Despite the importance of this extra-articular manifestation, there are relatively few identified risk factors for RA-ILD. Clinical risk factors consistently identified include older age, male sex, cigarette smoking, and severe articular disease<xref rid="R3" ref-type="bibr">3</xref>, <xref rid="R4" ref-type="bibr">4</xref>. There are also observed associations between prevalent RA-ILD and seropositivity with rheumatoid factor (RF), anti-cyclic citrullinated peptide (anti-CCP) antibodies, and anti-malondialdehyde-acetaldehyde adduct (anti-MAA) antibodies<xref rid="R4" ref-type="bibr">4</xref>, <xref rid="R5" ref-type="bibr">5</xref>. In addition to clinical measures and autoantibody status, genetic risk factors have been increasingly recognized for their contribution to RA-ILD development, specifically when it occurs in a usual interstitial pneumonia (UIP) pattern. The strongest genetic risk factor identified to date is the MUC5B rs35705950 promoter variant, a finding validated in multiple studies and independent cohorts<xref rid="R6" ref-type="bibr">6</xref>&#x02013;<xref rid="R10" ref-type="bibr">10</xref>.</p><p id="P7">Beyond clinically available autoantibodies and these genetic risk factors, there are a number of peripheral biomarkers associated with the presence of RA-ILD. A recent systematic review by our group identified 104 unique peripheral biomarkers associated with RA-ILD among 70 different studies, including a variety of cytokines/chemokines, growth factors, extracellular matrix proteins, tumor markers, lung epithelial or surfactant proteins, and others<xref rid="R11" ref-type="bibr">11</xref>. Many of these peripheral biomarkers are also recognized to be associated with RA, regardless of ILD status<xref rid="R12" ref-type="bibr">12</xref>, <xref rid="R13" ref-type="bibr">13</xref>. Most studies have focused on a single or select number of peripheral biomarkers for RA-ILD. The few studies that have applied more broad peripheral biomarker analyses in RA-ILD have been performed among small, selected samples<xref rid="R14" ref-type="bibr">14</xref>, <xref rid="R15" ref-type="bibr">15</xref>.</p><p id="P8">Given the vast range of recognized and candidate biomarkers for RA-ILD, a multi-dimensional evaluation of composite biomarkers for RA-ILD risk is needed, accounting for the interrelated nature across peripheral biomarker measures. Machine learning approaches have previously been leveraged to identify unique biomarker profiles in RA and other disease states. In this study, we aimed to derive unique peripheral biomarker signatures using an unsupervised machine learning approach among a large, multicenter RA cohort and test whether these biomarker signatures would improve RA-ILD prediction beyond clinical risk factors.</p>
<title>Patients and Methods</title>
<title>Study design, population, and demographics</title><p id="P9">This cross-sectional study was conducted within the Veterans Affairs Rheumatoid Arthritis (VARA) registry, which is a prospective, multicenter cohort of U.S. Veterans with RA fulfilling the 1987 American College of Rheumatology classification criteria that was initiated in 2003<xref rid="R16" ref-type="bibr">16</xref>, <xref rid="R17" ref-type="bibr">17</xref>. The disease activity score with 28-joint count (DAS28) and multidimensional Health Assessment Questionnaire (MD-HAQ) were documented by treating rheumatologists at enrollment. Additionally, PC13 (IgA/IgM antibodies to MAA-adducted collagen and adiponectin) was more closely associated with non-UIP ILD and PC7 (IgA/IgG/IgM antibodies to MAA-adducted fibrinogen, collagen, and vimentin) was more closely associated with severe ILD (FVC&#x0003c;80%), though 95% CI for estimates were overlapping.</p><p id="P23">While a clinical model performed similarly in the UIP subgroup (AUC 0.635 vs. 0.630), there was a substantial improvement in performance of the PC model (AUC 0.768 vs. 0.713) and combined models (clinical+PC+MUC5B model AUC 0.834 vs. 0.755; <xref rid="SD1" ref-type="supplementary-material">Table S3</xref>).</p>
