Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

At the end of the workday, the operator and a coworker were responsible for fueling the chipper at the landing site, and were using a skidder to go to retrieve the fuel truck, located about 750 feet away up a dirt skid road. The operator rode on the step outside the cab - which is not intended for passengers - holding onto handrails on the door and rear of the cab for support. The skidder was running in second gear at half throttle on nearly level ground. The two skidder operators were talking during the ride, until the coworker noticed his passenger did not respond. The coworker discovered the operator lying in the road behind him, about three-quarters of the way to the fuel truck. The victim had been crushed by the right rear wheel of the skidder. The coworker alerted other workers onsite and basic first aid was attempted. The victim died at the scene shortly after the arrival of first responders from the local fire department. Cause of Death: Severe head trauma. Recommendations: 1. Never ride as a passenger on a machine unless an appropriate seat and operator restraint system is available. 2. Employers must develop a formal training process for operators of mobile machinery, including written documentation and regular evaluation. 3. Employers must demonstrate a commitment to safety and encourage employee participation. It begins with information on fatal occupational injuries collected by the FACE (Fatality Assessment and Control Evaluation) project. The second section provides an overview of non-fatal injuries to teens using the available statewide emergency department data (2002-2004) and data on workers' compensation claims for injuries resulting in five or more lost work days. In the final section, findings based on interviews with injured teens are highlighted. For this report, injuries in the ED database are defined as ED visits with a primary ICD-9 diagnostic code between 800-999. Follow-up visits are not included unless the original injury was not already in the database. Injuries in the workers' compensation claims database are defined as claims with Occupational Injuries and Illnesses Classification System Nature of Injury codes between 0000-0990. The rates presented in this report are defined as the average annual number of injuries (ED visits or WC claims filed) per 100 full-time workers. Information on the number of teens employed was obtained from the 2000 Census. Rates are computed only for workers 16- and 17-years-old because employment statistics exclude those younger than age 16. The ED and WC cases are not mutually exclusive. Some of the cases treated in EDs may have also filed workers' compensation claims for lost wages. Data Highlights: 1) From 2000 through 2004, six teenagers under age 18 were fatally injured while working in Massachusetts. Three of these teens were doing tasks or jobs prohibited by the child labor laws at the time of injury. 2) From 2002 through 2004, there were 3,012 emergency department visits for work-related injuries to teens. 3) From 2000 through 2004, there were 1,410 workers' compensation lost wage claims filed by teens in Massachusetts. 4) The majority of injuries were to 16- and 17-year-olds, and males had higher rates of injuries than females based on both emergency department visits and workers' compensation claims. 5) The largest numbers of non-fatal injuries to young workers from 2000 through 2004 occurred in restaurants (30%), followed by grocery stores (13%). The highest rate of injury was in nursing homes. 6) Of 798 injured teens who were interviewed since 1993, 51% reported they had received no on-the-job training about how to work safely and avoid injury, 34% reported they had no work permits for their jobs at the time they were injured, and 17% reported one or more anticipated permanent effects from their injuries. The victim's employer was a construction company hired by the county to build underground tunnels to transport sewage as part of a wastewater treatment system. The victim was working with a crane operator, and two carpenters serving as riggers and signal persons, constructing a sound barrier wall that would encircle the construction site. The sound wall consisted of 40-foot-long, 1,400 lb. H-beams that were sunk 15 feet into the ground to serve as pilings for the support of wooden panels. The victim was preparing a welding torch for burning a hole in the piling once it was set so that braces could be attached to the piling. He was working about 10 feet from where the horizontal beam was laying prior to being lifted by a 50-ton mobile crane and placed vertically in the hole were it was to be set. The rigging for the beam/piling lift consisted of a locking lifting clamp attached to a 12-foot polyester lifting sling which was attached to the crane's hook. The rigger/signal person attached the clamp to the web of the beam and signaled the crane operator to begin the lift. As the beam was being lifted from its horizontal to vertical position it swung to the side and began to twist, causing the clamp to be side-loaded. The beam slipped out of the clamp and fell about 30 feet, striking the victim. He was transported to a hospital where he died. An investigation determined that the victim, while not directly under the load, was in its swing radius; the clamp was not centered on the beam's web; mud on the clamp's cam and pad may have contributed to side loading the clamp; and the rigger/signal person was inexperienced. An investigation determined that the victim, who was not wearing the provided seat belt, had been ejected from the seat of the skid steer loader and was crushed under the loader's bucket. The victim had not been trained in how use a skid steer loader. The head deacon told the decedent to go downstairs and get into fresh air. The decedent collapsed in a nearby upstairs room. When the head deacon went downstairs, he could not find the decedent. Two female church members were working in