Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The work crew was pouring the footing for a retaining wall as part of a bridge for a private runway being built. The concrete form collapsed, trapping one worker, and almost trapping two others. The decedent had left to retrieve a shovel to help free the trapped co-worker. As he retrieved the shovel, he walked into a blind spot of the track hoe operator at the same time another worker asked the track hoe operator if there was a chain in the cab of the track hoe. The operator looked down at the floor and as he did so, his hand slightly moved the hand control causing the bucket to move, thus striking the decedent. He was transported to a hospital where he was declared dead by the coroner from blunt force injuries to the head. In order to prevent similar incidents from occurring, the Kentucky Fatality Assessment and Control Evaluation program recommends: 1. A hazard assessment of the work area should be performed every time work conditions change. Workers should not be in a blind spot of heavy equipment operators without the operator's knowledge. When blind spots are identified, a spotter should be used. 2. During emergency situations employ safe work practices. After being struck, with co-workers following him trying to convince him to sit down, he walked away from the site until emergency rescue workers arrived and convinced him to sit down on a stretcher. He was transported to a local hospital then air lifted to a larger facility where he died five days later from a closed head injury. To prevent similar incidents from occurring, KY FACE recommends: 1. Employees should practice safe operating procedures when using dangerous equipment. 2. Equipment manufacturers should re-evaluate the design of the handle to direct auger bits and research the possibility of a new design. The victim was assisting the loader operator by bucking and limbing logs at the log loading area, which was adjacent to the drop zone, when a turn of logs was brought into the landing by a helicopter. The victim left the log loading area and ran towards the drop zone to help with the turn. When the helicopter released the logs from its suspended hook, a tagged out choker hung up on one side of the double hook. Within seconds after it had hung up, the choker released, dropping a large Douglas fir log, which struck the ground, then the victim on the head. First aid and CPR were given to the victim immediately. The victim was airlifted to a local medical facility but he died in flight. To prevent future similar occurrences, the Washington Fatality Assessment & Control Evaluation (FACE) Investigative team concluded that persons working in the logging and helicopter logging industry should follow these guidelines and requirements: 1. All ground personnel need to be in a safe zone when a helicopter approaches with a turn, until the chokers have been released from the remote controlled hook and the helicopter has left the landing's drop zone. 2. After a turn has been dropped, chasers and other personnel must wait for the logs to settle before entering the drop zone. 3. Consider using an audible and/or visual alarm system to establish an all clear signal to notify ground personnel that it is safe to enter the drop zone. 4. Landings should be constructed to ensure that there are safe distances between people and support operations taking place near the drop zone. 5. Consider conducting a research or engineering study on possible hook failure mechanisms. 6. Proper personal protective equipment (PPE) needs to be worn by all ground crew personnel. At approximately 6:00pm, he left his farm and drove the tractor on a paved, asphalt, 2-lane road with a narrow, paved shoulder that went up a hill to help his neighbor. Hidden by grass/weed growth approximately 5-6 feet from the paved right shoulder of the road was a steep 3-foot ditch. He had traveled eastbound up the hill approximately 500 feet when he left the right shoulder and attempted to re-enter the roadway. He was unable to re-enter the roadway and the tractor wheels entered the ditch. The tractor flipped over, probably striking the victim as it overturned and pinning him under the tractor. A passing motorist saw the overturned tractor and called 911. Emergency response arrived and the victim was declared dead at the scene. Toxicology results indicated an elevated blood alcohol level of 0.29 grams of alcohol per 100 milliliters of blood and 0.16 grams of alcohol per 67 liters of urine. Recommendations: 1. The incident happened at a job site where a parking lot was being constructed and water utilities were being installed. The construction crew was repairing a storm drain in a trench that was 95 feet long and at the site of the incident it was 3 feet 8 inches deep and 6 feet wide. The victim, the competent person on the site, was standing in the trench along with another worker and was directing the operator of the front-end loader to deposit sand in the trench. The loader was at a 90-degree angle to the trench and delivering a bucket of sand when it slipped into the trench, striking the victim with the bucket. It was raining and the ground was muddy and slippery. He was transporting a plastic-wrapped four-foot by five-foot hay bale to his wife who was breaking open the bales and feeding the buffalo. His