Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

As the forklift operator tried to remove the top pallet, the forks made contact with a portion of the wooden pallet causing it to move and tilt backward slightly. While the forklift operator pulled the forks out and attempted to reposition, the spotter moved behind the pallet, out of line of sight of the operator. When the operator reinserted the forks, the cased water on the pallet shifted against the shrink wrap, which gave way and dumped the cased water on the spotter. The forklift operator did not realize the spotter was behind the pallet, and it was a couple of minutes before he noticed that the spotter was under the cases of bottled water. The operator and several co-workers moved the spotter, who was unresponsive but breathing, away from the water and called emergency services. Paramedics arrived but were unable to revive the spotter who died from his injuries. California FACE (CA/FACE) investigators concluded that to help prevent similar occurrences, employers using forklifts to move palletized merchandise should: Ensure employees follow SOPs for forklift work to include constant communication between operators and spotters, adhering to safe working zones while forklifts are operating and ensuring operators always have line of sight to employees working around the forklift. Provide First Aid/CPR training to volunteer employees on all shifts so that First Aid is readily available to all employees per Occupational Health and Safety Administration (OSHA) requirements. On the day of the incident, he was part of a four-person crew removing a 127- foot fir tree from a residential property. The employer was not on site to supervise. The crew planned to use a lowering device (port-a-wrap) to lower tree sections and limbs. However, they did not have a rope long enough, so they tied two ropes together. The knot tying the ropes together would not fit in the port-a-wrap so they did not use it. Instead, they wrapped the rope twice around the tree for friction to lower the tree sections. A climber then worked from the tree rigging and cutting sections while the tree trimmer and another worker held the rope for lowering the sections to the ground. The climber cut off a 330-pound section from the tree. It fell about one foot before the rope holding it became taut. The weight of the section caused shock loading on the rope, which lifted the tree trimmer and the other worker off the ground. The workers struggled to control the section as its momentum pulled them toward an embankment. The tree trimmer lost his grip on the rope, stumbled, and fell backwards down the embankment, hitting his head on a rock. He died at the scene. Following the incident, investigators found: Due to the rigging design and weight of the section they were lowering, the ground workers were unable to control it during descent. The employer did not develop a formal, written accident prevention program (APP) tailored to the needs of the particular workplace or operation and to the types of hazards involved. There was no qualified person to supervise at the site. REQUIREMENTS: Provide and use safety devices, safeguards, and use work practices, methods, processes, and means that are reasonably adequate to make your workplace safe. Develop a formal, written APP. RECOMMENDATIONS: FACE investigators concluded that, to help prevent similar incidents: Develop APP policies that require a Job Hazard Analysis (JHA) with worker participation for each job. A JHA would have identified the hazardous rigging and lowering practices and the potential for being struck by the uncontrolled tree section. Have a qualified arborist: Make decisions about safe and controlled tree removal practices. Train and directly supervise workers involved in tree work. Use ANSI Z133 American National Standard for Arboriculture Operations - Safety Requirements to: Train workers and ensure they use proper tree removal methods. Understand and account for additional forces resulting from the rigging design, rope angles, and the number of lines and/or line parts that will act on any rigging or anchor point. A car carrying two gunmen pulled up to the store. While the car waited outside, the gunmen entered the store, drew handguns, and fired a warning shot into the ceiling. A budtender instantly pressed a panic button under a cash register. Everyone was ordered down on the floor, and workers were given plastic bags to put money in. One of the robbers aimed his gun at the worker and forced him to open one of the store's seven cash registers. A fight suddenly ensued between the worker and gunman and ended with them struggling on the floor. The fight alerted the second gunman, who was watching the front doors. He walked to the cash register counter, leaned over it, and shot the worker in the neck. The worker was shot three more times. The gunmen stole $250 from the opened register and fled in the getaway car. First responders arrived quickly but were