Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The worker was standing near the left rear of his vehicle using a locator device when struck. The 18-year-old motorist in the incident was reportedly using a cell phone when her vehicle drifted off the roadway. The victim was pronounced on scene by the medical examiner. Recommendations: 1. Analyze the work site including traffic patterns and plan the work zone, before you begin working. 2. Position work vehicle to create an obstacle to prevent oncoming traffic from hitting you. 3. Minimize exposure to moving traffic. 4. Drivers should not engage in activities that distract them from driving or hinder driving performance. Ensure that all machines are properly guarded. 2. Ensure that machine operators are trained to operate machines safely. 3. The hooktender, the employer's representative responsible for supervising the logging crew, and a choker setter, had extended the skyline to a new tailhold, without running the lines in and pulling a new layout in a straight line to the new location as originally planned. The jump put a 90-degree angle in the cable, with the slash pile in its path as it was drawn taut. The hooktender gave a signal to raise the skyline before he and the choker setter were in the clear. They were standing in front of the tailhold stump, less than 15 feet away, and had just turned to move uphill to a safe spot, when the skyline cable broke through the slash pile and whipped outward and back. The heavy cable struck the hooktender in his chest, knocking him into the choker setter. The choker setter sustained a fractured arm. The hooktender was knocked unconscious. The call of a man down brought the whole crew of eight to the scene, plus the employer and landowner who were also onsite. The victim was put on a stretcher and carried to a nearby logging road. He regained consciousness briefly and complained of intense pain. He tried to stand, but immediately fell, no longer breathing. Resuscitation efforts were unsuccessful. Local ambulance personnel arrived and transported the victim to a landing site for a life-flight helicopter, where he was pronounced dead. Recommendations: 1. Where possible, string lines to avoid hang-ups, and always position yourself in the clear of all lines being moved. 2. The hooktender must plan operations in advance and effectively communicate with coworkers to assure the safety of the logging crew. 3. Employers and employees should avoid extended work schedules, or drug and alcohol use during work hours, to reduce the risks created by fatigue or impairment. On the morning of January 2, 2002, the foreman instructed the victim to check to see if the door was operating properly before the vacuum cooler was placed back into service. Using the control on the pneumatic valve, the victim partially raised the door. As he positioned himself between the door and the rear of the cooler tube, one of the air lines on the pneumatic valve blew off, allowing the door to lower, pinning the victim between the door and the rear of the cooler tube. The foreman heard the rush of air and walked around the tube, finding the victim pinned. He held the air line on the pneumatic valve by hand and raised the door allowing the victim to fall to the shop floor. Coworkers initiated cardiopulmonary resuscitation while the foreman called 911 from the shop office. The emergency medical service (EMS) responded and transported the victim to the hospital where he was pronounced dead. The victim, who was working alone at the time, was constructing a skid road. It appeared that the victim had finished constructing a road parallel to the hillside and then backed down onto the steep hillside below the road. Tracks indicated that as he attempted to go back up the hill, his forward progress ended with the dozer's tracks spinning and digging deep ruts. He then began to back further down the slope. At some point the dozer began to slide backwards until reaching a near vertical grade above a creek. The dozer continued to slide down the steep grade approximately 70 feet until it rolled and landed on its side in the creek. The unrestrained operator was pinned under the rear support member of the dozer's rollover protective structure (ROPS). Later that evening, two acquaintances of the victim became concerned after seeing his car parked on the road above the logging site. They discovered the overturned dozer and the victim pinned beneath and immediately contacted EMS who arrived shortly thereafter. The victim was transported to a medical facility and pronounced dead. The WV FACE investigator concluded that to reduce the likelihood of similar occurrences, employers should: 1. Ensure that machines, including those provided by employees, are maintained in serviceable condition and that available safety devices are used while the equipment is being operated. Two days prior to the incident, the decedent had worked for her brother cleaning and organizing materials on the ground at another location. It was the decedent's first day on this job site. It is unknown whether or not the decedent had worked on other roofing jobs. Her job was to organize materials and hand her brothers tools. Just before falling, she had handed one brother a hammer. Neither brother saw her fall, but they heard the crash of guttering falling and found their sister on the ground below. She was not wearing any personal fall protection nor was a fall protection system in place at the time of the incident. She was transported to a local hospital where the coroner pronounced her dead due to multiple blunt force injuries, due to fall from a great height. To prevent future occurrences of similar incidents, the following recommendations have been made: 1. Contractors hiring subcontractors should take responsibility for ensuring through contract language that the subcontractors have an adequate safety program and understand and follow general safety practices and applicable Occupational Safety and Health Standards. 2. Both roofing employers and subcontractors should provide comprehensive fall protection training and personal fall protection for their employees that is site specific. 