Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The victim walked into the roadway crosswalk and signaled for all traffic to stop. As the student stepped into the crosswalk and started crossing, the victim noticed a motor vehicle quickly approaching. At this point, the victim realized that the motor vehicle was not stopping and the child was going to be struck and got the child to stop. The victim was then struck by the motor vehicle. After striking the victim, the operator of the motor vehicle drove a few hundred feet before stopping. Multiple people placed calls for emergency medical services (EMS). EMS, police and fire department personnel arrived within minutes and transported the victim to a local hospital where she died eight days later. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, municipalities should: 1.) Develop a school route plan that meets the standards set forth in the national Manual on Uniform Traffic Control Devices (MUTCD) AB - 2.) Ensure that school crosswalks are clearly visible to motorists AB - 3.) Ensure that the design of equipment supplied to workers, such as stop paddles, does not prevent employees from using the equipment AB - and 4.) The CA/FACE investigator determined that, in order to prevent future incidents, tree trimming companies and self-employed tree trimmers who trim or remove palm trees should ensure that: 1.) Palm tree trimming or removal is performed or supervised by workers trained in hazard recognition and accepted safe work practices. In addition, homeowners should: 2.) Examination of tire tracks at the scene suggested that the tractor was travelling at a fairly moderate speed down slope when it overturned after either striking a hay bale or making a hard left turn. Emergency responders made no attempt at rapid extrication, and no resuscitation effort was performed. Traumatic asphyxia accompanied by blunt force trauma was listed as the cause of death. RECOMMENDATIONS 1. Farmers should install rollover protective structures (ROPS) on tractors. 2. Farmers should consider equipping tractors and other farm machinery used for operations between sunset and sunrise with additional lighting to improve the operator's ability to identify hazards in low-light conditions. 3. Tractor operators should be trained in the safe use of tractors including behaviors that reduce the risk of tractor rollover such as reducing speeds when traveling down slopes. 4. Equipment operators should be aware of the limitations that some medications may impose on safe equipment operations. The co-worker went to the victim and then placed a call for Emergency Medical Services (EMS) and a call to other co-workers. Within minutes EMS and the local police arrived at the incident location and the victim was transported to a local hospital where he was pronounced dead. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, municipalities should: 1.) Ensure that when performing work in roadways that work zones are set up, at a minimum, in accordance with the Manual on Uniform Traffic Control Devices (MUTCD), Part 6, developed by the U.S. Department of Transportation Federal Highway Administration AB - 2.) It appears that when the victim exited the pickup truck, he fell into the water that then swept him down stream. The victim was found approximately two miles down stream by a search and rescue team the next day. The storage room had a large wooden outer door (cooler door) and an interior aluminum door, which had been sealed with caulking and weather stripping and which had a Danger, Do Not Enter sign affixed. To maintain the atmosphere but still allow for visual observation of the produce, the door to the CA room had a 2- by 3-foot Plexiglas window attached to the door by 18 bolts/wing nuts. The event was un-witnessed. The decedent removed the wing nuts and the Plexiglas window and climbed through the door opening, and entered the CA room. The owner found him collapsed on the floor on his knees with his head leaning against an apple bin. A bag of apples was found next to his body. The owner leaned through the window opening into the CA room, grabbed the decedent by his coat, pulled him toward the door through the window opening, and checked for a pulse. He did not find a pulse. The owner left the area to call the farm office to have them call for emergency response. Upon returning to the decedent, he took a deep breath, leaned through the window opening, grabbed the decedent under his arms and pulled him through the window opening and out of the CA room. The owner attempted CPR after he pulled the decedent out of the CA room but was unsuccessful in resuscitating him. Emergency response arrived and he was declared dead at the scene. A bag of a different type of apple obtained from another CA room was found on the propane-powered forklift that was parked in the aisleway nearby. The Plexiglas window was found leaning against a wall with the wing nuts nearby. Recommendations: Recommendations that directly address the factors contributing to the fatality: 1.) Evaluate the workplace to determine if a confined space and/or permit-required confined space (PRCS) is present. If a PRCS is/are present, the commercial facility owner and/or farmer should develop and implement the necessary sections of MIOSHA Occupational Health Standard Part 490, Permit-Required Confined Space and MIOSHA Safety Standard, Part 90, Confined Space Entry. 2.) The employer should develop and implement a written safety and training program that includes the MIOSHA Right-to-Know standard (Parts 92 and 430). 