Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The forklift started to tip and he attempted to either jump away from the forklift or was thrown from the operator's seat when the forklift overturned. The forklift's falling object protective structure (FOPS) landed on the victim's chest and abdomen area. A coworker found the victim and yelled for help. Other coworkers ran to assist the victim and they were able to lift the forklift enough to free the victim. Coworkers then began cardiopulmonary resuscitation (CPR) while Emergency Medical Services (EMS) were notified. EMS responded to the incident site within minutes and transported the victim to a local hospital where he was pronounced dead. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Provide adequate supervision for young workers, new employees and inexperienced workers. 2. Establish work policies and procedures that comply with federal and state child labor laws prohibiting youth less than 18 years of age from performing hazardous work, including operating forklifts. 3. Ensure that forklift keys are kept in a locked location and only accessible by trained and authorized employees. 4. Develop, implement and enforce a written comprehensive safety program that includes training in safe operation of forklifts. In addition, employers, distributors, and manufacturers should: 5. Place warning stickers on forklifts that state that forklifts are not to be operated by workers under 18 years of age. The silage defacer was attached on the front of the tractor, approximately four feet off the ground. The worker energized the defacer unit while the tractor was idling near the farm shop, then proceeded to do some work in the shop area, and while doing this walked in front of the energized defacer unit. The sleeve of the victim's coat became caught in the unit and he became entangled around the moving parts of the machine. The farm owner, who was working nearby in the farm shop, heard an unusual sound and went to investigate. Upon arriving at the scene, he immediately turned off the power to the defacer unit, shut off the tractor engine and then called 911. The farmer disentangled the victim from the machinery and began CPR until a local EMT arrived. Shortly after, the rescue squad and state police arrived. The victim was subsequently pronounced dead at the scene. New York State Fatality Assessment and Control Evaluation (NY FACE) investigators concluded that to help prevent similar incidents from occurring in the future: 1. Silage defacers and other powered equipment attachments on tractors should only be energized while the operator is in the machine operator position AB - 2. Workers should not come in close proximity to operating pieces of equipment such as defacers or other machines with aggressive entanglement points and AB - 3. Employers should ensure that all operators of equipment are thoroughly trained on hazardous pieces of equipment and ensure employees follow safe operating procedures. Additionally, equipment manufacturers should consider: 4. Incorporating automatic shut off features into equipment designs such that when the operator leaves the operator seat position, the equipment deenergizes and AB - 5. The loading dock leveler had a safety bar underneath the dock plate to hold it when in the raised position, but the safety bar was not used at the time of the incident. The method used to perform the lift was not the one that the company recommended. The CA/FACE investigator determined that in order to prevent future occurrences, employers, as part of their Injury and Illness Prevention Program (IIPP), should: 1. Ensure employees never place themselves under a suspended load. 2. Ensure employees follow recommended procedures when replacing the dock levelers. Methods: The Michigan Fatality Assessment and Control Evaluation (MIFACE) program investigated this work-related fatality to identify the factors that contributed to his death. The investigation included site visits, personal interviews, and a review of pertinent documents. Results: Factors contributing to his death included: the vehicle type; lack of engineering controls; lack of a company safety and health program, including a personal protective equipment program, medical monitoring, and employee training; and a lack of recognition of work-relatedness of the deceased's respiratory problems by a health care provider. Conclusions: This work-related fatality investigation demonstrates the need for isocyanate manufacturers/formulators to inform end users of their products of appropriate health and safety work practices associated with new technologies. In addition, health care providers need more education and assistance to recognize and manage work-related asthma. The victim entered the facility's pumping plant to take some measurements for a planned project. Due to a verbal miscommunication, he believed the area he was accessing was de-energized and did not use the established lockout/tagout procedure agreed upon by the contractor and the Bureau. The victim also did not use a high voltage tester to determine if the circuit was energized. He climbed a 6-foot ladder to access a breaker cabinet, opened the panel of the enclosed disconnect cabinet, and reached inside where he made contact with the energized 13,800 volt bus of the disconnect switch. He