Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The bulldozer evidently did not slip into gear, but the front blade was elevated, and the bulldozer was on a slight incline and the tracks rested on branches and other woody debris, which may have contributed to instability. The victim was pinned under the bulldozer, and was pronounced dead at the scene.Recommendations 1. Before exiting a bulldozer, the machine operator must apply the brake and lower the front blade to the ground. 2. Never rely solely on the brakes of a bulldozer to prevent movement. 3. Be aware of conditions where bulldozer tracks may be unstable. 4. Develop a site-specific safety plan that includes an assessment of hazards and plans to minimize the risks. He then noticed the coworker had left a jacket in the cab, so he lowered the tractor's blade to the ground, put the machine into idle, and stood on the track to reach out and give the jacket to the coworker. While in this position, the tractor suddenly moved backward, and the operator lost his balance and fell feet first between the track and the deck. The bulldozer continued to roll 8-10 ft backward, and the operator was pulled beneath the tractor and crushed. Coworkers freed the conscious victim, but he was confirmed dead when emergency medical responders arrived at the scene. The medical examiner reported a blood alcohol content of 0.08, the level of legal intoxication in Oregon for operating a motor vehicle. Recommendations: 1. Before exiting a bulldozer, the operator must secure hazardous energy by following all manufacturer recommended steps in parking, including setting the parking brake. 2. Operators should not use alcohol or other impairing drugs prior to or while operating equipment. 3. No passengers should be permitted to ride on mobile machinery, unless a properly equipped seat is provided. Currently, 14 other states also participate in the NIOSH FACE program. FACE is a data collection and research project designed to identify and prevent occupational fatalities. Data are collected on all work-related fatalities. Oklahoma FACE staff investigates incidents involving highway work zones, machinery, youth under 18 years of age, and immigrant workers. Prevention recommendations are made, and information is disseminated to related businesses/industries, safety professionals, and other stakeholders. Each year in Oklahoma, approximately 109 occupational fatalities occur; 20 are machine-related. Forklifts, tractors, and other machinery can be very dangerous and all possible precautions should be taken to prevent injuries and deaths. From 1998 to 2003, 85% of machine-related agricultural fatalities in Oklahoma involved tractors. On average, one forklift-related fatality occurs annually. Jump-starting or bypass-starting mobile equipment can increase the likelihood of serious injury or death. Jump-starting and bypass-starting are dangerous because the operator is required to work closely to the wheels and other moving parts of the equipment (Figure 1). Operators may be run over while attempting to jump-start or bypass-start equipment when the transmission is not in neutral and/or the emergency brake is not engaged. Operators with safety training and up to 15 years of experience have been killed attempting to jump-start/bypass-start equipment. Familiarity with equipment does not protect operators when a risky practice, such as jump-starting/bypass-starting, is being performed. Between July 1997 and February 2005, eight jump-start/bypass-start fatalities occurred in Oklahoma. Seven of the victims were male and one was female, with ages ranging from 35 to 70 years (average age 57 years). Seven victims were operators and one was a secretary/treasurer assisting in a jump-start. Six of the eight incidents involved tractors; one involved a forklift; and one involved a dirt compacting machine. Farming was involved in five of the jump-start/bypass-start-related incidents, while one occurred in road construction, one in mining, and one in machinery repair. Four victims used jumper cables during the incident, and the other four victims used a hand tool to bypass-start the equipment. Seventy-five percent of the victims were performing routine job tasks of their usual occupation during the time of incident. The Nebraska Workforce Development, Department of Labor's Investigator concluded that to help prevent future similar occurrences, employers should: 1. Ensure employees wear appropriate personal protective equipment (PPE) when required. 2. Ensure that tools being used above other workers are secured to prevent them from falling and striking other workers. 3. Retrain those employees that have not retained the requisite understanding and/or skills necessary to protect themselves and others during daily tasks. The tractor and two trailers jack-knifed, traveled through the median with the cab and trailers flipped onto their right sides, slid across the eastbound lanes, then the cab slammed through the guardrail. The cab came to rest on the steep embankment of the shoulder. A passing motorist called emergency services. Emergency personnel arrived, climbed down the steep embankment and found the top of the cab crushed. Both driver and passenger were thought to be alive. However, because of the steep slope and the condition of the cab, rescue personnel could not administer first aid to the two men in the cab. Tow trucks were called to the scene to move the two trailers and pull the cab up the slope and onto the pavement. Using torches, rescuers cut away the metal of the cab to reach the two men. Both men were wearing seatbelts which needed to be cut in order to free them. Emergency personnel assessed both men. The driver was found to be alive and was transported to the nearest hospital. However, the passenger did not have vital signs. The local coroner was contacted AB - he arrived and declared the passenger dead at the scene. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Livestock owners should ensure boundary fences are appropriate for the animal type and regularly maintained to ensure both animal and public safety. Recommendation No. 2: Police should warn motorists when errant farm animals are on interstate highways. Recommendation No. 3: Companies should provide professional training for company truck drivers. Recommendation No. 4: Parkway medians should be designed and constructed with median barriers to deter crossover median crashes. Risk increases greatly over age 35, and again over age 65. Fall hazards are everywhere. Falls from ladders are #1. Please observe the following safety tips. Reccomendations: Make sure ladder is in good condition, base and locks are secure. Set base 1/4 working length from wall, supported at top by both rails extending 3-4 ft above dismount level. Three-point rule: Get a firm grip with three of four limbs, especially in icy conditions. Beware losing your balance from the unexpected release of a weight you are carrying or pulling, or from overreaching. Avoid standing up from a kneeling position next to a ledge, where momentary dizziness can cause you to lose balance. Cover and guard holes securely. Cierpich rccendy spoke al the 12th Annual AgSafe Conference in Seaside, Calif., and had these suggestions for tree care com· panics and other employers that have Hispanic workers: 1. Make your safety training site·specific. Tmin on the job (versus in a classroom or another location). 2. Use "real" photos in your training. In other words. don', be afraid to show your Hispanic workers graphic photos of what could happ::n to them. 3. Provide "hands-on" training. Never assume that one of your workers understands how to operate a chain saw, chipper, or other equipment until a supervisor has monitored (and signed off on) the person demonstraling actual use of the equipment. 4. Encourage employee participation in safety presentations. 5. Keep it simple. Prepare low-literacy materials (including materials in Spanish), and "don't be using college words or $10 words when you can use a 50 cent word." 6. Use "show and tell" sessions to ensure that knowledge has been gained. 7. Encourage teamwork. 8. Make training interesting. 9. Don't tell your workers to "go do it." Instead, "do it with them." 10. Show your Hispanic workers that you care. "Show them that you respect them, and that they are human beings not entities you can push aside." The 53-year-old victim was employed by a heavy construction company. He had worked as a mechanic in the company's heavy equipment and construction vehicle maintenance repair shop for 12 years, servicing and repairing equipment, and replacing and fabricating components. On the day of the fatal incident, the victim and another employee were servicing a dump truck. They thought their work was completed but then realized that they needed to adjust a control cable. To access this cable they raised the bed of the truck and went up onto the truck's frame. They did not use, as they had earlier in the day, the truck's installed safety pole which is used during servicing to prevent the accidental lowering of the bed. According to the surviving employee, their work was only going to take about a minute. The victim was sitting on the frame of the truck adjusting the cable when he realized that the bed was about to come down and yelled at his coworker to get off the bed. The coworker escaped without injury, but the victim was struck and crushed by the falling bed. While performing the role of a spotter, the victim apparently stepped in front of a pile of gravel to direct a truck driver to move his truck when he was struck by a bulldozer that was traveling in reverse. The company owner was operating the bulldozer in reverse, with its back up alarm sounding, and spreading gravel over the excavated area. A coworker, who was sitting in a skid-steer loader located near the gravel pile, saw the victim directing the truck driver and saw the bulldozer backing toward him. He yelled a warning to both the victim and the company owner. The company owner did not hear the warning and the victim did not react. The track of the bulldozer struck the victim on the back of his legs and rolled over his legs and torso. The company owner, still unaware of the run over, drove the bulldozer forward about 50 feet and stopped. The coworker ran to the bulldozer operator and told him what had occurred. The company owner looked back toward the gravel pile and saw the victim lying on the ground. He called 911 using his cell phone. He ran to the victim and began cardiopulmonary resuscitation (CPR). A police officer arrived approximately one minute after the 911 call and assisted in providing CPR. Emergency medical services (EMS) personnel arrived approximately two minutes after the 911 call and found, through examination and cardiac monitoring, that the victim had no signs of life. The county coroner pronounced the victim dead at the site. The lead mechanic had left the work area just before the incident occurred, and the owner was in his office adjacent to the shop. The victim stood over the mounted tire and used a pneumatic impact wrench to either tighten or loosen the bolts on the wheel flange when the tire exploded. The CA/FACE investigator determined that in order to prevent future occurrences, employers, as part of their Injury and Illness Prevention Program (IIPP), should: 1. Ensure employees never perform maintenance or repairs on an inflated tire. 2. Ensure new employees are adequately supervised in the performance of their duties. 