Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

He was an experienced journeyman carpenter and a crew foreman on this project. On the day of the incident, a supervisor held a morning job safety analysis meeting with crew members to discuss hazards associated with the day's task. This task involved dismantling and removing concrete forms suspended underneath the bridge. The victim and another employee assigned to this task had done this work many times during the project. That morning, the two employees working from the concrete forms suspended under the bridge removed the north wing wall portion without incident. After lunch, the two employees began preparing to remove the south wing wall. It was supported by rods extending down from the bridge that were attached to the form by bolts. The plan was to lower the wing wall by using a machine on top of the bridge to lower the rods. Then a crane located on top of the bridge would hoist the wing wall up onto the bridge deck. One of the rods was lowered three feet. An attempt to lower another rod was made, but it would not lower. He and his coworker then decided to try and lift the wing wall with rigging attached to a chain hoist in hopes of lifting some of the weight off the rod. When this did not relieve the pressure, they decided to cut the rod with a torch. Keeping the chain hoist in place, the victim wearing a full body harness with a retractable steel cable lanyard went to the top of the wing wall and cut the rod. Cutting the rod caused the wing wall to shift. The victim lost his balance and slid down the wing wall and fell into an opening between the wing wall and the main concrete form. His lanyard cable severed when it went over the steel edge of the main form and he fell 60 feet to a concrete pier below. The investigation identified that the victim's fatal fall was due to improper use of the retractable lanyard. The victim was working more than 6 feet above his anchor point and during the fall the cable went over a steel edge of the form, both of which the lanyard manufacturer warns against. Also, there were retractable lanyards located above the victim that could have been used as an anchor point. The CFD contains detailed data on 768 fatalities in the construction industry reported by NIOSH and individual states from 1982 through June 30, 2015. The results show that falls accounted for 42% (325) of the 768 fatalities included in the CFD. Personal fall arrest systems (PFAS) were not available to more than half of the fall decedents (54%); nearly one in four fall decedents (23%) had access to PFAS, but were not using it at the time of the fall. Lack of access to PFAS was particularly high among residential building contractors as well as roofing, siding, and sheet metal industry sectors (more or less 70%). Although the findings may not represent the entire construction industry today, they do provide strong evidence in favor of fall protection requirements by the Occupational Safety and Health Administration (OSHA). In addition to stronger enforcement, educating employers and workers about the importance and effectiveness of fall protection is crucial for compliance and fall prevention. The fall was not witnessed by any of the co-workers onsite, but the site foreman heard a noise and while trying to determine what the noise was he found the victim on the ground. The site foreman placed a call for emergency medical services (EMS). EMS and local police and state police arrived within minutes. The victim was med-flighted to a regional trauma center where he was in the intensive care unit and was pronounced dead the next day. Contributing factors identified in this investigation included: employees were not protected from fall hazards and employees were not provided training. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Provide fall protection for all employees exposed to fall hazards of six feet or more AB - 2. The victim stepped between the trash compactor and the back of the receiving container to check their alignment. At the same time, the truck driver released the container which slid rapidly down the inclined truck bed, crushing the victim against the trash compactor. The California Fatality Assessment Control Evaluation (CA/FACE) program concluded that employers who deliver or receive roll-off compactor receiving containers take the following steps to prevent similar incidents: 1) Establish formal written rules for the safe off-loading of roll-off receiving containers. These rules should minimize and, where possible, eliminate the involvement of ground personnel. Where ground personnel are involved they should be formally designated, have a clearly delineated role, and have adequate training in hazard recognition, experience, and authority to conduct the job safely. 2) There should be sufficient employee supervision and enforcement to ensure that new employees conduct assigned duties in a safe manner. 