Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The paving company was contracted by the New York State Department of Transportation (NYSDOT) to build a roundabout intersection and reconstruct the roads leading to the roundabout. On the day of the incident, the victim was assigned to take down ten mobile light towers at the site and return them to a local rental company one at a time in a company pickup. By 9:20 AM, he had returned three light towers and started taking down the fourth. The victim was working alone on the roadside while other employees were working at a staging area. The incident was not witnessed. The light tower was positioned off the pavement on grass under a 7,620-volt powerline that was approximately 24.6 feet above the ground. For an unknown reason, the victim apparently extended the telescopic mast and raised the tower instead of retracting and lowering it. The tower came into contact with the powerline and the victim was electrocuted. The electric current ignited the diesel in the tower's fuel tank causing an explosion and setting the light tower on fire. The other workers heard the explosion and rushed to the site. They called 911 to summon emergency services. The police and fire department responded within minutes. The fire department had to wait for the power company to turn off electricity before fighting the fire. It took approximately an hour to extinguish the fire. The victim was pronounced dead at the scene. CONTRIBUTING FACTORS: 1. Jobsite survey failed to identify the electrocution hazard associated with positioning a mobile light tower and or around powerlines. 2. Victim was not trained on jobsite hazards and control measures. 3. Victim may not have been familiar with the controls of the light tower. 4. Victim worked alone while handling mobile light towers positioned under a powerline. 5. The potential electrocution hazard of placing a mobile light tower under a powerline was not addressed in the Lighting Plan. KEY RECOMMENDATIONS: 1. Employers should conduct a thorough jobsite survey to identify hazards including overhead powerlines and implement appropriate control measures prior to placing mobile light towers. 2. Employers should ensure that all workers know the safety and health hazards and control measures specific to their job assignments. 3. Employers should assign additional personnel to observe and maintain clearances when equipment is being operated or handled in the vicinity of powerlines. 4. Agencies that manage road construction projects should require contractors to address overhead powerline hazards in the nighttime lighting plan. 5. Agencies that manage road construction projects should train their field representatives on basic knowledge and skills to identify high hazard working conditions at road construction sites. 6. On the day of the incident, the victim and another worker began work at 7 a.m. They were working on a new two-story single-family residence project. The victim's job was to finish sheathing the roof. Before they started work, the employer told the victim to put on a safety harness and use fall protection while working on the roof. The employer told investigators the victim refused to wear fall protection, because he said the rope (lifeline) got in his way and that he would trip over it while moving about the roof. He also considered the victim to be a subcontractor, so he did not believe that he had the authority to compel him to use fall protection. The victim used a ladder to access the 12/12 pitch roof and began installing sheathing. The other worker went to work in the building's basement and the employer left the site to get building materials. A light rain began to fall. A few minutes later, the worker in the basement heard the victim sliding down the roof, then saw him fall head first onto the concrete walkway 18 feet below. He called 911 and then began CPR on the unconscious victim. Emergency responders arrived on the scene and attempted to revive the victim, but were unsuccessful. Washington State Department of Labor and Industries' Division of Occupational Safety and Health (DOSH) investigators determined that in this instance, the victim was an employee, not a subcontractor. The victim was working with an unlicensed plumbing contractor, who was working for a licensed general contractor. The California Fatality Assessment Control Evaluation (CA/FACE) program concluded that the following steps should be taken to prevent similar incidents and recommends: 1. Only electric-powered concrete saws should be used in enclosed spaces. If a generator powers the electric saw, the generator must be placed outdoors, well away from the enclosed space. 2. Employers should train employees to understand that all fuel-powered tools emit carbon monoxide and represent a potentially fatal hazard when used in enclosed spaces. Employees should also be trained to recognize the signs and symptoms of carbon monoxide poisoning, and the limitations of respiratory protection. 3. Concrete saw manufacturers should label fuel-powered concrete saws with a clear warning. For example: WARNING -- DO NOT USE INDOORS OR IN ENCLOSED SPACES -- CARBON MONOXIDE PRODUCED DURING USE CAN KILL. Equipment rental companies should also attach such warning labels to all fuel-powered concrete saws. CONTRIBUTING FACTORS: Occupational injuries and fatalities are often the result of one or more contributing factors or key events in a larger sequence of events that ultimately result in an injury or fatality. The CA/FACE team identified the following contributing factors in this incident that ultimately led to the fatality: 1. Using fuel-powered tools in an unventilated indoor space causes carbon monoxide to accumulate over time to lethal levels. 