3. Ensure that after employees receive specific training, their achievement of skills is verified through a testing program and that all training is documented. The 21-year-old victim was employed by a land subdivision contractor who bought and sold vacant land to be developed for the building of new residential homes. The victim was a college student who had taken the job for the summer and had been working for this employer for about two months. His job was to operate a Kubota compact tractor with a rear-mounted mower or "brush hog" to cut grass and brush so as to maintain vacant land for the housing development. He was not trained in the safe operation of this tractor. Prior to the fatal incident the victim was driving the tractor with the brush hog attachment along an old tractor trail. He was being followed by two co-workers who were riding 4-wheeled all-terrain vehicles. According to a witness, the victim was driving the tractor fast and they were "horsing-around." The victim looked over his shoulder and lost control of the tractor as he was going around a bend. He was ejected from the tractor as it rolled over, and the rollover protective structure (ROPS), or roll bar, crushed him. The tractor had a seatbelt, but the victim was not wearing it. He was declared dead at the scene from blunt chest trauma. Calls were placed to Emergency Medical Services (EMS) and the fire department. Within minutes, EMS and fire department personnel arrived at the site to attend to the victim and control the fire. The victim was pronounced dead at the scene. The co-worker was able to exit the house without injuries. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Use wood floor finishing products that are less flammable (products with flash points greater than 100 degrees F) for indoor applications. 2. Ensure that work areas are adequately ventilated during indoor application of wood floor finishing products. 3. Ensure that ignition sources including gas pilot lights are extinguished prior to beginning work. 4. Conduct job hazard analyses and implement and enforce a safety checklist to be completed prior to beginning work. 5. Develop, implement, and enforce a written hazard communication program that includes training employees about the chemicals they work with and the associated hazards and controls of these chemicals. The wood floor finishing industry and other stakeholders, such as contractors, insurance companies, and government agencies should: 6. Educate consumers about the hazards associated with finishing wood floors and actions that can be taken to minimize these hazards, such as the use of less flammable floor finishing products. Homeowners finishing wood floors themselves or through hired contractors should: 7. Ensure that only less flammable floor finishing products (products with flash points greater than 100 degrees F) are used inside their homes. In addition, policymakers should: 8. Consider developing regulatory approaches to minimize the risk of fires and explosions during wood floor finishing. The press operator put his head between the frame and the closed top lid of the press in order to see where to latch the glue tray. While he was in this position, the top plate suddenly lifted at the end of a programmed automatic cycle, and crushed the operator's head against the frame. Coworkers witnessed the event and immediately extricated the operator from the machine. The victim was transported to the hospital, where he was pronounced dead. Recommendations: 1. Completely shut down and lockout machinery before conducting maintenance. 2. Employers should develop, implement, and enforce a comprehensive hazardous energy program. 3. Ensure that all machinery moving parts and pinch points are fully guarded. 4. Employers should conduct regular hazard surveys and follow through with corrective action. 5. Employers should investigate on-the-job injuries and review work procedures in order to correct hazards and prevent similar incidents. 6. The victim was alone in the shop at the time of the incident, but was discovered within a half hour by his employer, lying on the floor. Medics arrived and found the victim dead at the scene. Recommendations: 1. Points of operation and all pressurized hoses should be guarded on a hydraulic press. 2. Employers should ensure that all equipment is manufactured and maintained in a safe, operable condition. 3. Adequate training should be provided to machine operators and fabricators, to include hazards associated with the equipment, safe operating procedures, equipment inspections, and locking out of hazardous energy sources. He was in the process of setting up a portable rubble plant which is used for crushing concrete. Part of the setup involves rotating a support leg 90 degrees from the transport position to the stationary position. The support leg pivots from the transport position to the stationary position on a 3-1/2" bolt with a one-inch diameter. The threads on the bolt stripped and the nut fell off allowing the support arm to fall on the victim. The Nebraska Department of Labor investigator concluded that to prevent future similar occurrences: 1. Employers should ensure that procedures are available for setting up and breaking down the portable rubble plant. 2. Employers should ensure a periodic inspection schedule is developed for checking the condition of the portable rubble plant which includes checking the condition of the bolts/nuts and/or pivot device on the support leg. 3. The manufacturer of the portable rubble plant should consider a redesign which would provide a safer pivot point for the support leg. He had climbed up an electrical transmission tower to perform the task and had attached his personal protective grounding jumper between the structure (clamp end) and the shield wire (hook end) and was aligning the suspension clamp for the shield wire. The 345,000 volt conductor lines, which are approximately 35 feet below the shield wire, were energized, which was normal for this procedure. The flat faced grounding clamp that was installed on a section of beveled angle iron on the tower structure became disconnected and contacted the victim resulting in his electrocution. The Nebraska Department of Labor investigator concluded that to prevent future similar occurrences employers should: 1. ensure employees attach personal protective grounding jumpers to appropriate attach points. 2. ensure information on proper grounding procedures for use with a flat faced grounding clamp are included in training. 3. consider implementing a company policy requiring that shirt tails must be tucked in when working in an environment where they could become caught in equipment (such as the grounding clamp in this incident).
