Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The victim was part of a seven-man crew performing a milling operation on a 2,200-foot-long and eight-foot-wide blacktop berm of a recently paved section of a four-lane state highway. Due to a quality control problem, the blacktop berm had to be taken up and replaced. The milling machine had the capacity to remove a six-foot-wide section of the berm at a time. The crew had completed the six-foot-wide milling operation on the 2,200 foot section of the berm and had decided to mill the remaining two feet of berm before quitting for the day. A tractor equipped with a road sweeper was positioned behind the milling machine and the water truck was positioned beside the milling machine in the closed right side, westbound lane of the highway. The supervisor jumped onto the running board of the water truck and instructed the driver to return to the starting point of the milling operation and to apply water to the remaining two feet of the berm. The driver began to back the truck the 2,200 feet to the starting point. After dismounting the truck's running board, the supervisor walked around the tractor and into the path of the backing water truck. The two rear passenger-side truck axles backed over the supervisor before the tractor operator could alert the truck driver. A crew member called 911 from a company cell phone while other crew members moved the victim from underneath the truck. The victim was breathing and vital signs could be detected. The victim was transported to the hospital where he was later pronounced dead by the attending physician. Each riser was equipped with a built-in shut-off plate that was opened by a removable valve with a crank handle. Following standard procedure, the ranch hand closed the valve and drained the supply line so the valve could be removed from the riser. When he began to remove the valve, the cap of the riser blew off and pressurized water struck him in the face. He was knocked unconscious and fell face down in a growing pool of water. The ranch manager saw the geyser from the ranch house and drove to the site to investigate. He discovered the ranch hand and pulled him from the pool of water. Emergency responders arrived in about 10 minutes. The victim was transported to a hospital, where he died in intensive care 5 days later. The explosion of water caused a severe penetrating injury of the eye and resulting brain injury. Recommendations: 1. When working with a pressurized irrigation system, stay clear or guard yourself from the outlet path where valves and hoses connect. 2. Components of a pressurized irrigation system should be inspected for defective parts at least annually. 3. Employers should establish an emergency plan for personnel who work alone. After calling the warning "clear," the coworker lowered the platen by activating a pneumatic toggle, so he could make a final adjustment. Discovering a small part was missing, the coworker climbed off the machine and looked for the machine operator. He found him injured under the machine. The coworker called on his cell phone to the supervisor for help, and 911 was called. The victim was taken to a hospital and underwent several surgeries. He remained in a comatose state until his death 53 days following the incident. CAUSE OF DEATH: Traumatic head injury. RECOMMENDATIONS: 1. Perform all appropriate lockout/tag out procedures to control hazardous energy before conducting maintenance inside machinery. 2. Before starting a machine, make sure all safety mechanisms are in place, such as interlock devices and machine guards. 3. Employers must train all workers assigned to operate or maintain machinery on appropriate energy-control procedures for that specific piece of equipment. 4. Employers should conduct regular hazard inspections and follow through with necessary corrective actions. CAUSE OF DEATH: Massive abdominal injury RECOMMENDATIONS: 1. On hillsides, stay alert for loose objects. 2. Make sure the landing area is large enough to keep logs from rolling or sliding back down the hill. 3. Make sure that workers at the landing site can communicate with the crew on the ground. 4. Training and supervision is necessary for inexperienced personnel working in the woods. The alley was "L" shaped. One side of the alley was the wooden barn wall with concrete support posts and a concrete quarter-wall slab. The other side of the alley was constructed of portable wooden gates made of 2-inch by 6-inch pieces of lumber. The livestock trailer was at the front of the barn. To be loaded into the trailer, the heifer entered the alley from the yard through a barn door, walked down a 36-foot-long alley, made a 90-degree right turn, walked down a 23-foot-long alley, and then was directed by an angled portable gate into the livestock trailer. The incident occurred about 20 feet into the 36-foot-long alley. The decedent was kicked in the head by the heifer. This was an unwitnessed event, so it is unknown how the decedent was positioned at the time of the incident. The hauler heard the commotion inside of the barn and discovered the decedent on the floor. He called 911 and emergency response arrived. The decedent was transported to a local hospital where he died the next day. Recommendations: 1. Livestock farmers should ensure that the alleys to loading chutes are designed and constructed to maximize safe cattle handling. 