Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

One of the victims in the bucket made contact with the lines, causing a large arc and fire. The electric utility company was not notified of the work being performed in close proximity to its high voltage lines. There were no written safety procedures or an Injury and Illness Prevention Program (IIPP) in place at the time of the incident. The CA/FACE investigator determined that, in order to prevent future incidents, tree trimming companies and self-employed tree trimmers working in close proximity to high voltage power lines should: 1) Ensure that a daily job hazard analysis of the work area is conducted, including any electrical hazards from high voltage power lines. 2) Ensure there is a minimum of ten feet between the work area and high voltage power lines. 3) Ensure that workers are trained on safety policies and procedures on electrical hazards around high voltage power lines. 4) Ensure that the electric utility company is notified whenever tree trimmers will be working in close proximity to high voltage power lines. CONTRIBUTING FACTORS: Occupational injuries and fatalities are often the result of one or more contributing factors or key events in a larger sequence of events that ultimately result in an injury or fatality. The CA/FACE team identified the following items as contributing factors in this incident that ultimately led to the fatality: 1) A daily hazard analysis was not performed. 2) Working in close proximity to high voltage electrical power lines. 3) No safety policies or procedures. 4) Electric utility company was not contacted prior to beginning work. RECOMMENDATIONS: To prevent future incidents, tree trimming companies and self-employed tree trimmers working in close proximity to high voltage electric lines should: a) Recommendation #1: Ensure that a daily job hazard analysis of the work area is conducted, including any electrical hazards from high voltage power lines. b) Recommendation #2: Ensure there is a minimum of ten feet between the aerial lift bucket and the high voltage power lines. c) Recommendation #3: Ensure that workers are trained on safety policies and procedures on electrical hazards around high voltage power lines. d) Recommendation #4: Employers should always inform the electric company whenever working in close proximity to their high voltage power lines. The California Fatality Assessment Control Evaluation (CA/FACE) program concluded that geothermal facility operators and onsite contractors should take the following steps to prevent similar incidents: 1) Hazardous openings should be enclosed with guardrailing and affixed with a warning sign. Alternatively, the openings should be covered with a tool-secured cover strong enough to support foreseeable loads AB - the cover should be affixed with an appropriate warning placard. 2) Employers sharing a worksite should notify others of activities which may create new hazards. This is part of their Injury and Illness Prevention Program (IIPP) responsibility on a multi-employer worksite. 3) Supervisors and foremen, as part of their employer's IIPP responsibility, should assess worksites for newly-created hazards. This should be done at the beginning of the shift and periodically as needed. CONTRIBUTING FACTORS: Occupational injuries and fatalities are often the result of one or more contributing factors or key events in a larger sequence of events that ultimately result in an injury or fatality. The CA/FACE team identified the following contributing factors in this incident that ultimately led to the fatality: 1) Lack of established geothermal plant procedures for safely enclosing or covering 'can' openings created when pumps are removed. 2) The geothermal facility did not notify the employer that by removing the pump they had created a new hazard in the employer's jobsite. 3) The employer's foremen did not conduct inspections of their jobsite for newly-created hazards. The inmate work crew was employed by a state agency to conduct forest thinning on a plot of state owned land. The victim had been working with the crew for approximately 3 weeks before the incident. He had previously worked on a similar crew in another part of the state, and was generally considered the most experienced worker in the group. At around 10:00 a.m., the forest crew supervisor conducted the on-site morning meeting. He discussed details of the work area and assignments, and some-site specific safety information, but no mention was made of the power transmission lines running through the work unit, or the risks and hazards associated with falling trees around overhead power lines. Following the meeting, the crew moved out in a line along the boundary of the site with their chainsaws to begin work. Around 11:00 a.m., the supervisor made a count of the crew and observed the victim and his partner thinning appropriate trees. At approximately 11:30 a.m., the victim's work partner, who had been cutting up the trees that the victim fell, turned and saw that a large tree had fallen onto the nearest power line, and the victim was using his chainsaw to cut the tree while it was still leaning on the line. He saw that the victim