Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

He then used a ladder to climb to the roof of his house, and began to shovel deep rooftop snow onto the fire in an attempt to extinguish the fire near the chimney. The roofing next to the chimney collapsed, and the victim fell into the attic space below. The victim apparently tried to escape from the attic fire by crawling toward the furthermost point from the chimney, where a cubbyhole might have provided an exit. Within about ten minutes of the victim's call for help, the fire department volunteers and vehicles arrived at the scene. The captain saw the hole in the roof, but not the victim so the captain turned fire fighting duties over to one team, and led a second team of five fire fighters to search for the victim. The search team, equipped with turnout gear and self contained breathing apparatus (SCBA), searched the first floor of the house without finding the victim. They then used chain saws to cut through the first-floor ceiling into the attic area for about 20 minutes before locating the victim. He was not breathing, and was pronounced dead at the scene. The FACE investigator concluded that, to prevent similar occurrences, fire fighters should: 1. Ensure that at least four fire fighters are on the scene before initiating fire fighting operations on the roof of a structural fire. 2. Ensure that all fire fighters wear and use personal alert safety system (PASS) devices when involved in fire fighting, rescue, or other hazardous duties. The victim had inspected and approved the work completed the day before, and was going to retrieve two rope grabs, which had been left in a pile. There were a total of five retractable lines with which workers were required to tie off whenever they went outside the caution lines along the roof peak. The victim was involved in a conversation with the witness when he stepped under a caution line to pick up two of the rope grabs, onto a weakened section of roof that collapsed under his weight. The witness later realized that he also had stepped beyond the caution line without noticing, but had apparently stepped onto a stronger section of the original roof. The other crew members were just arriving, and they ran to the guard booth to call 911. Emergency Medical Service (EMS) personnel arrived and transported the victim to a hospital, where he was pronounced dead at 6:50 am. In order to prevent similar occurrences, the KY FACE investigator recommends that employers should: 1. ensure that appropriate fall protection equipment is available and correctly used when working from elevations where there is a danger of falling. 2. ensure that, when equipment must be left on the job site, it is stored in an area where it can be retrieved safely. A resident who was in the area noticed that hydraulic fluid was spraying from the left side of the loader and notified one of the workers. When the worker went to the machine to check, he found the victim seated in the operator's seat with his head caught in the boom linkage and hydraulic fluid leaking from the bucket cylinder hydraulic line. It appears he slipped or inadvertently stepped on the right foot pedal, which immediately lowered the lift arms and the bucket, pinning him to the frame of the machine. Another possibility is that while pulling out the shrub, the chain slipped or the shrub gave way, causing the loader to buck up and down, throwing the man forward out of his seat. The machine had a seatbelt, but the man was not using it. He was working alone, out of sight from the farmstead AB - it was two hours before he was found dead at the scene by a family member. The skid-steer loader was still running when the man was found. The farmer's son, who had occasionally operated the machine, observed that it was having jerky movements during the few weeks prior to the injury, movements that could have been a factor in this fatality. A seat-actuated electro-hydraulic interlock had been by-passed long ago, therefore there was nothing to prevent the bucket from falling if the pedals were moved, even if the operator was not in the seat. The machine was manufactured in 1972. Recommendations based on our investigation are as follows: 1. Owners/operators of skid-steer loaders should not disable or alter factory-installed or retrofitted safety features. 2. Operators of skid-steer loaders should be educated regarding safe use of their machines. 3. Manufacturers should provide reliable mechanisms to prevent the loader bucket from falling unintentionally. 