We used data from the Centers for Disease Control and Prevention&#x02019;s COVID-19 Vaccine Pregnancy Registry (C19VPR) to assess risk of spontaneous abortion (SAB) among women receiving &#x02265;1 mRNA COVID-19 vaccine up to 30 days before last menstrual period or during pregnancy. We calculated risk of SAB from 6 to 20 weeks&#x02019; gestation using a fetuses-at-risk approach (n = 12,097). Pregnancies entered the risk pool at gestational age at vaccination. Risk of SAB in C19VPR was compared with pre-pandemic estimates. Cox proportional hazard models were used to compare risk by vaccine manufacturer. Cumulative risk of SAB was 10.79% (95%CI 10.01, 11.56). Risk of SAB among Moderna recipients was similar to Pfizer-BioNTech (adjusted hazard ratio 0.92; 95%CI 0.79, 1.07). Risk estimates of SAB were below or within pre-pandemic estimates. A qualitative analysis of pregnant women&#x02019;s hesitation about COVID-19 vaccines reported hesitancy due to lack of research about safety in pregnancy [<xref rid="R2" ref-type="bibr">2</xref>]. Understanding risk for adverse pregnancy outcomes after COVID-19 vaccination can inform decisions and recommendations about vaccination in pregnancy. As part of comprehensive vaccine safety monitoring, the US Centers for Disease Control and Prevention (CDC) established the COVID-19 Vaccine Pregnancy Registry (C19VPR) in January 2021 to collect data on COVID-19 vaccination, pregnancy outcomes, and participant and infant health [<xref rid="R3" ref-type="bibr">3</xref>]. To provide early findings, a preliminary report assessed the risk of spontaneous abortion (SAB) (14.1%) [<xref rid="R4" ref-type="bibr">4</xref>]; however, only approximately 25% of the C19VPR cohort data were available at the time of the preliminary report. This analysis updates findings to include all C19VPR participants and adds to the literature by comparing risk of SAB across vaccine manufacturers.</p>
<label>2.</label><title>Methods</title>
<label>2.1.</label><title>Study design and cohort selection</title><p id="P3">The C19VPR identified potential participants through V-safe [<xref rid="R5" ref-type="bibr">5</xref>]. Women aged 18&#x02013;54 reporting COVID-19 vaccination to V-safe between December 15, 2020, and June 21, 2021, were screened for C19VPR eligibility. Based on reported last menstrual period (LMP) and estimated due date (EDD), women who received COVID-19 vaccination up to 30 days before their last menstrual period preceding pregnancy or during pregnancy were eligible for enrollment. C19VPR collected pregnancy-related information directly from participants. Vaccination information was originally collected through V-safe and then confirmed through C19VPR. Participants could also report additional vaccination directly to C19VPR. Participant interviews varied based on when vaccination and pregnancy outcome were reported. Interviewer-administered phone surveys were scheduled for each trimester and twice in the postpartum period based on reported estimated due date. Some participants may have been interviewed only once. For example, if a woman reported vaccination at 8 weeks&#x02019; gestation, experienced a loss at 12 weeks, and was first contacted at 14 weeks, she would only complete one interview. Detailed C19VPR methodology is published elsewhere [<xref rid="R3" ref-type="bibr">3</xref>]. Data collection occurred from January 2021 to September 2022. We used a fetuses-at-risk approach to estimate week-specific and cumulative risks of SAB overall and by manufacturer. Cumulative risk provides a more accurate estimate of risk compared with crude incidence, as it accounts for differing exposure timing through censoring; crude incidence may underestimate risk by ignoring time-at-risk variability. Left censoring was used to ensure fetuses did not enter the analysis until gestational week of COVID-19 vaccination prior to or during pregnancy. Including unexposed time in the analysis (i.e., not applying left censoring) would dilute the estimated risk by attributing person-time without any exposure-related event potential, leading to underestimation of risk. Right censoring was used to ensure that fetuses contributed to the analysis only until pregnancy outcome had occurred. Other pregnancy outcomes (i.e., induced abortion, ectopic or