3. Employers should ensure workers receive safety training in their native language and be informed of hazardous and unsafe working conditions. Fire investigators theorize that a small pinhead leak developed in the lines while the victim was welding. This leak allowed the pressurized hydraulic oil to escape and atomize into the immediate work area. Sparks from the welding process were the ignition source. There were no eyewitnesses to the incident. However, it is estimated that the victim was exposed to the flames for between 2-4 minutes. Coworkers were successful in extinguishing the fire, but the victim sustained burns to (80%) of her body and critical inhalation injuries to her airways and lungs. The victim was airlifted to a local hospital's burn unit where she remained in critical condition until her death. Recommendations: 1. Employers and employees should assure that equipment is appropriate for the task and maintained to manufacturer's specifications. 2. Daily checks of all hydraulic equipment should include inspection of hydraulic hoses and connections. 3. Hydraulic lines should be relocated and protected from physical damage. 4. Check all safety equipment and ensure that it is operational, appropriate for the task and that employees know and understand how to use it. 5. Employers should maintain a current list and copy of MSDS's for the chemicals in use in the workplace, and employees should be appropriately equipped and trained for emergency response. The Massachusetts Department of Public Health concluded that to prevent similar occurrences in the future, schools that have woodworking shops with have table saws should: 1. Ensure that the manufacturers' supplied guards are installed on all table saws, and that the guards function properly and are used at all times. 2. Provide woodworking class conditions that at a minimum meet occupational safety and health standards set by the Occupational Safety and Health Administration (OSHA), and train students about these standards. 3. Position tables at the backside of table saws to prevent the work pieces from falling off the saws and onto the floor. 4. Design the woodworking shop layout to reduce distractions to equipment operators. The boy was operating a utility tractor equipped with a homemade rollbar and seatbelt. They had successfully removed several posts using the tractor's three-point hitch lift arms, but encountered a post that wouldn't budge. The grandfather switched to another method to pull posts using the rear wheel, something he had learned from his father many years ago. They backed up the tractor into position, adjacent to the 6-inch (15 cm) diameter wooden fence post. The tractor was facing up a slight slope. An old chain with 1/4 inch (6 mm) links was secured to the bottom of the post and the other end was looped around the rear tire. Normally, as the wheel would turn, it would pull the post upward and forward. The boy had the tractor in first gear, with throttle applied. As he released the clutch, the post, buried 4 feet (1.2 m) deep in dry ground, did not move, but the tractor's front end jumped upward. The grandfather yelled, but the boy had no time to react, and the tractor rolled completely over backwards. The self-made rollbar was sturdy and did not break or bend, but large pieces of the axle housing snapped off from both sides as the rollbar collapsed. The boy was instantly crushed underneath. The tractor had been previously used for city maintenance, and originally had a two-post factory-installed Roll-Over Protective Structure (ROPS). The tractor was sold through an auction and was bought by the family without this ROPS. The original mounting brackets were still in place, but the long bolts connecting the top and bottom brackets were missing. The new rollbar was welded to the existing top bracket, which was bolted only to the topmounting base of the cast iron axle housing. Without the long bolts between the top and bottom brackets, this installation did not provide adequate strength for the rollbar attachment to the axle, and the axle housings broke, making the rollbar ineffective. Recommendations based on our investigation are as follows: 1. Factory installed Roll-Over Protective Structures (ROPS) should not be removed from tractors. 2. Non-ROPS tractors should be retrofitted with approved ROPS AB - otherwise, they should be recycled, or retired to stationary power source use. 3. Self-made rollbars are not recommended, as they may not provide adequate structural strength in an overturn. 4. The rear wheel of a tractor should not be used to pull posts from the ground. 5. Youth under 16 years of age should not operate tractors, unless they have received tractor operator training. The other operator and the victim's father believe that he may have stepped off the machine to observe its operation or check on something. The victim fell under the left rear wheel and was crushed. The reason the victim fell remains undetermined but it is believed that he stumbled or tripped on the uneven ground. Recommendations: 1. Farm operators should not assess for seed loss while the combine is operational or moving. 2. Stop the engine before doing any checks, adjustments, repairs, lubricating or clearing any obstruction, or when leaving the cab or operating platform for any reason. 3. Always maintain a safe distance from all moving machinery. 