4. His wife went to search for him when she realized he had been gone from the house for quite some time. She found him in the field pinned under the disc that was attached to the tractor. She returned to the house to call 911. Rescue personnel were dispatched at 4:16 pm and arrived on the scene within minutes. The county coroner was summoned to the scene and pronounced the victim dead. In order to prevent similar incidents from occurring, FACE investigators recommend: 1. Never attempt to start a tractor unless seated in the driver's seat. On August 04, 1997, Alaska Department of Labor (AK-DOL) notified the Alaska Division of Public Health, Section of Epidemiology. An investigation involving an Injury Prevention Specialist for the Alaska Department of Health and Social Services, Section of Epidemiology ensued on August 04, 1997. The incident was reviewed with AK-DOL officials. Alaska State Troopers and Medical Examiner reports, as well as AK-DOL reports, were requested. The company in this incident was a privately owned logging operation that had been in business for 24 years. The company currently had 396 employees. The victim was an experienced cutter and had been employed by the company for five weeks. The company had a written employee's manual that included safety rules and practices for camp and work. In addition, employees received a copy of the Alaska Timber Insurance Exchange logging safety handbook. Safety meetings were held at least monthly. All employees were required attend an initial hire orientation and CPR and first aid training, if necessary. In addition, employees were assessed for individual skills and knowledge by the cutting supervisor or "bullbuck." Intermittent observations were made while working in their assigned area to provide recommendations and corrective actions including re-assignment, if necessary. This was the second fatality for the company. The first fatality was a heli-logging incident in 1996. 1. Employers should ensure that all cutters working on steep terrain fell trees according to safe methods in accordance with 29 CFR 1919.266. 2. Employers should consider excluding trees that should not be felled for harvesting due to extreme terrain conditions. Thereafter, they practiced cutting a fire line for 1 hour just prior to being called to the fire scene. Fire fighters were wearing standard gear including work pants under Nomex pants and a Nomex shirt with no lining worn over a white T-shirt. Of the crew interviewed by the employer, 80% had symptoms of heat illness. The decedent had been ill the day prior to the incident. The ambient temperature at the time of the incident was 98 degrees F and the relative humidity was approximately 30%. The CA/FACE investigator concluded, that in order to prevent future occurrences, fire agencies should: 1. Require supervisors to regularly monitor firefighters, using generally accepted methods, during periods of high heat stress. 2. Assure firefighters workloads are appropriate for their level of acclimatization. 3. Assure firefighters workloads are appropriate for ambient weather conditions and clothing. The victim had been raised in a steel-framed cage-type safety platform that had not been secured to the forklift. The victim removed a fluorescent light bulb from its fixture and stepped to one side of the safety platform. When the victim shifted his weight from the center of the platform to the outer edge, the safety platform toppled off the forks of the forklift. The victim, along with the safety platform, fell about 7 feet to a concrete floor where the victim struck his head and was also struck by the steel safety platform. The forklift operator called 911 and the Emergency Medical Service (EMS) arrived about 7 minutes later. The EMS transported the victim to a local hospital where he was pronounced dead on arrival. NIOSH investigators concluded that, to prevent similar incidents, employers should: Implement 29 CFR 190.178 (m) (12) (i), which requires the use of a safety platform firmly secured to the lifting carriage and/or forks when lifting personnel with a powered industrial truck (e.g., forklift). Ensure that personnel assigned to operate forklifts are thoroughly trained. Ensure that workers continually adhere to the safe work procedures that have been established by the employer. Routinely conduct scheduled and unscheduled worksite safety inspections. She was rolling the transition of the fresh asphalt and a concrete driveway. In an attempt to drive the roller back onto the main road it slipped off the edge of the road and slid toward a small ditch. The equipment rolled over on its side. The victim, who was not wearing her seat belt, was struck by the Roll Over Protective Structure (ROPS). The victim was pronounced dead at a local hospital with estimated time between injury and death listed as "immediate". The WV FACE Investigator concluded, that to reduce the likelihood of similar occurrences, employers, including the self-employed, should: 1. Ensure that tree fellers properly evaluate the area around timber to be felled so that potential hazards can be identified and appropriate control measures implemented. 2. Ensure that tree fellers prepare an adequate escape path and move a safe distance from the base of the tree as the tree is falling. 3. Develop, implement, and enforce a written safety program which includes, but is not limited to, training in hazard identification, avoidance, and abatement. 