3) Put safeguards in place to reduce the risk of someone eentering the 'danger zone' between the trash compactor and the receiving container when it is being off-loaded. CONTRIBUTING FACTORS: Occupational injuries and fatalities are often the result of one or more contributing factors or key events in a larger sequence of events that ultimately result in an injury or fatality. The CA/FACE team identified the following contributing factors in this incident that ultimately led to the fatality: 1) There were no formal, written rules for the safe off-loading of receiving containers at the trash compactor. 2) There may not have been sufficient employee supervision and enforcement to ensure that the victim, a new employee, was conducting assigned duties in a safe manner. 3) There were no safeguards in place to reduce the risk of someone entering the 'danger zone' in front of the trash compactor during the off-loading of a receiving container. RECOMMENDATIONS: Employers who deliver or receive roll-off compactor receiving containers should take the following steps to prevent similar incidents: a) Recommendation #1: Establish formal written rules for the safe off-loading of roll-off receiving containers. These rules should minimize and, where possible, eliminate the involvement of ground personnel. The CA/FACE investigator determined that, in order to prevent future incidents, HVAC contractors working in close proximity to skylights should: 1) Perform a daily jobsite hazard analysis to be aware that skylights pose a risk even if adjacent work is of short duration. 2) Ensure that building owners install skylight safety nets, screens, covers, or guardrails around skylights within six feet of HVAC equipment. CONTRIBUTING FACTORS: Occupational injuries and fatalities are often the result of one or more contributing factors or key events in a larger sequence of events that ultimately result in an injury or fatality. The CA/FACE team identified the following items as contributing factors in this incident that ultimately led to the fatality: 1) A daily job hazard analysis of the work place was not performed. 2) Working in close proximity to an unprotected skylight. 3) The skylight was not designed to support the weight of a worker. The victim was employed to help during the harvest of wheat on a family member's farm. He was being paid for his work. He had over 25 years experience driving commercial vehicles. The truck, manufactured in 1988, was purchased by the employer the previous year. For the past three weeks, he had been working 10 to 11 hours per day, 6 days per week. The victim primarily made his living as a pastor in the local community. He also drove a school bus and worked as an on-call fireman. During previous harvests, he had worked for other farmers in the area and had driven many types of trucks. He had a valid CDL. On the day of the incident, his last before he went on vacation, he was driving a tandem-axle grain truck pulling a pup trailer, both fully loaded with wheat. This vehicle was used only during the harvest. According to the employer, before being put into service for the harvest, the truck had been checked and everything was adjusted properly. He was on the way to a nearby town to make a delivery to a grain elevator. He had made many trips with full loads to this elevator, including one earlier in the day. As he was driving along a state highway carrying approximately 60,000 lbs of wheat, he started down a long hill on the outskirts of town. The brakes were insufficient to slow the truck's descent. Attempting to slow down the truck, he used the Jake brake, downshifted, and applied what little brakes he had. As the truck entered the town a witness estimated that it was travelling at about 50 mph. The truck left the road, entered a dirt covered lot and then crashed into the grain elevator's scale house where he was to make his delivery. The victim died at the scene. Investigators determined that the brakes had been in disrepair for some time and that out of twelve brakes, only four were working properly. According to State Patrol, as the victim downshifted while descending the hill he likely missed a gear and was unable to get the transmission back into gear; this caused the truck to gather speed. The four remaining functional brakes were not sufficient to stop the truck. The student was operating a combination belt and disc sander in shop class when the students gloved right hand thumb came in contact with the rotating disc sander. The glove and the students thumb were partially pulled into the machine. Immediately after the incident occurred, a junior who was supervising the freshman student immediately turned off the machine and started to disassemble the machine to free the student. The student was brought to the school nurse who placed a call for emergency medical services (EMS). The local police and EMS arrived at the incident location within minutes. The injured student was transported to a hospital in another state where the partially amputated thumb tip was reattached with 12 stitches and the injured student was then released from the hospital. Contributing factors identified in this investigation included, greater than 1/16 of an inch