Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The CA/FACE investigator determined that in order to prevent future occurrences, employers, as part of their Injury and Illness Prevention Program (IIPP), should: 1. Ensure that all heavy equipment tires are deflated and the valve stem removed prior to any work being performed on the tires or attached components. The upper boom actuating chain connecting the upper and lower boom sections broke and the upper section fell, striking the truck bed and ejecting the victim from the bucket. The victim struck his head and neck on the truck bed before landing on the road surface. A co-worker (the victim's son), who witnessed the collapse from the ground, determined that the victim was unresponsive and began resuscitation efforts. A passing motorist called emergency medical service (EMS) personnel. The victim was transported to a local emergency department where he was pronounced dead. The bales had shifted during the trip from the originating farm to the purchasing farm. We postulate that after arriving at the delivery site, the decedent, beginning on the driver's side at the cab end of the trailer, removed each bale stack's ratchet binder. At the rear of the trailer, some of the stacked bales in the last row fell from the trailer to the ground after he removed this last ratchet binder. The purchasing farm's worker #1 transported some of the fallen bales from the rear of the trailer to the storage area. We postulate that the decedent, while standing on the passenger side of the trailer, began to roll up the securement straps. It appears that while rolling one of the straps, the decedent was struck by a falling bale. Worker #1 noticed that more bales had fallen to the ground. Because he could not see the decedent, he called for help. Another worker who arrived at the incident scene found the decedent under a bale on the ground. 911 was called. Another worker arrived and used a forklift to remove the bale from on top of the decedent. Emergency response arrived and the decedent was taken to a local hospital and where he was declared dead. Recommendations: 1. Employers should ensure employees use the correct roof brackets and are trained and follow the "best safe work practice" when installing a roof bracket scaffold platform - using the correct roof bracket for the roof pitch and securing the bracket by 16d nails anchored into a roof rafter. 2. Farm employers should ensure cargo subject to motor vehicle cargo securement rules is appropriately secured in a manner to meet or exceed these requirements. 3. Farm employers should develop a cargo securement assessment protocol upon arrival at the destination and develop unloading procedures specific to shifted loads, such as additional means of load support prior to unloading. 4. Farm employers should ensure that all loads conform to the Michigan Motor Vehicle Code and Federal Motor Carrier Safety Act for cargo transport width and height requirements. The two workers arrived at the site in the morning, where a barn was being converted into a shop, and set up a scaffold to a height of about 7 feet. The scaffold was placed about 1 foot from the wall, the lead worker locked the wheels on his end, and the two workers climbed onto the scaffold to hang 12-foot pieces of sheetrock on the shop wall. The installer was holding the sheetrock against the wall when the scaffold suddenly moved backward, causing him to lose his balance and fall. He hit his head on the concrete floor. The victim was transported to a hospital, where he died 2 days later. Cause of Death: Blunt force head injuries. Recommendations: 1. Workers on mobile scaffolding should double-check the structural stability of the scaffold and ensure that wheels are locked before mounting the work platform. 2. A competent person must supervise the erection and use of scaffolds. 3. Employers must ensure that construction workers are trained to identify, understand, and control fall hazards. 4. Employers should consider using guardrails on scaffolds wherever practicable. The workers were removing old roofing material and hand-carrying the debris in metal trash cans from the roof, and dumping the debris into a truck located adjacent to the roof. The roofing contractor used a ladder-jack scaffold plank as a walking platform for the workers to use while carrying the debris from the roof to the truck. The victim was hand-carrying an armful of debris across the plank to the truck, when he fell approximately 9 feet to an asphalt driveway below. He was unconscious and unresponsive with an obvious serious head wound suffered from the fall. Medical assistance was called and he was transported to an area trauma center where he died of his injuries several days later. To prevent similar occurrences in the future, the Washington State Fatality Assessment & Control Evaluation (FACE) investigative team concluded that employers engaged in roofing work, or similar operations, should follow these guidelines: 1.) Employers are responsible for assessing hazards and implementing fall prevention and protection measures before starting a job, and for re-evaluating the hazards and protection measures during the course of the roofing work. 2.) Jobs should be designed to minimize fall exposures. An alternate method could be used to eliminate the need for walking between the roof and debris truck. 3.) Conduct pre-job safety meetings to discuss site safety & health issues and to review potential fall hazards and appropriate fall prevention measures. 4.) Provide employees with fall hazard assessment training by a competent person. 5.) Temporary employment service agencies should work with secondary employers to ensure that specific job descriptions, training criteria, and hazard analyses have been completed for each job assigned to temporary employees. 