the church area. The head deacon asked them if they had seen the decedent and they replied they had not seen him. All three individuals went upstairs and found the decedent on the floor in the room. One of the female church members stayed with the decedent and provided CPR. The other church member and the head deacon went downstairs and the female church member called 911. When the ambulance arrived, they could smell the exhaust from the generator and decided not to enter the church. The ambulance called for fire department assistance. When the fire department arrived, they entered the church and proceeded to the second floor room where the decedent and the now unconscious female church member were located. The fire department brought the decedent and this church member outside and they began resuscitative efforts. Fire department personnel transported the decedent and the unconscious church member to a local hospital. The decedent was declared dead at the hospital. The church member was hospitalized and recovered. Recommendations: 1. Generators or similar fuel-burning equipment, such as propane radiant heaters, should not be operated indoors or in enclosed or partially enclosed spaces. 2. Carbon monoxide (CO) detectors/alarms that meet the requirements of the current Underwriters' Laboratory (UL) standard 2034 or the requirements of the International Approval Services (IAS) 6-96 standard should be installed on each building level in areas when fuel-burning devices are present. 3. Portable generator manufacturers should label their products and should supply stores carrying their equipment with the Danger label required by the 2007 Consumer Product Safety Commission. In 2006, a 54-year-old male mechanic died from CO poisoning while sitting in a box truck's cab with a gasoline-powered generator operating in the back of the truck. In 2006, a 43-year-old male finish carpenter died from CO poisoning while working inside a metal storage container with a gasoline-powered generator operating to power a light. In 2006, a 38-year-old male dock worker died from CO poisoning while using a gasoline-powered pressure washer to clean a freshwater tank on a fishing vessel. As with most work-related fatalities, these deaths could have been prevented. Place fuel-burning equipment outdoors, away from open windows, doors and vents. What is Carbon Monoxide? Carbon monoxide (CO) is a poisonous, colorless, odorless and tasteless gas produced by burning fuel, such as gasoline, kerosene, oil, propane, coal or wood. When fuel-burning equipment, tools and appliances are used in enclosed places, or places without good ventilation, CO levels can build up quickly. How to prevent CO poisoning in the workplace: Avoid using fuel-burning equipment indoors or in enclosed or partially-enclosed spaces, such as inside houses, garages, crawl spaces, basements, storage areas and tanks. Place fuel-burning equipment outdoors away from windows, doors or vents which could allow CO to enter and build up in the work area. Use tools powered by electricity, batteries, or compressed air when working indoors. If fuel-burning equipment must be used indoors, be sure to vent equipment exhaust outdoors and provide fresh air ventilation to the work area. Even with doors and windows open, CO levels from fuel-burning equipment can still reach dangerously high levels quickly. Ensure a trained professional annually inspects and services fuel-burning equipment and appliances, such as an oil or gas furnaces, gas hot water heaters, or portable compressors. When measuring tailpipe exhaust of LPG fork trucks to minimize CO output, use a CO analyzer specifically designed for that purpose. 5. Use tools powered by electricity or compressed air approved for indoor use when working indoors. 6. Place gasoline- or fuel-powered equipment outdoors and away from windows, doors or vents. CO could enter the building through the openings and build up in the work area. 7. If gasoline- or fuel-powered equipment must be used indoors or in enclosed or partially enclosed spaces, such as houses, garages, crawl spaces and basements, vent equipment exhaust outdoors and away from air intakes such as doors, windows or fresh air vents. Provide supplemental fresh air inside the building because even with doors and windows open, CO levels can reach lethal levels in a short period of time. The gas company provider whose pipeline was involved in the incident did not make a Positive Response. Beginning at the transformer location, the decedent began to plow the cable approximately 36 inches deep into the ground, heading south towards a corner marker where he had to make a 90-degree turn to the west. Approximately 58 yards after he turned the corner, the vibratory plow hit a 24-inch high-pressure natural gas pipeline that was buried approximately 36 to 40 inches below the existing grade. The rupturing pipeline created an explosion throwing dirt and debris into the air and created an 80-foot crater. His coworkers, who were uninjured, called 911. After the explosion occurred, Coworker #1 and Coworker #2 noticed two yellow natural gas pipeline markers at the edge of the road. Approximately eight hours later, the pressure in the line was stabilized and reduced to a condition so firefighters could approach the crater to remove the decedent and the vibratory plow from the crater. The decedent was pronounced dead at the scene. Recommendations: 1. Employers conducting excavating or tunneling operations should ensure employees receive hazard recognition training pertaining to utility markers. 2. Employers conducting excavating or tunneling operations should instruct and ensure employees conduct a thorough site inspection to determine if all potential underground utilities are marked prior to beginning digging operations. 3. Employers conducting excavating or tunneling operations should contact MISS DIG if public utilities/agencies listed on the MISS DIG ticket have not marked their lines or provided a Positive Response in response to the MISS DIG request. 