<title>Discussion</title><p id="P24">In this study, we identified unique peripheral biomarker signatures occurring among a large, diverse RA population and evaluated their association with the presence of RA-ILD. PCs that were positively associated with RA-ILD were characterized by biomarkers that encompassed inflammatory cytokines (IL-3, IL-5, IL-8, IL-15, INF&#x003b1;&#x02212;2a), autoantibodies (anti-MAA, anti-CCP, RF), adipokines (FL, leptin, fractalkine, FGF-21), alarmins (IL-17e-IL-25, TSLP), and matrix metalloproteinases (MMP9). The variety of immune pathways and inflammatory processes associated with RA-ILD highlight the diversity of pathways that likely contribute to RA-ILD risk and pathogenesis, or that are impacted by the presence of RA-ILD. Furthermore, combining these biomarker signatures with clinical risk and genetic factors improved RA-ILD risk stratification, illustrating the potential role of peripheral blood biomarkers in facilitating RA-ILD identification.</p><p id="P25">The current lack of RA-ILD screening strategies is due in part to a paucity of informative clinical risk factors for RA-ILD. This was evident in our study, where models with only clinical risk factors yielded modest AUC values. Adding the biomarker PCs significantly improved the predictive performance of a model for the identification of RA-ILD over clinical risk factors alone. This improvement suggests that utilizing peripheral blood biomarkers could help overcome the limited predictive value of clinical risk factors for RA-ILD. While the performance of a model with clinical risk factors and peripheral biomarker signatures was statistically improved with the further addition of the MUC5B promoter variant, the absolute improvement in AUC was relatively modest. However, this degree of improvement was similar to the effect observed by adding the MUC5B promoter variant to clinical prediction models of RA-ILD<xref rid="R8" ref-type="bibr">8</xref>, suggesting that this genetic variant identifies features of susceptibility not captured through peripheral biomarkers. Additional research investigating the utility of integrating more comprehensive genetic and proteomic measurements into such models is warranted.</p><p id="P26">Biomarker themes that emerged through the PCA included myeloid differentiation, the innate immune and allergic responses, autoantibodies (specifically anti-MAA, anti-CCP, and RF), adipokines/metabolism, alarmins, tissue remodeling, and neutrophil chemotaxis. These findings may hint towards the diverse pathophysiologic pathways involved in RA-ILD, many of which are supported by prior literature. It has been demonstrated that serum anti-citrullinated protein antibodies (ACPA) correlate with local ACPA production in the lung mucosa and mucosal inflammation, leading to the &#x0201c;mucosal origins&#x0201d; hypothesis of RA<xref rid="R29" ref-type="bibr">29</xref>, <xref rid="R30" ref-type="bibr">30</xref>. Inflammatory cytokines such as TNF, IL-1, IL-4, IL-5, IL-13, and several chemokines are thought to likely play a role in the transition from epithelial injury to the development of lung fibrosis<xref rid="R29" ref-type="bibr">29</xref>. MAA expression in lung tissue has been shown to be higher in RA-ILD than in other forms of ILD, emphysema, or controls, and autoreactive B cells to MAA have been found in the lung and joint tissue of RA patients<xref rid="R5" ref-type="bibr">5</xref>, <xref rid="R31" ref-type="bibr">31</xref>. Adipokines, specifically adiponectin and leptin, have been shown to play a regulatory role in the inflammatory response, which has been suspected to have a potential role in ILD pathogenesis<xref rid="R32" ref-type="bibr">32</xref>. MMPs (including MMP9) have been shown to be overexpressed by alveolar epithelial cells within fibroblastic foci of the lungs in IPF and drive lung fibrosis, in a highly similar disease process to UIP RA-ILD<xref rid="R29" ref-type="bibr">29</xref>, <xref rid="R33" ref-type="bibr">33</xref>. Thus, our findings are in line with these prior studies and emphasize the need for further translational and mechanistic efforts to determine how these processes are involved in RA-ILD pathogenesis.