The intervention consists of 6 consecutive 2- to 3-hour sessions delivered during a weekend retreat (half-day Friday and all-day Saturday and Sunday). Rather than a singular emphasis on condom use, 3MV uses a menu of behavior change options for HIV/STI prevention. Session 1 (The Culture of black MSM) helps participants recognize how racism and homophobia are related to sexual and substance use risk behaviors. Session 2 (STI/HIV Prevention for black MSM) describes the roles of ‘‘Tops’’ and ‘‘Bottoms’’ as they relate to sexual relationship dynamics and the risk of STI and HIV transmission. Session 3 (STI/HIV Risk Assessment and Prevention Options) helps participants personalize their own risk by building a menu of behavioral options (e.g., abstinence, mutual monogamy between 2 HIVseronegative partners, consistent condom use) to reduce HIV/STI transmission risk. Session 4 (Intentions to Act & Capacity for Change) enhances participants’ intentions to change their own risky behaviors and guides them toward safer sex behaviors. During Session 5 (Relationship issues: Partner selection, Communication & Negotiation), participants recognize power and control dynamics in their relationships, and are encouraged to select and implement a relationship-focused risk-reduction behavior change option with their partner(s). During these conversations, the POL corrects misperceptions, discusses the importance of HIV prevention, describes strategies he uses to reduce his own risk (e.g., keeping condoms nearby, avoiding sex when intoxicated, resisting coercion for unsafe sex), and recommends that the peer adopt safer sex behaviors. Popular opinion leaders wear buttons displaying the project logo, which also is on posters around the bars, as a conversation-starting technique. Each POL agrees to have at least 14 conversations with peers and to recruit another POL. POLs attend four weekly 90-minute training sessions that involve didactic and group discussions, modeling of effective health promotion messages, and extensive role play. The team also creates their own social events (e.g., dances, video parties, picnics, discussion groups) to attract young gay men. M-groups are peer-led, 2–3-hour meetings of 8-10 young gay men who discuss factors that contribute to unsafe sex (misconceptions, beliefs that safer sex is not enjoyable, poor sexual communication skills). Through skillsbuilding exercises, the men practice correct condom use and safer sex negotiation. Participants receive free condoms and lubricant and are trained to conduct informal outreach, where they are encouraged to discuss safer sex with their friends. The ongoing publicity campaign attracts men to the project by word of mouth and through articles and advertisements in gay newspapers. A PrEP navigation tool is used to identify and decrease barriers to PrEP initiation for persons with new PrEP prescriptions. The intervention has two cycles. Cycle 1 focuses on identifying barriers to picking up initial PrEP prescriptions. Cycle 2 focuses on staff trainings and system improvements informed by patient feedback about the PN tool. In Cycle 1, the nurse practitioner (NP) follows all PrEP patients. For those who do not pick up their PrEP prescription within 7 days, the NP uses the PN tool to identify barriers to initiation. In Cycle 2, the NP continues to monitor prescription pickup rates and assesses whether patients continue to face system level barriers that were identified in Cycle 1. Cycle 2 includes an assessment of needs to improve communication among medical providers, front desk staff, pharmacy personnel, and patients. Weekly medical, community health, and pharmacy meetings are held to review patient responses and current procedures and facilitate discussion on system-level responses. Other strategies include training pharmacy staff on counseling patients about possible home delivery of refills, retraining pharmacy technicians to ask all eligible patients if they want to enroll in the pharmaceutical company copay assistance plans, and sending messages through the electronic medical record (EMR) messaging about patients who are starting PrEP and have insurance coverage concerns. Text messages were used to remind participants to take their medication and confirm that the medication was taken. Int ervention participants tailored and personalized text message reminders to their medication regimen, including the number and time of dosages. Text message reminders and follow-up messages were also personalized to reflect content meaningful to their culture or other sources of identity. To protect confidentiality, intervention participants were encouraged to delete messages after taking medication, use messages that would not reveal their HIV status or mention medication, and received information about phone confidentiality (e.g., passcode protection). Message delivery was timed to coincide with the participant’s dosing schedule. Youth-friendly motivational or encouraging follow-up messages were delivered that were based on the youth’s responses to taking their medication (e.g., “Well done!” or “You can do it!"). Two interventions were evaluated against standard clinic ART delivery: (1) community-based ART initiation with quarterly monitoring and ART refills through mobile vans and (2) a hybrid approach that consisted of ART initiation at a clinic site followed by mobile van monitoring and refills. Participants in the community-based ART group receive same-day ART initiation including standardized counseling and the national HIV program’s regimen of ART. Seven days after ART initiation, participants receive a call asking about symptoms, side effects, and adverse events. Participants return to a mobile van parked at a known location and scheduled time for five in-person follow-up visits at 1, 3, 6, 9 and 12 months for ART resupply, clinical monitoring, counseling, and ascertainment of adverse events and social harms. Participants receive an automated text message reminder the week before their visit is due to schedule appointments for their mobile van visits. Participants can reschedule visits by text message, request additional ART if traveling, and nominate someone else to collect their medication. Participants who missed appointments are contacted and their visit is rescheduled. The mobile phone service is regularly available on evenings and