was shaking and his feet appeared to be smoking, and ran to get help. Around the same time, the supervisor was informed by another crew member that a tree had been felled onto the power line. He looked and saw an electrical arc from the power line to the tree, and what appeared to be a worker shaking the tree. The supervisor shouted for the worker to get away from the tree. He reported hearing a loud noise, and seeing flames travel down the tree to the ground and the tree falling from the power line. As the supervisor went to his truck to notify dispatch of the incident, other workers hurried to the victim and found him burning, lying under the scorched tree, with severe burn injuries covering much of his body. They pulled the victim out from under the tree and put out the flames on his body with their drinking water. One worker attempted CPR, but it was apparent that the victim was deceased. When first responders arrived, they indicated that the victim had died from electrocution, and due to the extent of the injuries, no lifesaving measures would have been effective. Investigators later found that the tree the victim felled was larger than the maximum size that the crew was allowed to cut, and it was outside of the work boundary. The direction of the face cut the victim had placed in the trunk led the tree to fall in a straight line toward the power lines, and no hinge wood had been left during cutting to guide the tree's fall. The victim had attempted to cut the tree from the power line with his chainsaw. He made one undercut successfully near the base of the tree, but, when that did not release the tree from the line, he attempted another cut further up the trunk and was electrocuted. He worked the night shift from 9 p.m. to 6 a.m. Prior to the incident he was working alone operating the loader to push feed to the penned cows. When he finished his task, he drove the loader along a dirt road on the employer's property that would take him to the milking area, where he would spend the rest of his shift. On the way, he turned off the road away from his destination and travelled across a field back toward the cow pen. It is unknown why he went toward the pen. In the field between the road and the pen, there was an unfenced, unmarked ground level manure pit. This manure pit was 50 feet long by 15 feet wide and 4-to 6-feet deep. As he was driving in a 15-footwide area between the pit and fence, the loader tipped over into the pit. It landed on its side, submerging the operator's cab. The victim drowned. The coroner reported to the media that he had methamphetamine in his system. She received burns to 90% of her body and died four days later. RECOMMENDATIONS - To prevent similar occurrences in the future, Washington State Fatality Assessment and Control Evaluation (FACE) investigators concluded that employers and business owners should follow these guidelines: 1. Employers and business owners should recognize and train employees on the potential fire, explosion, and burn hazards associated with propane gas vapor in the vicinity of an open flame, such as the pilot light of a propane-powered space heater. 2. Employers and business owners should not use adapters to transfer propane from larger cylinders to refill one-time use only 1-pound propane cylinders, as this practice creates a risk of fire, explosion, serious injury, or death. 3. Employers and business owners should follow National Fire Protection Association (NFPA) building fire codes and local building codes to ensure safe building egress (exit). 4. Employers and business owners should provide adequate heat in a safe manner to the workplace. It was a low pitch roof with a 1 in 12 slope. The victim was working on the edge of the roof nailing down roofing materials. Neither of the roofers was using fall protection, nor was there any in place on the roof or on site. The father looked up from his work and did not see his son on the roof. He went to the side of the roof where his son had been and looked down. His son was lying on the sidewalk below, bleeding. He had fallen 25 feet from the roof edge. The roadway was unlit and curved slightly to the west. Paved shoulders were present on both sides of the expressway. The decedent had been on his cell phone with his spouse when he told her that another vehicle driver motioned to him that there was a problem with the trailer's tail lights. He told her that he was going to pull over to check his tail lights because he did not want to get a ticket. The decedent exited the freeway to the right shoulder, parking his semi and trailer fully on the shoulder. He was not wearing a high visibility vest. Leaving the tractor unit running and prior to exiting the tractor cab, he activated the hazard lights. The hazard lights were operational and the tractor's front headlights were lit. While either walking back to or returning from the back of the trailer near the fog line near the rear of the trailer on the roadway side, he was struck by the passenger side of an oncoming car. Passing motorists found him lying face down in the roadway between the two right lanes, just north of his semi-truck. Police found his left shoe wedged under the driver's side front rear trailer tire (the trailer had two rear axles). The tractor cab was not equipped with a forward-facing camera. MIFACE identified the following key and possible contributing factors in this incident: 1. Inattention of oncoming driver. 