route between the hay storage area and his wife was rutted and snow-covered. Metal fence pipes were also in his path. He placed the hay bale in the front-end loader bucket but did not secure it apparently because of the short distance to where his wife was feeding buffalo. He was backing up, looking over his shoulder when the 2-wheel drive tractor's unweighted rear tires became stuck in a rut and the right front tire became lodged on some of the metal pipes. To free the tractor, he rocked the tractor back and forth. While rocking back and forth, and unknown to the victim because he was looking over his shoulder at his wife, the front-end loader arms raised independently. The setscrew in the joystick mechanism that tightens the spring to operate the loader's hydraulics was loose, allowing the loader arms to rise independently. While rocking back and forth, the hay bale fell out of the elevated bucket striking the victim. The bale then rolled off and landed on the ground to the victim's left. Both the tractor seat and steering wheel were damaged and bent from right to left. Emergency response was called while a family member administered CPR. Emergency response arrived and took the victim to a local hospital where he died. Recommendations: 1. When using a tractor loader to move a bale, properly secure the bale with a manufacturer's approved device such as a bale spear, clamp or grapple. 2. Inspect work areas and equipment before performing a task to identify actual and potential hazards and determine adequate controls. 3. Tractor owners should consider using a 4-wheel drive tractor with rollover protection and rear counterweights when using a tractor equipped with a front-end loader. 4. Ensure proper hydraulic maintenance occurs during tractor service and when aftermarket accessories are installed. 5. The joystick manufacturer should consider reevaluating the design or assembly methods of the joystick control valve to ensure that it cannot loosen once adjusted. 6. Although not a factor in this incident, MIFACE recommends that the tractor PTO stub shaft be guarded at all times to prevent entanglement. 7. The front-end loader manufacturer should consider reevaluating the loader model design in terms of number of and material used to construct the loader support system. After he was finished, he took the tractor to the shop. The forklift hydraulics were drifting during the loading operation, and he may have been attempting to adjust the forklift position when he started the tractor. While standing on the ground in front of the rear tire, he pressed down on the clutch and started the tractor. The tractor was left in gear and the tractor's rear wheel ran over the victim, causing crushing injuries to his pelvis. He was found by another employee and taken to the hospital for surgery and stabilization. The next day, a blood clot developed, and he died. Recommendations: 1. Tractors should only be started when the operator is on the tractor platform sitting in the operator's seat. 2. Employers should provide annual operator training to tractor operators. 3. A farm safety plan should be established for the agricultural operation that includes work rules such as starting the tractor from a seated position on the operator's platform. The victim had completed the weld on the disc and appeared to be repositioning to repair the stone guard when his face touched the mower. The mower was energized, and the victim was electrocuted. Upon finishing his chores, the brother walked over to the mower to see the progress on the mower repair and found the victim lying motionless on the ground. He went to get help at the farmhouse. His mother came down to the area, touched the mower and received a shock. The dog also touched the mower and yelped, at which time the victim's brother then turned off the circuit breaker. Emergency response was called, and the victim was declared dead at the scene. Recommendations: 1. Equipment should be maintained in good operating condition. 2. Appropriate personal protective equipment should be worn and ensured that it is in proper working order. 3. All circuit breaker circuits should be labeled as to the outlets covered by each breaker. Emergency medical assistance responded, and he was transported to the hospital where he was pronounced dead. At the hospital, a rectal temperature of 107.6 degrees Fahrenheit was recorded. Recommendations: 1. Employers should ensure that supervisors/managers regularly monitor workers during periods of high heat stress/strain. 2. Have medical care providers identify workers who take medications or who have medical conditions that would predispose them to heat-related illnesses. 3. Train supervisors and employees regarding heat stress, heat strain and heat-related illnesses in early spring. 4. Ensure all employees are able to recognize the signs and symptoms of heat-related illness in themselves and in others. 5. Stress the importance of drinking nonalcoholic beverages before, during and after working in hot conditions. 6. Periodically remind workers of the signs of heat-related illness and of the importance of drinking copious amounts of water during hot conditions. The embankment berm was insufficient to stop the trucks when they got close to the edge. The dump truck was supplied with a lap-type safety belt that the victim was wearing. The CA/FACE investigator determined that, in order to prevent future occurrences, employers, as part of their Injury and Illness Prevention Program (IIPP) should: 1. Ensure berms are adequately built to prevent trucks from over-traveling and over-turning at dumping locations. 