molar pregnancy) were censored at gestational age at pregnancy end [<xref rid="R6" ref-type="bibr">6</xref>]. Participants who did not report date or gestational age at outcome or were lost to follow-up were censored at gestational age of last contact. Sensitivity analyses include outcomes reported to have occurred prior to 6 weeks&#x02019; gestation (n = 154) and exclusion of participants who received COVID-19 vaccines only prior to LMP (n = 1012). We compared cumulative risk of SAB in the C19VPR cohort to pre-pandemic estimates and the preliminary C19VPR report by graphing cumulative risk at each gestational week [<xref rid="R4" ref-type="bibr">4</xref>,<xref rid="R7" ref-type="bibr">7</xref>&#x02013;<xref rid="R9" ref-type="bibr">9</xref>]. As most C19VPR participants were non-Hispanic White, we graphed an approximation of Mukherjee et al.&#x02019;s (2013) estimates for White participants as a comparator (<xref rid="SD1" ref-type="supplementary-material">Supplement Table 1</xref>) [<xref rid="R7" ref-type="bibr">7</xref>]. To understand differences in estimates between the current analysis and preliminary C19VPR report estimates, we stratified the complete cohort by timing of enrollment in a post-hoc analysis (<xref rid="SD1" ref-type="supplementary-material">Supplement 2</xref>).</p><p id="P9">A higher proportion of Moderna participants were vaccinated prior to 6 weeks&#x02019; gestation (<xref rid="T1" ref-type="table">Table 1</xref>); earlier inclusion in the risk pool can bias risk estimates because risk of SAB is higher early in pregnancy and then gradually declines. For manufacturer comparisons, we matched individual pregnancies in the Pfizer-BioNTech group to individual pregnancies in the Moderna group using 4-week intervals of gestational age at vaccination. Participants receiving doses 1 and 2 from different manufacturers were excluded from manufacturer-specific analyses (n = 10). We assessed for effect modification by vaccine manufacturer among the matched subset (n = 10,052) using Cox proportional regression to calculate hazard ratios adjusted for variables associated with SAB and vaccine manufacturer (i.e., race and ethnicity and gestational age at vaccination).</p>
<label>3.</label><title>Results</title><p id="P10">Among 71 participant-reported pregnancy outcomes reviewed for consistency with medical records, four participant-reported SABs were reclassified as other pregnancy outcomes. We found no association between vaccine manufacturer (i.e., Pfizer-BioNTech versus Moderna) and risk of SAB. Findings are based on the first monovalent (index) mRNA COVID-19 vaccine formulations for the original SARS-CoV-2 variant. Few studies have assessed risk of adverse pregnancy outcomes following receipt of COVID-19 vaccines formulated after index virus COVID-19 vaccines. Newer mRNA COVID-19 vaccines are formulated using similar processes [<xref rid="R10" ref-type="bibr">10</xref>], and data suggest similar safety profiles between formulations [<xref rid="R11" ref-type="bibr">11</xref>]. Post-authorization vaccine safety studies provide critical data in helping patients and providers compare the risks of COVID-19 vaccination and illness.</p><p id="P15">Few studies in the United States have assessed the risk of SAB after COVID-19 vaccination. An analysis of data from the Vaccine Safety Datalink found no increase in odds of having received COVID-19 vaccination in the 28 days prior to SAB compared to ongoing pregnancies [<xref rid="R12" ref-type="bibr">12</xref>]. International studies have found no association between pre-pregnancy COVID-19 vaccination and SAB but were limited in sample size (n &#x0003c;2000) [<xref rid="R13" ref-type="bibr">13</xref>,<xref rid="R14" ref-type="bibr">14</xref>]. A 2022 metanalysis that pooled cohorts to increase analytic power found no increased risk of SAB following COVID-19 vaccination [<xref rid="R15" ref-type="bibr">15</xref>].