Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

On the day of the incident, the employer, his sons, ages 17 and 10, and the victim were working on a sewer installation project on private property in an apartment complex. Shortly before the incident, the employer parked his backhoe next to the trench and left with his older son to get a backhoe with a smaller bucket. The victim remained in the trench leveling the soil in preparation for laying sewer pipe. The 10-year-old boy remained at the site to watch the victim and to be available to get help if anything went wrong. Shortly after his father left, the 10-year -old boy called several warnings to the victim as he saw soil caving in from the trench walls. Minutes later he called a warning when he saw a large section of trench wall breaking loose and beginning to fall. The section struck the victim before he had an opportunity to move and completely covered him. The boy called out to the victim several time and, hearing no response, ran to the apartment complex for help. One resident called 911, while a second ran to the trench. When the apartment residents arrived at the trench, they observed the soil pile at the bottom of the trench but could not see the victim. Realizing that the trench walls remained unstable and hazardous, they warned others to keep out of the trench and waited for the police who arrived approximately 3 minutes later. A police officer entered the trench and saw the victim through a 2 to 3-inch gap between the fallen section of earth and the trench wall. He was unable to detect any sound or movement from the victim. Police and Fire Department personnel, after evaluating the incident scene, determined that this was a body recovery operation, not emergency rescue. They summoned a specially trained Technical Rescue Team (TRT) from another fire department to direct recovery operations. The victim's body was recovered approximately 10.5 hours following the incident and taken to the local morgue for autopsy. The bundle struck the victim as it fell, knocking the victim to the ground. A customer exiting the company building heard the load crash to the ground and crossed the parking lot to the trailer's location. The victim was lying on the ground beneath the pipes. The customer went back into the store to notify workers and call medical emergency services (EMS). Coworkers lifted the pipes off the victim and began CPR. The victim was transported to a nearby medical center, and he died shortly thereafter from his injuries. Based on the findings of the investigation, to prevent similar occurrences, employers should: 1. Ensure a competent person inspects all incoming freight before accepting the shipment AB - 2. The victim had finished cutting for the day and was on his way back to the landing site. He decided to fell one last tree on the way out. The victim was felling a tree on terrain with a 70% slope. The slope was rocky and laden with grapevines. He was in the process of felling a yellow poplar. The poplar was approximately 30 inches in diameter at breast height and 90 feet tall. Eighteen feet directly behind and uphill from the victim was a small tree approximately 6 inches in diameter. The smaller tree had grapevines which were attached to the top of the tree being felled. His felling technique was un-controlling, often referred to as "stump jumping." As the tree began to fall, the entangled vines snapped off the smaller tree approximately 9 feet above ground level. The felled tree continued pulling the smaller tree down hill until it struck the victim who was 4 feet from the base of the tree. He was not wearing a hard hat and was struck in the head. Fellow workers who had also finished for the day and were on their way out heard the fell but got no response from the victim when they yelled to him. A cable puller was first to discover the victim. He ran to the landing site, at which time a cellular phone was used to summon rescue. Fellow workers administered first aid and CPR. The EMS arrived within approximately 25 minutes. The victim was transported to the local trauma center and died two hours later. The WV FACE Investigator concluded that, to reduce the likelihood of similar occurrences, employers should: 1. Ensure that tree fellers properly evaluate the timber and the area around the timber to be felled so that potential hazards can be identified and appropriate control measures implemented. 2. Ensure that tree fellers prepare an escape path and move a safe distance from the base of the tree as it is falling. 3. Ensure that tree fellers utilize proper directional felling techniques. 4. Develop, implement, and enforce a written safety program which includes, but is not limited to, training in hazard identification, avoidance, and abatement as well as methods dealing with worker non-compliance. A flat roof over the first story portion of the company was covered with drifted snow, which varied in depth from several inches to over 3 feet in places. The victim was using large steps to walk through the snow to the south side of the roof, where the snow was deepest. It completely covered the tops of skylights in this area. He apparently failed to see the unguarded, three-foot square skylight and stepped onto it while walking. The plastic bubble of the skylight broke, and the victim fell to the concrete floor. His co-worker had been walking toward the north side of the roof, and turned to look when he heard a noise from the victim's direction. Seeing the broken skylight, the co-worker yelled for help. Workers on the main floor were already assisting the victim, where he had fallen, and called emergency services. EMS responders were on the scene within minutes, and a physician pronounced the victim dead at the scene. To prevent future fatalities of this type, the FACE investigator recommends employers should: 1. Guard skylight openings. 2. Lock all doors that provide access to unguarded rooftops. 