This study assessed substance use, polysubstance use, and use of select prescription medications during pregnancy.</p></sec><sec id="S2"><title>Methods:</title><p id="P2">We analyzed 2019 data from the Pregnancy Risk Assessment Monitoring System in 25 United States jurisdictions that included questions on prescription medications, tobacco, and illicit substance use during pregnancy. Alcohol and electronic cigarette use were assessed during the last three months of pregnancy, and all other substances and medications were assessed throughout pregnancy. Weighted prevalence estimates and 95% confidence intervals (CIs) were calculated.</p></sec><sec id="S3"><title>Results:</title><p id="P3">Nearly one-fifth of respondents who reported use of any substance reported use of at least one other substance during pregnancy. Cigarettes (8.1%; 95% CI 7.6&#x02013;8.7%) and alcohol (7.4%; 95% CI 6.7&#x02013;8.1%) were the most frequently reported substances, followed by cannabis (4.3%; 95% CI 3.9&#x02013;4.7%). Substance use was higher among individuals who reported having depression or using antidepressants during pregnancy compared with those who did not report depression or antidepressant use. Illicit drug use prevalence was low (0.5%, 95% CI 0.4&#x02013;0.7%); however, respondents reporting heroin use also frequently reported use of illicit stimulants (amphetamines: 51.7%, 95% CI 32.1&#x02013;71.3% or cocaine: 26.5%, 95% CI 11.9&#x02013;41.1%). Although prenatal clinician screening for alcohol and cigarette use was approximately 95%, fewer respondents (82.1%) reported being screened for cannabis or illicit substance use during pregnancy.</p></sec><sec id="S4"><title>Conclusions:</title><p id="P4">One in five individuals who reported use of any substance during pregnancy engaged in polysubstance use, highlighting the importance of comprehensive screening and evidence-based interventions including harm reduction.</p></sec></abstract><kwd-group><kwd>Pregnancy</kwd><kwd>Polysubstance use</kwd><kwd>Alcohol</kwd><kwd>Cigarettes</kwd><kwd>Cannabis</kwd><kwd>PRAMS</kwd></kwd-group></article-meta></front><body><sec id="S5"><label>1.</label><title>Introduction</title><p id="P5">Approximately one in five people use alcohol, tobacco, marijuana, or other illicit substances during pregnancy in the United States (<xref rid="R60" ref-type="bibr">U.S. Department of Health and Human Services, 2023</xref>). Nationally, tobacco products are the most commonly used substances during pregnancy (10.8% of people who are pregnant), followed by alcohol (9.8%) and marijuana (7.7%) (<xref rid="R60" ref-type="bibr">U.S. Department of Health and Human Services, 2023</xref>). Nearly 8% of women who are pregnant have a substance use disorder (SUD) or need SUD treatment (<xref rid="R41" ref-type="bibr">Martin et al., 2020</xref>). Substance use during pregnancy can lead to miscarriage, stillbirth, preterm birth, low birthweight, neonatal abstinence syndrome or neonatal opioid withdrawal syndrome, and fetal alcohol spectrum disorders (<xref rid="R51" ref-type="bibr">Smid and Terplan, 2022</xref>).</p><p id="P6">While the prevalence of using individual substances during pregnancy is well-documented (<xref rid="R17" ref-type="bibr">Creanga et al., 2012</xref>; <xref rid="R26" ref-type="bibr">Handmaker et al., 2006</xref>; <xref rid="R35" ref-type="bibr">Ko et al., 2020a</xref>, <xref rid="R36" ref-type="bibr">2020b</xref>; <xref rid="R39" ref-type="bibr">Luke et al., 2019</xref>; <xref rid="R51" ref-type="bibr">Smid and Terplan, 2022</xref>; <xref rid="R53" ref-type="bibr">Substance Abuse and Mental Health Services Administration, 2009</xref>), less is known about polysubstance use. Use of multiple substances during pregnancy may exacerbate adverse health outcomes for the person who is pregnant and the infant over single substance use. For example, compared with infants of people who used only marijuana or tobacco during pregnancy, neonates of mothers who reported co-use of marijuana and tobacco during pregnancy had higher odds of cardiac, musculoskeletal, and gastrointestinal birth defects, small head circumference, and poor infant self-soothing (<xref rid="R15" ref-type="bibr">Coleman-Cowger et al., 2018</xref>; <xref rid="R19" ref-type="bibr">De Genna et al., 2019</xref>). A recent study found that 38.2% of women who are pregnant who drank alcohol in the past 30 days also reported use of another substance, most commonly tobacco or marijuana (<xref rid="R22" ref-type="bibr">England et al., 2020</xref>). Another found that approximately half of women who were pregnant and reported nonmedical opioid use also reported having more than five alcoholic drinks in one day in the past month; 42.1% reported cigarette use and 35.1% reported marijuana use (<xref rid="R30" ref-type="bibr">Jarlenski et al., 2017</xref>). These studies suggest that among people who are pregnant and use at least one substance, polysubstance use is common.