This study assessed CASN memberships' interests, priorities, and use of resources to help plan future activities and assess member engagement and satisfaction. We electronically surveyed 201 CASN members; 60 responded (29.85%). Respondents were asked about their affiliations, participation in agricultural associations, past and future level of engagement, agricultural safety interests, use of agricultural resources, and future activities of interest. Latent class analysis (LCA) was used to understand heterogeneity in the CASN members based on their selected interests. Classes were then tested in logistic regression models to determine how they might differ based on how much of a problem they perceived injury in youth to be, their level of past and future engagement, and their years with CASN. Odds ratios and 95% confidence intervals were used to estimate effect sizes. Exploratory factor analysis was used to assess the patterns of use of resources by respondents. Frequencies were reported for use of resources and future interests. Two groups of CASN members were identified based on reported topics of interest. Specialists (58.9%) were focused on the use of motorized vehicles and roadways. The second group, Generalists (41.1%), had more diverse interests. The two groups differed on how seriously they viewed the problem of agricultural injury in youth (Generalist vs Specialist: OR = 6.27; 1.56, 25.2) and their interest in participating in future CASN activities (Generalist vs. Specialist: OR = 8.75; 1.76, 43.5). The use of 11 resources fell into two patterns. Nearly half of respondents reported current use of the Ag Youth Work Guidelines, Ag Injury News, and the Child Ag Injury Prevention Workshop. CASN is heterogeneous in their agricultural interests around youth safety and their use of resources. In planning future campaigns, it is important to meet the needs of both groups, with activities planned around ATV/UTV and roadway safety and keeping young children out of the worksite, as well as specific topics of concern such as manure ponds, skid steers, and agritourism. Scores were based on 16 community-level characteristics categorised into 4 themes: socioeconomic status; household characteristics; race and ethnicity and housing type and transportation. Counties were divided into quartiles from least to most vulnerable, based on the score distribution among all counties. Deaths were identified from the 2019 Fatality Analysis Reporting System. Death rates per 100 000 population were stratified by vulnerability quartile and urbanicity. Large central metropolitan county results used the second least vulnerability quartile as reference. AB - Among 328 320 065 people across 3140 counties, there were 22 942 occupant and 6272 pedestrian deaths. Occupant death rates were higher in most vulnerable counties compared with least for all urbanicity levels (large central metropolitan: 4.0 vs 2.8; large fringe metropolitan: 7.4 vs 5.2; medium/small metropolitan: 8.9 vs 7.0; non-metropolitan: 18.5 vs 12.2) with disparities prominent in counties experiencing more socioeconomic and household vulnerability. Pedestrian death rates were highest in most vulnerable counties compared with least (large central metropolitan: 2.5 vs 1.4; large fringe metropolitan: 3.3 vs 1.0; medium/small metropolitan: 2.8 vs 0.8; non-metropolitan: 2.4 vs 0.9) with disparities prominent for all four vulnerability types. AB - To develop a practical approach for any workplace, we reviewed a variety of sources and used an international panel of interdisciplinary experts. Evidence included data from experimental, observational, and review studies. Thirty-two subject matter experts were invited to review the document; twenty-two completed the review and unanimously endorsed the ototoxicity management system as proposed. AB - Six key action steps were proposed to: (1) identify workers exposed to ototoxic chemicals, (2) perform auditory and vestibular assessments, (3) follow-up after monitoring health, (4) document worker data, (5) maintain healthy safety culture, and (6) review ototoxicity management approach. These steps focus on the management of workers who are at-risk for workplace ototoxic chemical exposure at any level (with or without concurrent noise exposures). In 2018, the TB COEs conducted a multiphased assessment among U.S. staff involved in TB service delivery to identify needs related to TB training, resources, and medical consultation. Representatives from each TB COE and CDC's Division of TB Elimination formed a workgroup to guide the design of the needs assessment. The group used an online survey for data collection. Participants were staff working in some capacity on TB within the United States, Puerto Rico, and the U.S. Virgin Islands. Staff could be in non-public health (e.g., community health center, hospital, laboratory, private practice) or public health (state or local health department staff responsible for TB) settings and did not have to be a clinical health care provider (N = 1,482). We identified four priority areas for future TB training and education efforts. These areas include (1) focus on key topics; (2) tailor training and products to different