4. Employers conducting excavating or tunneling operations should describe the area to be staked as specifically as possible when calling MISS DIG. 5. At the base of the excavation was a French drain system that permitted the water to be drained away. A part of the excavation had been laid with geofoam blocks and had already been ballasted with soil. The incident occurred on the section of geofoam foundation that was not ballasted. The French drain had backed up causing the geofoam blocks to float on the water. After the water was drained, a section of geofoam did not settle to grade. After entering the excavation and standing on the top layer of geofoam pad, the decedent spoke with a representative from the company that had laid the block (Company A) about upcoming work at the site. A wind gust of approximately 53 miles per hour (mph) came from the southwest direction. The force of the wind caused geofoam layer #3 to rise up about 12 feet like a wave. The wave of geofoam block collapsed and the wind began to blow the individual blocks at the decedent and the Company A employees who were removing the block from the pad. Six or more Company A employees and the decedent were struck by the wind-blown blocks. Emergency response was called when the decedent was found lying on the pad, bleeding from his nose and having difficulty breathing. The decedent was transported to a local hospital where he was declared dead. Recommendations: 1. Contractors should secure/ballast geofoam block edges in accordance with manufacturer specifications for installation and storage. 2. Construction employers should conduct a daily hazard assessment to determine if environmental working conditions have changed or will change. They should inform their employees of their findings and how the changing conditions may affect the work to be performed. 3. Trade groups involved in the manufacture and installation of geofoam should develop a guideline for geofoam applications as foundation material in excavations. The ladder's safety feet were in an up position on the frozen soil. He called to his coworker to hold the ladder while he accessed the roof area. The coworker stood underneath the ladder and held rung #5 with his right hand and rung #7 with his left hand. The decedent climbed the ladder holding the wood, to either rung #8 or #9 when the base of the ladder slipped away from the house. The falling ladder struck the coworker on his shoulder and arm and knocked him to the ground. The decedent fell to the coworker's left and landed on his back. 911 was called and the decedent was taken to a local hospital. After assessment, the decedent was transported to another hospital where he died six days later. Recommendations: 1. Employers should ensure that ladders are used in accordance with the requirements of existing safety standards and good standard practice. 2. Employers should develop and implement a comprehensive written safety program. 3. Construction employers should conduct a daily hazard assessment to determine if environmental working conditions have changed or will change. They should inform their employees of their findings and how the changing conditions may affect the work to be performed. 4. Employers should consider having at least one person on the jobsite certified in First Aid/CPR, should strongly consider having an individual certified as a Medical First Responder or Emergency Medical Technician (EMT), and hold at least semi-annual workplace rescue/first aid practices. 5. He went on to the roof wearing his fall harness, attached his safety rope, and engaged his fall protection system. He then began laying felt paper. At 8:59 a.m. the victim fell 23 feet from the roof, landing on the ground below. The homeowner found the victim and called emergency medical services and the victim was transported to a hospital. The victim was conscious and stated that he had disconnected his safety rope from his fall harness so that he could untangle it. He then stepped into the opening of a small vent pipe that was covered with felt paper and lost his balance, causing him to fall. Sixty-seven days later the victim died from medical conditions aggravated by his fall injuries. Every year workers get sick and some die from carbon monoxide poisoning caused by exposure to fuel-powered tools and equipment. Small gasoline and other fuel-powered engines and tools present a serious hazard with serious consequences. This incident occurred in a janitorial/commercial cleaning company; however, CO exposures exist in workplaces across industries where fuel-powered engines are used. CO is a colorless, odorless gas that is produced by burning fuel, such as gasoline, kerosene, oil, propane, coal or wood. It can overcome you without warning. Gas levels can build up quickly in enclosed spaces. Types of equipment that have caused similar incidents include: propane buffers, generators, space heaters, pumps, blowers, compressors, concrete saws and forklifts. Preventing CO Poisoning from Powered Equipment: 1. Identify all potential sources and exposures to carbon monoxide gases. Refer to equipment manufacturers' safety guidelines and operating procedures. 2. Employers should educate workers on hazards, symptoms, sources and control of CO exposure. Training should be compatible with workers' language abilities. 3. Do not use gasoline or other fuel-powered engines, equipment or tools inside buildings or enclosed areas where carbon monoxide levels can quickly build up. Simply opening doors and windows may not provide adequate ventilation. 4. Use engines powered by electricity or compressed air approved for indoor use when operating indoors or in enclosed spaces. 5. Use local exhaust ventilation to remove CO and other exhaust gases from areas where there are no other options but to use gasoline-powered equipment. 