3.) Controlled atmosphere generating equipment and door manufacturers should include a confined space warning sign(s) on the equipment/door exterior. Recommendations to commercial fruit and vegetable storage and warehousing facilities and to farmers storing fruits and vegetables in a similar way: 4.) Instruct employees/family members/visitors not to enter a confined space or other designated area to rescue an ill/injured individual unless trained and equipped with appropriate rescue equipment. 5.) Utilize electric forklifts when loading and unloading product in a controlled atmosphere (CA) room. 6.) Maintain and tune propane-powered forklifts for operations associated with CA rooms and any interior building use. If the forklift was manufactured prior to 2007, equip the forklift with a catalytic converter to further minimize carbon monoxide (CO) emissions, or lease/use a forklift manufactured after 2007. Since neither good maintenance nor a catalytic converter eliminate all carbon monoxide, this approach is not as effective as switching to an electric forklift. 7.) If propane-powered forklifts are used to load and unload product from a CA room, ensure the ventilation equipment in CA rooms is operating. Conduct air monitoring to determine the CO concentration in the room to protect the forklift operator from an overexposure. Consider using direct reading CO monitors (room or personal). 8.) Place oxygen and carbon monoxide air monitoring air portal lines or equipment at various locations in the CA room to better characterize the atmosphere in the room. 9.) Alert employees/family members/visitors to the hazard of sealed storage areas even when no oxygen drawdown has been done. These areas may have lower oxygen levels than expected. 10.) Establish a CA room opening procedure to minimize inhalation hazards posed by low oxygen levels. Editor's Note: Operations involved with agricultural commodities need to determine the "industry" status of the work activities within their operations. The same work activity can be either "Agricultural" or "General Industry" and be covered by different regulations. For the purposes of this report an "agricultural operation" means the work activity designated in major groups 01 and 02 of the Standard Industrial Classification manual, United States Bureau of the Budget, 1972 edition. Agricultural operations include any practices performed by a farmer or on a farm as an incident to or in conjunction with farming operations including preparation for market, delivery to storage or market, or to carriers for transportation to market. Individual state definitions may vary. Recommendation to MSU Extension: 11.) Update the Controlled Atmosphere Storage and Warehousing Clinic manual to comply with MIOSHA health and safety standards. This motion caused the victim to lose his balance and fall approximately 20 feet to the ground below. The victim landed to the right of the ladder on cement steps. A call was placed for emergency medical services (EMS) by the co-worker. EMS and police personnel arrived within minutes and transported the victim to a local hospital where he was pronounced dead. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1.) Ensure that ladders are not used as work platforms, when feasible AB - 2.) Ensure that ladders are equipped with stabilizers and are set up properly AB - 3.) Co-workers freed the victim from the fallen tree and called for assistance. When emergency responders finally arrived on the scene the victim was pronounced dead. The CA/FACE investigator determined that in order to prevent future incidents, employers who employ tree trimmers should ensure that: 1.) Every tree removal job is assessed for potential hazards posed by dead trees, and safe work procedures to eliminate the hazards are implemented. 2.) Tree trimming and felling should be performed or supervised by workers trained and certified by organizations such as the ISA (International Society of Arboriculture). Department of Public Health AB - On April 4, 2008 a 61-year-old male forklift/baler operator (victim) was fatally injured when he was crushed between the cage and mast of the forklift he was operating. The victim was positioning a pallet loaded with waste corrugated cardboard boxes on top of other stacked pallets inside a tractor trailer. The pallet loaded on the forklift tines was lowered onto the stacked pallets inside the trailer, but would not slide off the raised forklift tines. The victim exited the forklift operator's area to assess where the pallet was caught. While climbing up onto the exterior front section of the forklift and then reaching up between the forklift cage and mast, the victim's foot came in contact with and engaged the forklift's mast tilting control lever. The mast tilted back, crushing the victim between the mast and forklift cage. The victim was found by a co-worker who placed a call for emergency medical services (EMS). Co-workers then freed the victim from the forklift. The local fire department arrived within minutes and started cardiopulmonary resuscitation (CPR). The victim was then transported to a local hospital where he was pronounced dead. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1.) Ensure that forklift operators never position themselves or any part of their bodies between the forklift operator's cage and the upright of the mast while the forklift is running AB - 2.) Ensure that standard operating procedures (SOP) are followed during forklift operation and that SOPs include alternative procedures for non-routine tasks, such as inability to lower the tines AB - 3.) Ensure adequate light is provided in locations where forklifts are operated AB - Ensure that all federal and state required trainings and licenses for forklift operators are up to date AB - and 4.) Routinely evaluate the effectiveness of health and safety programs and activities in the workplace. Forklifts manufacturers should: 5.) Design forklifts so that operators cannot place any parts of their body in between the cage and mast and unintentionally engage the mast controls. They were driving a semi-tractor trailer loaded with air freight. It was nighttime AB - the husband was driving, and the wife was in the sleeper berth. There was an unoccupied car parked on the right shoulder. The husband crossed the fog line on the right, sideswiped the car, traveled through a guardrail, and down an embankment toward a road below the interstate. During the trajectory of the semi-tractor trailer, the unit jackknifed AB - the trailer detached from the semi then struck the semi, and both the husband and wife were ejected. The husband was found on the embankment, and the wife was found under the contents of the semi. A witness to the crash called emergency services. They arrived and transported the husband to the nearest trauma hospital. The coroner was contacted, and upon his arrival to the scene, declared the wife dead at the scene. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Companies should provide new and refresher truck driver safety training for company drivers that includes driver distraction and defensive driving techniques as they pertain to fatigue. Recommendation No. 2: Passengers using the sleeper berth in a moving semi should use the restraint system currently provided by the manufacturer. Recommendation No. 3: Sleeper berth occupant protection systems should be redesigned to provide comfort as well as safety. Recommendation No. 4: Electronic stability systems should be mandatory equipment on all commercial vehicles. Recommendation No. 5: Commercial vehicle carriers should implement and enforce a workplace policy that requires drivers to wear seat belts while operating a commercial vehicle. Recommendation No. 6: A comprehensive motor vehicle safety assessment of Kentucky's interstate system needs to be performed in the area where the collision took place. Employees perform a thorough hazard assessment and implement effective controls prior to using aerial lifts. 2.) Employees not maneuver scissor lifts forward or backwards while in the raised position. In addition, manufacturers should install safety interlocks that prohibit scissor lift travel while the platform is raised. Department of Labor and Industries AB - On September 12, 2008, a cellular tower construction crew foreman was killed when he fell from a cellular tower mast to the roof 24 feet below. The 33-year-old victim was employed as a crew foreman by a company that erects and maintains cellular towers. He had three years experience in the industry and was supervising a crew of five workers to replace a series of antennas on a mast located on the roof of an elevator housing on top of a building. The victim and his crew began their shift at 10 pm. A previous crew that had done prep work for the job. The crew was working by lights from their head lamps. At about midnight, the victim and a coworker climbed onto the mast to begin work when the victim fell to the roof below. He was not wearing fall protection. These deaths are all the more tragic because they are largely preventable. Information about where and how they occur is essential in order to develop effective prevention programs. In Massachusetts, the Occupational Health Surveillance Program (OHSP) in the Massachusetts Department of Public Health (MDPH) collects information on fatal occupational injuries as part of the national Census of Fatal Occupational Injuries (CFOI), conducted in cooperation with the Bureau of Labor Statistics (BLS), US Department of Labor. OHSP also conducts in-depth investigations of fatal occupational injuries as part of the national Fatality Assessment and Control Evaluation (FACE) project, sponsored by the National Institute for Occupational Safety and Health (NIOSH). The purpose of the FACE project is to develop a detailed understanding of how fatal injuries occur and to identify effective countermeasures to prevent similar incidents in the future. Excerpts from selected FACE investigations are highlighted in this report. This update provides an overview of fatal injuries at work that occurred in Massachusetts during 2007 and 2008. These include fatalities traditionally linked to the work environment such as falls, electrocutions, and exposure to toxic chemicals. They also include workplace homicides and suicides as well as motor vehicle-related fatalities that occurred during travel on the job. Deaths resulting from occupational illnesses and heart attacks at work are excluded from this fatality update. Simon and Jose may have been trying to clear a jam when they were crushed. What Went Wrong? Simon and Jose were able climb into the balers. Simon and Jose climbed into balers that were turned on. What Should Be done? Make sure balers have guards that keep workers out of the points of operation. Make sure workers are trained in and follow a lockout/tagout program to shut off power before they reach or climb into balers.