</p><p id="P7">Overdose is a leading cause of pregnancy-associated death (<xref rid="R6" ref-type="bibr">Campbell et al., 2021</xref>), with mortality rates increasing approximately 81% from 2017 to 2020 (<xref rid="R5" ref-type="bibr">Bruzelius and Martins, 2022</xref>). Polysubstance use was involved in a majority (83%) of drug-induced deaths among women who were pregnant or postpartum in one study (<xref rid="R52" ref-type="bibr">Smid et al., 2019</xref>); in another, nearly half (48.9%) of people with pregnancy-related overdose deaths were diagnosed with depression (<xref rid="R31" ref-type="bibr">Kountanis et al., 2023</xref>). Access to substance use and mental health screening and treatment, including medications for opioid use disorder (MOUD), is a critical component of efforts to reduce harmful substance use and overdose during pregnancy (<xref rid="R51" ref-type="bibr">Smid and Terplan, 2022</xref>; <xref rid="R6" ref-type="bibr">Campbell et al., 2021</xref>). Unfortunately, SUD treatment services remain limited for people who are pregnant; only about 13% of women who were pregnant with a past-year need for SUD treatment actually received it (<xref rid="R41" ref-type="bibr">Martin et al., 2020</xref>).</p><p id="P8">In addition to adverse pregnancy, birth, and infant outcomes linked with substance use, polysubstance use, and overdose, some prescription medications may also be associated with birth defects or may have interaction effects with certain substances if taken during pregnancy. An estimated seven in ten women take at least one prescription medication during pregnancy (<xref rid="R43" ref-type="bibr">Mitchell et al., 2011</xref>). In 2019, 6.6% of women took prescription opioids during pregnancy, and approximately one in five of these women reported prescription opioid misuse (<xref rid="R36" ref-type="bibr">Ko et al., 2020b</xref>). Prescription stimulant use has risen among young adult women, and prescription stimulant use during pregnancy has been linked to birth defects including gastroschisis, omphalocele, and transverse limb deficiency (<xref rid="R3" ref-type="bibr">Anderson et al., 2020</xref>). Prescription benzodiazepine use during pregnancy may also be associated with birth defects such as Dandy-Walker malformation, anophthalmia or microphthalmia, gastroschisis, esophageal atresia or stenosis, and pulmonary valve stenosis, although additional research is needed (<xref rid="R57" ref-type="bibr">Tinker et al., 2019</xref>). It is important that patients take medications as prescribed and do not start or stop using prescribed medications during pregnancy unless advised to do so by a physician (<xref rid="R10" ref-type="bibr">Centers for Disease Control and Prevention, 2022</xref>; <xref rid="R40" ref-type="bibr">March of Dimes, 2022</xref>; <xref rid="R58" ref-type="bibr">Food and Drug Administration, 2019</xref>). Patient and clinician communication is important to ensure patients understand the risks involved in stopping medications or taking medications along with certain substances during pregnancy (<xref rid="R33" ref-type="bibr">Lynch et al., 2018</xref>) and that patients are screened for substance use and referred as indicated for treatment. Comprehensive information surrounding medication and substance use during pregnancy is lacking (<xref rid="R23" ref-type="bibr">Franks et al., 2020</xref>; <xref rid="R42" ref-type="bibr">McCance-Katz, 2011</xref>; <xref rid="R62" ref-type="bibr">Widnes et al., 2017</xref>); enhancing knowledge can inform clinical practice and messaging to improve health outcomes without stigmatizing people who take medications or use substances.</p><p id="P9">This study estimates the prevalence and patterns of substance use, polysubstance use, and use of select prescription medications during pregnancy. A secondary aim of this study was to estimate the prevalence of respondent-reported clinician screening for alcohol, tobacco, and other substance use during pregnancy. Understanding these patterns can inform public health actions and clinical care, reduce the potential for harmful interactions between prescription medications and substances, improve overall family health, and prevent adverse infant developmental outcomes related to substance use. Additionally, understanding polysubstance use during pregnancy can inform evidence-based interventions, and the role of harm reduction in addressing associated health risks.