4. Ensure that tree fellers use appropriate personal protective equipment for the work being performed. The tractor was not equipped with a rollover protective structure and a seat belt. The east end of the field was adjacent to a county road ditch. The victim had nearly completed plowing the field and it appeared that he was making the last few trips across the field plowing only from the east end of it to the west end. After reaching the west end he drove the tractor on the farm driveway back to the east end. After reaching the east end of the field, he was unable to make a sharp 180 degree left turn and enter the open furrow to again plow across the field. Since he was unable to make the sharp left turn, after turning left from the driveway (90 degrees) and entering the field he drove past the open furrow and made a circular turn to the right to enter the open furrow. While making this circular turn the left wheels of the tractor entered the road ditch and the tractor overturned. A passing motorist discovered the overturned tractor and drove to a nearby farm where a call was made to emergency medical personnel. They arrived at the scene shortly after being notified. The farmer from whose place the emergency call was made also arrived at the scene with a tractor to assist in freeing the victim. Shortly after the victim was freed, emergency personnel pronounced the victim dead at the scene. MN FACE investigators concluded that to reduce the likelihood of similar occurrences, the following guidelines should be followed: 1. all tractors should be equipped with a rollover protective structure and a seat belt. As the operating crane traveled along its runway, it struck the aerial platform, knocking it off balance and causing it to tip over to the floor. When the platform and victim hit the floor, the victim sustained massive head trauma and internal injuries. Co-workers notified the emergency medical service who responded within 3 minutes, and the victim was transported to a local hospital where he was pronounced dead. A ground man, facing away from the victim, was picking up the fallen limbs when he heard a snapping sound similar to that of a limb breaking. When the ground man turned, he saw that the bucket's extended boom had broken, 5 feet from its base, and had fallen to the ground. The victim, tied off to the anchor point in the bucket, had fallen to the ground inside the bucket. The ground man immediately alerted the supervisor, who summoned the rescue squad from a radio in the company truck. The rescue squad arrived within 10 minutes and transported the victim to the hospital, where he was pronounced dead by the attending physician. The tractor continued down the hill and into a drainage ditch where it was found out of gear. The rotary mower ran over the victim's legs, and evidence at the scene indicated that he crawled a short distance from the tractor and then came back and attempted to operate it before he died. In order to prevent similar occurrences, the KY FACE investigator recommends that: 1. older tractors be retrofitted with rollover protective structures (ROPS) and seatbelts. Department of Health & Human Resources AB - On July 26, 1997, a 61-year-old male farmer (victim) died of injuries sustained when the tractor he was driving rolled end-over-end and then barrel-rolled down a steep slope. The victim had gotten his tractor out of deep ruts in a field road and was driving uphill when his back tires slid over the edge of the hillside precipitating the rollover. He was working alone at the time of the incident. The tractor did not have rollover protective structures or a seat belt. The victim's sister became concerned when he did not arrive home before dusk and, along with a neighbor, searched the field in which he had been working. Upon finding the victim, they went to the victim's home and called the EMS. The EMS arrived within 27 minutes. With the approval of the sheriff's office, the EMS transported the victim to the local hospital where he was officially pronounced dead. The coroner estimated that the victim had been dead several hours before he was found and that he had died immediately after sustaining his injuries. Damage to the steering wheel and to the victim's head, neck, and chest suggest that the tractor steering wheel crushed the victim during the roll before he was ejected. The WV FACE Investigator concluded that, to reduce the likelihood of similar occurrences, the following guidelines should be followed by tractor owners: 1. Equip all tractors with rollover protective structures (ROPS) and a seat belt. 2. Evaluate environmental conditions and terrain prior to using field roads and make necessary adjustments to accommodate for them. The victim and two coworkers were removing soil and clay from the site and had nearly completed the excavation process. The victim used a measuring stick to check the grade along the site to ensure that it was excavated to the proper depth. A few moments before the incident, the Caterpillar operator drove to the west end of the excavation area and the victim walked to a position about fifty feet behind the Caterpillar. While the victim checked the grade of the area, the operator shifted the Caterpillar into reverse and began backing it across the construction site. The operator did not see the victim and continued backing until he saw the victim laying in front of the Caterpillar. A call was immediately made to emergency personnel. They arrived shortly after being notified and pronounced the victim dead at the scene. MN FACE investigators concluded that to reduce the likelihood of similar occurrences, the following guidelines should be followed: 1. mobile equipment should be equipped with sensing units to detect pedestrian workers in the blind spots of the equipment. He had worked with the employer for many years and had a good knowledge of most the farm's equipment.