weekends and an application is used for standardized monitoring. For HIV and ART monitoring, participants complete a clinical questionnaire to screen for symptoms of ART adverse events, TB, and other common opportunistic infections. HIV plasma viral load is assessed for treatment success at month 6. Participants also receive individualized adherence support. Participants who require additional clinical services are referred to care and followed until they are linked to a clinic. Participants in the hybrid group are counseled to notify the study team once they initiate ART at the clinic to facilitate transition to the community for refills and monitoring. Until notification of ART initiation, they receive quarterly phone calls to inquire whether they have initiated ART and record adverse events. Once participants initiate ART at the clinic, they follow the identical procedures described above for the community-based ART delivery group. A PrEP navigation tool is used to identify and decrease barriers to PrEP initiation for persons with new PrEP prescriptions. The intervention has two cycles. Cycle 1 focuses on identifying barriers to picking up initial PrEP prescriptions. Cycle 2 focuses on staff trainings and system improvements informed by patient feedback about the PN tool. In Cycle 1, the nurse practitioner (NP) follows all PrEP patients. For those who do not pick up their PrEP prescription within 7 days, the NP uses the PN tool to identify barriers to initiation. In Cycle 2, the NP continues to monitor prescription pickup rates and assesses whether patients continue to face system level barriers that were identified in Cycle 1. Cycle 2 includes an assessment of needs to improve communication among medical providers, front desk staff, pharmacy personnel, and patients. Weekly medical, community health, and pharmacy meetings are held to review patient responses and current procedures and facilitate discussion on system-level responses. Although the National HIV Surveillance System (NHSS) collects information about persons with diagnosed HIV infection [1], other surveillance systems provide more detailed information about care seeking, health care use, use of ancillary services, and key behaviors among people at risk for, and with, diagnosed HIV [2, 3]. In 2005, in response to an Institute of Medicine (IOM) report outlining the need for representative data on persons with HIV [4], the Centers for Disease Control and Prevention (CDC) implemented the Medical Monitoring Project (MMP), which from 2009 to 2014 collected data from a 3-stage probability sample of adults receiving HIV medical care [5]. In 2015, in response to recommendations stemming from an IOM review of national HIV data systems [6], MMP sampling and weighting methods were revised to include all adults with diagnosed HIV infection, regardless of HIV care status. MMP is a cross-sectional, nationally representative, complex sample survey that assesses the behavioral and clinical characteristics of adults with diagnosed HIV infection in the United States. MMP also provides information on behaviors and clinical outcomes affecting the risk of HIV transmission, morbidity, and mortality that are critical for achieving the goals of the National HIV/AIDS Strategy [7] and the Ending the HIV Epidemic in the U.S. (EHE) initiative [8], which seek to reduce new HIV infections in the United States by 90% by 2030. AB - The 2020 MMP data were collected from a 2-stage probability sample. During the first stage, 16 U.S. states and Puerto Rico were selected from all U.S. states, the District of Columbia, and Puerto Rico. A total of 23 project areas from 16 states, including 6 separately funded jurisdictions within these states, and Puerto Rico were funded to conduct data collection for the 2020 cycle (Table 1). In the second stage, simple random samples of persons aged ≥18 years with diagnosed HIV infection who were reported to NHSS as of December 31, 2019, were selected from each participating jurisdiction [9]. AB - This report presents unweighted frequencies and weighted prevalence estimates with 95% confidence intervals for all characteristics. The estimates describe the characteristics of adults with diagnosed HIV infection who are living in the United States, hereafter referred to as persons with diagnosed HIV or persons. The period referenced for estimates is the 12 months before the participants’ interviews and medical record abstractions unless otherwise noted. Statistical software (SAS, version 9.4) was used for analysis of weighted data [10]. Data are not reported for estimates derived from a denominator size 130%, and estimates of 0% or 100% are marked with an asterisk and should be interpreted with caution. Division of HIV/AIDS Prevention AB - The Patient-Centered Appointment Reminder Intervention is designed to decrease the number of missed appointments. Prior to the intervention, patients had the choice of receiving appointment reminders via phone or e-mail. Appointment reminders, based on patients’ preference, are made 2 weeks before, and within 24-48 hours of the scheduled appointment. The intervention includes a risk-stratified protocol developed and implemented to identify patients at risk for missing the next appointment. Patients at medium risk for missing their next appointment receive a pre-visit planning call from an assigned case manager. The case manager attempts to reach the patient via phone to provide a reminder about the appointment, identify and remove barriers to appointments (e.g., transportation and insurance issues) or offer incentives (e.g., gift card for food, bus pass, etc.). If the patient was not reached by phone, attempts are made to reach patient by text or e-mail. Patients at high risk for missing appointments receive a home visit by the patient-peer navigator who is a person with HIV and trained on motivational interviewing and peer support. The peer navigator uses a questionnaire to assess reasons for chronically missing appointments and offers a same-day medical appointment which includes transportation incentives. All no-show patients are contacted by clinic staff (i.e., physician/medical assistant, administrative director) within 24 hours of a missed appointment to reschedule and discuss barriers to appointment attendance. These cases are then referred to the case manager for mitigation.