space between the sanding table and disc, wearing gloves while operating the sander, not being fully trained on the sander prior to use and inexperience. The Massachusetts Department of Public Health concluded that to prevent similar occurrences in the future, schools with shop classes should: 1. Ensure that students are not wearing gloves when operating machinery with accessible rotating or moving parts AB - 2. Ensure that the space between a disc sanders sanding table and sanding disc is not greater than 1/16 of an inch when being used AB - 3. Ensure students have a clear understanding of the task they will be performing and prohibit any machine operation before being fully trained AB - and 4. Develop comprehensive safety and health plans, as required by Massachusetts regulations, to protect both students and school staff by ensuring that schools are providing conditions that, at a minimum, meet occupational safety and health standards set by the Occupational Safety and Health Administration (OSHA). There had been over three inches of rain during this time. Several trenches were dug in soil to allow workers to remove old pipes and install new pipes. The soil was unstable wet, loamy sand, classified as "Type C" soil. On the day of the incident, the crew was nearly finished with the project. The victim entered a trench to finish work on the sewer line connection to the house. The trench was 21 inches wide by 60 inches long and 7 feet deep. It was alongside a house foundation with a cement walkway on the other side. There was no protective system in the trench (though there was a hydraulic shoring cylinder near the bottom on one side). At 10:30 a.m., a wall of the trench collapsed, burying the victim. The coworker contacted emergencyservices and then attempted to dig out the collapsed trench. Fire department emergency response team personnel arrived on the scene within a few minutes. The rescue attempt soon changed to a recovery effort. The victim was declared dead at the scene. He died of compressional asphyxia. The driver was hauling cinder on a 2-lane road in a truck with two loaded dump trailers attached. The driver lost control while making a turn on a downhill slope. The trailers tipped over, pulling the truck down the embankment. This was only the driver's second day on the job, and he did not hold the proper CDL endorsement for this type of truck-trailer combination. It is not known whether he was wearing his seat belt. The equipment was found to be in sound operating condition. The driver died at the scene of multiple blunt force injuries. Information about the occupations, industries and circumstances in which these fatalities occur is essential to guide efforts to prevent future fatalities. Since 1991, the Massachusetts Department of Public Health has collected information on all fatal occupational injuries in the Commonwealth as part of the national Census of Fatal Occupational Injuries, conducted in cooperation with the Bureau of Labor Statistics in the U.S. Department of Labor. The Department of Public Health also conducts on-site investigations of fatalities with the aim of identifying workplace factors that increase the risk of fatal injury. This report provides a comprehensive summary of fatal occupational injuries in Massachusetts during 2008-2013. It includes a comparison of the occupational fatality experience in Massachusetts during this time with that of the nation as a whole as well as with the Massachusetts findings for the previous surveillance period (2000-2007). Findings are intended to guide the many stakeholders - government agencies, employers, unions, safety professionals, advocacy organizations, researchers, job trainers, and equipment design engineers - all of whom have important roles to play in preventing fatal injuries at work. The truck driver was standing on the bed of the trailer and both he and the bridge crane operator had their backs turned to the bridge crane, while the crane operator actuated the crane's controls. The crane's sling became entangled in the eye of a steel coil, causing it to become unstable and topple over onto the truck driver. The truck driver was pinned to the bed of the trailer beneath the steel coil and died at the scene. Recommendations for prevention: 1. Steel coils and other cargo that could tip over should be properly secured to the trailerbed prior to detachment from bridge cranes. 2. Workers should maintain visual contact with the cargo and crane at all times during crane operation.3. Written policies for loading and unloading should include that personnel be restricted from the loading zone during mechanized loading and unloading activities. 