2. Using air-purifying respirators which do not protect against carbon monoxide exposure. 3. Working alone, the victim could not be immediately removed from the dangerous atmosphere when he lost consciousness. On the day of the incident, he was working to grease the bearings on a hydraulic straw spreader attached to a tractor. This task was performed by farm employees as needed, up to twice a week. The worker told investigators that the farm mechanic had trained him to grease the straw spreader while it was running so that grease would go into the bearings. The employer's accident prevention program included information about lockout/tagout procedures, but not specifically for the straw spreader. The worker said that he had never received training in lockout/ tagout procedures. When the worker had finished greasing the bearings on one part of the spreader, he started to duck under the rotating beater bars to get to the next set of bearings underneath the machine. As he bent down, the metal teeth caught hold of his loose hooded sweatshirt and pulled his head and neck into the beaters. The straw spreader had a mechanism to shut off automatically when jammed, but when the beater bars stopped rotating, he remained entangled in the machine and lost consciousness. Another employee found the injured worker caught in the teeth and shut off the power to the straw spreader. The co-worker and a supervisor were able to cut him loose and call emergency services. He underwent surgery for fractures to his neck and skull. After more than six months, he had still not been able to return to work due to the extent of his injuries. Department of Labor and Industries AB - In July 2016, a 40-year-old construction foreman died and a 57-year-old laborer was injured when a truck struck the elevated aerial lift platform they were working in, ejecting them from the platform. The two men worked for a civil construction contractor that does, among other projects, bridge construction. The Seattle Department of Transportation (SDOT) hired the contractor to replace rubber bearing pads on an expressway bridge. Their task that day was to install replacement retaining rods underneath the bridge. Also known as seismic restrainers, these rods prevent excess movement of bridge elements. They were using a self-propelled telescopic boom-supported elevating work platform or aerial lift. They positioned the lift underneath the bridge in a dirt median between two roadway ramps. After loading equipment, they both stood on the platform while the foreman raised it and boomed out so that they could access the work area between girders on the bridge above. The work area was about 30 feet above a single lane of an elevated exit ramp. As the foreman was positioning the platform, it was struck by a box truck traveling in the lane below. The impact spun the platform 180 degrees and ejected the workers from the platform. The foreman was thrown 50 feet horizontally, landing on a railroad track 48 feet below. He died of his injuries. The laborer fell about 10 feet and dangled from his lanyard above the road until the truck driver and a passerby helped him down. Emergency responders took him to a hospital where he received treatment for abdominal injuries. The investigation determined (1) temporary traffic controls were not used, exposing the two workers to an open traffic lane, (2) traffic controls had been used on this project by the employer previously, though ramp closure was not permitted on business days, per the contract with SDOT, (3) though the two workers were wearing full body harnesses, neither of them fixed their lanyards to the manufacturer's provided and approved attachment points. The foreman's lanyard was not attached to anything, the laborer tied off to the platform's handrail. Lots of people drive as a part of their job - some more than others. Motor vehicle crashes continue to be a leading cause of work-related death in Massachusetts and across the country. In the past four years (2013-2016), 36 workers have died in motor vehicle crashes while driving for work in Massachusetts. It's not just truck drivers who are dying in these crashes. Only 11 of these 36 victims were truck drivers. Many different jobs require employees to drive or be a passenger in a vehicle while at work. Some examples are: home health aides, landscapers, sales representatives, and police officers. How can employers keep workers safe while driving or riding in motor vehicles? Prevent distracted driving by: 1. Banning texting and hand-held phone use while driving for work (both work and personal phones). 2. Requiring employees to pull over in a safe location if they must text, look up directions, or make/answer a call. This includes texts or calls from management. 3. Preparing employees before implementing these policies by communicating: a. How distracted driving puts them at risk of a crash; b.That driving requires their full attention while they are on the road; and c.What action the company will take if they do not follow the policies. 4. Ensuring that employees program navigation devices (e.g., GPS, phones) before they start driving, and that these cannot be operated manually when the vehicle is in motion. Also, make sure a vehicle mount is used to secure the device and eliminate the need to hold it while driving. Require the use of seat belts at all times by all vehicle occupants. 