2. Ensure that heavy equipment operators follow all operating signals they are given. 3. He and his partner were replacing non-functioning lights on two-light light poles. One of the new lights installed did not work. His partner was at the top of the pole in an aerial work basket checking the ballast. The victim was kneeling on damp grass at the base of the light pole so he could open the handhole to inspect the wiring and fuses. He was not wearing or using any protective equipment. The wires were energized and carried 277 volts of electricity. Although exactly what occurred is not known, it is possible that the plastic cover over the fuse inside the pole was broken, and when he reached into the handhole to extract the wires, he made contact with the electricity. It is also possible that he extracted the wires from the handhole, and as he attempted to untwist the plastic cover over a fuse, it broke in his hands. However it happened, he made contact with the electricity. When his partner realized what had happened, he descended immediately and severed the victim's contact with a wooden board. Emergency medical care was given at the site. He was pronounced dead at the hospital. Recommendations: 1. Employers should ensure that a responsible person such as a supervisor/manager periodically monitors workers who are assigned to remote locations. 2. Do not work on energized faulty or damaged equipment. 3. Ensure that appropriate safety equipment is available and that workers use it. 4. Disconnect and lock out electrical lines from their energy source before working on them. 5. Periodically reinforce training of supervisors and workers regarding the hazards associated with specific work assignments and work practices. When the farmer dismounted from the backhoe to connect the chain that would be used to pull a heavy piece of equipment, he left the backhoe idling with the transmission in gear and did not set the parking brake. As he was working at his position in front of the left rear tire, the backhoe began to move, knocking the victim down and running over his head. The backhoe proceeded on until coming to rest against a large steel holding tank. The victim was discovered approximately fifteen minutes later by a farm employee who immediately went for help. County sheriff investigators responded to the scene in addition to the local emergency squad. The county coroner was called to the scene and pronounced the victim dead at the location. New York State Fatality Assessment and Control Evaluation (NY FACE) investigators concluded that to help prevent similar incidents from occurring in the future, individuals operating backhoes should: 1. The Nebraska Workforce Development, Department of Labor's Investigator concluded that to help prevent future similar occurrences, employers should: 1. ensure that all equipment safety devices are in operational condition before being placed in service. 2. ensure that all workers are aware of the basic elements of a Lock-out/Tag-out program. 3. ensure that workers do not stand over, on or across equipment or machines while in operation. 4. ensure that equipment is completely shut down prior to performing any cleaning or maintenance tasks. Department of Labor and Industries AB - On August 1, 2002, a mason working from a scaffold to build a block wall, leaned forward over the wall he was working on and fell 25 feet to a concrete surface below. The 32-year-old member of the bricklayer's union was working for a masonry contractor helping to construct a warehouse-style building. At the time of the incident the victim was level trowelling a masonry block wall, the top of which was 24 inches above the scaffold plank on which he was standing. He fell forward over the wall and landed on a concrete surface 25 feet below. Front guardrails or fall arrest systems were not in use. The victim and her partner were working on the shoulder of a six-lane interstate highway. The shoulder was 11 feet wide where they were working. A guardrail was placed at its edge to prevent cars from driving over the drop off onto the embankment. The workers were therefore constrained from moving farther away from the moving traffic. Several areas of signage identifying the work zone were in place before the work zone including one approximately one-quarter of a mile before the area where they were working. An electronic message board, message side facing traffic, was mounted on the back of a trailer. The victim, on the hitch side of the electronic message board, had plugged her laptop computer into the board to check the calibration of the "Do Not Pass" message. She was positioned with her back to oncoming traffic. The company vehicle in which she and her co-worker had driven to the site was parked a slight distance beyond her. A car swerved off the road onto the shoulder striking the sign, trailer, and workers. The victim was crushed between the message board trailer and the company vehicle. Her co-worker was propelled 50 feet from the point of impact down the embankment and received serious injuries to his legs, pelvis, hands and face. He was hospitalized in critical condition. He is continuing to recover from his injuries. Recommendations: 1. Employers/highway construction contractors should develop and implement an accident prevention plan prior to beginning a construction project that addresses specific worksite hazards and how to minimize or eliminate them. 2. A truck-mounted attenuator should be placed in the proper position to protect traffic control installers working on road shoulders from high-speed traffic when they are setting up signals. 