received an electrical shock and fell 4 feet from the ladder. He was found a few minutes later by members of his crew who administered emergency aid. The victim was taken unconscious to a hospital where he died 18 days later of complications from the effects of contact with electrical current. Working alone very early in the morning, the farmer prepared the manure for removal by agitating the pit with a manure pump attached to a tractor power take off (PTO). After starting the agitation process, the farmer entered the building to retrieve a dead hog that he had noticed inside of the building (Photo 1). The mechanical ventilation system in the building was not in use and the building was totally enclosed at the time of his entrance. Later in the afternoon the farmer was found lying face down in the center of the building by his fiancé, who immediately called 911 for help. Upon arrival, emergency rescue personnel donned appropriate personal protective equipment and removed the farmer from the confinement building. Examination of the farmer revealed that he had been deceased for several hours prior to the discovery of his body. Recommendations based on our investigation are as follows: 1. Hazards of working in and around manure pits should be regularly and effectively communicated to farmers. 2. Pits and confinement areas should be ventilated before, during and after the agitation of manure. 3. No workers should be inside animal confinement buildings during agitation of manure pits beneath the buildings. 4. Farmers should have proper respiratory protection and a rescue capability in place when entering a manure pit. The victim had just finished filling and disconnecting the nurse tank from the anhydrous ammonia transfer system, when a seam at the base of the cylinder near the front split open, causing a rapid expansion, and subsequent explosion of the escaping anhydrous ammonia. The force from this event threw the coworker against a nearby pickup, and knocked him unconscious. The victim shut off the anhydrous ammonia transfer system and went to aid his fallen coworker. Emergency response personnel arrived at the scene and stabilized the victim and coworker. Both men suffered chemical burns to more than 50 percent of their body surface area, and had eye and respiratory system injuries. The coworker survived his injuries but the victim died thirteen days later from complications associated with the incident: pneumonia due to inhalation burns. Recommendations based on our investigation are as follows: 1. Owners of liquefied anhydrous ammonia nurse tanks should institute periodic testing programs to ensure that the tanks remain structurally sound. 2. Employers and operators should ensure that emergency response procedures are in place and utilized by workers to adequately protect them from hazards. The elevator was powered by a typical tubular PTO (power take-off) shaft from the tractor. After emptying one side of the crib, two of the men were cleaning out the tunnel on that side while the victim was unloading corn from the other end of the crib. They heard the tractor engine slow down momentarily and went to investigate. The victim was found wrapped around the unshielded PTO shaft powering the elevator, with obvious severe injuries to one arm, leg, and his chest. After rescue crews arrived, the victim was transferred to a local hospital, where he was pronounced dead. Recommendations based on our investigation are as follows: 1. All PTO drivelines and other exposed drivetrain components on farms should be completely shielded to prevent accidental contact by workers or helpers AB - 2. The boy jumped out of the truck to hitch up the wagon, and when he lifted the tongue on the wagon, the wagon apparently started to slowly roll forward toward the truck. The boy failed to get the tongue hitched to the rear of the truck, and the wagon continued its slow forward roll, pinching the boy's head between the wagon and the rear of the truck's metal bed. The farmer, sitting in the driver's seat of the truck, felt nothing unusual. Suddenly, not seeing the boy, he jumped out and ran back to find him slumped over the tongue of the wagon, and a large pool of blood on the ground. The boy was dead at the scene. The tongue of the wagon had bulldozed into the ground, indicating that the wagon had moved forward. Recommendations based on our investigation are as follows: 1. Farm wagons without parking brakes should always have their wheels chocked to prevent them from rolling, especially when parked on rolling or sloping ground. 2. When hitching wagons to towing vehicles, farmers and farm workers should stand safely to one side, out of the hitching area, where they can see the hitch and communicate directions to the vehicle operator. 