</p><p id="P16">Published estimates of SAB in the United States use data sourced primarily from health systems&#x02019; birth cohorts, which often lack national representativeness and, critical to the evaluation of SAB risk, lack early pregnancy data and data on non-live births [<xref rid="R16" ref-type="bibr">16</xref>&#x02013;<xref rid="R18" ref-type="bibr">18</xref>]. Current safety monitoring systems in the United States often identify participants based on exposure (e.g., vaccination) rather than pregnancy status, resulting in a lack of data on early pregnancy and an inability to construct a comparison group of unexposed pregnancies. Generally, observational cohorts assessing SAB are subject to left truncation bias, as an unknown proportion of pregnancies is not identified because of pregnancy losses prior to enrollment [<xref rid="R19" ref-type="bibr">19</xref>]. Enrolling participants at the start of pregnancy or when trying to become pregnant, rather than after exposure event during pregnancy (e.g., vaccination), would facilitate a more comprehensive understanding of pregnancy outcomes in the US.</p><p id="P17">Understanding risk of SAB is challenging, requiring vaccination and enrollment of participants early in pregnancy. Our analysis has several strengths. Data on timing of vaccination during pregnancy enabled us to calculate week-specific risk, with pregnancies entering the risk pool only after participants were vaccinated. Week-specific and cumulative risk using the fetuses-at-risk approach is a more precise measure than overall prevalence because denominator at each gestational week includes only pregnancies wherein vaccination and no other outcome has occurred. Although this analysis was based primarily on participant report, we conducted clinical review of medical records to reduce misclassification of late fetal loss and early infant death.</p><p id="P18">Our analysis has several limitations. First, differences in estimated cumulative risk of SAB in this report versus preliminary findings may suggest that potential participants who had an SAB were less likely to enroll if contacted several months after the SAB [<xref rid="R4" ref-type="bibr">4</xref>]. However, analysis of the complete cohort was sufficiently powered to detect an increase in cumulative risk of SAB, whereas the preliminary analysis was not. Second, due to the limited number of women who received only one COVID-19 vaccine dose prior to pregnancy outcome, we were unable to assess risk by number of doses in pregnancy. Third, the registry cohort is largely homogenous. As many C19VPR participants were older, White, living in urban areas, and had low-to-normal BMI, it is likely that the cohort had differential access to prenatal care when compared to the pre-pandemic comparators. Though we used an approximation of Mukherjee et al.&#x02019;s (2013) estimates for White participants as a comparator, generalizability remains limited. Fourth, the C19VPR cohort may be subject to healthy vaccinee bias, wherein healthier people participate in vaccination programs. Further, people who experienced a spontaneous abortion may be less likely to participate to avoid talking about their experience, further biasing towards the null. However, therapeutic abortions may have been reported as miscarriages, biasing away from the null. Among 20 outcomes reported as SABs &#x02265;17 weeks&#x02019; gestation for which medical record review was conducted, 4 (20%) were reclassified as induced abortions whereas only 4 (7.7%) of 52 reviewed stillbirths were reclassified as SABs. Fifth, our cohort includes only vaccinated women, resulting in the need to use previously published data for comparison. Use of pre-pandemic estimates as comparators rather than a contemporaneous control group may bias results due to temporal trends in the rate of SAB. Sixth, use of self-reported data presents challenges, especially regarding gestational age at outcome, timing of exposure, and certainty of pregnancy outcome among participants who did not undergo medical record review.</p><p id="P19">Despite its limitations, this updated report adds to the literature by comparing risk of SAB across vaccine manufacturers. While comparisons to pre-pandemic rates should be interpreted with caution due to demographic homogeneity of the C19VPR cohort, caveated dissemination of findings is critical in an environment where women report a desire for additional safety data. Acknowledgement of limitations in comparing contemporary data to historical data can be used to inform future emergency response public health safety monitoring programs that aim to assess risk of early pregnancy outcomes.