2.Walking on roadway side of tractor trailer. 3. Not wearing a high visibility vest. 4. Possible - Inadequate vehicle inspection prior to leaving lot. 5. Possible-Malfunction of trailer lights. RECOMMENDATIONS: 1. Always wear a high visibility vest when exiting the tractor on a roadway or while walking in a truck yard. 2. When possible, after exiting the tractor, minimize exposure to oncoming traffic by walking around the cab (face traffic) and then walking on the non-traffic side of the trailer, keeping the tractor/trailer between you and active traffic. If not possible to minimize exposure to traffic, park the truck/trailer as far to the right as possible to maximize space between truck/trailer and oncoming traffic. 3. Perform an adequate pre-trip vehicle inspection, and include both the tractor and the trailer. 4. Use a flashlight, moving the flashlight back and forth, to light way and alert drivers when walking/working on a dark roadway. 5. When possible, travel to nearest exit/area with appropriate pull-off parking facilities to check vehicle function rather than stop on an active roadway. 6. Trucking companies should develop and implement specific policies, such as quarterly safety meetings and safe work practices, such as wearing high visibility vests to reduce the potential for injury. The policies and safe work practices should apply to and be shared with both company and independent contracted drivers. 7. Michigan's Move-Over law should be expanded to semi-trucks parked on a roadway shoulder. 8. Michigan Department of Transportation should consider retrofitting existing roadways, especially those roadways in highly traveled areas with rumble strips along the roadway pavement edges to warn motorists when their vehicle is approaching the shoulder. Work-related transmission has been documented in a research laboratory setting via a needle stick. Healthcare and laboratory employers need to assess Zika risk and protect employees under their blood-borne pathogen exposure control plan. Outdoor workers are at risk in areas with Zika. So far, that does not include California. In our state, Zika virus infections acquired from mosquitos have been documented to date only among people infected while travelling outside the United States (and possibly to Florida). For outdoor workers and workers who travel to Zika areas, employers need to inform them of the risk and provide them with the means to prevent mosquito bites and possible infection. Many people infected with Zika do not know they are experiencing its mild symptoms. If a pregnant woman is infected, however, the virus can pass to her fetus and cause serious birth defects. CDC recommends that pregnant women not travel to areas where Zika is transmitted via mosquitos. CDPH's Zika website includes information for healthcare providers on what to do if they treat someone suspected to have Zika. Bureau of Occupational Health and Injury Prevention AB - On March 10, 2014, a 39 year-old male worker of an automobile body shop was fatally crushed by a 2002 Ford F-550 XLT Super Duty diesel flatbed truck in the shop. At the time of the incident, the victim was working alone cleaning the truck in a wash bay. The truck was parked in first gear with the key in the ignition. The parking brake was not set and the wheels were not chocked. At approximately 11:00 am, the victim was cleaning the passenger side of the truck with a pressure washer hose when the truck suddenly rolled forward. It rolled 16 feet and crashed into a paint booth before it stopped. The victim was crushed and pinned between the paint booth door and the passenger side of the truck. A shop worker called 911 immediately. The other workers pulled the door away from the victim and freed him. The emergency medical technicians arrived within minutes. The victim was transported to a hospital where he later died from the crush injuries. A Ford certified mechanic from a local Ford dealer examined the flatbed truck after the incident. The solenoid was replaced by one of the shop's mechanics 20 days prior to the incident. The Ford certified mechanic found that the battery cable on the solenoid was not installed correctly. He identified a short circuit between the battery cable terminal and the signal wire terminal on the solenoid. According to the Ford certified mechanic, the short circuit allowed battery power to activate the starter motor that moved the truck forward. He also found that the truck's neutral safety switch was broken. The broken neutral safety switch did not contribute to the spontaneous starting of the truck. Contributing Factors: 1. The solenoid was not installed correctly resulting in a short circuit between the signal wire terminal and the battery cable terminal on the solenoid. 2. The parking brake of the truck was not set and the wheels were not chocked. Key Recommendations: 1. Employers should ensure that workers always set parking brakes and chock the wheels when working on or around a vehicle. 