unable to save the fatally injured worker. Following the incident, investigators found: The employer's Accident Prevention Program (APP) had a standard operating procedure (SOP) for emergencies and robbery responses. The SOP instructed workers to comply with robber's demands and call 911 or press panic buttons. Safe drops were required every 30-45 minutes or when a cash register reached a certain dollar limit. Store security included two unarmed guards, surveillance cameras, and an alarm system with panic buttons under each cash register. REQUIREMENTS: The employer met all applicable late night retail establishment crime prevention and worker safety requirements in WAC 296-832 and laws in 49.22 RCW. RECOMMENDATIONS: FACE investigators concluded that to help prevent similar occurrences, employers should: Develop, review, and enhance security plans and train all workers on the security procedures. Hire armed security guards and consult a security company to assess store vulnerabilities. Make frequent cash deposits to minimize the amount of cash in the cash register. Use "drop safes" and post signs in store explaining that staff cannot open the safe. Post signs stating that video surveillance is in use. Ensure adequate outdoor and indoor lighting. Use walk up window(s) instead of allowing people in to the store. Have an initial entry area separated by a security door that opens into the retail area. RECOMMENDATIONS: Washington FACE investigators concluded that, to help prevent similar occurrences, employers should: Install manufacturer-approved, certified ROPS and seat belts on tractors. Develop a formal, written APP with job hazard analysis (JHA) and ROPS and seat belt safety training requirements. Improve tractor safety on embankments by assessing road and slope hazards, installing lights, and marking edges of roads with visual cues, such as reflective driveway stakes. The injury happened around 4 a.m. when the worker's shift was ending. Getting ready to clock out, he saw a long bine sticking out from the hop harvester's arm picker. The picker stripped leaves and stems from hop bines using a chain and sprocket drive that operated between 190 - 210 rpm. When the worker reached over into an open 11-inch gap above the picker to remove the bine, the energized rotating chain pulled his left hand in, shredded his work glove, and badly mangled his fingers and palm. Co-workers gave first aid and drove him to the hospital, where doctors amputated his index and middle fingers and part of his thumb. He was hospitalized for ten days. Months later, he has not returned to work and needs more treatment for his physical and mental trauma. Following the incident, investigators found: The hop harvester's arm picker had a horizontal steel mesh guard that was too low to prevent someone from reaching over into an 11-inch unguarded gap above the chain and sprocket drive. The worker did not follow his training from the employer to notify a lead mechanic, who was nearby, to perform lockout/tagout (LOTO) procedure requirements before cleaning out debris. REQUIREMENTS: Unless guarded by location, all sprocket wheels and chains must be enclosed. RECOMMENDATIONS: FACE investigators concluded that to help prevent similar occurrences, employers should: If a machine cannot be guarded by distance and location, protect workers from hazards created by rotating or revolving parts by using one or more safeguarding methods. Make sure guards are: Made of strong, durable materials, such as angle iron, solid metal, expanded metal, and wire mesh. Securely fastened to the machine or the building structure if they cannot be attached to the machine. Regularly inspected and maintained or replaced if not in serviceable condition. Make sure guards protect workers by preventing hands or other body parts from reaching through, over, under, or around the guard into the hazard area; and preventing objects or debris from falling onto or being thrown towards a worker. Install warning signs on and near machines to remind workers of operating hazards and LOTO requirements written in a language all workers can understand. Conduct Job Hazard Analysis (JHA) to identify machine operation hazards and solutions before use. Two dump truck drivers employed by a solid waste recycling company were emptying dumpsters and hauling away the debris. While emptying a dumpster, a 5-gallon bucket of paint fell out and spilled on the street. The superintendent came over to organize the cleanup. He assigned one of the subcontractors to get sawdust to absorb the paint and told the drivers he was going to direct vehicles away from it. The drivers then entered their trucks to go pick up the next dumpster located close to the spilled paint. The trucks had to be parked side-by-side as the grapple on one truck needed to pick up the dumpster and empty it into the other. The driver of truck #1 drove out of the alley, turned right, and parked on the side of the street near the superintendent. The