Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The victim climbed onto the side step of the forklift, which was not designed to accommodate passengers, and held on to the forklift frame. As they neared the gated lot, the victim volunteered to open the gate. Jumping from the forklift while it was moving, the victim's foot caught in the forklift's back wheel guard, and he fell face first to the dirt lot. Unaware that the victim had fallen, the second laborer continued driving the forklift forward until he realized the back left tire had tread onto the victim. The second laborer immediately rolled the forklift back off of the victim. A customer, who was also a physician, heard the forklift driver's screams for help and ran to offer assistance. The physician ordered the employees not to move the victim while a third laborer at the distribution center immediately contacted 911. Emergency medical services arrived at the scene within 15 minutes and called air transport. The victim was air transported and arrived at a major trauma center at 6:15 pm. He died from his injuries at 6:52 pm. Recommendations for prevention: 1. Do not allow passenger(s) to ride on a forklift unless the forklift is specifically designed to accommodate passengers. 2. Employers should provide and enforce forklift operator safety training. 3. Employers should review regulations pertaining to safe operation of forklifts and other equipment prior to allowing their use on the jobsite. There were 143 work-related deaths in 2014, an increase of 9 deaths compared to 2013 (134 work-related deaths) representing 139 employers and 138 separate incidents. A narrative summary of each work-related fatality is in Appendix I. MIFACE educational material, including on-site investigation reports, summaries of MIOSHA investigations, and hazard alerts are located on the MIFACE webpage on the Michigan State University Department of Occupational & Environmental Medicine (<a href="http://www.oem.msu.edu/"target="_blank">MSU OEM</a>) website. Key findings for 2014: The number of work-related deaths (143) and the fatal injury rate (3.2 deaths/100,000 workers) were up compared to 2013 (134 work-related fatalities, 3.1/100,000 workers, respectively). The number and rate of acute traumatic fatalities peaked in the years 1997-2001, were at their lowest from 2004-2005 and during the economic depression in the years 2007-2009 but otherwise have fluctuated during this time period from 134-155 deaths per year with a rate 3.1- 3.3/100,000 workers. Agriculture (25, 17.5%) had both the largest number of work-related deaths and the highest risk of death (30.5 deaths/100,000 workers). Construction was second in both number and risk (23, 16.1%; 16.3 deaths/100,000 workers), and Transportation and Warehousing was third in number and risk (19, 13.3%; 14.7 deaths/100,000 workers) of a work-related death. Retail Trade, although 4th in the number of deaths (13, 9.1%) and tied with Other Services, (13, 9.1%) had one of the lowest risks of death (2.8 deaths/100,000 workers compared to Other Services 7.6 deaths/100,000 workers). The two most common causes of death were from motor vehicles and struck-by incidents (28 each, 19.6 %), then falls (24, 16.8%) and then homicides (19, 13.3%). The number of work-related suicides in 2014 decreased dramatically compared to 2013 (9 suicides in 2014 compared to 22 suicides in 2013). Individuals who died were most likely to be men (87.4%) and Caucasian (83.9%). The average age was 47.9 years old and ranged from 20 to 84 years of age. Foreign-born workers accounted for nearly 10% of all work-related deaths in Michigan in 2014. The most common cause of death among foreign-born was homicide (4 deaths), followed by falls (3 deaths) and then machines (2 deaths). Illegal drugs, alcohol or side effects of prescribed medication was a potential factor in approximately 24% of the deaths. The Management occupation had the largest number of work-related deaths (32) followed by Transportation & Material Moving (30) and then Construction & Extraction (21). There was a work-related fatality in 44 of Michigan's 83 (53.0%) counties. Wayne County had the highest number of work-related fatalities (29, 20.3%), followed by Genesee and Macomb Counties (8 each, 5.6%). Of the 143 work-related fatalities, 37 (25.9%) were MIOSHA program-related and were investigated by a MIOSHA compliance officer. The employer had been involved in demolition work and scrapping for 40 years. The victim had not previously done demolition work. The employer had arranged with a food processing plant to demolish the steel structure that had supported a retired cyclone air separator and then sell the steel for scrap. The structure was supported by four steel column legs. It had been moved from its original location and placed on slightly sloping pavement in the