2. Employers should ensure that a starter is installed properly according to the manufacturer's specifications. 3. Emergency response staff should set the parking brake, chock the wheels, and disconnect main battery cable to prevent movement of a vehicle when responding to a vehicle emergency. 4. Manufacturers should consider modifying the design of solenoid terminals to prevent short circuits. 5. Manufacturers should warn mechanics in the service manual to strictly follow the installation instructions to avoid short circuits when installing a solenoid. 6. The interior of the tank was partitioned into containment bays using stacked concrete ecology blocks to separate the different materials. Semi-truck trailers filled with bulk fertilizer were driven into the storage tank and backed into a containment bay, which was bordered on three sides by ecology block walls. Using a mobile conveyor belt system, or piler, connected to the rear of the trailer, fertilizer was unloaded into a pile on the floor. As the pile grew larger, an employee operating a front end loader would push the material to the back of the bay. In the early evening, the victim and one other employee were working to unload the last truck of the day. The victim was operating the front end loader, pushing fertilizer further into the bay after it dropped from the conveyor. At one point, the victim stopped the front end loader and went to investigate whether material was leaking through the rear wall of the bay. He climbed onto and walked along the ecology block wall to the back wall of the bay, which blocked a large opening in the tank. The second worker in the tank shut off the piler and heard several loud booms. He yelled for the victim, but there was no answer. He ran up the pile and saw that the blocks had collapsed, so he ran outside and around the storage tank and found the victim pinned by a fallen block against the wall of the tank. He yelled to another employee to call 911. Investigators suspect that the victim saw that material was leaking between the blocks of the wall and climbed down a ladder on the back of the tank to take a closer look. The victim was apparently trying to brace the unstable block wall with some long pieces of lumber when it collapsed. The top block of the wall fell directly onto the victim's head and neck, crushing him against the wall of the storage tank. First responders arrived within five minutes, but the victim was declared dead at the scene. RECOMMENDATIONS - To prevent similar incidents, Washington State Fatality and Control Evaluation (FACE) recommends that employers should: 1. Plan bulk material storage facilities for safety and stability by choosing structures designed specifically for bulk material containment, whether they are permanent or modular. 2. Consult a registered professional engineer prior to constructing bulk material containment walls or other structures using ecology blocks. 3. Provide employees with training on the hazards of working around ecology block walls, and safe practices, including to: a. Inspect ecology block wall storage areas before and after loading in material. b. Immediately stop work around ecology block walls showing signs of material leakage, leaning, moving, or other damage. c. Never attempt to brace or stabilize a leaning ecology block wall on your own. d. Avoid walking on top of ecology block walls. e. Avoid contacting ecology block walls with heavy or other equipment. f. Notify the proper person if structural integrity issues in an ecology block wall are found. 4. Regularly assess and audit the structural stability of ecology block walls. If integrity appears compromised, work around the wall must cease and the wall must be repaired or rebuilt. CONTRIBUTING FACTORS: 1. Lack of proper design, construction, and maintenance of material storage areas. 2. Lack of training regarding the dangers of working around bulk material and ecology blocks. 3. Possible destabilization of block wall due to granular material leaking between the blocks. 4. Possible destabilization of block wall through contact with the front end loader bucket. Once the hydraulic line was removed, the victim was crushed against the loader's frame when the lift arm and bucket came down. The victim's employer arrived on site and found the victim. A call was immediately placed for emergency medical services (EMS). EMS and local police arrived within minutes. The Jaws of Life were used to free the victim. The victim was pronounced dead at the incident location. Contributing factors identified in this investigation were working under an unsupported raised load, lift arm support device not engaged to prevent the lift arm from lowering, and lack of a safety and health program and lockout/tagout procedures. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Ensure that skid-steer loader lift arm locks are engaged prior to beginning maintenance tasks that require lift arms in the raised AB - 2. Develop, implement and enforce lockout/tagout procedures for skid-steer loaders that include the use of lift arm support devices during maintenance tasks AB - 3. Provide training on maintenance tasks employees will be performing and ensure that only trained employees perform maintenance tasks AB - and, 4. Develop, implement, and enforce a safety and health program that addresses hazard recognition and avoidance of unsafe conditions. In addition, forklift manufacturers should adopt and implement the concept of Prevention through Design (PtD) and design skid-steers to eliminate known hazards. Operating a riding mower on a slope that exceeds manufacturer's specifications. 2. The SCAG turf tiger riding mower was not equipped with a rollover protection structure. 3. The employee was working alone. Recommendations for Prevention: 1. Never operate a lawn mower on a slope that exceeds the angle limits specified by the lawn mower manufacturer AB - use lawn maintenance equipment designated for steep slopes. 2. To prevent rollover injuries, riding lawn mowers should be equipped with roll-over protective structures (ROPS) and seat belts. 3. Employees should receive safety training on slope mowing with riding lawn mowers that includes slope calculation, manufacturer specifications, and terrain assessment. 4. Employers should consider implementing a buddy system when working in hazardous areas such as steep slopes. The day prior to the incident, an employee of the site general contractor informed the victim that one of his crew was using a stepladder improperly by standing on its top step with the ladder propped against a building in the closed position. On the day of the incident, members of the general contractor's crew saw the victim propping a stepladder over and across a stairwell, with the foot of the ladder placed against the outside railing and the top of the ladder leaning on a wall above the stairwell. They told him not to use the ladder like that. The victim was trying to reach a portion of the window located over the stairwell in order to finish removing tape that had been used when caulking was applied. He moved the ladder to the other side of the half wall above the stairwell and then placed the 8-foot stepladder against the building in its closed position. No one observed what happened next, but ten minutes later the two employees who had warned him about the ladder found him seriously hurt at the bottom of the stairwell, where he had apparently fallen from the ladder. He had fallen 12 feet 8 inches. He was taken to a hospital with injuries to his spine and head. He died of his injuries six days later. The worker was repairing communication equipment on top of a light pole. The repair was complete and the lift was lowered to allow the coworker to exit the lift bucket and conduct tests on the ground. The worker re-entered the lift bucket to ascend and take photographs of the completed work. He raised the boom lift up to its maximum angle with the counterweight downhill on a slope. This caused the lift to tip over ejecting the worker from the bucket. He had a full body harness on but it was not connected to the anchorage point on the lift. During a remodel, the electrical subcontractor disconnected lighting circuits at ceiling junction boxes mounted from the hallway outside of the bathroom without tagging all points where circuits could be energized. Wiring was run to adjacent junction boxes: one for general lighting and the other for emergency lighting. The apprentice electrician located the circuit breaker for the emergency lighting on a different floor, shut it off and locked out the circuit breaker. He then reconnected wires to one of the existing junction boxes in the hallway. A witness observed him finishing up connections in the general lighting boxes rather than the emergency lighting junction box. The apprentice mentioned to the witness that he had just been shocked. The witness heard the apprentice electrician fall and then found him collapsed on the restroom floor. It appeared that the apprentice electrician reconnected the wrong wires in the hallway junction box and energized the general lighting circuit. The following notable trends occurred in 2014: The total number and rate of fatalities was higher in 2014 relative to the 4 previous years (see fatality rate chart, p. 10). Forestry/logging industry had the highest number of fatalities followed by transportation. It has been since 2011 that this industry had the most number of fatalities. Fatal cases from contact events exceeded all other events. Half of these cases occurred in forestry/logging industry. Second to contact were events in the violence category. Six of the 11 violence cases were suicides. At the time of the incident, the maintenance worker was working below the elevated forks, replacing a hydraulic seal on the forklift carriage lift mechanism while the carriage and forks were supported by resting the fork tips on the edge of a shipping container. The forklift operator was standing next to the forklift. As the maintenance worker tightened a hydraulic fitting, the fork tips slipped off the container edge. The carriage and forks fell, striking his head and pinning him underneath. The forklift operator radioed to call 911. Emergency medical services dispatched to the incident, and the maintenance worker was transferred to the hospital where he succumbed to his injuries.