3. Manufacturers of farm wagon running gear and other wagons should consider providing parking brakes, chocks, or other reliable means to secure wagons when they are parked. Normally, workers were required to use their own vehicles for commuting between farm locations, but the tractor was available and was used this time. While driving down a gravel road early in the morning, he evidently lost control and veered into the right-side ditch, where the tractor rolled over 3/4 of one revolution. The operator was found dead by a coworker, pinned to the ground by the left rear tire. This tractor was about seven years old and was equipped with a foldable Rollover Protective Structure (ROPS). The ROPS was not in the upright position at the time, and provided no protection for the operator in the overturn. The operator was not wearing a seatbelt, and was thrown from the tractor when it rolled. There were no known environmental factors that contributed to this incident. The machine was being repaired, and the loader's seat, battery cover, and battery were removed from the floor of the operator's station. Hand operated controls had been disconnected from their control rod linkages, which connect them to the valves that govern hydraulic flow to the power lift arms, forklift, and skid steer movement. The lift arms, with mounted forklift attachment, were partially or fully raised. The tall repairman was in the zone bounded by the frame across the lower front of the skid steer loader, the back frame of the raised forklift attachment, and the lift arms extending forward along each side of the protective metal structure around the operator's station. He was leaning forward into the operator's station to accomplish his repair tasks. The repairman was working alone inside his shop in the middle of the afternoon. A customer had stopped by sometime after noon with additional work to be performed, and planned to return in an hour and a half. Upon returning to the shop, the customer discovered the repairman pinned against the front of the skid steer frame by the back of the forklift attachment, which was now resting near the ground. The repairman, nearly to his knees on the dirt floor, was found facing into the operator's station with his head tilted outward near the left lift arm. The customer ran to the repairman's residence adjacent to the shop to summon Emergency Medical Services. He returned and placed a floor jack under the forklift attachment in an attempt to extricate the repairman, who was subsequently pronounced dead at the scene. Actuation of the control linkage allowing the lift arms to lower is the key contributing factor in this unwitnessed incident. Recommendations based on our investigation are as follows: 1. Skid steer loader operators should lower lift arms to the ground before exiting the operator's station. Operator's and those repairing skid steer loaders should always use lift arm locking mechanisms provided by the manufacturer and ensure that they perform properly before exiting or working under any portion of the raised lift arms of the loaders. 2. Operators and repair personnel should be educated and trained to recognize, assess, and act to avoid the risks of lift arm movement during skid steer loader operation, maintenance, and repair. 3. Skid steer loader manufacturers should consider means to prevent lift arm lowering when the control to lower lift arms is actuated and the skid steer's engine is not running. 4. Means to secure skid steer loader lift arms such as capable blocks, jacks, and hoists, which prevent unintended movement of lift arms during repairs performed with the lift arms in a raised position, should be employed prior to performing such repair tasks. During the process, the anchor began to wobble and the extended boom contacted the overhead power line. Apparently unaware that the boom was in contact with the overhead power line, the victim grabbed the energized anchor with both hands in an attempt to stabilize it and remained in contact with the energized anchor until the boom was moved away from the power line. Electrical current moved through the victim's body from his hands to ground through his feet. The boom truck operator immediately called 911 on his cell phone and emergency medical services (EMS) arrived in about 4 minutes. CPR was immediately initiated and the victim was transported to the hospital where he was pronounced dead. NIOSH investigators concluded that, to help prevent similar occurrences, employers should: 1. conduct a jobsite survey to identify potential hazards and develop and implement appropriate control measures for these hazards AB - 2. follow existing OSHA regulations and safe work practices concerning the operation of cranes or equipment in close proximity to overhead power lines, and take steps necessary to de-energize or insulate power lines before work begins AB - 3. ensure that when working near a high voltage overhead power line where visibility could be obstructed or clearances difficult to determine, an observer is used to help the operator maintain the required clearance AB - and, 4. develop, implement, and enforce a comprehensive written safety program for all workers which includes training in hazard recognition and the avoidance of unsafe conditions. A written training plan should require training for all ground crew in electric utility operations. His wife returned to her work and within seconds she heard the tractor start to roll over. She saw the tractor roll upside down over her husband. She ran over to the tractor and yelled to her 11-year old son to call 911. She called for help as she tried to get him out from under the tractor. There were no neighbors in sight at the time. She ran to the house and notified the emergency medical system (EMS). She quickly returned to her husband and stayed with him until the emergency personnel arrived. They arrived within six minutes after they received the call. A medical helicopter arrived and their physician pronounced the victim dead at the scene. FACE investigators conclude that to help prevent similar occurrences, farmers should: 1. Retrofit all tractors with rollover protection structures (ROPS) and seatbelts. 