professions, settings, and skill levels; (3) keep learners updated on the latest resources and best practices; and (4) use a mix of training methods and formats. The findings highlighted future priorities for TB training and education and were shared with health department TB programs throughout the United States. AB - The MEASURE assay was transferred to UK Health Security Agency (UKHSA) and US Centers for Disease Control and Prevention (CDC) laboratories. Here, a cluster of locally transmitted NDM-1 CP-CRPA infections in Southern California is reported. AB - Epidemiologic investigations involving both patient screening and environmental surveillance by microbiological culture, carbapenemase testing, and bacterial whole-genome sequencing were conducted. AB - Through extensive epidemiologic investigations, a sink environmental reservoir was identified. A genetically unique strain of NDM-CRPA was identified to be transmitted among seven patients in a single hospital over the course of 1.5 years. Changes in antimicrobial resistance genes harbored by a mobile genetic element were identified between the index and later cases, and the gene encoding NDM-1 resided within a mobile genetic element on the chromosome displaying evidence of widespread transmission between different species. Several interventions were implemented, including sink P-trap replacement and disinfectant against Pseudomonas biofilms. No additional cases were identified after the interventions. The methodologies and instrumentation currently used to support the emission-assisted maintenance (EAM) programs for previous generations of diesel engines are in need of improvement to allow for monitoring low concentrations of complex aerosols emitted by the advanced diesel engines. The results showed that of the test conditions currently used in EAM programs, the torque converter stall and hydraulic stall are the most suitable for assessing the effectiveness of the DPF-based advanced aftertreatment systems. The low idle and high idle test conditions, frequently used in EAM programs for traditional engines, did not produce reliable and reproducible data. The solid particle number (SPN) concentrations proved to be more suitable than total particulate number concentrations as a metric for EAM monitoring of diesel aerosols emitted by advanced diesel engines. The findings of this study should provide the underground mining industry with valuable information needed to enhance their EAM programs. This study investigated the influence of Social Vulnerability (SV), barriers to access to vaccination and Racial Disparities (RD) on COVID-19 infection and death rates among Georgian residents using Georgia Department of Public Health data and County Health Rankings & Roadmaps. AB - We adjusted analyses for other predictors of outcomes by using the Poisson Generalized Linear Mixed Models (with county as the unit of analysis). We iteratively modeled county-specific infection/death rates as a function of the Social Vulnerability Index (SVI, % Racial Population Gap (RPG) [(60+years % White - %African Americans/Blacks (AA)/Black)], education, %unemployed, %uninsured, % obese, % fully vaccinated, racial differences in respiratory infection discharge rates and %AA /Black residents (w/o RPG in the model). AB - Per adjusted models' results of COVID-19 related death,, I) AA/Blacks, relative to Whites, were 51% more likely to die (p-value <0.0001), 1) by age-specific and overall estimates(p-values <0.0001 and 2) at a younger mean age (p-value < 0.0001), II) 1% increase in SVI increases the risk of death by 25% (p-value <0.0001) and III) risk of death decreases by 2.3% for every % increase in 60+ years old Whites vs. Black males county residents (p-value <0.0001). The case infection rate a) decreased by a.1) 0.1% for every percent population increase in the racial gap (i.e., more Whites than AA/Blacks in a county) (p-values = 0.0122) and a.2) 27% for every % increase of those fully vaccinated(p-value < 0.0001). The rates also increased by a) 17% with every 1% increase in SV Index p-value <0.0001) b) 1% for every 0.1% increase in those a) obese (p-value < 0.0001) and b) uninsured (p-value < 0.0001). Historically, clinicians have considered dermatophytosis as a mild, easy-to-treat condition; however, the epidemiology of dermatophytosis has changed dramatically in the past decade because of the emergence of dermatophyte strains causing increasingly severe and difficult-to-treat infections. We review three recently emerged dermatophytes of public health concern: Trichophyton indotineae, which is causing outbreaks of frequently terbinafine-resistant and difficult-to-treat tinea in South Asia, with cases also reported across six continents; Trichophyton mentagrophytes genotype VII (TMVII), associated with oral and anogenital tinea infections particularly among men who have sex with men in France and the United States; and terbinafine-resistant Trichophyton rubrum, noted as a cause of difficult-to-treat tinea infections, although data are limited. We discuss practical considerations for identifying these pathogens, which relies on DNA sequencing or MALDI-ToF rather than on morphological