Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

Kentucky State Police arrived and called Kentucky Vehicle Enforcement to the scene. The driver had not been wearing his seatbelt. Toxicology detected methamphetamine and doxylamine in his system at the time of the crash. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Owner-Operators should follow Kentucky laws and wear seat belts while operating a commercial vehicle. Recommendation No. 2: Owner-Operators should comply with mandated participation in a consortium that provides the central coordination of drug screening programs and other services within Federal Motor Carrier Administration guidelines and Department of Transportation regulations. The victim's job was to stand in the pit and guide the beam that was being hoisted by the pile driver (which functions as a crane and a hammer) to the appropriate position to be hammered. While the victim was guiding the pile, before it was properly located, the hammer dropped from its upper position. Since the pile was not in the proper location, the hammer struck the pile on its side, instead of the top. The fatal injury occurred when the beam then fell to the side, striking the victim. NJ FACE investigators recommend following these safety guidelines to prevent similar incidents: 1. Employers should ensure that pile driver operators are properly trained on the safe operation of pile driving equipment. 2. Employers should ensure that pile driver operators are periodically reevaluated on the safe operation of pile driving equipment. 3. A safety and health plan based on a job hazard analysis should be developed and followed. 4. Employers should incorporate specific excavation procedures into the safety and health plan. The Washington Fatality Assessment and Control Evaluation (FACE) Program has evaluated truck driver fatalities and presents the following summary of facts related to the incidents that occurred between 1998-2007. Summary of Key Facts: 1. Eighty-one truck drivers died when their trucks traveling on a public roadway crashed into another vehicle (30 incidents) or left the roadway and crashed (51 incidents). 2. Forty-two truck drivers died in non-crash type incidents which included, among others: a. being struck by falling freight during loading and unloading (14) b. struck by another vehicle or their own unattended vehicle while out of their truck (10) c. falls from truck or loading dock (5) d. electrocution when truck part contacted an overhead power line (3) e. 3. Of the types of trucks driven by the drivers, the most prevalent types included unspecified semi-tractor-trailer (42); log hauling (20); tanker (11); dump (9); and flatbed (9). 4. The fatalities were widely distributed among industries with highest numbers in freight trucking (62); construction (12); logging (10); retail trade (9); and wholesale trade (8). 5. Truck transportation industry (NAICS 484) had 62 fatalities to truck drivers which broke down to the industry sub-classifications of General Freight Trucking, Long-Distance, Truckload (30); General Freight Trucking, Local (20); Specialized Freight (except Used Goods) Trucking, Local (6); and General Freight Trucking, Long-Distance, Less Than Truckload (5); Specialized Freight (except Used Goods) Trucking, Long-Distance (1) . 6. The contractor was hired by a bulk petroleum distributor to apply an epoxy coating to the inside of four 15,000 gallon above ground fuel (gasoline) storage tanks. Prior to the incident, two of the 20' x 11' tanks had been drained of fuel and their interiors, vented with a blower, abrasively blasted, cleaned, and coated with a two-part epoxy. A third tank was being prepared for a permit-required confined space entry by five workers, including the victim, a co-worker, a temporary help agency contracted worker, and two oil company employees. A worker placed an electric blower outside the tank near its base by an open hatch in order to ventilate the tank by sucking air out. The vents at the top of the tank were not opened to allow fresh air to circulate through the tank. The blower was not rated for use in areas where flammable gases or vapors may present. The victim approached the tank to move the blower to do air testing to determine whether it was safe for workers to enter the tank. When the victim moved the still operating blower, a flame shot into the tank where flammable vapors were ignited and an explosion occurred. The victim was killed and a co-worker and two oil company employees received extensive burn injuries. An investigation determined that the probable cause of the explosion was 1) the blower and its extension cord were not rated for use in flammable atmospheres and 2) there may have been a static discharge when the victim touced the plastic housing on the blower. A narrative text analysis of 69 Kentucky Fatality Assessment and Control Evaluation (FACE) agricultural tractor fatality reports from 1994 to 2004 was performed. The FACE reports were developed using the National Institute for Occupational Safety and Health, Division of Safety Research-recommended FACE report format that incorporates Haddon's matrix. Haddon's matrix separates the fatal incident into three event phases and is used to develop points of intervention based on human, organizational, and environmental factors. A multivariate logistic regression analysis for association between identified exposure variables and the outcomes of interest was undertaken. The operation of a tractor with an attached bucket, muddy terrain, and being thrown from the tractor were independent risk factors for being declared "dead at the scene". A tractor rollover and operation of a tractor on a slope were independent risk factors for being "crushed" by a tractor. Narrative text analysis of FACE fatality investigation reports is a valuable tool for the identification of additional factors contributing to tractor fatalities that can inform farm safety training, identify new areas for agricultural interventions, and support the