2. Wear a seat belt when operating a tractor equipped with ROPS. 3. Use extreme care when operating tractors on or near sloped terrain. 4. He was mixing and feeding silage to his herd of heifers. Hay silage from an oxygen-limiting silo was mixed with corn silage from another silo, and the conveyor belt carried the mixture to a feed bunk 60 feet (27 m) long. The mixing took place in a shed between the two silos, which were approximately 10 feet (3 m) apart. Controls for the silo unloaders were mounted on the walls of each silo. The hay silo had a control switch box, while the corn silo was controlled from a breaker panel. This panel also controlled the conveyor belt motor. This configuration of switches required the farmer to walk around the exposed end of the conveyor belt at least three times during each feeding cycle. The shaft for the conveyor belt pulley extended about three inches (75 mm) beyond the bearings on each side. A keyway was cut into the length of the 1-inch (25 mm) diameter shaft, and there was no guard around the conveyor belt or the shaft. The injury occurred when the farmer apparently leaned or bent over too close to the shaft. The hood of his nylon jacket became entangled in the exposed shaft end. The hood and arm of his jacket wound up in the shaft strangling him and stalling the conveyor. Silage kept piling up from the silo, and the conveyor motor and drive kept running, eventually burning through the west end of the stalled conveyor belt. The victim was discovered by his wife. He had suffered fatal injuries before rescuers arrived. Recommendations based on our investigation are as follows: 1. Machinery and equipment must be guarded properly to avoid entanglements. 2. Control switches and stationary machinery should be placed so that there is no need to work near hazardous machine parts. 3. Farmers should carry a communication device for emergency situations. 4. Farmers should wear appropriate, well-maintained clothing for farm work. This mixing and feeding system had been used for the past 20+ years to supply an outdoor cattle feeding operation. The configuration of conveyors and controls in the shed required the farmer to crouch down and crawl under the feed bunk conveyor several times during each feeding cycle. There was only about 3 foot (90 cm) clearance between the conveyor and the floor. The farmer was accustomed to ducking under the conveyor and had done so for many years. There were many unguarded moving parts in this work area. The shaft for the pulley at the end of the conveyor was too long, extending about 2.5 inches (6 cm) beyond the bearings on both sides of the pulley. The shaft had a keyway, which created sharp-cornered entanglement points on both ends of the shaft. The injury occurred while the farmer was ducking under the conveyor belt. His coat collar was caught and wound up in the exposed rotating shaft. He became entangled and the conveyor belt stalled as the coat wrapped around the shaft. He was found dead at the scene several hours later in the afternoon by a family friend. Recommendations based on our investigation are as follows: 1. Injury Prevention Service AB - A 49-year-old production worker died on April 10, 2005 from chest and abdominal trauma he received after being pulled into a tread scrap machine. The victim was employed by a temporary staffing agency and placed in a manufacturing facility as a production worker to operate a tread scrap machine. As the victim was feeding tread liner into the take-up spool of the tread scrap machine, his right hand became entangled between the liner and take-up spool and he was pulled into the machine. No witnesses were present during the incident and coworkers did not hear the victim call for help. A cleaning employee found the decedent's body wedged in the take-up spool and notified the occupational health staff who immediately called 911. Employees dismantled part of the machine to free the victim and cardiopulmonary resuscitation (CPR) was initiated. Emergency medical services (EMS) arrived and transported the victim to a local hospital where he was pronounced dead in the emergency room. Oklahoma Fatality Assessment and Control Evaluation (OKFACE) investigators concluded that to help prevent similar occurrences, employers should: 1. Ensure that all machines have adequate engineering controls and guards to minimize employees' exposures to hazards. 2. Monitor compliance of safe machine operating procedures through random inspections and observations by a competent person. 