4. Rural emergency medical systems must have the ability to rapidly locate trauma victims and transport them to the appropriate level of care. Federal, state and local agencies should work together to encourage the development and deployment of Enhanced 911 (E-911) technology and systems. It had three main components, a three-bar reel, a pick-up head and a steel collection hopper. The pick-up head was controlled by two hydraulic cylinders, one located on each side of the unit. These cylinders controlled the pick-up head and also raised and lowered the hopper to empty the unit. The day before the incident occurred, an employee of the victim used a tractor and the rock picker to clear rocks from a field until a failure occurred in the unit's hydraulic system. The employee left the tractor and rock picker in the field and notified the victim of the break down. The next day, the victim drove his pickup truck to the field where the rock picker was to repair it. Although a failure had occurred within the hydraulic system, he was able to raise the hopper to its dumping position. He also hooked one end of a chain to the front edge of the hopper and the other end to the hitch of his truck. After raising the hopper he entered the area directly behind the pick-up head which is the area the hopper occupies when lowered. Several wrenches were found near the victim which indicated that he entered the area to repair the hydraulic system. Apparently while repairing the unit, something changed such that the raised hopper could not be held by the parked truck. The hopper lowered to its down position and the victim was pinned beneath it. Two employees of the victim happened to drive by the field, thought things seemed out of place and investigated. They found the victim pinned beneath the hopper and placed a call to emergency personnel who arrived shortly after they were contacted. They checked the victim for signs of life and determined that he was deceased. They placed inflation bags under the rock picker, inflated the bags and removed the victim. MN FACE investigators concluded that, in order to reduce the likelihood of similar occurrences, the following guidelines should be followed: 1. Equipment should be designed with safety devices to support raised components while maintenance or general repair work is being performed AB - and, 2. On July 29, 2003, a bulldozer owner/operator drowned in the cab of his machine as he was trying to find a leak in a pond dam. The operator had been hired by a local farm owner to find and repair a leak in one of the ponds on her farm. As the operator dug a trench through the earthen dam, a section at the top collapsed sending sludge into the trench. The water/sludge mixture filled the cab of the bulldozer, trapping the operator inside. When the owner/operator did not return home as expected, his wife called the pond owner. The pond owner and the operator's wife searched the pond area for the bulldozer operator. They found the water-filled trench next to the pond, but not the bulldozer or the operator. Emergency medical services (EMS) were called to the pond. After arriving at the scene, EMS contacted a diver who then found the bulldozer submerged in the water with the decedent inside. The coroner stated the cause of death as due to drowning. 1. Ponds should be drained down before repairs begin. 2. Bulldozer owner/operators should follow Occupational Safety and Health safety guidelines. 3. Bulldozer operators should leave themselves an escape route when working in dangerous spaces. It is thought that he was looking at a structural area previously identified by a building inspector as needing a support plate on the floor. The roller coaster operator did not see the victim enter the area or see the coaster strike the victim. When the roller coaster came around the track curve, it either struck him or a piece of the front roller coaster car caught on a piece of his clothing and dragged him on the track. The coaster ran over the back of the victim's neck and derailed. 911 was called and emergency response arrived. The victim was declared dead at the scene. Recommendations: 1. Ensure that lockout/tagout procedures are followed when servicing machines and equipment. 2. Provide lockout/tagout training to employees to ensure that the purpose and function of the energy control program is understood. 3. Prohibit employee entry into a ride operation area while the ride is in motion. 4. Place warning signs at the back entrance of the ride area prohibiting entry via this entrance unless the ride is locked/tagged out. 5. Ensure that all forklifts are inspected daily and those not in safe operating condition are taken out of service until competent maintenance work can be performed. 2. Ensure that all company employees are trained on forklift pedestrian safety. 4. Ensure that all forklift operators receive documented formal training on the Occupational Safety and Health Administration's Powered Industrial Truck Standard. 5. The mixer had undergone repair, and its u-joint and PTO shaft were unguarded. The tractor PTO lever was located on the operator's right side while seated in the tractor. The mixer had a moveable scale that was attached to a stationary arm that could be moved to face either side of the mixer. The victim loaded the mixer with supplements and then had driven the tractor/mixer to the bunker silo area to add haylage to the supplements. He drove the tractor into the bunker area from a north to south direction instead of his usual approach, which is in a south to north direction. He had dismounted from the tractor and left the tractor running AB - it is unknown if the PTO was engaged or disengaged when he dismounted. He was working alone, and the event was unwitnessed. His left hand/arm became entangled in the rotating PTO. His clothing was ripped off and his left arm severed at the shoulder. Another employee found the victim with his head by the driver side rear tractor wheel and his feet facing the mixer. This employee turned off the tractor and contacted another employee for help. The second employee ran to assist the victim AB - when the second employee determined there was nothing he could do, he called the farm owner to obtain the exact farm location address and then called 911 with this information. Emergency response arrived and the victim was declared dead at the scene. Recommendations: 1. All rotating shafts, including PTO drivelines, should be covered by shields/guards in good condition to prevent worker contact with rotating parts. 2. Tractor operators should engage/disengage the PTO while seated in the tractor operator seat. 3. Loose or frayed clothing that can get caught in machinery should not be worn. 4. Employers should establish a safety plan that includes a farm emergency plan that identifies emergency responsibilities of individuals working on the farm, an emergency contact list, and farm location by the telephone.
