Data in this report may be used to accelerate action to reach disproportionately affected populations and achieve national goals, including the Department of Health and Human Services (HHS) core indicators [1, 2], Healthy People 2030 [3], the National HIV Strategic Plan for the United States: A Roadmap to End the Epidemic 2021–2025 (NHSP) [4], and the United States—Ending the HIV Epidemic in the U.S. (EHE) initiative [5]. HIV surveillance data have highlighted differences in HIV burden by sex at birth, race/ethnicity, and transmission category in the United States. Factors other than these (i.e., social determinants of health) contribute to disparities, and a better understanding of the social determinants that affect the health of populations can be beneficial. AB - The term social determinants of health (SDH) refers to the nonmedical factors that influence health outcomes. They are the conditions in which people are born, grow, work, live, and age, and the wider set of forces and systems shaping the conditions of daily life. These forces and systems include economic policies and systems, development agendas, social norms, social policies, and political systems that are responsible for most health inequities [6], including the disproportionate effect of HIV on certain populations. Achieving equitable health will require focusing on factors associated with disparities and directing resources to disproportionately affected populations and geographical regions. This approach will improve the health of all persons. AB - Stratifying public health data by key SDH, such as income and health insurance coverage, is useful for monitoring health inequities. Addressing SDH makes it possible to measure and identify health differences between populations or geographic areas and can provide insight for identifying populations or areas that may benefit from HIV testing, prevention, and treatment initiatives. AB - For this report, the Centers for Disease Control and Prevention (CDC) uses geocoded HIV surveillance data reported by state and local HIV surveillance programs that have been linked with SDH indicator variables on factors that may affect HIV transmission. HIV surveillance data are geocoded to the census tract level by address of residence at the time of diagnosis and then linked to census tract-level SDH data from the U.S. Census Bureau’s American Community Survey (ACS). Using census tract-level surveillance data increases CDC’s ability to assess the geographic distribution of HIV, the social determinants associated with HIV, and the relationship of HIV to other diseases and healthcare resources in a defined area. Addressing SDH that adversely affect health outcomes may advance efforts to reduce disparities in HIV diagnosis rates among populations and areas. AB - This surveillance supplemental report presents data on persons aged 18 years and older (adults) with HIV infection diagnosed in 2019 and reported to the National HIV Surveillance System (NHSS) through June 2021. Data were limited to persons aged 18 years and older to align with the population in the ACS from which SDH indicator variables were collected. Numbers, percentages, and rates of diagnoses of HIV infection were based on data reported from the 50 states, the District of Columbia, and Puerto Rico after the jurisdictions geocoded cases to the census tract level, linked to ACS estimates, and transmitted SDH information to CDC on cases (after personal identifiers were removed) diagnosed during 2019. AB - SDH data presented in this report are the result of the linkage of geocoded HIV diagnosis data (at the census tract level) to SDH indicator variables from the 2015–2019 ACS and are based on where the person was living at the time of diagnosis. One of two amounts are offered: 10000 Tanzanian shillings ($4.50 in US dollars – the smaller incentive group) and 22500 Tanzanian shillings ($10.00 US dollars – the larger incentive group). The incentives are intended to motivate clinic attendance, with the amounts chosen in consultation with local and national stakeholders and designed to motivate clinic attendance and to partly cover the costs of transportation, food, and lost wages for the day spent at the clinic. Cash transfers are delivered a maximum of once monthly via an automated mobile money system that is linked to a biometric attendance monitoring system. Specifically, participant fingerprints and mobile banking account details are registered in the mHealth system. Subsequent clinic attendance is logged in the mHealth system upon a fingerprint scan administered by a pharmacist or research assistant at the pharmacy. Participants who do not have access to a mobile banking account receive money in hand from a research assistant. Participants can receive a maximum of six transfers totaling a potential $27 to $60 USD depending on study group and visit attendance. Streamlined Care improves patient-provider relationships and increases patient and provider HIV-related knowledge and skills. In addition, the intervention addresses structural barriers including long wait times, fixed clinic hours, lack of efficient appointment reminder and tracing mechanisms, and transportation cost and inconvenience associated with accessing care. A nurse-driven triage system is implemented to efficiently tailor clinic visits to what patients need to reduce wait times. Scheduled visit intervals and ART pick-up are lengthened to every 3 months for stable patients to decrease wait times by reducing patient volume and increase convenience associated with less frequent clinic visits. The intervention integrates multidisease care for hypertension and diabetes and includes appointment reminders and phone access for health-related questions or appointment rescheduling. Off-hours clinic access is also offered. For patients who missed an appointment, re-engagement efforts of increasing intensity are offered, including an initial phone call followed by physical tracing and facilitated transportation to return to care. Streamlined Care also includes provider and staff training on providing care. LINCS uses three D2C referral strategies – referrals from (1) health care providers, (2) HIV surveillance epidemiologists, and (3) LINCS navigators using a clinical electronic medical record (EMR) registry matched to surveillance. All referrals are identified via the absence of viral load or CD4 test results or the presence of a recent high viral load (VL) value (VL >1500 copies/mL within last 4 months or no VL in >15 months). HIV surveillance epidemiologists use the viral load or CD4 criteria to identify patients in the Enhanced HIV/AIDS Reporting System (eHARS) for referral. Provider referrals also include patients with no evidence of care post-diagnosis, who have not accessed care over many months, or who are not adherent to medication. The clinical EMR match uses a registry developed through Health Resources and Services Administration HIV Ryan White quality improvement activities and includes all PWH receiving care in the public health clinics where LINCS navigators serve. The eHARS and clinical EMR are matched, and lists are sent to LINCS to determine current NIC status for referrals from all three