3. Employers/highway construction contractors should use traffic control variable message boards that can be programmed in advance or programmed from a remote location, so they can be put into position with minimum time spent in close proximity to high-speed traffic flow. 4. All entities concerned with work zone worker safety including the Department of Transportation, Traffic Enforcement, and the Michigan Occupational Safety and Health Administration (MIOSHA) should continue their efforts to remind drivers of their responsibility to proceed through highway work zones attentively and according to the posted signs. Since July 1997, the Injury Prevention Service of the Oklahoma State Department of Health has been collecting data on work-related injury deaths in Oklahoma through the Fatality Assessment Control and Evaluation (OKFACE) project, which is funded by the National Institute for Occupational Safety and Health. Data were collected from death certificates, the Office of the Chief Medical Examiner, Occupational Safety and Health Administration, Public Employees Occupational Safety and Health Program, Worker's Compensation Court, and newspapers. Department of Health AB - A 20-year-old farmer (victim) died after he became entangled in a silo auger. The inside of the silo was illuminated by natural light that entered the silo through skylights in it's dome and a small opening near the bottom of the silo. This made it difficult to see anything inside the silo without looking through the small opening at the base of the silo. In the floor of the silo was an unloading auger that extended to the center of the floor. A gearbox in the center of the floor at the auger intake opening connected one end of the unloading auger to a sweep auger located inside the silo. The sweep auger extended from the center of the silo to the silo wall and was used to remove feed when it no longer flowed to the center of the silo. When running the sweep auger traveled clockwise around the silo floor. The unloading auger extended beyond the outer edge of the silo and dumped into a horizontal conveyor located beneath it. One end of the conveyor was inclined to provide an elevated discharge point so a portable feed mixer could be parked beneath it. The victim and his brother were doing chores at the farm however they were not working together at all times. The victim parked a feed mixer at the end of the silo conveyor and was filling it with corn, hay and commercial dairy feed supplements. The silo containing the corn only had about 6-8 inches of corn in it. The victim turned on the unloading auger, the sweep auger and the horizontal conveyor to transferring corn from the silo to the mixer. While mixing the feed, his father stopped at the farm, picked up his younger son and drove to a nearby town. When they returned about 30 minutes later, the father dropped his younger son off and he drove to a nearby farm where he lived. The victim's brother soon noticed that the mixer was still parked where it was when he left the farm. He investigated and found all of the equipment running but he could not find his brother. He looked through the opening at the base of the silo and saw his brother caught in the sweep auger. He stopped the augers and then drove to his father's farm and informed him of the incident. Emergency personnel were called and then the victim's family returned to the scene. Emergency personnel arrived shortly after family members returned to the scene. They and the victim's father entered the silo and determined that the victim was deceased. A coroner arrived and after the victim was examined by the coroner, he was removed from the silo. The victim's cell phone and his pliers were found on the ground outside the silo and directly below the small opening at the base of the silo. The victim was wearing a hooded sweatshirt that had a drawstring to tighten the hood. Based on the evidence at the scene, the victim probably looked into the silo to determine how much corn was still in it. While doing so, the sweep auger apparently moved past the opening and the ends of the drawstring in the victim's hooded sweatshirt became entangled in the auger. Unable to free himself or stop the augers, the victim was pulled into the silo. MN FACE investigators concluded that to reduce the likelihood of similar occurrences, the following guidelines should be followed: 1. all machines and equipment should be turned off and completely stopped before any component or aspect of a work place environment is evaluated, 2. an adequate artificial light source should be available to illuminate those areas of work place environments that are poorly illuminated AB - and 3. loose-fitting clothing should not be worn near operating machines and equipment.