1. Ensure that there are enough seats for each passenger and that each seat has a functioning seat belt. 2. Require more than one trip or an additional vehicle if there are more passengers than seats. Develop a Motor Vehicle Safety Program that includes policies on: 1. Training employees on the importance of being attentive while driving. 2. Routinely reminding employees that while behind the wheel, driving is their primary job. 3. Schedules that allow employees to obey speed limits, follow applicable hour-of-service regulations, and prevent drowsy driving. 4. Zero tolerance for speeding and aggressive driving practices. 5. Procedures for reporting and investigating crashes and vehicle breakdowns. 6. Routine maintenance procedures for employer provided vehicles. The following notable trends occurred in 2015: The total number of fatalities was lower, whereas the rate of employment was higher, relative to the previous 12 years (see fatality rate chart, p. 11). The transportation industry had the highest number of fatalities, followed by forestry and logging. These two industries have been among the top 4 industries for fatalities in 11 of the previous 12 years. Fatal cases from Contact events exceeded all other events, followed by Motor Vehicle Accidents. Nearly one-third of these occurred in the transportation industry. Fatality Assessment and Control Evaluation (FACE) Program AB - On Sunday, December 16, 2012, at approximately 6:37 p.m., a 44-year-old male excavator operator died after being struck in the chest by a section of 16-inch gas pipeline. Two excavator operators were using a tandem lift procedure to transport a 128-foot section of the pipe from a construction area (top of hill) to an installation area (bottom of hill). The two excavators were on the right side of the pipe, and the operators needed to maneuver around the pipe prior to moving it down to the installation area. Both excavators were attached to the pipe with slings, approximately 20 feet from each end of the pipe. The lead operator lowered his end, detached from the pipe, and positioned his excavator on the left side of the pipe AB - once on the left side, he reattached to the pipe and raised it off the ground. The operator of the rear excavator attached at the opposite end (rear) of the pipe, then raised the pipe off the ground to cab height and maneuvered the excavator around the end of the pipe while swinging the boom of the excavator. When the pipe was in line with the excavator cab, the pipe plunged through the front windshield, entering the cab and striking the operator in the chest. Police, emergency medical services, and the medical examiner arrived, and the excavator operator was pronounced dead at the scene. He had worked for the multi-state potato harvesting and transport company for about four months. On the morning of the incident, the victim arrived at around 7 a.m. and began loading four tables onto a lowboy flatbed trailer so they could be transported to another site. Each table was 41 feet long and weighed approximately 3000 lbs. The surface of the lowboy trailer was 38 in. from the ground and was arched slightly at the center. The semi-truck was parked in a location with a slight grade, and there was a 3-degree slope from the front of the trailer to the rear. It is not clear why the victim attempted to load the tables onto the trailer by himself. Company procedure was to call at least one other worker for help when loading this kind of equipment. He had done the same task the day before with the help of other workers. This morning, however, the victim worked alone with a forklift to place a stack of two tables on the driver's side of the trailer. Next, he loaded the second stacked pair on the other side, offset toward the rear of the trailer. The position of the table wheels on the flatbed were straight in-line with the trailer surface. He did not chain or chock the wheels of the tables to keep them from moving. After loading the tables, the victim parked the forklift and went back to the trailer and began laying out equipment to tie down the load. Company procedure would have been to leave the forklift in place to support the stacked tables while the wheels were secured with chains. Around this time, a truck driver from another company drove onto the site. He saw the victim get out of his cab and walk around behind the loaded trailer. As the truck driver went past the victim's trailer, he noticed that a stack of two tables had fallen onto the ground. When he walked over to see what had happened, he saw that the tables had fallen onto the victim's back. He and another worker used a forklift and a front-end loader to lift the tables off the victim, and then called 911. The victim was pronounced dead at the scene. He was pronounced dead at the scene by the coroner. Recommendations for prevention: 1. A job hazard analysis should be performed by a trained, competent person, before beginning a job. 2. Workers should always use personal fall protection when exposed to a vertical drop of 6 feet or more. 3. All employees expected to work from heights of 6 feet or more should receive training on fall protection use prior to beginning such work. 4. The victim was found dangling from the tree by his fall arrest system, and was pronounced dead at the scene at 11:00 am. Recommendations for prevention: 1. Insulating equipment, such as insulating rubber gloves and sleeve combinations, or insulated power line sleeves, should be used when tree trimming work is to be performed within the minimum approach distance (MAD). 