2. Livestock farmers should review each individual cow's behavioral characteristics and determine the flight zone around each cow so that the handler can provide a safe loading experience for the cow as well as for the handler. 3. Livestock handlers should conduct a risk assessment, including how to minimize unintended animal contact. After the driver exited his driveway, he drove down the street parallel to the tracks which ran north and south. There was a train on the tracks traveling approximately 33 miles per hour. The train engineer, after clearing some trees located between the railroad tracks and the dump truck driver's house, observed the dump truck driver approach the railroad crossing, applied the train brakes and shut down power to the engine. The train struck the dump truck on its passenger side, pushing it over onto its right side. The driver, who was not wearing a seatbelt, was ejected from the truck and pinned underneath the front right tire. The train engineer called emergency medical services who was dispatched immediately. Emergency medical services arrived and transported the driver to a nearby trauma hospital where he arrived at approximately 7:07 AM. He died 5 ½ hours later due to multiple blunt force injuries sustained in the crash. To prevent future occurrences of similar incidents, the following recommendations have been made: 1. Dump truck drivers should obey all traffic guidelines pertaining to railroad track crossings. 2. If a commercial vehicle stalls on a railroad grade crossing, the driver should immediately exit the vehicle and contact emergency services. 3. Commercial drivers should always wear safety belts while operating a motor vehicle. 4. The state of Kentucky should adopt a vegetation clearance code/regulation pertaining to sight of clearance along railroad tracks and crossings. The three-person work crew consisted of the rotomill operator and two ground men, one of whom was the decedent. The rotomill had made two passes on an asphalt road and was in the process of a third pass. The dump truck into which the mill was emptying needed to be changed out because it was filled. The decedent decided to perform some required measurements during the downtime in the operation while the dump trucks were being changed out. The decedent sketched the jobsite to record his measurements on a notepad. With a digiroller in hand, he walked across the road. Apparently unbeknownst to the decedent, as the full dump truck was exiting (Truck #1), another dump truck driver (Truck #2) was backing his empty dump truck into position by the rotomill from his staging position. It appears the decedent waited for Truck #1 to pass him, and then he walked behind Truck #1 into the path of the Truck #2. He was struck by the driver's side rear wheel and run over. Emergency response was called and the decedent was taken to a local hospital where he was declared dead. Recommendations: 1. Employers should develop, implement, and enforce an internal traffic control plan (ITCP) that minimizes backing distances through work zones and exposures of workers on foot to moving vehicles and equipment. Because many times multiple contractors are present at a job site, the ITCP must be shared with all employers and their employees. 2. Employers should develop and implement specific training for mobile equipment operators and workers on foot regarding driver blind areas on equipment. 3. Employers should develop, implement, and enforce a comprehensive written safety program, which includes training in hazard recognition and the avoidance of unsafe conditions. 4. Employers should ensure backing procedures are in place for the use of mobile construction vehicles and that drivers have communication with workers on foot and use a designated spotter to direct backing. 5. Employers should consider the use of proximity warning devices such as radar and sonar based systems and/or rear-view camera systems to enable operators to detect when someone is near or approaching a vehicle or piece of machinery. 6. Employers should ensure that pedestrian workers wear a high visibility vest, shirt or jacket when working in a road construction zone. 7. The U.S. Occupational Safety and Health Administration (OSHA) and MIOSHA should initiate rulemaking to require new safeguards for employees on roadway construction worksites. The 49-year-old victim, a member of the Laborers' International Union of North America, had worked in the construction industry for 25 years, of which the last 1 ½ years were with his current employer, a company that installs underground utilities such as water and sewer lines and other structures. On the day of the incident, the victim was assigned the task of repairing a broken section of a concrete stormwater catch basin. The catch basin was erected in a 14 ft. deep