driver of truck #2 then turned left onto the street, drove forward, and stopped. He checked his mirrors and got a hand signal from the superintendent to begin backing up. As he was backing up, he lost sight of the superintendent and ran him over. The incident was unwitnessed. It is unknown why the superintendent was in the backing zone, or why the driver could not see him. Following the incident, investigators found: The truck did not have a backup camera, nor was an observer signaling that it was safe to back up. The truck's backup alarm was working as it backed up. The truck drivers were not trained on procedures for backing up at construction sites. REQUIREMENTS: Before backing a dump truck, the driver must determine that no one is currently in the backing zone and it is reasonable to expect that no employee(s) will enter the backing zone while operating a dump truck in reverse. If employees are in the backing zone, you must make sure the truck is backed only when: An observer signals that it is safe to back or an operable mechanical device that provides the driver a full view behind the dump truck is used, such as a video camera. It is the responsibility of management to establish, supervise, and enforce, in a manner which is effective in practice training programs to improve the skill and competency of all employees in the field of occupational safety and health. RECOMMENDATIONS: FACE investigators concluded that to help prevent similar occurrences: General contractors at multi-employer job sites should: Continuously assess the hazards of vehicles to workers on foot and ensure hazards are corrected. Require workers to wear ANSI Class 2 high-visibility garments, such as vests, when exposed to vehicular traffic. Employers who use dump trucks should: Consider installing pedestrian proximity detection systems on trucks to alert drivers of workers on foot. Train drivers that they must use a signaler or back up camera when backing near workers on foot. Create and enforce policies that: 1) Drivers maintain visual contact with workers on foot. When visual contact is lost, drivers should stop and not resume movement until visual contact is re-established. 2) Workers on foot stay out of backing zones unless trained and acting as an observer signaling the driver. He was laying floor sheeting while one was taking measurements and the other was busy cutting the sheeting. They finished framing the room and installing the roof trusses about a week before. He went down a ladder to the ground floor to get some nails. After coming back up the ladder, he started nailing down a sheet. Shortly after, he fell through the joists of the unfinished floor along with a sheet of OSB to the concrete 14 feet below. Despite working close by, neither brother saw him fall. He was not wearing a personal fall protection harness when he fell. He died the next day at the hospital. After the incident, the brothers demonstrated to investigators how they tied-off their fall protection lifeline ropes by wrapping them around roof trusses. They did not have fixed or temporary anchors installed. Investigators determined their method of tying off was inadequate since diagonal truss braces are not rated for that. Investigators also found that the employer did not have an Accident Prevention Program (APP) and did not develop a fall protection work plan for the project. REQUIREMENTS Employers must: Ensure that a fall arrest system, fall restraint system, or positioning device system is provided, installed, and implemented in accordance with this chapter when employees are exposed to fall hazards of six feet or more to the ground or lower level while constructing a leading edge. Develop and implement a written fall protection work plan including each area of the work place where the employees are assigned and where fall hazards of ten feet or more exist. Develop a formal accident-prevention program, tailored to the needs of the particular plant or operation and to the type of hazard involved. RECOMMENDATIONS: FACE investigators concluded that to help prevent similar occurrences employers should: Frequently remind workers of the need to use fall protection 100% of the time they are at risk of falls. Develop policies and train workers to: Tie-off to approved fixed or temporary anchors only. Wood beams of roof trusses are not designed to support fall protection systems. Not store raw materials like OSB over floor joists or other areas where fall hazards exist. Near the end of the night shift, the harvester operator instructed the crew to stop working and return to the maintenance facility to clean the machine. Upon arriving at the maintenance facility, they noticed that the victim was missing. Driving back to the field where the victim was last seen, the operator found his body, apparently crushed by the tomato harvester. Emergency services were called to the scene but were unable to revive him and he died from his injuries. CA/FACE investigators concluded that, to help prevent similar occurrences, employers using harvesting equipment should: 1) Provide all employees working at night with appropriate reflective clothing. 