</p><p id="P27">In the subgroup analyses, we found that the associations of PCs with UIP RA-ILD were generally consistent with those for all RA-ILD, likely reflecting the preponderance of UIP in RA-ILD. Interestingly, PC10 (IL-17e-IL-25 and TSLP) was positively associated with a UIP pattern and negatively associated with other ILD patterns. This has not been previously demonstrated, and this hypothesis-generating finding warrants further investigation into factors that differentiate specific ILD patterns and severity. Notably, because of the limited number of ILD cases in subgroup analyses, we were unable to detect modest differences in PC associations with RA-ILD subgroups. There was improvement in clinical/biomarker/MUC5B model performance for the outcome of UIP over all RA-ILD. This was limited by power but supports the idea that risk stratification efforts in RA-ILD may need to be pattern specific.</p><p id="P28">Strengths of this study include a large, diverse, multicenter U.S. cohort of well-phenotyped RA patients with comprehensive RA-ILD identification and peripheral biomarker measurements<xref rid="R19" ref-type="bibr">19</xref>. Limitations include a lack of chest CT imaging for the entirety of the cohort, as RA-ILD testing was performed as part of routine clinical care. This may result in misclassification of subclinical ILD as non-ILD. However, our results illustrate the utility of peripheral biomarker signatures for RA-ILD risk stratification in a real-world practice setting. ILD pattern (UIP, NSIP, etc.) was unavailable for some cases (n=9) and based on clinical determinations, rather than a separate research evaluation. While this cohort is predominantly male, it is reflective of the national VA population and RA-ILD preferentially affects males. While the lack of external validation is a limitation, internal validation was performed via bootstrapping and models underwent optimism correction, which did not impact the interpretation of findings. Given the cross-sectional nature of this study, there is potential for reverse causality between RA-ILD and peripheral biomarkers, although with the goal of identifying prevalent disease this should not impact the interpretation of our results. We included a broad range of peripheral blood biomarkers implicated in RA and RA-ILD pathogenesis but were unable to include all previously reported biomarkers associated with ILD. This remains an evolving area of research and future studies will need to continually reassess other novel biomarkers that would benefit from inclusion<xref rid="R34" ref-type="bibr">34</xref>&#x02013;<xref rid="R36" ref-type="bibr">36</xref>. Lastly, the PCA method used allowed for the identification of distinct biomarker profiles within a broad range of potential peripheral biomarkers and their association with RA-ILD. However, alternative approaches would be needed to identify individual biomarker cut-offs or evaluate the clinical application of predictive models, including the use of other machine learning techniques to guide efficient biomarker selection for clinical prediction models.</p><p id="P29">In conclusion, PCA identified distinct peripheral biomarker signatures that implicate a wide range of biologic pathways associated with RA-ILD. These peripheral biomarker signatures significantly improved ILD discrimination among a large RA cohort beyond clinical risk factors alone. The addition of the MUC5B promoter variant resulted in a modest improvement over clinical and peripheral biomarker models. Further investigation is needed regarding the effective use of peripheral biomarkers in the identification of RA-ILD, as well as applications towards ILD incidence and progression. This includes their additive contribution to other clinical and genetic risk factors and their relationship to the various pathways involved in RA-ILD pathogenesis that could impact therapeutic targeting.</p>
<title>Supplementary Material</title><supplementary-material id="SD1" position="float" content-type="local-data"><label>Supplementary Materials</label><media xlink:href="NIHMS2099354-supplement-Supplementary_Materials.docx" id="d67e613" position="anchor"/></supplementary-material>
</body><back><ack id="S18"><title>Acknowledgements and affiliations:</title><p id="P30">AMW is supported by the Rheumatology Research Foundation.