trench excavation. The 84" type-2 catch basin was 8 ft. 8 inches high and was set on a 1 ft. base. A protective shield 8 ft. tall x 10 ft. wide x 10 ft. long was set in the excavation surrounding the catch basin. The victim attempted to access the basin to begin the repair work by stepping down from the edge of the excavation onto the protective shielding and then onto the concrete edge of the catch basin. As he stepped down, onto the edge of the catch basin, he lost his footing and fell head first into the basin, landing 9 ft. below. He was taken to a hospital where he died two days later of injuries to his head and spine. The employer should install after market devices (i.e., camera, radar, and sonar) on construction vehicles and equipment to help monitor the presence of workers on foot in blind areas. 3. Employers should ensure that safe work procedures are in place for the use of mobile construction vehicles, a spotter is designated to direct movement of these vehicles, and drivers are in communication with workers on foot. Coworkers attempted to free the victim and notified Emergency Medical Services (EMS). EMS responded to the incident site within minutes and pronounced the victim dead at the incident site. The Medical Examiners Office was notified and arrived to remove the victim's body. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Ensure that all exposed and accessible sections of conveyors are guarded. 2. Routinely conduct a job safety analysis (JSA) to ensure equipment and current practices are safe. 3. Designate only workers who have been trained to perform or assist in required machine maintenance tasks. 4. Ensure that all workers have knowledge of worksite hazards by providing safety training in the workers' primary language(s) and at appropriate literacy levels. 5. Ensure that workers employed through temporary agencies are provided site and task specific safety and health training. 6. Develop, implement, and enforce a comprehensive hazardous energy control program including lockout/tagout procedures and routinely review and update the program and training. In addition, manufacturers of equipment with conveyors should: 7. Ensure that all moving sections of conveyors are guarded. 8. Seeing the dump truck driver running and waving his arms in his mirror, the Tack truck driver stopped. At the front of the Tack truck both drivers found the victim lying face down on a manhole cover on the ground. A laborer ran and notified a flagger who then called 911. Approximately 4 minutes later, Emergency Medical Services (EMS) and the police arrived on the scene. EMS determined that the victim was not breathing and pronounced him dead on the scene. A coroner arrived on the scene and at approximately 2:21 p.m., the victim was transported to the morgue. These deaths are all the more tragic because they are largely preventable. Information about when and how they occur is essential in order to target effective prevention programs. In Massachusetts, the Occupational Health Surveillance Program (OHSP) in the Massachusetts Department of Public Health (MDPH) collects information on all fatal occupational injuries in the Commonwealth as part of the national Census of Fatal Occupational Injuries (CFOI), conducted in cooperation with the Bureau of labor Statistics (BLS), U.S. Department of labor. OHSP also conducts in-depth work site investigations of targeted fatal occupational injuries as part of the national Fatality Assessment Control and Evaluation project (FACE), sponsored by the National Institute for Occupational Safety and Health (NIOSH). The purpose of the FACE project is to develop a detailed understanding of how fatal injuries occur and to identify effective countermeasures to prevent similar incidents in the future. Excerpts from selected FACE investigations are highlighted within this report. This update provides an overview of fatal injuries at work that occurred in Massachusetts during 2005. These include fatalities traditionally linked with factors in the work environment such as falls, electrocutions, and exposure to toxic substances. They also include homicides and suicides at work, as well as motor vehicle-related fatalities that occurred during travel on the job. Deaths resulting from occupational illnesses and heart attacks are excluded from this fatality update. In Massachusetts, 75 individuals were fatally injured at work during 2005; 69 were men and six were women. The annual rate of fatal occupational injury in Massachusetts for 2005 was 2.3 deaths per 100,000 workers. This rate is similar to fatal occupational injury rates for Massachusetts reported for the previous two years. The victims ranged in age from 21 to 75 years, with an average age of 44 years. The 75 fatalities resulted in a total of 2,315 potential life years lost, an average of 31 potential life years lost per death. Potential life lost is the difference between the victim's age and 75 years. Fifty-seven (76%) victims were White non-Hispanic; six (8%) victims were Black non-Hispanic and 4 (5.3%) were Asian non-Hispanic. Six victims (8%) were Hispanic, four