6.) Employers should provide appropriate fall protection equipment for all workers who may be exposed to a fall hazard. The tractor had a wide front axle but was not equipped with a cab, roll-over protective structure (ROPS) or the seatbelt which is an integral component of standard ROPS installations. The farmer was mowing weeds when the wheels on the right side of the tractor ran over a tree stump. The farmer was pitched from his operating position on the tractor to the ground ahead of the tractor's left rear wheel. The tractor and rotary cutter combination stalled with the farmer under the rotary cutter. Discovered by a neighbor, who immediately summoned emergency assistance, the farmer died from an open laceration to his chest before emergency personnel arrived. RECOMMENDATIONS: 1. Operators should follow tractor and rotary cutter operator's manual instructions, safety messages and safety signs, and all applicable safe operating practices when using rotary cutters. 2. Rotary cutters, especially when operated on adverse terrain, should be used with tractors that have ROPS and seatbelt. 3. The area to be mowed should be scouted, and cleared of debris, with all obstacles effectively marked if they cannot be removed. 4. Rotary cutters should be operated when lighting is sufficient to expose marked and unmarked obstacles, debris, uneven terrain, and other hazards. Tools to operate the valve were stored reachable from above ground level inside the manhole vault. He worked alone, his death was not witnessed, and his body was not immediately discovered. First responders found no abnormal oxygen levels, combustibles, or toxic air contaminants in the manhole vault at the time of the rescue. The victim was extricated and transported. It was determined that his death resulted from positional asphyxia. RECOMMENDATIONS: 1. Identify property and workplace hazards such as confined spaces. 2. Eliminate or reduce confined space hazards and associated risks. 3. Develop and implement a comprehensive and effective confined spaces program. 4. Establish and enforce a contractor safety program. One lane of the road was open to traffic and the other was being used as a staging area for the paving operation, where dump trucks were dropping their trailers. The victim and a co-worker had been laying, or tacking, a string line down the center of the road and using paint to mark the pavement so as to guide the crew laying asphalt and ensure its proper alignment. The coworker was called away and the victim stood up and waited for another worker to take his place laying string. At this moment, a dump truck that had just dropped its trailer began backing up toward the victim, who had his back turned away from the truck. The dump truck driver was backing down the lane avoiding his parked trailer and the live-traffic lane, but he did not see the victim who was in his blind spot. The victim was struck and run over by the reversing dump truck. He died at the scene of his injuries. The victim had been wearing a class 2 safety vest, yellow pants, hard hat and was using earplugs. The dump truck's back-up alarm was found to be working. He was wearing an orange high visibility vest. The two-person survey crew had not set up any road signage indicating that survey work was being conducted. The crew had not established a proper lane closure nor had they set up traffic cones around the area where he was standing, holding the prism pole. The decedent was holding the prism pole in the southbound lane when an oncoming vehicle traveling in the southbound lane struck him. The collision caused him to become airborne and he was struck again by a northbound vehicle. 911 was called and the decedent was declared dead at the scene. The driver of the southbound vehicle was driving on a revoked license due to "vision problems." Recommendations: 1. Road Commission employers should ensure employees have a copy of the Michigan Manual on Uniform Traffic Control Devices (MMUTCD) and required signage in the work vehicle, use the required signage for work performed, and that the manual requirements are implemented (such as adequate MMUTCD-required personnel are available for a work crew) when working on or near a roadway. 2. Road Commission employers should ensure effective health and safety training for their employees. 3. Each county Road Commission should form a joint Health and Safety Committee. 4. Employers investigating additional technologies to provide supplementary employee protection such as intrusion alarms or lighted vests should consult the National Work Zone Safety Information Clearinghouse for the advantages/disadvantages of these technologies. 5. The Michigan legislature should consider increased funding for public education campaigns to educate motorists about Work Zone Safety. After emptying the tote, the supervisor leaned forward over the edge of the hopper, apparently to shake the plastic liner to dislodge remaining product, or retrieve a box liner that had fallen inside the hopper. As he leaned into the hopper, the supervisor's right jacket pocket caught the end of the tote-dump control lever and pushed it downward, causing the dump arm to descend. He was struck on the back and crushed against the hopper. A coworker witnessed the incident and responded to raise the dump arm. The victim was transported to a local hospital where he was pronounced dead. CAUSE OF DEATH: Multiple traumatic injuries. RECOMMENDATIONS: 1. Never place your hands or body in the operating areas of a machine without first shutting down the machine completely and locking out all forms of hazardous energy. 2. Machine activation mechanisms should be guarded to prevent unintended startup. Guarding should also restrict access to all moving, shear, and pinch-point areas on machinery. 3. Employers should develop a comprehensive hazardous energy program that includes machine-specific lockout procedures. 4. Employers should conduct regular hazard surveys of the workplace, and a job safety analysis of each job to correct unsafe work practices. 