3. Provide training in the recognition and avoidance of unsafe conditions to workers who are assigned tasks outside their normal duties. The tower being constructed was a three-sided, 10-foot-face, 1,040-foot tall structure to be utilized for a high definition digital television antenna. The victim was at the 940-foot level when the incident occurred. He was wearing a positioning safety belt with a T-Bar attached to the D-rings of the belt. Attached to one end of the T-bar was an adjustable length lanyard with a large hook and the other end was a large hook. It appears the victim placed the hook attached to his T-Bar over the guy wire. Apparently the victim did not attach his other positioning lanyard to the tower before sliding out onto the guy wire. As the victim slid out onto the wire he discovered he was not attached to the tower. At this point he was able to hang on and not slide down the wire. The closest co-worker who was aware the victim was in trouble climbed out to help him back to the tower. The victim had one foot over the top of the wire but could not keep himself from sliding. He started sliding and could not stop. The victim then slid rapidly down the wire striking the anchorage point on the ground. He was pronounced dead at the scene. The MO FACE Investigator concluded that in order to prevent similar occurrences employers should: 1. provide employees with a 100% fall protection system compatible with the work being performed, instruct employees in the proper use of the system and equipment, and ensure their use AB - 2. provide workers with a proper work-positioning system, instruct workers in the proper use and limitations of the system, and ensure its use AB - 3. develop, implement, and enforce a comprehensive written safety program which includes a commitment to 100% tie off and written procedures to implement 100% fall protection. Additionally, manufactures of tower components and tower owners should: 4. consider installing fall-protection fixtures on tower components during fabrication or erection that would facilitate the use of fall protection. As they were traveling west, they encountered a forklift traveling east on the same roadway. Just before the electric cart and forklift passed each other, the forklift driver turned left into the cart. The decedent and his co-worker were thrown from the cart. The forklift ran over the decedent. The co-worker was thrown clear of the forklift's path and received cuts and bruises. The forklift was being driven with the load held high. The electric cart did not have a high warning flag to increase its visibility. The CA/FACE investigator determined that, in order to prevent future occurrences, employers should as part of their Injury and Illness Prevention Program (IIPP): 1. ensure employees drive forklifts with the forks as low to the ground as is safe. 2. equip electric carts and similar small vehicles with high warning flags. He moved his rig forward after weighing and got out of the cab. He had waved to a second tractor-trailer rig to indicate that it was okay to back into the trash pit. As he was walking near his rig the second tractor-trailer rig was backing up alongside. The decedent stepped too far away from his rig and was run over by the trailer of the backing rig. The backing rig's backup alarm, located on the tractor, was not functional. The trailer did not have a backup alarm. None of the three spotters on site were able to react in time to prevent the incident. The CA/FACE investigator determined that, in order to prevent future occurrences, employers should as part of their Illness and Injury Prevention Program (IIPP): 1. ensure no one walks near tractor-trailer rigs when they are moving in close quarters. 2. install backup alarms on the trailers of tractor-trailer rigs. 3. ensure pre-trip inspections include checking whether or not backup alarms are functional. 4. stop moving traffic when pedestrians enter an area of danger. The propane-powered tractor was parked in a machine shed, which had a dirt floor. The farmer was working alone and evidently assumed the tractor transmission was in neutral, then started the engine while standing on the ground to the left of the tractor or while standing on the side step of the tractor. The ignition switch was easily reachable from the ground on the left side. The tractor started and lurched forward, smashing equipment in front of it and partially crashing through the tin wall of the shed. There were no witnesses to the event, but it appears that the farmer was standing on the ground, holding in the clutch with his left hand, starting the tractor with his other hand, then letting out the clutch assuming the transmission was in neutral. The tractor moved ahead about 3 m (10 ft), stalled, and stopped when it hit a corner post of the machine shed. The farmer was found trapped partially under the left rear wheel of the tractor, dead at the scene. The tractor engine was cold, with the ignition key in the on position. The tractor originally had a safety switch built-in to the transmission, but it had been bypassed by the previous owner of the tractor. This enabled starting the tractor in gear when the clutch was pressed down. Recommendations based on our investigation are as follows: 1. Farmers and tractor operators should not bypass factory-installed safety devices. 2. Tractor operators should not attempt to start their machines from any position other than sitting in the operator's station. The incident occurred outside a condominium complex where the victim was clearing and grading a drainage retention pond. As he was working, the left front wheel of the backhoe loosened and came off. Before fixing the wheel, the victim and his coworker left the site, reporting the problem to the employer when they met during lunch. The two workers returned to work on the machine. Planning to lift the wheel by digging the backhoe's front bucket down against the ground, the victim stood by the side of the machine and reached in to turn on the ignition. The backhoe was in gear and immediately lurched forward, striking the victim, and pulling him under the rear tire. A police chief was also injured during an attempt to move the backhoe. Despite rescue efforts to lift the backhoe with a larger front-end loader, the victim was asphyxiated and died at the scene. To prevent similar incidents in the future, NJ FACE recommends the following safety guidelines: 1. Employers should ensure that heavy equipment operators are properly trained before using the machines. 