development of new agricultural engineering strategies. As logging intensifies in Asia, Africa, and South America, more workers are exposed to extreme hazards in unmanaged conditions. According to data supplied by the Food and Agriculture Organization of the United Nations (<a href="http://www.fao.org/index_en.htm"target="_blank">http://www.fao.org/index_en.htm</a>), the world's softwood timber harvest since 1965 has been dominated by Russian states, China, and the USA. Many other countries with high production in 1965 have depleted their resources significantly, including Japan, Canada, France, Romania, and Finland, among others. Substantial growth has occurred meanwhile in New Zealand (with plantation timber), Hungary, and developing countries such as Brazil, Viet Nam, Malaysia, and Tanzania. Experience with state regulation and employer commitment to safety in logging activities in the Pacific Northwest may help others to improve safety and reduce injuries and death in logging operations. Kentucky Injury Prevention and Research Center AB - Early in the morning during the winter of 2005, a 31-year-old male semi-truck driver died when the semi-tractor trailer he was driving left the roadway and rolled over into a ditch. He had just picked up a load of raw lumber from a lumber yard and pulled out of the gate onto a county road and driven about a mile when the incident occurred. His destination was a lumber mill approximately 150 miles away. He was driving on a straight stretch of the road when the right tires left the pavement and dropped onto the sloped grassy area next to the road. There was evidence the driver attempted to correct the tractor trailer and return it to the pavement. However, the ground was wet and the slope was steep. The tractor trailer hit a tree and rolled over onto a fence. There were no witnesses to the incident. Emergency medical services were contacted. Upon arrival, they found the driver on the inside of the roof of the overturned, upside down cab without vital signs. The local coroner was contacted who arrived and declared the driver dead at the scene. He had not worn his seatbelt. Toxicology results showed acetaminophen, Doxylamine, and Dextromethorphan (cold medications) were in his system at the time of death. To prevent future occurrences of similar incidents, the following recommendations have been made: 1. Commercial vehicle carriers should implement and enforce a workplace policy that requires drivers to wear seat belts while operating a commercial vehicle. 2. Employers should implement and enforce a policy that prohibits commercial drivers who are ill or taking over-the-counter medications with potential side effects for impaired driving from operating a commercial vehicle. 3. Commercial vehicles should be in compliance with Department of Transportation regulations. 4. Companies should provide professional training for company truck drivers. 5. County roadways in Kentucky should be designed and constructed with an adequate shoulder area, or have guardrails that will deter commercial vehicle drivers from leaving the roadway. 6. Transportation companies should consider equipping semis with global positioning satellite systems. The driver of a coal tractor-trailer (Truck 1) and the decedent driver of a second coal tractor-trailer (Truck 2) had descended a hill in the lane next to one another on a four-lane undivided highway. Truck 1 stopped at a traffic light at an intersection at the bottom of a hill while Truck 2 approached the intersection from behind in the left lane. The light turned green so that Truck 2 did not need to stop for the traffic light. Truck 1 advanced from the traffic light and began to climb another hill. Truck 2 approached Truck 1 on the hill, struck the right rear of Truck 1, veered to the left, struck an embankment, and rolled over. Upon impact, driver of Truck 2 was thrown into the sleeper compartment. Emergency medical services and the local coroner arrived on the scene. The coroner pronounced the driver dead at the scene. To prevent future occurrences of similar incidents, the following recommendation(s) are being made: Recommendation No. 1: Owner-Operators should follow Kentucky laws and wear seat belts while operating a commercial vehicle. Recommendation No. 2: Companies should provide new and refresher truck driver safety training for company drivers including driver distraction , defensive driving techniques and Hours of Service as it pertains to fatigue. Recommendation No. 3: Vehicle stabilizer and sensory systems should be mandatory equipment on all commercial vehicles. Recommendation No. 4: To better prevent rear collisions, companies should establish a safety program to install flashing amber LED lights on the rear of all slow moving trucks. Make sure the tire is deflated before beginning any work on a wheel. Employers should make sure workers complete a comprehensive safety training course before starting any work on wheels. The 49-year-old victim had worked with his employer, a heavy and civil engineering construction firm, for almost a month. The victim was working on a project to construct a tunnel for a light rail transit system. He had never driven a locomotive prior to this job and had received limited on the job training. At about 4 a.m. on the day of the incident the victim was working inside the tunnel, when he discovered that he needed to get some parts to complete the job he was working on. He hitched a ride on a 30-ton tunneling locomotive pulling four rail cars containing construction materials that was going out of the tunnel to a staging area There was no seating for a passenger, so the victim stood beside the operator. The locomotive began to move down an incline and gathered speed on the wet, muddy track. The operator made several attempts to slow it down but the locomotive was going about 50 mph when it came out of the tunnel and crashed into a parked locomotive at the end of the track. The passenger was thrown or jumped from the cab and was fatally