</p></sec><sec id="S6"><label>2.</label><title>Material and methods</title><sec id="S7"><label>2.1.</label><title>Data source</title><p id="P10">We analyzed 2019 data from the Pregnancy Risk Assessment Monitoring System (PRAMS), a population-based surveillance system that collects data on experiences and behaviors before, during, and shortly after pregnancy. Participating jurisdictions sample between 1000 and 3000 individuals with a U.S. live birth occurring in the preceding two to six months each year from birth certificate records, and contact sampled individuals by mail and telephone (<xref rid="R8" ref-type="bibr">Centers for Disease Control and Prevention, 2021a</xref>; <xref rid="R50" ref-type="bibr">Shulman et al., 2018</xref>). The PRAMS protocol has been approved by the institutional review boards of the Centers for Disease Control and Prevention and all participating jurisdictions.</p><p id="P11">This analysis included data from the 25 U.S. jurisdictions that opted into a PRAMS opioid supplement survey (<ext-link xlink:href="http://www.cdc.gov/prams/special-projects/opioid-supplement-call-back-survey/index.html" ext-link-type="uri">http://www.cdc.gov/prams/special-projects/opioid-supplement-call-back-survey/index.html</ext-link>) and achieved a response rate of &#x0003e;50%. Two jurisdictions only included the opioid supplement substance use questions for a partial year: Louisiana (six months) and Illinois (nine months). Five jurisdictions (Kentucky, Louisiana, Massachusetts, Pennsylvania, and Utah) oversampled individuals in counties with high rates of opioid overdose mortality (<xref rid="R9" ref-type="bibr">Centers for Disease Control and Prevention, 2021b</xref>).</p></sec><sec id="S8"><label>2.2.</label><title>Measures</title><p id="P12">The opioid supplement asked whether, at any time during pregnancy, respondents had taken certain prescription medications for any reason (hereafter referred to as &#x0201c;prescription medications&#x0201d;, including opioids, such as hydrocodone and tramadol; MOUD, including methadone, buprenorphine, Subutex, or Suboxone; benzodiazepines, such as Valium or Xanax; and stimulants, such as Adderall or Ritalin), marijuana or hash (hereafter referred to as &#x0201c;cannabis&#x0201d;, not including cannabidiol products), or illicit substances (including illicit amphetamines, cocaine, and heroin). Cigarette use at any time during pregnancy and e-cigarette use in the last three months of pregnancy were captured using the birth certificate and core PRAMS questions (<xref rid="SD1" ref-type="supplementary-material">Supplemental Table 1</xref>). Of the 25 jurisdictions that participated in the opioid supplement, 11 also included an optional standard question on their PRAMS surveys about alcohol use during the last three months of pregnancy. Alcohol use was assessed only for those jurisdictions. In this paper we define substance use as alcohol use during the last three months of pregnancy; e-cigarette use during the last three months of pregnancy; or cigarette use, cannabis use, heroin use, illicit amphetamine use, cocaine use, or prescription opioid misuse at anytime during pregnancy. Prescription opioid misuse was defined using responses to questions in the opioid supplement and has been described in detail elsewhere (<xref rid="R36" ref-type="bibr">Ko et al., 2020b</xref>). Polysubstance use was defined as use of two or more substances during the described timeframe, but not necessarily use of those substances on the same day or at the same time.</p><p id="P13">Sociodemographic variables such as age, race/ethnicity, and education level were obtained from birth certificate data. Other sociodemographic variables, behaviors, and experiences during pregnancy, including whether clinicians asked about alcohol, tobacco, or other substance use during pregnancy, were obtained from questions asked on the core PRAMS survey (<ext-link xlink:href="http://www.cdc.gov/prams/questionnaire.htm#current" ext-link-type="uri">http://www.cdc.gov/prams/questionnaire.htm#current</ext-link>). One such variable was a combined variable that captured depression or anti-depressant use based on a core PRAMS question and an opioid supplement question (<xref rid="SD1" ref-type="supplementary-material">Supplemental Table 1</xref>).