4. After placing and securing the steel coil in the intended location, authorized loading zone personnel should ensure that slings and other attachments are free of the coil's eye before actuating the crane away from the coil. While climbing the ladder, the victim fell to the ground below when the base of the ladder slipped out and then the ladder fell to the left. The co-worker went to assist the victim and the co-worker and the homeowner placed calls for emergency medical services (EMS). EMS and local police arrived within minutes. The victim was transported to a local hospital where he died 16 days later. Contributing factors identified in this investigation included: an incorrect ladder type/duty rating was being used AB - the ladder was overloaded AB - and the ladder was set up on a surface that was not flat. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Ensure that the correct equipment, both types and ratings, are provided and used for the tasks being performed AB - 2. Ensure ladders are set up properly before use and equipped with accessories that will help stabilize the ladder AB - 3. The victim wanted to inspect a sprinkler mounted on the building exterior. He placed a ladder against the side of the building. The ladder was a 24-foot, Type 2 medium duty/ commercial use fiberglass extension ladder with a load capacity of 225lbs. The other worker stood at the bottom of the ladder holding its rails while the victim climbed up the ladder. As he was climbing, the ladder broke at its upper section. It bent and broke at both rails. The victim fell 12 feet to the asphalt surface of the ground. He suffered multiple injuries and was in a coma for several weeks before he died. Investigators were unable to determine exactly why the ladder failed. A contributing factor was that the ladder had pre-existing fractures that were not the result of a manufacturing defect. The victim's weight was not determined and it is unknown whether it was a factor. A call was immediately place for emergency medical services (EMS). EMS and state police arrived within minutes. The victim was pronounced dead at the incident location. Contributing factors identified in this investigation were the narrow highway shoulder, the consequent need to work on the side of the tow truck closest to passing traffic, the motorist stopping on the narrow shoulder and not continuing to drive to a safer location to stop, and the box truck driver not moving over one lane or slowing down while passing a stopped recovery vehicle with lights flashing as required by Massachusetts state law. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, employers should: 1. Consider the option of wireless controls for tow truck beds and winches AB - 2. Ensure that employees use portable emergency warning devices to help alert approaching motorists of the stopped emergency vehicles ahead AB - and 3. Provide Traffic Incident Management (TIM) training to all tow operators. Motorist experiencing vehicle problems while driving should: 1. Continue driving, if it is safe to do so, until a location is reached where the vehicle can be moved completely off of the roadway and away from traffic. Drivers of larger vehicles should: 2. Oregon Institute of Occupational Health Sciences AB - On November 4, 2014 a 55-yearold log quality specialist employed by a timber leasing organization was killed when her vehicle was struck by a tree that was cut by a faller. This occurred within an active logging area that included cable yarding at the southeast end of the unit, and active timber falling at the northwest end of the unit. The faller was working at the northwest end and uphill from the road (see illustration at right). A single cable flagger associated with the cable yarding at the southeast end allowed the log quality specialist to drive under the cable and proceed northwest. Shortly after she passed under the cable she was met by the owner of the logging company who was driving from the northwest end toward the southeast end of the unit. During this stop, the log quality specialist and logging company owner had a brief conversation. The owner was the only witness to the conversation, and his report of what he said was limited and ambiguous. The logging company owner then left to perform some work at the east end of the site. A witness indicated the log quality specialist waited at the location of this encounter for at least 20 minutes before proceeding to the northwest, driving toward the timber falling activity. There were no warning signs or flaggers present in advance of the active falling area. The faller working at the northwest end cut a tree that fell downslope and into the road, 135 degrees from its intended lay. He went down to clear the tree from the road and discovered a vehicle had been struck by the tree and come to rest further down the road. He discovered the log quality specialist severely injured in her vehicle and called 911. First responders arrived within about 30 minutes but pronounced the log quality specialist dead at the scene. RECOMMENDATIONS: 1. Employers responsible for active logging operations should assure that entry intohazardous logging areas is controlled, including correct placement of flagging, roadclosures, and adequate and proper signage and warnings. 2. Employers should assess tree fallers' skills for felling and bucking logs, and requirethat novice or inadequately performing workers are directly supervised by aqualified person until the faller demonstrates the ability to safely perform thesetasks independently. 