injured when he struck a beam. The locomotive operator remained in the cab and received minor injuries. Kentucky Injury Prevention and Research Center AB - During the fall of 2006, a 25-year-old male company dump truck driver (decedent) died when he became entangled in the drive shaft of the power-take-off pump (PTO) underneath a dump truck. He and four other dump truck drivers were waiting to have their trucks filled with asphalt when one of the other drivers (Driver 1) began having difficulty with the bed of his truck raising and lowering properly. Another driver (Driver 2) attempted to assist Driver 1 with the problem, but was unsuccessful. Driver 1 decided to inform the plant manager (Owner 1) of the situation and request assistance from the company's mechanics to fix the problem. While Driver 1 was speaking with Owner 1, the decedent (Driver 3), unbeknownst to anyone else, crawled under the dump truck of Driver 1 to see if he could fix the problem. As Driver 3 lay under the idling truck's spinning PTO shaft, his coat sleeve became entangled in the grease fitting on the front universal joint. He was twisted onto the shaft and under the bed of the dump truck. When Driver 1 returned to his truck to wait for assistance from the company mechanic, he found the decedent caught in the PTO drive shaft underneath the truck. Driver 1 yelled to Owner 1 in the tower to call emergency medical services to the scene. While Owner 1 called 911, he called Owner 2 to the scene. Owner 2 found the decedent entangled in the PTO shaft and with the assistance from Driver 4, they proceeded to cut his clothing to free him from the shaft. Owner 2 tried to resuscitate Driver 3 (decedent) while EMS was enroute. Emergency medical services arrived six minutes later, determined the driver was in critical condition, and notified emergency air care. As emergency medical personnel placed Driver 3 in an ambulance and transported him to a nearby field where the air ambulance waited, he died. Air transport was cancelled and the local coroner was contacted who arrived and declared Driver 3 dead at the scene. To prevent future occurrences of similar incidents, the following recommendations have been made: 1. Ensure that trucks are turned in the "off" position and keys removed, prior to driver exiting and leaving the motor vehicle unattended. 2. Employers should provide comprehensive safety training for all newly hired personnel including temporary and substitute drivers before they are allowed to work in the field. 3. Loose clothing should not be worn while repairing machinery. 4. Employers should require that operators or other competent persons perform daily safety checks on the mobile equipment prior to operating them using an expanded Department of Transportation Inspection form. 5. Educate employees about the hazards of PTO's and ensure they do not crawl under, on, or over unguarded power take-off (PTO) drivelines while the PTO is operating. 6. The decedent and two other carpenters were framing a new construction, single-family residence located on a hillside which was rough and steep. The three carpenters were framing the wall at the rear corner of the house which was high because of the sloping terrain. To reach the area, the crew decided to use a homemade platform as a makeshift scaffold. A fourth worker operated a rough-terrain forklift and elevated the platform with the three carpenters holding the wall to be framed up in the air. The forklift operator lifted the unsecured platform and at approximately seven feet in the air, the platform fell off the forks. Two of the carpenters fell off the platform, which fell on the edge of the foundation above the men's heads. The decedent fell off the left end of the platform which struck him in the head. Emergency medical services were summoned to the scene. Upon their arrival they found the other two carpenters who had fallen together alive and requiring medical attention. They were transported to a nearby hospital and later released. Emergency medical service personnel found the third carpenter to be without vital signs and contacted the local coroner who arrived and declared the carpenter dead at the scene. To prevent future occurrences of similar incidents, the following recommendations have been made: 1. Companies should not attempt to build any "homemade" type personnel platform for a forklift, all personnel platforms must comply with ANSI / ASME standards and the OSHA regulations. 2. Those who use a personnel platform on a rough terrain forklift must use approved fall-protective equipment. 3. Scaffolding should be used in difficult-to-reach framing locations. 4. Personnel should not be transported by material hoists. 5. Forklift operators should be trained by a competent person to operate forklifts correctly and safely. 6. Employers with multicultural workers should design safety training in languages suited for the company workforce. 7. General contractors should ensure through contract language that all subcontractors implement appropriate safety and health programs and training specific to the work to be performed. Small business owners should institute and enforce drug testing policies. The sprayer struck the trailer of the truck. After striking the trailer, the decedent was ejected from the sprayer and landed in a ditch by the side of the road's southbound lane. Emergency response was called and transported him to a local hospital where he died an hour later. Recommendations: 1. Employers should require employees to wear seat belts or seatbelt/shoulder harness restraints when the equipment is equipped with these personal restraint systems. 2. Employers should include the seat belt requirement in their safety manual and safety training programs. 