plant's yard. The structure's legs were of different lengths, which, combined with the slope, caused it to lean about four feet in the direction of the slope. The employer and his crew decided to lower it in the direction of the lean using a rope attached to a forklift. Working from a platform elevated by the forklift, the victim used a cutting torch to make several cuts to the structure. The forklift was repositioned and a rope was attached to it and rigged to several parts of the metal structure. The victim then climbed up a couple of rungs on a stepladder and cut through a cross bracing. As he was doing this, the structure unexpectedly collapsed. It fell in the opposite direction than was planned. He was crushed under a steel beam and died at the scene. A trench box measuring 8 feet deep and covering an area of 2 feet 1 inch by 16 feet had been placed into the trench. The trench box walls were 4 feet 2 inches too short and left the construction laborers vulnerable to a large section of the spoil pile and concrete above. The two construction laborers were laying sewage pipe in the trench for approximately 6 hours when an above section of the spoil pile and concrete gave way, dumping the majority of the spoil pile into the trench, burying the victim and trapping the other construction laborer from the waist down. Three construction laborers outside the trench witnessed the incident and immediately contacted Emergency Medical Services at 2:22 pm. While waiting for the EMS to arrive, a worker climbed into the trench and began clearing the debris in an attempt to free the construction laborers. EMS and a special trench rescue team arrived at the scene within eight minutes, at 2:30 pm, to begin the rescue and recovery process. The trapped second construction laborer was alert, calm, and responsive throughout the extraction process, while the victim showed no signs of verbal responsiveness and was entirely covered by debris. The trapped second construction laborer was extracted after four and a half hours and transported to a local hospital for treatment, while the victim required two additional hours of extraction and was pronounced dead at the scene. Recommendations for prevention: 1. Trench boxes used in excavation projects should have a wall height greater than or equal to the wall height of the surrounding trench. 2. Pre-job site hazard analysis should be performed before work commences. 3. Employers should ensure a trained, competent person is on the jobsite prior to starting work and that they receive regular refresher training in order to maintain competent person status. 4. Employers should ensure all employees receive proper trench safety training. The SUV immediately stopped after striking the victim and stayed at the scene. A call was placed for emergency medical services (EMS). EMS and local police arrived within minutes and transported the victim to a local hospital where she died later that same day. Contributing factors identified in this investigation were outdated traffic control signage to warn motorists of crosswalk location, crosswalk markings were starting to fade, and parking was permitted within 20 feet of crosswalks. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, municipalities should: 1. Ensure that, at a minimum, crosswalk traffic control signage meets the standards set forth in the Manual on Uniform Traffic Control Devices (MUTCD) AB - 2. Provide crossing guards with American National Standards Institute compliant high-visibility safety apparel and MUTCD compliant STOP paddles to help ensure they're visible to motorists while working along roadways AB - 3. Consider additional measures to ensure crosswalks at unsignalized intersections and mid-block locations are clearly visible to motorists AB - 4. Ensure that crossing guards are provided with initial training and annual refresher training AB - and 5. Provide work environments for employees that, at a minimum, meet all relevant Occupational Safety and Health Administration (OSHA) regulations and industry accepted standards of practice per the Department of Labor Standards policy. The platform was secured to the forks of a rough terrain telescopic forklift truck. At approximately 11:30 am, they signaled for the foreman, who was operating the forklift, to lower the platform. The foreman tilted the frame of the platform in order to avoid scraping the wall of the building during the platform descent, and lost control of the lever handle. The tilt became more pronounced and the weight of the 40 foot extended platform caused the forklift to turn over onto its left side slamming the platform onto the concrete ground below, killing the 23-year-old siding installer victim, and injuring the second siding installer. A laborer witness also performing work at the site called Emergency Medical Services (EMS). EMS arrived within four minutes. The victim was pronounced dead at the scene while the second injured siding installer was airlifted to a major trauma center for treatment. Recommendations for prevention: 1. Employers should provide appropriate siding equipment on the job site and ensure all employees are trained to operate the siding equipment. 