The victim used at least four 3/8-inch chains rated at 6,600 pounds (the exact number is unknown) to secure the loader. Five binders were found at the scene. One chain appeared to be improperly attached to the loader's maintenance ladder, one chain appeared to be attached to an attachment point on the loader. The remaining chain attachments to the loader are unknown. If four chains were used, the load exceeded the aggregate working load limit of the chains. Also unknown is where each of the chain hooks and/or binder hooks were attached to the trailer. Based on the police report of the incident, at least one chain hook/binder was improperly attached to the trailer bed. One chain appeared to have an oversized hook. During transport, the driver stopped suddenly between two bridges. Skid marks from the truck were present on the roadway. It appears that a sudden stop caused sufficient force on the chains to break them. The loader rolled forward over the 5th wheel and onto the top of the truck cab. The cab was crushed, and the driver was declared dead at the scene. Recommendations: 1. Companies should require and train employees to use manufacturer identified cargo attachment points and "D" rings on the trailer using the appropriate number of tiedown assemblies. 2. Companies should ensure that consistency is maintained with the working load limits of all components of a tiedown assembly. 3. Companies should ensure employees inspect all components of a tiedown assembly to ensure that they are in good working order and are an appropriate and consistent size. 4. Additionally, MIFACE recommends that all employers develop standard operating procedures and provide employee training to educate workers about the January 2004 FMCSA new cargo securement rules. The victim was part of a five person crew that was hoisting coaxial cable up on the outside of a 1524 foot high telecommunications tower. The victim and two other workers were approximately 1100 feet up on the tower, guiding the coaxial cable to prevent it from banging into the tower side. The victim was outside of the tower structure, standing on a "platform" which was an ice guard above an FM antenna. He was wearing a harness/lanyard and was tied off to the guard structure. One worker was a few feet above him and another a few feet below him, both inside the tower structure and tied off. The coaxial cable had a 350 pound headache ball attached to the end to prevent cable twisting. This was several feet above the victim. With approximately 75 more feet to go, the winds had started to become a factor at that height. The supervisor on the ground radioed the victim to have him tie the cable off where it was and they would start again the next morning if the wind died down. The victim told the owner they still had a few minutes before the wind was too high, and since they were about done, to go ahead and finish hoisting the cable. The owner went to the trailer that contained the winch, revved the motor and started to pull the handle to raise the cable. As he pulled back, he heard a "snapping" sound from somewhere up on the tower, looked up, and saw the cable, wire rope and headache ball falling towards the ground. He yelled to the other workers to get out of the way and ran to the corner of the adjoining support building. As the materials fell they struck the victim, knocking him off the platform. The materials continued downward until they fell across a guy wire, causing them to slide off to the east of the tower structure. This motion caused the entire tower structure to shake violently. The two workers, one above and one below the victim, immediately started down the structure. They radioed the owner that the victim was badly hurt. They attempted to check for a pulse but the victim was too far out on, and below the platform for them to reach. The owner told them to exit the tower immediately, as he was also afraid it was going to collapse. Local rescue personnel were summoned, but no one had the experience necessary to attempt a rescue at that height. A climbing team trained in rescue procedures was brought in approximately 9 hours later and the victim was removed. The Nebraska Workforce Development, Department of Labor's Investigator concluded that to prevent future similar occurrences: 1. Employers should comply with established industry safety guidelines. 2. Employers should ensure equipment is adequate in size and strength, properly maintained and safely utilized for the job being performed. 3. Employers should ensure that adequate rescue equipment/personnel are available. 4. Employers should routinely conduct scheduled and unscheduled worksite safety inspections. 5. Employers should consider supplementing worker training on safe work practices with discussions of FACE case reports and other related training aids. 6. Tower owners should, through the use of contractual language, ensure that employers/workers adhere to OSHA-required safety measures (including Compliance Directive 2-1.29) while construction or maintenance is being performed on their towers. The metal framework involved in this incident was free-standing, not on a pallet. The handrail side of the framework was facing the open end of the trailer AB - the opposite side of the framework had a hopper and the equipment motor and was facing the truck cab. To remove the framework, a chain was wrapped around the bottom rails and the framework was pulled to the end of the trailer bed by the forklift. While pulling the framework toward the end of the truck, the framework became "cocked" to one side and lodged or caught on something in the trailer and would not continue to move. The forklift was positioned so the forks were placed under the lower framework cross beams and the handrail side placed against the backrest. The victim climbed into the trailer, went under the framework and kneeled on the bed floor while under the framework to see what the framework was caught on and to guide the forklift operator. The forklift operator lifted the framework a short distance from the trailer bed floor. As it was being lifted, the framework became dislodged. The sudden movement allowed it to lurch off of the forks toward the hopper/motor side. The victim attempted but was unable to get out of the way of the falling framework. The hopper landed on the victim's back pinning him to the trailer bed floor. The semi-truck driver called 911 and emergency responders arrived. The victim was declared dead at the scene. Recommendations: 1. Employers should ensure that powered industrial truck operators properly position and secure all loads before lifting and prohibit employees from working under raised loads. 2. Employers should develop and implement a comprehensive safety and health program that includes a powered industrial truck safety program and employee training. 3. Employers should conduct a job safety analysis for each individual rigging job. 4. Employers should develop and implement a health and safety committee that includes representatives from both management and labor. 5. He went around the facility and went to the propane tank located outside the building and shut it off. He went to his house and notified his wife about the gas leak. Together they went back to the shed and turned on two exhaust fans, one for the north side of the building, one for the south. The victim and his wife removed a broken shut off valve and reconnected the pipe without replacing the valve. His wife went outside to the propane tank and turned the valve on. When the victim attempted to light the south heater, his wife saw a flash of light from the eves and roof vent and immediately shut the valve off. The victim ran out of the hog shed with his clothes on fire. His wife was able to get him to the ground and used a coat to pat out the fire. Emergency personnel were called and the victim was life-flighted via helicopter to a nearby hospital burn center where he died five days later. The Nebraska Workforce Development, Department of Labor's Investigator concluded that to help prevent future similar occurrences, employers should: 1. Install gas/leak detectors. 2. Ensure that atmospheric testing equipment is available for use and personnel are trained on its operation. 3. Allow adequate time for accumulated flammable/explosives gases to be ventilated safely outside the facility. 4. Prior to re-lighting, check all joints, valves and associated piping for gas leaks. Two truck drivers parked within the construction site noticed the dump truck was backing in line with the walking officer. They attempted to warn the officer and the backing truck operator. The dump truck struck and knocked the victim to the ground and then backed over him with the left rear wheels. On site police officers placed emergency assistance calls and responded to the victim who was transported to a local hospital where he was pronounced dead a few hours later. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers/roadway construction contractors should: 1. Carefully evaluate project scheduling by considering the type of work zone set up, the time of day and the time of year the work will be performed. 2. Develop, implement and enforce an internal traffic control plan (ITCP) specific to each construction site to reduce backing of construction vehicles. 3. Ensure backing procedures are in place and that designated individuals are assigned as signalers to direct backing construction vehicles on construction sites. 4. Immediately repair equipment when problems, such as broken back-up alarms, arise In addition, local and state government agencies should: 5. Consider offering work zone safety training for all municipal officers who perform traffic details on roadway construction site. 6. Consider applicants' past work practices and safety record for all submitted bids to perform roadway construction work. In addition, manufacturers of heavy construction equipment, such as dump trucks, should: 7. Explore the possibility of incorporating new monitoring technology on their equipment that will assist the operator while backing.