2. Employers should frequently inspect construction equipment to ensure that all operational controls and safety devices are in good working order. 3. Employers should become familiar with available resources on safety standards and safe work practices. The decedent had climbed a fixed ladder to gain access to a platform built on top of the boiler. A valve in the steam line needed to be opened to release pressure. In order to turn the valve wheel that was 6 feet, 1 inch above the platform floor, the decedent climbed onto the guardrail surrounding the platform. He was using a 3-foot-long pipe wrench to open the 16-inch valve wheel when the decedent slipped, or the pipe wrench slipped causing the decedent to topple over the guardrail. The decedent landed on the concrete floor below. The employer did not have an Injury and Illness Prevention Program (IIPP) or a code of safe practices. The decedent, who was substituting for the boilermaker, was not trained in the task he undertook. The employer did not have a person on site to which safety responsibilities were assigned. No recent safety inspection had been performed. The CA/FACE investigator determined that, in order to prevent future occurrences, employers should as part of their Injury and Illness Prevention Program (IIPP): 1. ensure employees do not stand on guard railing to gain access to a work area. 2. ensure employees do not perform tasks for which they are not trained. 3. make work areas safely accessible to employees. 4. supply proper tools that are commercially available for tasks employees must perform. 5. develop and implement a formal, written Injury and Illness Prevention Plan to include a code of safe practices. 6. assign a qualified safety person to fully carry out safety responsibilities. The front tires had not been chocked. As he was performing a brake job underneath the bus, the bus slipped off the jack stands. The CA/FACE investigator determined that, in order to prevent future occurrences, employers should as part of their Injury and Illness Prevention Program (IIPP): 1. ensure employees use only tools and equipment that have been tested, certified and rated. 2. ensure employees chock the wheels of vehicles that remain in contact with the working surface. 3. ensure employees do not place jack stands on portions of a vehicle that are not fixed in place. Department of Health & Human Resources AB - On May 18, 1999, a 39 year-old male tree feller (the victim) died of injuries sustained when he was struck by and then pinned to the ground by the tree he was felling. The victim was felling trees on terrain with a 16% slope. He was in the process of felling an oak. The oak was approximately 16 inches in diameter at breast height and 85 feet tall. He asked the dozer operator working with him to clear a small area next to the base of the tree. He then made a few additional brush clearing cuts for base access. He made a small, open faced notch. His third and final cut left little if any hinge wood. After making his final cut he, moved approximately 4 feet to the side and watched the tree fall. In the process, the tree contacted an oak 24 feet from the base of the tree. The contacted oak was 8 inches in diameter at breast height. As the falling tree contacted the smaller oak, it rolled and the butt cantilevered into the air striking and pinning the victim to the ground. He was pinned diagonally across the abdomen and pelvis. Witnessing the incident, the dozer operator responded by lifting the tree enough to free the victim using the dozer blade to slide the tree up the now-slanted smaller oak. He then slid the victim out. The victim remained conscious. After reassuring the victim, the operator reached in the victim's pocket and acquired the keys for his truck which was located approximately one quarter of a mile downhill at the landing site. Upon reaching the truck, he realized he had the wrong set of keys and began a lengthy downhill run through the woods. His goal was a house located approximately two miles away at the beginning of the site's access road. At the home, he called 911 and waited for the ambulance. Riding in with the ambulance, he guided them to the victim. The dozer operator then cleared a landing area for life-flight. Approximately three hours after the incident, the victim was flown to the nearest trauma center where he died later that evening. The WV FACE Investigator concluded that, to reduce the likelihood of similar occurrences, employers should: 1. Ensure that tree fellers prepare an escape path and move a safe distance from the base of the tree as it is falling. 2. Ensure that tree fellers utilize proper directional felling techniques. 3. Develop, implement, and enforce a written safety program which includes, but is not limited to, training in hazard identification, avoidance, and abatement as well as methods dealing with worker non-compliance. 4. Designate a competent person to conduct frequent and regular site safety inspections. 6. Ensure that a timely means for summoning emergency services exists on each job-site. According to co-workers, the hammer was lowered by the crane operator and struck the pile before it was set causing it to swing into the victim. The victim was pushed into the wall of the trench box and killed. Emergency medical assistance was immediately summoned while co-workers attempted to assist him. The victim was pronounced dead at the scene and transported to the medical examiner's office. To prevent future similar occurrences, the MA FACE Director concluded that employers should: 1. Employ only competent crane operators experienced in the type of equipment to be operated, in this case, a piledriver. 