5. Employers should investigate on-the-job injuries and review work procedures in order to correct hazards and prevent similar incidents. There were 13 grates that required removal. Each grate lay in a track, which held them in position in the shredder box. Five grates had been removed. To remove a grate, the decedent attached a separate steel alloy chain to each end of the grate and a mobile crane lifted the grate out of the machine via the grate track. The incident occurred during the removal of the sixth grate. The grate was located at a five o'clock position. The decedent had attached the chains to the grate and the hoisting process began. The grate became stuck in the grate track. To facilitate its removal, the decedent used a torch to partially cut through the grate while it was still under tension from the crane. The crane again attempted to lift the grate from the machine. Because the grate again did not move, the decedent cut through the grate end while the grate was still under tension. When the grate was cut through, the momentum of coming to center caused the grate to continue past center and strike the decedent in the side of his head. 911 was called and the decedent was transported to a local hospital where he died three days after the incident. Recommendations: 1. Employers should conduct a job safety analysis (JSA) when performing non-routine tasks or using non-routine tools, develop a standard operating procedure for the identified tasks, and train applicable employees on the procedure. 2. Employers should provide rigging training to employees involved in lifting a heavy load with load-bearing equipment. 3. Employers should form a joint Health and Safety committee. Although not a factor in this incident, MIFACE recommends: 4. Employers evaluate the workplace to identify any confined spaces and develop and enforce written work procedures for entry into these areas. 5. Employers establish written lockout/tagout procedures to provide employee protection from injury due to unexpected/unintended motion, energization, start-up or release of stored energy from the machine, equipment or process when working in, on, or around the equipment during repair, maintenance or other associated activity. The victim had spent the majority of his work shift cutting plywood sheathing and handing it up to coworkers who were sheathing the roof. He was working from the second floor of a two-story, single family home under construction where he and other workers had completed most, but not all, of the subfloor (plywood sheathing secured over floor joists) on previous workdays. They had left a floor area open in an attic space where a walk-in closet was to be constructed later. At the end of the shift, the victim's lead worker asked two workers to complete the second story subfloor in the attic space. One of these workers joined the roofers to lay roofer's felt. The other co-worker joined the victim and together, they cut two sheets of plywood sheathing and placed them over the joists in the open area in the attic space. They were not wearing fall protection. The co-worker reported that he was looking down at the sheets of unsecured plywood sheathing trying to make the pieces fit into an opening that was not square and when he looked up the victim was gone. The victim had apparently stepped onto a piece of the unsecured plywood sheathing that covered part of the floor opening, and when the plywood sheathing pivoted on the floor joist, he fell through the opening. The victim's co-worker immediately phoned their employer who was working off-site. The victim's employer then called the general contractor's field manager, who was working on-site, and the field manager called 911. Emergency medical services (EMS) arrived within minutes and immediately transported the victim to an area hospital. The victim was pronounced dead in the emergency room. The job site was at a commercial building under construction. The victim was trying to reposition a bundle of metal decking to be used for sheeting the building's roof. The 1,500 lb. bundle had been hoisted by a crane and left on the roof's steel girders. The bundle of decking was intended to span 3 girders, but because of crane limitations, it was placed spanning only 2 girders with its ends cantilevered over the girders. The victim was using a come-along to move the bundle into position to span 3 girders and bear weight on each end of the bundle. The bundles were moved using a comealong attached to a beam with a cable, and a hook attached to the bundle. The victim was standing on the bundle, ratcheting the come-along, to move it into position. He was wearing a full body harness and was using the bundle as an anchor point to which he had tied-off his deceleration lanyard. The victim was standing on the cantilevered end of the bundle as it was moved, causing it to tip sending him and the bundle falling 30 feet to the ground below. Emergency responders transported the victim to the hospital where he was pronounced dead. The mechanic was assigned to prepare a shipping container used to transport silicon tetrachloride for an inspection. The tank had been purged with an inert nitrogen atmosphere. While waiting for the inspector to arrive, the mechanic entered the tank for an unknown reason, apparently without first testing the atmosphere, and died of asphyxiation. When the victim was discovered about an hour later, a coworker jumped into the tank, again without testing the atmosphere first, and lifted the victim up to others standing on top of the tank. Testing by fire department responders showed the atmosphere at the bottom of the tank to be about 12% oxygen, below the minimum safe level of 19.5% oxygen. CAUSE OF DEATH: Asphyxiation. RECOMMENDATIONS: 1. Employers must evaluate confined spaces in the workplace. A permit-required confined space requires written entry procedures, including posted documentation of test results by an authorized person. 2. Never enter a confined space without first testing the atmosphere with an appropriate air-monitoring instrument. 