2. When lift equipment is required to elevate siding installers, choose only equipment specifically designated by the manufacturer for that purpose. 3. Avoid tilting a forklift truck when the platform is elevated. 4. When using a forklift approved by the manufacturer for personnel lifting, the width of a scaffolding platform should not exceed the width of the forklift wheel base. It was his first day on the job. A backhoe loader was being used to transport concrete blocks from a staging pile to the wall by connecting blocks to the bucket of the backhoe with a chain. The victim was standing near the backhoe loader preparing to connect a block to the bucket when the operator, not realizing the victim was there, rotated the boom. The bucket struck the victim's head and pinned him against the stack of blocks. The victim suffered blunt force head injuries and was pronounced dead at the scene about fifteen minutes later. Contributing Factors: 1. Worker standing within swing radius of backhoe loader boom. 2. No personal protective equipment. NJ FACE investigators recommend that these safety guidelines be followed to prevent similar incidents: 1. A safety and health plan based on a job hazard analysis should be developed by the employer and followed where workers are assigned tasks. 2. Training of all employees on job-specific tasks is extremely important, including day laborers. 3. Personal protective equipment such as hard hats and high-visibility vests should always be worn when working with backhoe loaders. TY - Guidance A2 - Oregon Fatality Assessment and Control Evaluation (Program) (OR-FACE) AB - Our safety talk today is about a 32-year-old carpenter foreman who was killed when the roof truss system he and his crew were installing collapsed. The truss manufacturer arranged for delivery of the trusses on a trailer pulled by a truck-mounted crane. The truck operator provided the foreman with the delivery packet containing the "Guide to Handling, Installing Restraining and Bracing Trusses." After each truss was lifted by the crane and set in place on the structure, the crew nailed temporary lateral restraints on the top chords but not diagonal bracing as instructed on the Guide. After the 13th truss was on the structure and disconnected from the rigging, the system collapsed. Two crew members fell off the top plates and the worker on the floor was struck by a truss and sustained a concussion. The supervisor fell to the floor and was fatally struck by a falling truss. Data from the 2014 Census of Fatal Occupational Injuries showed a downward trend in workplace fatalities for Latinos and all workers in California. The number of Hispanic or Latino workers who died on the job fell by 33%, from 194 to 130 deaths, from 2013 to 2014. Similarly, for all California workers, there was a 13% decrease from 396 deaths to 344 during the same period. While this decrease in fatalities is encouraging, there are many things employers and workers can do to continue keeping their workplaces safe. Employers must ensure that safety training provided for Latino workers is understandable and effective - this means considering employees' language and reading skills. In addition, all workers must be given the training, equipment, and safety measures needed so that they return to their families at the end of a work shift. OHB's <a href="https://www.cdph.ca.gov/Programs/CCDPHP/DEODC/OHB/FACE/Pages/FACE.aspx"target="_blank">Fatality Assessment and Control Evaluation Program (FACE)</a> program provides <a href="https://www.cdph.ca.gov/Programs/CCDPHP/DEODC/OHB/FACE/Pages/SpanishPubs.aspx"target="_blank">prevention materials in Spanish</a>. Falls continue to be a leading cause of workplace deaths in the construction industry for all workers, including Latinos. OHB encourages employers to join the <a href="https://www.osha.gov/StopFallsStandDown/"target="_blank">National Safety Stand-Down to Prevent Falls</a> in Construction on May 2-6. The victim was inside an excavated hole located in a roadway with three other co-workers. A backhoe loader was positioned near the trench with its outriggers extended and bucket attachment resting inside the dump section of a dump truck. The backhoe loader was unintentionally pulled forward when the dump truck was driven forward. As the backhoe loader was pulled forward, the outrigger on the right side struck the victim and dragged him out of the excavated hole. A police officer performing a traffic detail was on site and placed a call for emergency medical services (EMS). EMS, the fire department and the local and state police arrived at the incident location within minutes. The victim was pronounced dead at the scene. Contributing factors identified in this investigation included resting the backhoe bucket attachment in the dump truck when not being used, lack of communication among workers, not closing the roadway during the repair resulting in a limited work area, and absence of a safety and health program. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Ensure that when excavating equipment is not being used that the equipment is moved away from the excavation edge and all attachments are lowered to the ground before workers enter the excavation AB - 2. Ensure that before moving equipment located on work sites that the operator performs a walk around to warn workers on foot that the equipment will be moved and to identify potential obstacles AB - 3. Consider closing roadways when the work area is located in a roadway and there is limited space for the work area, and multiple large pieces of equipment are needed to complete tasks AB - 4. Develop, implement, and enforce a safety and health program that addresses hazard recognition and control and avoidance of unsafe conditions AB - and 5. Provide work environments for employees that, at a minimum, meet all relevant Occupational Safety and Health Administration (OSHA) regulations and industry accepted standards of practice per the Department of Labor Standards policy. He was discovered by fire fighters 170 feet below the bridge in a steep wooded area. The victim was pronounced dead at the scene. Recommendations for prevention: 1. Manufacturers should consider the redesign and relocation of fuel tanks on commercial vehicles to reduce risk of penetration and fire. 2. Drivers should always be focused on the road to avoid distracted driving. 3. Commercial motor vehicle drivers should be trained to know the signs of fatigue and steps to take to eliminate driving while fatigued. 4. Employers should ensure that their tractor-trailers are equipped with fire extinguishers. Kentucky Injury Prevention and Research Center AB - On Wednesday morning, March 11, 2015, a city police officer was driving southbound on a federal highway to a neighboring city for training. The driving conditions were not ideal, as visibility was low due to heavy fog in the area and the roadway was wet. As the officer approached an intersection, a motorist, who was at the intersection attempting a left turn, struck the officer's cruiser on the right side causing him to lose control and spin into the northbound lane, where another vehicle collided with the police cruiser's passenger side. Emergency Medical Services were notified of the incident at 7:18 am and arrived at 7:22 am. Three people were taken to a local hospital while the victim was transported to a major trauma center, where he succumbed to his injuries at 8:21 am. Recommendations for prevention: 1. Law enforcement offices should implement safe driving policies. 2. Law enforcement offices should implement an annual training for inclement driving. A co-worker immediately placed a call for emergency medical services (EMS). Then about 20 people, some who were co-workers, lifted the forklift off of the victim. The local police and EMS arrived at the incident location within minutes. The victim was transported to a local hospital where he was pronounced dead five days later. Contributing factors identified in this investigation included operating forklifts on loading docks with no protective edge barriers and the lack of a forklift operator restraint system. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Prohibit forklifts from loading dock locations that are not equipped with protective edge barriers AB - and 2. Consider installing protective barriers at the loading dock edge to minimize the possibility of employees and equipment falling off the loading dock. In addition, forklift manufacturers should adopt and implement the concept of Prevention through Design (PtD) and redesign the stand-up counterbalanced forklift operator area to include restraint systems. We think we know how to use ladders safely, but the frequency with which ladders are used and our casual attitude towards ladder use may actually lead to their incorrect use, resulting in injury and even death. In Michigan, falls involving ladders caused 41 deaths from 2001 to 2014 and 105 injuries in 2014 and 2015 that required hospitalization. The average age was 46, range 19 - 79, and 90% were men. Twenty-eight (27%) suffered a fracture of the lower limb, 22 (21%) had a fracture of the neck or trunk, and 15 (14%) had a fracture of the upper limb. The predominant industries where injuries involved ladders were construction (29), trade (14), and manufacturing (13).