2. Whenever tree limbs within the MAD are to be removed, a second line-clearance tree trimmer should be at ground level to provide verbal assistance from a second vantage point. 3. The victim was using the tractor and mower attachment to cut the grass between rows of pear trees. The incident was not witnessed, but investigators believe that it is most probable that he fell when he attempted to dismount the tractor to retrieve his hat that may have been knocked off by a low hanging branch. He likely either failed to put the tractor in neutral when he stopped, causing the tractor to lurch forward when he removed his foot from the clutch pedal, or inadvertently moved the gear selector lever out of neutral as he stood up to dismount the tractor. In either scenario, it is assumed that the sudden or unexpected movement of the tractor caused him to fall to the ground between the tractor and the mower. As the tractor continued to move forward, the victim's upper body was run over by the 1740 lb. mower. He was pronounced dead at the scene. Kentucky Injury Prevention and Research Center AB - On Monday, September 19, 2016 a 48-year-old male tow truck owner-operator (the victim) was loading a disabled vehicle onto his flatbed tow truck on the west bound shoulder of a four lane controlled access highway. At approximately 10:15 pm, the victim was on the traffic-facing lane side securing the vehicle to his tow truck when the operator of an oncoming Dodge Durango traveling in the same direction failed to move over, veered over the edge line, and struck the tow truck owner and the side of the tow truck. The victim later died at the hospital. Recommendations for prevention: 1.Tow truck operators should limit the amount of time spent on the traffic lane facing side of the tow truck. 2. High-visibility safety apparel, such as safety vests, should be worn at all times while working at roadside. 3. Tow truck operators should work in conjunction with law enforcement to secure the work area prior to loading and securing a vehicle. 4. Tow truck operators should utilize portable emergency warning devices such as bidirectional reflective triangles. 5. There should be increased public awareness of the "Move Over Law" in Kentucky. 6. Tow truck operators should consider National Traffic Incident Management Responder Training, regardless of company size. This is the first report on occupational crushing injuries in Michigan; it covers three years, 2013, 2014 and 2015. These are the key findings: Work-related crushing injuries were identified through multiple reporting sources - 1) There were 947 work-related crushing injury incidents that represent 946 individuals in 2013. 2) There were 1,080 work-related crushing injury incidents, including two deaths, which represent 1,078 individuals in 2014. 3) There were 1,110 work-related crushing injury incidents that represent 1,109 individuals in 2015. 4) Over the three years combined, there were 3,137 work-related crushing injury incidents that represented 3,131 individuals; 4 individuals each sustained 2 unique crushing injuries in the same calendar year and 2 individuals had 2 unique crushing injuries in two different calendar years. For 2013 through 2015, the Federal tracking system that relies on employer reporting, estimated only 1,260 work-related crushing injuries in Michigan or 40% of the total of 3,137 crushing injuries we identified in the three years (43.3% of our Michigan multi-source total in 2013, 55.6% of our total for 2014 and 22.5% of our total for 2015). The U.S. Bureau of Labor Statistics' estimated rate was 13 per 100,000 full-time equivalent (FTE) workers in 2013, 19 per 100,000 FTEs in 2014 and 7 per 100,000 FTEs in 2015, which was only 59.1%, 76.0% and 28.0% of the rate of 22, 25 and 25 per 100,000 workers of work-related crushing injuries identified in Michigan's multi-source reporting system. The most common type of medical encounter was an emergency room visit (2,411; 77.9%). Eighty percent of all work-related crushing injuries were among men and 85.2% were among Caucasians. The most common part of the body injured was an upper limb (2,287; 72.9%), followed by a lower limb (663; 21.1%). Primary Metal Manufacturing Industry had the highest number of work-related crushing injuries with 694 (26.7%) cases, followed by Construction with 247 (9.5%) cases, and Wood Product Manufacturing with 206 (7.9%) cases. These three industries combined accounted for almost half (44.1%) of all work-related crushing injuries. Mining, Quarrying, and Oil and Gas Extraction had the highest rate of crushing injuries with 154.9 per 100,000 workers, followed by the Agriculture, Forestry, Fishing and Hunting Industry with a rate of 59.7 per 100,000 workers, and then the Wholesale Trade Industry with a rate of 57.1 per 100,000 workers. "Pinched between objects other than door" and "Struck by falling object" were the two main causes of work-related crushing injuries in the Primary Metal Manufacturing (the industry with the most crushing injuries, 694; 26.7%), with 16.9% and 19.0%, respectively. Workers' Compensation was the expected payer for only 76.0% of the 3,135 crushing injuries that were identified in the hospital/ED records. The MIOSHA program completed inspections at 77 worksites identified by the surveillance system where individuals were injured in 2013 through 2015. MIOSHA issued 212 violations and assessed $276,425 in fines. In 45 of these 77 inspections the employer had not addressed the circumstances causing the crushing injury (e.g., no guard on the machine where the crushing injury occurred) even though the MIOSHA inspection was performed months after the occurrence of the injury.