2) Supervisory personnel should inspect equipment and conduct a safety meeting at the beginning of the shift. 3) Develop a Standard Operating Procedure (SOP) that includes a system of communication between operator and crew during the shift as well as a method for accounting of personnel prior to operation of agricultural equipment. 4) Provide training and instruction related to harvester equipment safety. 5) Consider conducting industrial hygiene sampling for dust and noise. 6) Offer high-intensity LED lighting as an option on all tomato harvesters. 7) Improve visibility for the harvester operator. The driver did not set the truck's parking brake before exiting the cab to access the top of the trailer to prepare his load for tarping. While the driver was walking on top of his load near the back of his trailer, his truck began to roll forward. He ran toward a ladder at the front of the trailer that went down to a walkway behind the tractor. While trying to descend the ladder, he went over the edge of his trailer and fell to the road. The only other driver at the mill left the site without seeing the incident. Almost two hours later, another truck driver entered the site and found the driver fatally crushed under his tractor's left front drive wheels. CONTRIBUTING FACTORS: Key contributing factors identified in this investigation include: Uneven road grade at truck tarping station area. Parking brake rules in Accident Prevention Program (APP) not followed. Inadequate truck parking brake and wheel chocking policy in APP. Insufficient safety audits of drivers and customer worksites located abroad. RECOMMENDATIONS: Washington FACE investigators concluded that, to help prevent similar occurrences, employers should: Install electronic parking brake systems that automatically apply the parking brake when the driver has not set it before exiting the cab. Develop APP to have a parking brake and wheel chocking policy with strict requirements for preventing rollaways and unsafe responses. Collaborate with owners of remote customer sites outside of their ordinary daily travel range to ensure the sites are safe for their drivers. Assist safety supervisors to obtain work visas or hire safety consultants abroad to do safety audits at customer sites in other countries. The next day, he coupled a yard tractor to the chassis and tried towing it indoors to avoid the rain. The chassis would not move as its parking brake was still engaged. The mechanic stopped and exited the tractor leaving it parked on a downgrade with the engine running, parking brake released, and wheels unchocked. He told another mechanic to reconnect the chassis' air lines so its parking brake could be released. He then kneeled between the chassis' wheels and waited while the other mechanic worked. When the air lines were reconnected, the restored air pressure released the chassis' parking brake allowing gravity to pull the tractor and chassis combo downhill. The mechanic was crushed by the wheels of the rolling chassis while the other mechanic, who was still under the chassis, was almost struck by the axles. CONTRIBUTING FACTORS: Key contributing factors identified in this investigation include: Parking brake not engaged when necessary. Wheels not chocked during vehicle repair operations. APP lacked vehicle parking brake and wheel chocking requirements. APP had ineffective progressive discipline policy. RECOMMENDATIONS: Washington FACE investigators concluded that, to help prevent similar occurrences, employers should: Install electronic parking brake systems that automatically apply the parking brake when the driver has not set it before exiting the cab. Develop APP to include vehicle rollaway hazard identification, assessment, and prevention procedure requirements. Develop APP to have progressive discipline policies that effectively address vehicle safety rule violations and prevent hazardous operation. The siding installer, who owned his own construction company, was working for a subcontractor to install siding at a new construction apartment complex. On the day of the incident, the installer accessed the scaffold platform at the building's third floor level by climbing an extension ladder. The scaffold was not equipped with a guardrail system; it only had a workbench, which alone does not meet the requirement of a guardrail system. The installer provided his own full body harness, but he was not wearing it. The subcontractor provided the pump jack scaffolds and the rest of the personal fall arrest system consisting of a vertical rope lifeline with a rope grab and connector. The subcontractor required that site workers use the personal fall arrest system. As the installer was working from the scaffold platform, he fell 23 feet, landing on a pile of construction materials on the ground. He died of multiple