sources prior to beginning outreach. Navigators attempt to locate individuals within 30 days of assignment using multiple electronic systems including the local STD surveillance database, the public health hospital EMR, and other disease intervention searching tools. PWH who are enrolled in LINCS are required to attend a relinkage appointment with an HIV care provider and receive short-term case management for up to 90 days; navigators provide a range of field-based services such as benefits navigation, appointment reminders, clinic accompaniment, motivational interviewing, and modified Anti-Retroviral Treatment and Access to Services (ARTAS) strengths-based case management. Special attention is given to ensure there is no lapse in combined antiretroviral therapy (cART) due to the transition from pregnancy Medicaid to AIDS Drug Assistance Program (ADAP), assessment of mental health of mother, facilitation of transportation and scheduling of follow-up visits for outpatient HIV care; (2) intense monitoring by information tracking of referred pregnant WWH on date of diagnosis, estimated delivery date, and any known barriers to care; (3) pamphlets that addressed the unique needs and concerns of pregnant and postpartum WWH and ASCC HIV postpartum care and treatment services; and (4) case management which includes face-to face meetings and telephone calls to facilitate communication, build trust, address barriers to engagement and retention in care, and provide ancillary services as needed. In addition, the intervention addresses structural barriers including long wait times, fixed clinic hours, lack of efficient appointment reminder and tracing mechanisms, and transportation cost and inconvenience associated with accessing care. A nurse-driven triage system is implemented to efficiently tailor clinic visits to what patients need to reduce wait times. Scheduled visit intervals and ART pick-up are lengthened to every 3 months for stable patients to decrease wait times by reducing patient volume and increase convenience associated with less frequent clinic visits. The intervention integrates multidisease care for hypertension and diabetes and includes appointment reminders and phone access for health-related questions or appointment rescheduling. Off-hours clinic access is also offered. For patients who missed an appointment, re-engagement efforts of increasing intensity are offered, including an initial phone call followed by physical tracing and facilitated transportation to return to care. Streamlined Care also includes provider and staff training on providing care. Division of HIV/AIDS Prevention AB - The Community Health Worker (CHW) program is a structural intervention that builds capacity of Ryan White HIV/AIDS Programs (RWHAP) to implement a CHW program in their HIV care settings and work toward integrating CHWs into their multidisciplinary HIV clinical care team. The program is part of a Health Services Resources Administration (HRSA) initiative conducted in RWHAP locations across the United States. To support CHW implementation, organizations receive training, technical assistance, and coaching while participating in collaborative learning sessions. The CHW program at each site is guided by the training curriculum and implementation guide tailored to HIV care and services based on the core constructs and common roles of CHWs from the Community Health Worker Core Consensus (C3) project. CHWs perform a variety of tasks depending on client needs, such as home visits, health education, treatment support, and provision of referrals to support services and assistance with transportation. CHW training consists of standard curricula that includes 16 hours of HIV core competency training on topics such as: education around the viral life cycle, treatment adherence support, addressing stigma and disclosure, harm reduction, and motivational interviewing techniques; and up to 64 hours of CHW core competency training related to working as part of a multidisciplinary team, professional roles and boundaries, communication skills, outreach, and navigation activities. Activities with clients include coaching, giving emotional support, making appointment referrals, accompanying clients to appointments, offering concrete services such as phones or bus passes, providing health care appointment reminders, arranging health care transportation, and updating care plans and medical records. In Puerto Rico, TCC is integrated into the existing organization operations of The One Stop Career Center of Puerto Rico (OSCC-PR) to address health care access and social determinants of health (e.g., housing, employment/income, food, transportation, social support) for incarcerated persons with HIV. Direct service staff at OSCC-PR are trained in care coordination, HIV treatment and prevention, stigma, and sexually transmitted infections (STIs), as well as crisis intervention. Direct service staff responsibilities also include accompanying PWH from correctional facilities to their home and offering community case management, transportation, accompaniment to HIV care, and support in accessing mental health and substance use services, as well as support to address other needs (e.g., housing, employment/income, personal hygiene products). In addition to the TCC services, the OSCC-PR also established a provider network across all jurisdictions in Puerto Rico to support reentry and address care needs for PWH. Clubs are managed by lay staff and nurses at the facility with support from community health workers. The goal is to keep patients engaged and adherent by providing social support and facilitating medication delivery and treatment monitoring while reducing patient burden at the clinic. The intervention sites allow clinically stable patients to choose between three prescription collection strategies available: ACs, Decentralized Medication Delivery (DMD) (i.e., prepacking and distribution of medication to pick-up points which are at locations other than the clinic pharmacy), or spaced fast-lane appointments, which the authors did not evaluate or elaborate on. Switching between strategies is also allowed, but this is monitored. Patients cannot enroll in more than one approach at the same time. In the first 3 months, all participants receive neutral, encouraging health promotional text messages with a number to text if additional help is needed. Participants in the intervention group only receive an additional immediate text message after visiting the clinic that provides information about entry into a lottery if they complete the next step in the HIV care continuum. One week later, another text message is sent to the intervention participants indicating the lottery result (win or not) and promoting additional opportunities to win the lottery. Lottery eligibility is conditional on clinical registration, ART initiation, completing ART monitoring and refill visits, and viral suppression at six months. Lottery prizes are either a mobile phone, data, or a gift card (all valued at South African Rand 1000/$100 US Dollars at the time of the study implementation). Staff provides information to the intervention group when a lottery is won and what prize is chosen.