characteristics. Additionally, we highlight the importance of antifungal susceptibility testing and practical laboratory considerations. Finally, we emphasize the importance of increased adoption of diagnostic testing for suspected dermatophyte infections, as well as the development of rapid, accurate, and affordable dermatophyte testing methods to help improve diagnostic accuracy and judicious antifungal use. Overall, the emergence of severe and antifungal-resistant dermatophyte infections poses a global public health concern. Clinical microbiologists can play a crucial role in addressing this threat by familiarizing themselves with techniques for identifying emerging dermatophyte species and performing antifungal susceptibility testing to guide patient management, monitor trends, and inform future public health interventions. Laboratory characterization of tailpipe emissions for three "clean" engines that meet U.S. Environmental Protection Agency (EPA) Tier 4 final emissions standards were used to assess the viability and effectiveness of those strategies. The evaluated engines were representative of those that achieve the emission standards through implementation of various in-cylinder emissions control strategies, use of crankcase filtration, and use of three types of exhaust aftertreatment systems: (1) diesel oxidation catalytic converter (DOC), (2) combination of DOC and the full-flow wall flow monolith diesel particulate filter (DPF), or (3) combination of DOC, diesel exhaust fluid (DEF)-based selective catalytic reduction (SCR) system, and ammonia slip catalyst (ASC). The study showed that the highest reductions in concentrations of diesel aerosols in underground workings, in terms of both mass and number, could be achieved if the engines, preferably in all power classes, are fitted with viable DPF systems. The use of U.S. EPA Tier 4 final engines equipped with DOC and DOC/SCR/ASC systems could help operators to considerably reduce mass, but not number concentrations of aerosols. The emissions of two of the evaluated engines, one equipped with DOC and the other equipped with DOC/DPF systems, were characterized by substantial secondary NO AB - emissions that would limit the viability of those engines for underground mining applications. The catalyst formulations used in the exhaust aftertreatment systems of the diesel engines marketed to the underground mining industry need to be formulated to minimize the potential for generation of secondary NO AB - emissions. Engines fitted with viable SCR/ASC systems present a low-NO AB - alternative. All three of the evaluated advanced engines were found to have low CO output. Due to nuances associated with the use of diesel-powered mobile equipment in underground mines, the selection and potentially optimization of advanced engines for underground mining applications deserves special consideration. AB - From October 2021-January 2022, we conducted an online survey of pregnant and postpartum women and interviews with clinicians who provide care to this population. The survey was administered to pregnant and postpartum women who used substances or received SUD treatment during the COVID-19 pandemic. AB - Two hundred and ten respondents completed the survey. All respondents experienced pandemic-related barriers to routine health care services, including delays in prenatal care and SUD treatment. Disruptions in treatment were due to patient factors (38.2% canceled an appointment) and clinic factors (25.5% had a clinic cancel their appointment). Respondents were generally satisfied with telehealth ( AB - = 3.97, AB - = 0.82), though half preferred a combination of in-person and telehealth visits. Clinicians reported telehealth improved health care access for patients, however barriers were still observed. Data from 121 individuals with Niemann-Pick disease type C were drawn from several studies conducted at 2 US sites. Direct standardized assessments included the Nine-Hole pegboard or Purdue pegboard, a portion of the Clinical Evaluation of Language Fundamentals, and the age-appropriate Wechsler IQ test or the Mullen Scales of Early Learning. The 5DNPCCSS domains were significantly related in the expected directions to their respective direct assessments, supporting their construct validity. In combination with previous evidence presented for the Ambulation and Swallow domains, these results support the fitness of purpose of the (5DNPCCSS for clinical studies in Niemann-Pick disease type C. AB - The Family Snapshot Survey was developed to assess the impact of COVID-19 on family life, including items for quantitative analysis and 2 open-ended responses. This national survey of 9000 parents recruited from an opt-in internet panel was conducted in 3 waves of 3000 in November 2020, February 2021, and July 2021 using the online YouGov platform. The 2 open-ended responses are analyzed here using qualitative thematic analysis based on the SF Approach and Protective Factors Framework for caregivers with children aged 0 to 5 years. AB - The SF factors were used to classify 770 responses, with many responses identified as containing multiple factors, representing their interrelated nature. A lack of concrete