of whom were foreign born. The rate of fatal occupational injury among White non-Hispanics was 2.1 deaths per 100,000 workers. The rate among Black Non-Hispanic workers was 3.8 deaths per 100,000 workers, and the rate among Hispanic workers was 2.9 deaths per 100,000 workers. Twenty-two victims - approximately one in four workers fatally injured at work in 2005 - were born outside of the US. Six of these workers were from Central or South America, five from Africa, four from Asia, and three from Southern Europe. Foreign-born victims worked in a range of industries. The rate of fatal injury among foreign-born workers was 4.0 per 100,000 workers compared with the rate of 1.9 deaths per 100,000 workers for US-born workers. Of the 75 workers fatally injured, 15 were self-employed. The fatal injury rate among self-employed workers was 4.7 deaths per 100,000 workers while the rate among wage/salary earners was 2.1 deaths per 100,000 workers. One-third (33%) of the self-employed victims were working in the construction industry as compared with one-fifth (20%) of the wage/salary earners. Thirty-four (45%) victims worked in small employer establishments (with 10 or fewer employees). Eleven (32%) of those victims worked in construction, nine of whom were fatally injured at residential construction sites. Occupational and Environmental Medicine AB - On January 7, 2006, a 34-year-old female firefighter was critically injured after being struck by a pickup truck that had lost control on an icy interstate highway. The fire department responded after being coded as a first call response by 911 dispatch that an accident had taken place on a local highway with occupants trapped. An engine was dispatched with three firefighters, the engine driver, lieutenant and the decedent. The incident occurred near a freeway entrance ramp. The entrance ramp was blocked by a car and pickup truck that had been involved in an accident at the ramp junction with the highway. The lieutenant and the decedent exited the engine and walked on the side of the freeway to the accident site while the engine driver repositioned the engine. As they were walking, a pickup lost control on black ice that had formed on the highway surface. The pickup slid out of control and struck the decedent. She was thrown off the highway shoulder onto the grass bank alongside the highway. Other emergency vehicles responded and she was transported to a local hospital. She died one week later from her injuries. After the incident, the fire department developed a standard operating procedure (SOP) for emergency response on roadways that incorporated the first four work practice recommendations listed in the RECOMMENDATIONS section of this report. Recommendations: 1. fire departments develop, implement, and enforce standard operating procedures/guidelines (SOPs/SOGs) regarding emergency operations for roadway incidents and employees should receive training in the proper procedures and the hazards associated with emergency operations for highway incidents. 2. Fire departments ensure that firefighters establish a protected work area on roadways before safely turning their attention to the emergency. 3. Fire departments establish pre-incident plans regarding traffic control for emergency service incidents and pre-incident agreements with law enforcement and other agencies such as highway departments. 4. Fire departments ensure firefighters wear suitable high-visibility apparel meeting American National Standards Institute (ANSI) 107-2004 requirements when working as an emergency responder on a roadway. 5. The State of Michigan should adopt a training module specific to operational practices in or near moving traffic as part of the Office of Fire Fighter Training firefighter training program. 6. Michigan Department of Transportation (MDOT) should continue expanding the use of a "changeable message sign" to inform motorists of hazardous road conditions or vehicular accidents. 7. The International Fire Service Training Association (IFSTA) should update the Fire Fighters Training Manual to include a chapter on emergency response vehicle positioning to protect emergency response workers. The victim was the only employee at the company who knew how to operate the machine, and had only received a few hours of instruction in its use by the manufacturer's representative. The CA/FACE investigator determined that in order to prevent future occurrences, employers, as part of their Injury and Illness Prevention Program (IIPP), should: 1. Ensure employee safety by installing guards around the moving parts and pinch points of the machine to protect employees from accidental contact. 2. Ensure employees only use machines that are properly guarded and do not place any part of their bodies into areas where they might become entangled with machinery when it is running. 3. Ensure employees are properly trained on machine operation and safety and their achievement of skills is verified through a testing program. 4. Ensure employee safety by preparing and periodically updating a hazard analysis for all equipment. In addition: 5.