3. Develop written, machine-specific safe operating procedures to assess and mitigate employees' exposures to hazards. In addition, temporary employment service agencies should: 4. Work with secondary employers to establish specific job descriptions, training criteria, and hazard analyses of each job assigned to temporary employees. He instructed the decedent to wait for him, but the decedent decided to leave without him once he was finished with his task. The forklift was only equipped with one flashing light on top AB - there were no slow-moving vehicle emblems. The decedent was not wearing the seat belt. After crossing to the inside lane of the highway to make a U-turn, the forklift was struck by a vehicle that had just come over a hill and did not have time to react. The decedent was ejected and pinned for 50 minutes under the forklift that landed on him. He was pronounced dead at the scene. Oklahoma Fatality Assessment and Control Evaluation (OKFACE) investigators concluded that to help prevent similar occurrences, employers should: 1. Consider using a trailer or other means for transporting mobile heavy equipment, such as forklifts, between work sites. 2. Ensure that forklifts and other powered industrial trucks are not driven off work premises and particularly not on public highways. 3. Ensure that employees wear a seat belt while operating forklifts or other powered industrial trucks. 4. Develop, implement, and enforce a training and certification program for operators of forklifts and other powered industrial trucks. He had been picking up tree limbs, removing brush piles, and working to extract a large tree stump in an area of the cemetery with sloping terrain. He used a chain saw to cut through the roots of the previously felled tree so he could hoist the old stump from the ground and place it into a nearby wagon, already partially filled with tree branches and trimmings. The tractor was aimed at an angle forward and downward across the slope, tilting to the left as he attempted to lift the heavy tree stump by raising the front-end loader. The stump was secured to the loader by a log chain around it and the loader bucket. As the loader bucket raised well above the height of the hood of the tractor, the tractor tipped onto its left side and continued to roll onto its top, stopping upside down on top of the operator. The victim was found by another worker who came to the cemetery to mow grass later in the day. The victim had been crushed between the ground and the steering wheel of the tractor and was pronounced dead at the scene. Because the loader lift arms, and therefore the loader bucket, were raised high the tractor was protected from damage in the overturn. Although there were other tractors reasonably available for the victim to use, none of them were equipped with a loader. The victim had also been drinking alcohol and this likely influenced his assessment of the equipment, loadings, terrain, and overall risk of an overturn. RECOMMENDATIONS: (1.) Tractors suitable for the task, properly equipped with a rollover protective structure (ROPS) and seat belt, configured and ballasted appropriately, should be used with a capable, properly-installed front-end loader recommended for the tractor. (2) Operators should be educated and trained to recognize and assess the risk of an overturn and the factors that contribute to an overturn. (3) Workers should not operate tractors while under the influence of alcohol or when taking medications for which doctors advise against the operation of machinery. Another worker had been assigned as a ground person during this construction activity. The ground person indicated he had walked to the front of the unit when he heard a scraping sound. He called out but received no response. As he called out, he and two other employees looked up saw the decedent pinned with his back against a 6-inch diameter iron fire protection water pipe and his chest against the control panel at the front of the work platform basket. They tried to override the basket panel controls the decedent had been using to operate the work platform, but could not do so, because the key that would allow them to access the controls from the ground was missing from the ground control panel switch. They estimated that three to four minutes passed until they could find a key that allowed them to lower the basket. Another employee used a non-JLG key to gain control of the work platform and lower it to the ground. When they did gain control, the basket and the boom fell abruptly to the dirt floor of the plant. The local fire department and its EMS squad responded to the incident. They found the decedent in the basket on the floor of the plant. Emergency treatment was administered at the plant. He was transported to a local hospital where he was pronounced dead approximately one hour after the incident occurred. Recommendations: 1. The American National Standards Institute (ANSI) should evaluate and consider modifying ANSI A92.S standard titled "The American National Standard for Self Propelled Elevating Work Platforms" and encourage manufacturers and distributors to follow the International Organization for Standardization (ISO) Standard for articulated boom-supported aerial work platforms regarding providing pressure sensor/relief valves on their equipment. 2. Limit the number of brands of aerial lifts purchased or leased by a facility. 3. Tether the key that operates the equipment from the ground controls to the ground control panel. 4. Conduct a written pre-work inspection on equipment and ensure workers do not operate equipment until it is repaired when the pre-work inspection reveals faults such as defeated or missing safety devices. 5. Ensure that employee training is appropriate and specific for each type of equipment workers will be using.