support was the most frequent challenge, and parental resilience was the most frequent support. AB - Access or lack of access to concrete support in times of need underscored many of the strengths and challenges, respectively. Participants described interactions between individual protective factors that allowed for safety, stability, or positive experiences. In future community policy creation, policymakers may look at how programs interact and allow families to access multiple protective factors at once, with a critical need for providing these concrete supports. AB - Using two state-level systems which likely capture severe injuries, the (i) Iowa Trauma Registry (ITR) and (ii) Workers' Compensation (WC) records, we compared worker and injury characteristics found in these two data sources over a ten-year period (2005-2014) and developed a predictive model to determine which injuries are most likely captured in one data source vs the other. AB - A total of 2398 unique injuries from ITR and 3419 unique injuries from WC were found. There was very little overlap between the two sources. In general, injuries among older agricultural workers (65+) had a higher probability of capture by the ITR, while injuries among younger agricultural workers were more likely captured by WC. Young agricultural workers injured in motor vehicle traffic incidents were more likely captured by WC, but after age 25, these injuries were more likely captured by the ITR. In contrast, more machinery injuries were captured by the ITR than WC across age. AB - Neither of these secondary systems fully captured all injuries. Hence, comprehensive surveillance would require multiple data sources. However, future studies involving targeted research questions could select one data source, especially when surveillance resources are limited. AB - People in the criminal justice system, especially those enrolled in drug treatment courts, have complex health profiles, limited access to medical and dental care, and a high prevalence of substance use disorders (SUD). SUD is associated with poor oral health, including severe dental caries and periodontal disease. Both poor oral health and SUD can contribute to low self-esteem and stigma that pose barriers to seeking care. The aim of this study is to examine dental care and oral health disparities among community-dwelling justice-involved people with SUD enrolled in the drug treatment courts. We surveyed clients (N = 59) and leadership team members (n = 23) in four drug treatment courts. Clients responded to questions about dental care needs and self-reported oral health; leadership team members provided their perspectives on clients' oral health needs and barriers to care. Most clients (61%) rated their oral health as fair to very poor, and 27% had not seen a dentist in at least 3 years. Most (70%) leadership team members indicated that clients had occasionally or frequently discussed an oral health problem and 26% noted that clients occasionally or frequently missed court due to dental pain. Both clients and team members identified cost, transportation, fear, and stigma as key barriers to addressing clients' dental health needs. Improving oral health for justice-involved people with SUD may entail a multifaceted approach to address these barriers, and might consider novel approaches, such as co-locating dental and SUD providers. Frequencies of adverse outcomes were reported by syphilis treatment status during pregnancy as defined by the 2021 Sexually Transmitted Infections Treatment Guidelines (inadequate, adequate, and no treatment). Adjusted risk ratios (aRRs) were modeled for each outcome comparing adequate treatment with no treatment and with inadequate treatment, controlling for the pregnant woman's age at infection, education level, insurance status, reported substance use, number of prenatal visits, and stage of syphilis. AB - As of June 7, 2024, 1,682 singleton pregnancies complicated by syphilis were reported, with more than half of pregnant women adequately treated for syphilis (57.6%). Pregnant women with no or inadequate treatment had higher relative frequencies of adverse outcomes (stillbirth, prematurity, low birth weight [LBW], and neonatal intensive care unit [NICU] admission) than those with adequate treatment. The aRRs for stillbirth (9.9% vs 1.4%, aRR 3.72, 95% CI, 1.73-8.03), LBW (30.1% vs 9.9%, aRR 1.51, 95% CI, 1.07-2.14), and NICU admission (66.7% vs 27.0%, aRR 1.60, 95% CI, 1.28-1.98) were higher in pregnant women with no treatment compared with those with adequate treatment. Inadequate treatment was associated with LBW and NICU admission (24.9% vs 9.9%, aRR 1.81, 95% CI, 1.29-2.52; and 57.2% vs 27.0%, aRR 1.61, 95% CI, 1.31-1.99, respectively) compared with adequate treatment. AB - The high relative frequencies of adverse pregnancy and neonatal outcomes associated with inadequately treated or untreated syphilis during pregnancy reinforce the importance of adequate treatment in mitigating the effects of syphilitic infection. Increased attention and systematic strategies are needed to address gaps in screening and treatment before and during pregnancy to reduce adverse pregnancy and neonatal outcomes.