3. Never enter a confined space without appropriate personal protective equipment for safe entry. 4. A trained attendant must monitor workers in a confined space and be prepared for an emergency response that does not involve direct entry of the space. Occupational and Environmental Medicine AB - On August 16, 2006, a 63-year-old male siding installer was critically injured when he fell from a roof while installing a J channel on a dormer for a home under construction. The decedent had erected a roof bracket scaffold platform on a high pitch roof (14/12) approximately three feet from the roof edge. The roof brackets that he installed were spaced approximately 11 feet apart and were secured by two 2-inch screws instead of 16d nails. The screws were installed in the roof sheathing, not secured into a roof rafter. The decedent had placed two 2x6-inch wood planks laid on their side across the span of the roof brackets. He then placed a ladder on the planks to use as a climbing device to reach the dormer to install the J channel. He was on the ladder when it appears that either the north roof bracket failed, or the decedent fell from the ladder. The decedent slid down the roof, off the roof edge and fell nine feet to the asphalt below. Another contractor's employee notified the decedent's employer who had just arrived on site that the decedent fell. 911 was called and the decedent was transported to a local hospital where he died approximately two weeks later. RECOMMENDATIONS: 1. Employers should ensure employees use the correct roof brackets and are trained and follow the "best safe work practice" when installing a roof bracket scaffold platform - using the correct roof bracket for the roof pitch and securing the bracket by 16d nails anchored into a roof rafter. 2. Employers should develop an accident prevention program compliant with MIOSHA Construction Safety standard requirements and train employees on its contents. His primary job function was that of a mechanical harvester operator. At some point, while working alone, the decedent attempted to fell a 20-inch diameter tree with a chain saw. The tree was not properly notched. This tree fell as directed by the notch and became lodged on a small branch in a nearby standing tree. The lodged tree was not removed prior to the decedent beginning delimbing work on another felled tree on the ground, which was in the fall path of the lodged tree. While the decedent was delimbing the tree on the ground with a chainsaw, the lodged tree fell, striking him in the back. His chainsaw was last heard running at about 1:30 p.m. A fellow logger walked to the incident area at approximately 5:20 p.m. and found the decedent's head and neck pinned against the tree he was delimbing by the tree that fell onto him. The site supervisor called 911. When emergency response arrived, the logger was declared dead at the scene. RECOMMENDATIONS: 1. Logging employers should develop, train employees in, and ensure the practice of safe felling procedures, such as but not limited to, a prohibition for working under "danger" trees and chain saw tree-felling techniques. 2. Logging employers should develop a procedure to check on the safety of fallers and buckers working beyond the hearing range of coworkers at regular intervals. 3. Employers should provide employee safety and health training as prescribed by appropriate MIOSHA standards. The dock plate had two standard guardrails on either side with a chain across the dock opening. There were no witnesses to the events prior to the incident or to the incident itself. The decedent was discovered when another employee traveling through the area noticed the forklift's lights shining on the wall and the truck's forks up and against the wall in the truck well. This employee summoned others to the area when he discovered the decedent pinned face down between the concrete floor and the overhead guard. Several employees manually lifted the forklift enough to move the decedent from under it. It is assumed that the forklift was traveling in reverse, because neither the forks nor the mast appeared bent, as would have been the case if he had been traveling forward and drove off of the dock edge. It is unknown if he jumped or was thrown from the operator's seat. 911 was called and paramedics pronounced him dead at the scene. RECOMMENDATIONS: 1. Employers should install drop-off protection capable of withstanding forklift impact on a loading dock entrance when the dock entrance is near a normal path of travel for a forklift or pedestrian. 2. Employers should enforce employee use of the operator restraint provided by the forklift manufacturer (in this case, a seatbelt). 3. If the truck is not transporting a load that obstructs forward vision, forklift operators should limit the distance that the truck travels in reverse. The decedent was delivering roofing materials to a firm contracted to re-roof the southwest section of a warehouse. He climbed a ladder to the roof and spoke with three roofing contractors about placement of the roofing materials. For reasons unknown, the decedent stepped over a roof parapet wall onto the low-sloped metal warehouse #2 roof that was not near the area where the roofing was to occur. One of the roofing contractor employees warned the decedent of the skylights on warehouse #2 roof. The decedent took several steps and then fell through a skylight that was flush with the roof. He landed on the concrete floor 20 feet below. The business owner called 911. Emergency response arrived and transported him to a local hospital. After evaluation at this hospital, the decedent was transferred to a second hospital where he died. RECOMMENDATIONS: 1. Employers should develop, implement, and enforce a comprehensive accident prevention program that includes, but is not limited to, job site hazard surveys and worker training in fall hazard recognition. 2. Building owners should install protective guard over skylights to prevent falls through skylights by maintenance or other personnel who may access the roof.