3. After receiving a call about the pothole, which formed during a rain storm, the victim retrieved a backhoe, loaded its bucket with cold patch, and drove the backhoe to the pothole's location. The victim was standing in front of the backhoe's bucket while filling the pothole when he was struck head on by a minivan, crushing him between the backhoe's bucket and the minivan. A call was placed to the local police department by the operator of the minivan. The local police and fire departments and emergency medical services (EMS) arrived within minutes. EMS transported the victim to a local hospital where he was pronounced dead. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, municipalities should: 1. Ensure that work zones are set up, at a minimum, in accordance with the Manual on Uniform Traffic Control Devices (MUTCD), Part 6, developed by the U.S. Department of Transportation Federal Highway Administration AB - 2. Ensure that employees' exposure to moving traffic is minimized when working in and around streets and highways by developing temporary traffic control plans AB - 3. Provide and ensure that employees wear appropriate personal protective equipment, including high visibility vests when working along roadways AB - 4. Develop, implement and enforce a buddy system for employees working in roadways and around moving equipment AB - 5. Develop, implement, and enforce equipment maintenance programs that include scheduled preventive maintenance and timely repairs to equipment AB - 6. Develop, implement, and enforce a comprehensive safety program that includes training on hazard recognition and avoidance of unsafe conditions AB - 7. Provide work environments that, at a minimum, meet all relevant Occupational Safety and Health Administration (OSHA) regulations and industry accepted standards of practice AB - and 8. The job site supervisor instructed the victim to remove two steel 1,850 lb. horizontal beams, also known as "strongbacks," from a steel sheet pile wall. The strongbacks to be removed were the bottom row of three rows on the 30' high by 45' long steel wall and were about 3 feet from the ground, where the victim was working. The victim was working alone, using an oxy-acetylene torch to cut the gussets on the strongback in order to remove it from the wall. When he cut the last gusset, the strongback fell 3 feet, striking his head and pinning him to the ground. Emergency personnel were called and he was declared dead at the scene. The 25-year-old victim had three months experience installing siding while working for his employer, a residential construction contractor. He was working on a remodeling of an apartment complex which included re-siding the building exterior. Prior to installing siding, the victim was working from a plank on a pump jack scaffolding sy applying tar paper around a third-floor window. He was not using fall protection equipment, nor was there a guardrail on the scaffold. The victim was reaching for tape that another worker was handing him from a plank below, when he fell about 30 feet to the ground below. While climbing the scaffold, he may have had a cerebral infarction (blockage of the flow of blood to the cerebrum, causing or resulting in brain tissue death). He died two days later. The scaffold's 60-foot long, 5-foot wide platform access was located 32 feet from the ground. The fixed ladder supplied by the manufacturer was not installed. Two members of the crew, Coworker #1 and Coworker #2, climbed the scaffold tower to the platform to prepare the wall and winterize the scaffold. The decedent arrived and began to climb the tower to access the platform. When the decedent was approximately six to ten feet above the ground, Coworker #1 witnessed the decedent suddenly fall backwards from the scaffold to the ground and then roll to his left side. Coworker #1 descended from the scaffold to assist the decedent. The decedent was unconscious but still breathing. Coworker #2 descended to stay with the decedent while Coworker #1 ran to the general contractor's work trailer for assistance. Emergency response was called. Emergency response arrived, and after approximately one hour, the decedent was airlifted to a local hospital where he died two days later. Recommendations: 1. Employers should ensure that scaffold safety components are provided and appropriately attached to the scaffold to provide safe access to the scaffold platform prior to use. 2. Employers should develop a checklist to ensure all unattached scaffold components are included in the shipment to the site. 3. Employers should periodically reevaluate their organizational commitment and leadership with respect to their safety programs. 4. Company management should consider developing a joint health and safety committee. 5. Hydromobile scaffold manufacturers should review current ladder system design for potential ergonomic modifications to improve ease of worker ascent to/descent from the working platform. The mill worker was working to unplug a jam in a wood chipper while the machine was still running. An electrical switch installed on the chipper bypassed the manufacturer's safety interlock mechanism, and allowed the machine to continue operating when the hood was open. The mill worker stood on a catwalk next to the feed chute with the hood open, trying to clear the jam with a stick, which was ground up without effect. He then threw in a large chunk of wood. A coworker joined him on the catwalk and handed him a picaroon (a metal pike with a hook) to maneuver the chunk of wood inside the chute. Once they heard the wood start to grind, they ran and ducked. This was their routine procedure when clearing a jam. The mill worker was struck on the back of the head, apparently by the chunk of wood he had thrown into the chute, and was killed instantly. Recommendations: 1. Never attempt to remove jammed material from a wood chipper while machinery is in operation. Shut down, isolate, and block all forms of hazardous energy before performing any machine maintenance. 2. Never disable the manufacturer's safety interlocks or other safety features on a machine. 3. Employers must train all workers assigned to operate or maintain machinery on appropriate energy-control procedures for that specific piece of equipment.