blunt force injuries due to the fall. Investigators found: All employees had received fall protection training. The site superintendent reported that he held weekly site-wide safety meetings, which included an emphasis on the requirement for workers to use personal fall arrest systems. In addition, he did safety walk-arounds once or twice per day to ensure workers were using fall protection. On the day of the fatal incident, he had not yet done a walk-around inspection. REQUIREMENTS: Employers must: Protect each employee on a scaffold from falling ten feet or more to a lower level, by providing either: (a) A personal fall arrest system; or (b) Guardrails. Ensure guardrails are installed along all open sides and ends of platforms. Develop and implement a written fall protection work plan including each area of the work place where the employees are assigned and where fall hazards of ten feet or more exist. (Note: A fall protection work plan is not required where a guardrail system eliminates the fall hazard.) RECOMMENDATIONS: FACE investigators concluded that general contractors and sub-contractors should: Use guardrails on scaffolds whenever practical instead of or in addition to personal fall protection. Emphasize the requirement that workers maintain 100% tie off. This should be written in the work contract. Inspect scaffold operations at the start of each day before work begins and periodically throughout the day to ensure workers use fall protection. The laborer had worked for his employer, a construction contractor, for five months. He had worked for two months at the incident site where a condominium complex was being remodeled. He was new to the trade and had only received informal on-the-job training. On the day of the incident, the laborer and two other workers were preparing to work on the building's exterior balconies. A multilevel supported scaffold allowed the workers to access and work on the building exterior. Ladders and safety hatches installed on the inside of the scaffold walking platforms allowed workers to access all levels of the scaffold. Using the scaffold's interior ladders, they were bringing tools up to balcony decks on the third and fourth floor work areas. The laborer did not use the scaffold ladders. Instead, he climbed up the outside of the scaffold using its cross braces in the 12 ½-inch gap between the scaffold and the building exterior. He was not wearing personal fall protection. It was not required because the scaffold was within 14 inches of the work face. He then retrieved a saw from the second floor level and holding it in one hand began climbing the cross braces to the next level. As he reached up to hand the saw to a coworker on a balcony, he slipped and fell through the gap, landing 25 feet below on pavement. REQUIREMENTS: Employers must: 1. Make sure cross braces are not used as a means of access. 2. Enforce the fall protection requirements for scaffolds. 3. Develop a formal accident-prevention program tailored to the needs of the particular plant or operation and to the type of hazard involved. 4. Train employees who work on a scaffold. RECOMMENDATIONS: FACE investigators concluded that, to help prevent similar occurrences: 1. Train workers to never climb on or use scaffold cross braces as a means of access. 2. Assess fall hazards and the need for personal fall protection, even when it is not required. 3. Add scaffold side brackets and planks just below each level of decks to prevent workers from accessing or falling through the gap between building and scaffold. 4. Provide one dedicated access point to each deck from the scaffold. The incident occurred at a client's site where the decedent was servicing the order picker with a co-worker. The carriage, weighing approximately 1,163 pounds (lbs.), consists of an operator platform, control console, and lift forks. The carriage can move up and down on the order picker's mast, and the movement is actuated by a center lift hydraulic cylinder and a pair of side lift cylinders along with a set of chains and pulleys. The hydraulic pressure for the center lift cylinder was over 2,000 pounds per square inch. The center lift cylinder had a minor leak, and the decedent and the co-worker were on site to repack the cylinder (replacing the cylinder parts) on the day of the incident. To access the cylinder, they removed the control console cover and raised the carriage approximately seven feet high. The co-worker went to his van to fetch a pair of locking chains so that they could secure the elevated carriage to the mast with the chains to prevent the carriage from falling. They then realized that they had the wrong packing kit with them. They could not repack the cylinder that day since they had to go back to their shop to get the correct kit. According to the co-worker, the decedent was holding a pair of snap ring pliers while standing directly underneath the carriage that was neither chained nor blocked. At about 8:15 a.m., the decedent