6. Monitor the workplace exposure for carbon monoxide. Use monitors that have alarms to warn workers of high levels of carbon monoxide in the work area. 7. Even if outside, place fuel-powered equipment away from doors, windows or vents that can cause CO to build up inside. At the end of the 8-inch line, there was a vic fitting end cap (also referred to as a victaulic, groove lock or gasketed end cap). The decedent was removing the 25-pound vic fitting end cap from the 8-inch line in the main boiler room in preparation for continuing the waterline project. The metal cap struck the decedent in the neck and upper chest area when it blew off the end of the pipe. Fellow workers called 911. Emergency response arrived, and the decedent was taken to a local hospital where he was declared dead. Recommendations: 1. Employers should ensure that their written accident prevention plan identifies and describes all hazards that could be encountered in the worksite and how to recognize and avoid them. 2. Employers should develop a specific standard operating procedure for pipe pressurization/depressurization including securely tagging appropriate piping and pipe components, such as caps, plugs, valves, etc. 3. Employers should use an end cap that remains attached to the pipe during loosening and prior to removal, or outfit the end cap with a pressure-relieving device. 4. Employers should periodically reinforce skill and safety training. The victim was wearing a full-body safety harness with a retractable lanyard that was anchored to the rolling roof outrigger. The victim's co-worker was suspended approximately five feet down the side of the building from the rolling roof outrigger. As the victim was trying to reposition the rolling roof outrigger, it rolled to the edge the roof and then rolled off of the roof dragging the victim over the roof edge. The victim, co-worker and the rolling outrigger all fell to the ground below. The rolling roof outrigger landed on top of the co-worker. Calls were placed to the local police and fire departments. Within minutes, police and fire department personnel arrived at the site to attend to the victim and his co-worker. The victim and co-worker were transported to a local hospital where the victim was pronounced dead and the co-worker sustained massive injuries but survived. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Consider eliminating the use of rolling roof outriggers on lower rise buildings with flat roofs and unguarded roof edges AB - 2. Ensure that rolling roof outriggers are properly tied back at all times during use to prevent outriggers from falling off the roof AB - 3. Ensure that anchor points for personal fall protection equipment are completely independent from descent equipment AB - 4. Ensure a competent person inspects and evaluates all anchor points and rigging before each descent AB - 5. Obtain owner's manuals for all equipment to ensure that equipment is being used as it was designed to be used AB - 6. Ensure that employees who are hired as window washers are provided training on the proper use of approved descent control devices and appropriate support systems prior to assigning employees any window washing tasks AB - 7. Devise a communication system when the workforce is multilingual to ensure employees can understand general safety and procedural commands. In addition, employers and commercial building owners should: 8. Develop and enforce a plan of service that addresses the availability of a competent person, safety training, and standard operating procedures specifically for window washing operations. On the day of the incident, the company was contracted to install curtain walls (large double paned thermal insulated glass) at the residence. Prior to installation, the workers staged the crates containing multiple plates of glass in such a way that when the crates were opened, the plates of glass would not fall out and the top glass sheet could be easily retrieved. The workers stood the crates on their sides, tilted them to a slight angle and supported the crates with either wooden bars (legs) or another crate of similar size and weight. When crates are staged in pairs, they form an upright triangle or "A" frame with the tops of the two crates touching and supporting each other. The incident happened while the workers were staging two crates (A and B) into an "A" frame. Crate A weighed approximately 1,859 pounds (Lbs.) and Crate B weighed approximately 1,943 Lbs. At the time of the incident, the foreman was standing at the north end of the two crates and a co-worker was at the south end. The victim was at the west side of the crates facing Crate B. The foreman directed the workers to push and tilt the crates towards each other. It was witnessed by other contractors working nearby that Crate A slowly overcame the weight of Crate B. Both crates started leaning towards the victim and fell over and the victim was crushed by Crate B. The foreman called 911 to summon emergency medical services (EMS) immediately. EMS arrived at the site within minutes. The victim was pronounced dead at the scene. New York State Fatality Assessment and Control Evaluation (NY FACE) investigators concluded that to help prevent similar incidents from occurring in the future, glass installation or ornamental metal working companies should: 1. Ensure that a crane is used to stage large and heavy crates of glass and keep the crates attached to the crane until they are stabilized and chocked AB - 2. Ensure that the employees always chock and brace the tilted crates to prevent them from kicking back and collapsing AB - 3. Conduct a job hazard analysis during the planning phase of a glass installation project to identify potential hazards and develop and implement appropriate control measures to protect workers AB - and, 4. Develop written standard operating procedures (SOP) for the safe handling and staging of glass crates and provide employee training and supervision. In addition, manufacturers of "A" frame carts should research and explore the possibility of safely adapting these products for use in construction settings.