2. Develop and maintain a health and safety program which includes but is not limited to regular consultation with employees on safety issues. Also, the state and/or other certifying agencies should consider issuing a special license for the operation of cranes as piledriver's. At 8:00 a.m., the victim began assisting his partner who was loading the truck using a knuckle boom loader. The partner left the loader and accessed the truck's cab. He then asked the victim to direct him as he backed the loaded truck out of the loading zone. The fully loaded truck which was moved prior to securing the load, was being relocated to make room for an empty truck, under contract waiting to be loaded. As the victim's partner was parking the truck, the victim approached the truck to begin securing the load. He approached one of the binding straps located on the side of the truck. As he did, a log rolled off of the truck pinning him to the ground. An individual who accompanied the empty truck's driver to the site was standing on the landing and witnessed the incident. The bystander ran to the victim's partner to inform him of the incident. The partner ran to the victim. It was reported the partner removed the log from the victim's body by hand. The empty truck's driver ran to the residence of the land owner and called 911. EMS responded, and he was taken to local hospital and pronounced dead at 9:35 a.m. The WV FACE Investigator concluded that to reduce the likelihood of similar occurrences, employers should: 1. Ensure that the height of the stacked logs does not exceed the height of the standards on the truck. 2. Ensure that stacked logs are secured immediately after the loading operation is complete prior to moving the truck. 3. Develop, implement, and enforce a written safety program which includes, but is not limited to, task-specific safety procedures and worker training in hazard identification, avoidance, and control. Additionally, manufacturers and employers should consider manufacturing and/or retrofitting log trucks with retention stakes high enough to adequately secure anticipated log loads. The tipping caused the driver to fall from the forklift truck, which then continued to tip over onto the victim. The police and emergency medical services were called immediately by a number of people driving on the state road in front of the store as well as a co-worker. The victim was transported to a nearby city hospital emergency room where he was officially pronounced dead within an hour of the incident. The MA FACE Program concluded that to prevent similar future occurrences, employers should: 1. Assure that forklift operators are trained in the safe operation of their vehicles. 2. Require that operator restraints when available on forklift trucks be used. 3. Develop, implement, and enforce a comprehensive safety program that includes, but is not limited to, worker training in controlling the health and safety hazards of warehouse work. The incident occurred at one of three plants operated by the company, whose central office was in another state. The plant maintenance and repair person quit the company about a month before the incident, and a replacement had not been hired. About two weeks before the incident, a machine guard had been removed from the molding machine in preparation for a visit from a maintenance worker from another company plant. At the time of the incident, the victim (shift supervisor) and a co-worker were examining the machine to determine the location of a hydraulic fluid leak. The molding machine was operating while the victim and co-worker peered into it so they could see the location of the leaks with the hydraulic hoses under pressure. The victim was bending forward into the back of the machine, with his head positioned next to a fixed metal bracket. The machine cycled automatically, causing a metal tie bar to move back and pinch his head against the bracket with about 500 pounds of pressure. The co-worker heard a sound, looked in the direction where the victim had been working, and saw the victim's head pinned in the machine. The co-worker called for help, the machine was turned off, and the victim was released. EMS workers arrived within four minutes, and transported the victim to the hospital where he was pronounced dead. To prevent future fatalities of this type, the FACE investigator recommends employers should: 1. Maintain guards in place over machine pinch points when machines are operating. 2. He died Thursday February 4, 1999, as a result of the injuries sustained in the fall. A local window cleaning company that has been in business for more than one hundred years employed the victim. The company had no history of any fatal injuries before this incident. The victim had worked for the company for more than three years. He had cleaned the windows of this building on other occasions. The victim was training an employee in the use of the boatswain chair technique to wash windows. According to the report prepared by the OSHA investigator, the two workers set up the equipment by anchoring the roof roller to the air conditioning unit with one line, the safety or lifeline. The other line, the work line, was affixed to the roof roller and was not anchored to any site on the roof. The two workers successfully washed the windows on the front (north) of the building. They then returned to the roof to reposition the roof roller (scaffolding) in order to wash the windows on the side of the building (east). The co-worker disconnected the safety or lifeline securing the roof roller to its anchorage on the building's air conditioning unit and had not yet attached it to its new anchorage. The co-worker had his back to the victim and did not see the victim attach his safety harness to the safety line of the roof roller and go over the edge of the building. The victim and the unsecured roof roller were found in the alley on the east side of the building. The victim was transported by fire department medics to the nearest level one trauma center, where he died the next day. Recommendations: 1. Test equipment before going over the side of a building to ensure proper anchorage of scaffolding. 2. Trainer should personally check all equipment especially when engaged in training exercises. 3. Suggest the use of specific anchorage devices for window cleaning equipment. 4. Emphasize communication and cooperation between co-workers. 5. Maintain separate anchorage points for safety lines and work lines. 6. Recommend the use of protective headgear.