3. Employers with employees who work in and around forests who may be exposed toproduction logging operations should train employees in hazard recognition andreporting, and assure reported hazards are tracked, documented, and resolved, andtheir resolution communicated. 4. Incident investigations should be utilized to identify action items to be addressed,and responsibilities should be assigned to assure their completion. 5. On multi-employer worksites, all employers with employees on site share theresponsibility for protecting workers from known hazards, and thus should establishinter-employer safety communication practices involving all employers at a givensite. On the day of the incident, the victim and three other employees planned to complete construction of an "L"-shaped material containment bunker on a base of dry, loose sand. They were assembling the bunker using dry-stacked concrete ecology blocks that interlocked on the top and bottom with a tongue and groove system. The blocks were lifted and moved into place using a chain attached to the bucket of an excavator that was looped through a rebar picking-eye on the top of the block. While the employer frequently constructed and used ecology block retaining walls for various purposes, this was to be the first of this height built on sand. The first section of the bunker had been completed the previous day. It consisted of two adjacent walls, each approximately 21 feet long and 10 feet (5 blocks) high. When completed, employees reported that the walls appeared to be sitting level on the sand. The second section of the bunker was to consist of an 18-foot wall, perpendicular to the first wall, with rows of 6-foot-long ecology blocks horizontally stacked 5 blocks high. On the morning of the incident, as the excavator operator was lowering the first block of the second bunker section into place, the victim stood near and used a shovel to guide the block's alignment. The two other employees were working near the victim. As the block was set onto the sand against the base of the inside wall of the first bunker section, both original walls began to destabilize and collapse. The victim and the two employees ran to avoid the falling blocks. At some point, the victim hesitated and turned back to look at the wall and was struck by a falling block, crushing his right leg and pinning him to the ground. First responders arrived within minutes and transported the victim by helicopter to a local hospital where he remained in critical condition for nearly a month, after which he died due to complications following multiple surgeries. RECOMMENDATIONS - To prevent similar incidents, Washington State Fatality Assessment and Control Evaluation (WA FACE) recommends that employers engaged in similar work: 1. Plan and design bulk material storage areas for safety and stability. Make sure all walls, whether temporary or permanent, are built on a stable foundation able to support the weight of the structure. 2. Develop, implement, and enforce a comprehensive written safety program that is tailored to ecology block wall construction. Create Standard Operating Procedures that address hazards in both construction of and working near ecology block walls. Ensure that all employees are trained in and follow the procedures. 3. Conduct a thorough job hazard analysis (JHA) before construction begins on any project involving stacked ecology blocks. 4. Encourage employees to recognize and report safety concerns, and train workers on the protocol for reporting hazards. Ensure that all safety concerns are effectively addressed before employees are further exposed to the hazard. CONTRIBUTING FACTORS: 1. Attempting to build a ten-foot-high ecology block wall on an unstable sand surface. 2. Lack of discussion of possible safety risks or hazards before beginning work. 3. Unwillingness to challenge the company owner about safety concerns. At 8 a.m. on the day of the incident, the four-member crew met for their daily safety meeting after which they began the roof tear-off. They were each using a personal fall arrest system consisting of a full-body harness, a rope, a rope grab, and roof anchors. The roof pitch was 8:12. The weather was cloudy with intermittent light rain and mist. They worked quickly and by afternoon they had removed the existing roof and were laying down plywood sheathing and felt paper. While placing sheathing next to the chimney, the victim decided that he needed another small piece of sheathing. The sheathing was located on the other side of the roof. He unhooked from his fall protection safety line then slipped and fell from the roof, landing 19 feet below on concrete. Coworkers saw him just before he fell, but did not see him fall. They found him unconscious on the ground and called emergency services. He was transported to a hospital. He died 12 days later from multiple injuries to his head and body. His coworkers did not understand why he unclipped from his safety line. They were all wearing fall protection.