removed the outer snap ring from the cylinder with the pliers apparently to verify the part number. The function of the outer snap ring (aka retaining ring) is to hold and secure the oil seal of the cylinder at the rod end. While the decedent was telling his co-worker that he just wanted to look at the ring, the carriage fell quickly crushing the decedent underneath. Meanwhile hydraulic fluid sprayed over the entire dock area. The police, fire department, and EMT arrived at the scene within minutes. However, the rescuers were unable to revive the service technician who was pronounced dead at the scene. CONTRIBUTING FACTORS - Occupational injuries and fatalities are often the result of one or more contributing factors or key events in a larger sequence of events. The NY FACE investigation identified the following key contributing factors in this incident: 1) The snap ring was removed from the cylinder causing the hydraulic system failure and rapid descent of the carriage. 2) The elevated carriage was neither blocked nor secured to prevent it from falling. 3) Occupant, the decedent youth, was in the silo with the sweep auger rotating and circling around the silo. The victim was standing under elevated carriage which was neither blocked nor secured. 4) The cylinder was not removed from the order picker before the snap ring was removed. 5) Employer did not have specific standard LOTO procedures for servicing hydraulic cylinders on order pickers. 6) Employer did not provide employee training on specific LOTO techniques and procedures. RECOMMENDATIONS - NY FACE investigators concluded that, to help prevent similar occurrences: 1) Employers should develop specific lockout/tagout procedures for service technicians to follow when servicing lift trucks such as order pickers. 2) Employers should ensure that service technicians follow manufacturer's safety requirements, and employees should strictly follow the standard safety procedure. 3) Employers should conduct job hazard analysis (JHA) to identify hazards and risk factors associated with lift truck maintenance and repair services. 4) Employers should ensure that all service technicians receive proper and adequate training on LOTO procedures. 5) Employers should develop checklists for high-risk maintenance tasks and require technicians to check each step and sign the checklist. 6) Employers should conduct periodic inspections at client sites to ensure the sites are free of hazards and workers follow the safety protocols. 7) Employers should implement a near miss reporting system to ensure worker safety and the compliance of company safety protocols. The operator and a coworker were installing large metal storage racks in a customer's warehouse. They were in the process of placing shims under the uprights to level the racks. The operator was using a forklift to raise the racks so they could install the shims while his coworker drilled holes in the concrete floor to anchor the uprights. The operator parked the forklift, shut off the engine, and set the parking brake. He then exited the forklift but left the forks raised to about 39 inches. He unknowingly parked on top of the cord of the drill his coworker was using. His coworker got in the forklift to move it and try to free the cord. He started the engine and put it in forward gear but did not know how to release the parking brake. He asked for assistance, so the operator reached into the cab and released the parking brake. Once the brake released, the forklift started moving toward the metal racks. The coworker swerved to avoid the racks and the operator was crushed between the rear of the forklift and one of the metal uprights. The coworker then panicked and jumped from the forklift, which finally came to a stop when its forks ran into a wall. The operator was pronounced dead shortly after arriving at the hospital. Following the incident, investigators found: 1. The employer had an Accident Prevention Program (APP) and had performed a Job Hazard Analysis (JHA) prior to this task, but this situation was not an expected hazard. 2. Neither the operator nor the coworker had completed a required operator training program before operating the forklift or other powered industrial tucks (PITs). 3. The operator had received on the job training and was authorized by the employer to operate the forklift. The coworker was not trained or authorized to operate the forklift. REQUIREMENTS: 1. Employers must make sure employees successfully complete an operator training program before operating PITs. RECOMMENDATIONS: FACE investigators concluded that to help prevent similar occurrences employers should: 1. Train workers to: 2. Identify unexpected situations, not specifically addressed in the JHA. 3. Evaluate the hazards associated with unexpected situations. 4. Control the hazards safely if possible and check with a supervisor if they cannot or have questions. 5. Instruct unauthorized workers never to operate PITs, even if only for a short time.