</p></sec><sec id="S9"><label>2.3.</label><title>Statistical analysis</title><p id="P14">Weighted prevalence estimates and 95% confidence intervals (CIs) were calculated for alcohol (11 jurisdictions) and all other substances and medications (25 jurisdictions) in SAS software version 9.4 (SAS Institute, Cary, NC). Chi-square tests were used to test for association between use of specific substances and sociodemographic characteristics, behaviors, and experiences during pregnancy (including depression or taking anti-depressants). Separate analyses were performed to estimate use of all substances except alcohol among the full 25 participating jurisdictions and all substances inclusive of alcohol among the 11 jurisdictions that asked about alcohol use during the last three months of pregnancy. Additionally, overall and for each substance and medication used during pregnancy, we calculated prevalence of respondent-reported clinician screening for alcohol, cigarette, and substance use.</p></sec></sec><sec id="S10"><label>3.</label><title>Results</title><sec id="S11"><label>3.1.</label><title>Single substance use</title><p id="P15">The substances most commonly used during pregnancy were cigarettes (8.1%, 95% confidence interval [CI] 7.6&#x02013;8.7%), alcohol during the last three months of pregnancy (7.4%, 6.7&#x02013;8.1%), and cannabis (4.3%, 3.9&#x02013;4.7%; <xref rid="T1" ref-type="table">Table 1</xref>). Individuals aged 35 years or older more commonly reported alcohol use (10.3%, 8.5&#x02013;12.1%) compared with individuals younger than age 25 (20&#x02013;24: 4.4%, 3.0&#x02013;5.8%; &#x0003c;20: 1.9%, 0.3&#x02013;3.5%). Individuals aged 20&#x02013;24 had higher prevalence of cigarette use compared to persons aged 30 years and older, and cannabis use compared to persons aged 25 years and older. Non-Hispanic White persons reported higher prevalence of alcohol use than non-Hispanic Black persons. American Indian/Alaska Native persons reported higher prevalence of cigarette, cannabis, and illicit drug use (heroin, cocaine, or illicit amphetamine use) compared with all other racial/ethnic groups. For all substances except alcohol, individuals with less than a high school education had higher prevalence of use compared to those with more than a high school education. Persons living in rural areas reported more cigarette use (16.8%, 14.8&#x02013;18.8%) compared with those in urban areas (6.8%, 6.3&#x02013;7.4%). Additionally, respondents who reported having depression or taking antidepressant medications during pregnancy had higher prevalence of all substances except prescription opioid misuse, compared with those who did not have depression or use antidepressants.</p></sec><sec id="S12"><label>3.2.</label><title>Polysubstance use</title><p id="P16">Among respondents who reported any substance use in the 11 jurisdictions that collected information on alcohol use, 18.3% (15.6&#x02013;21.0%) reported use of another substance during pregnancy (data not shown). Among respondents who used alcohol in the last three months of pregnancy, 8.9% (5.9&#x02013;12.0%) used cigarettes and 8.3% (5.0&#x02013;11.7%) used cannabis at any point during pregnancy (<xref rid="T2" ref-type="table">Table 2</xref>). Among individuals who used cannabis, 12.8% (7.8&#x02013;17.9%) reported alcohol use, and 41.6% (36.6&#x02013;46.7%) reported cigarette use (<xref rid="T2" ref-type="table">Table 2</xref>). Among respondents in all 25 jurisdictions, 22.2% (19.2&#x02013;25.3%) of persons who reported cigarette use at any time during pregnancy and 24.4% (16.4&#x02013;32.5%) of those who reported e-cigarette use in the last three months of pregnancy also reported using cannabis at any point during pregnancy.</p><p id="P17">In all 25 jurisdictions, among persons who used heroin, illicit amphetamines (including methamphetamine), and cocaine, 81.2% (65.5&#x02013;96.8%), 87.4% (79.8&#x02013;94.9%), and 88.8% (79.0&#x02013;98.6%) also reported cigarette use, respectively. Persons who reported heroin, illicit amphetamine, or cocaine use during pregnancy also frequently reported cannabis use (47.7%, 28.6&#x02013;66.8%; 65.8%, 51.4&#x02013;80.3%; and 65.2%, 37.7&#x02013;92.8%, respectively). Nearly one-fourth of persons who reported cocaine use also reported heroin use. Respondents who used heroin also reported prescription opioid misuse (31.1%, 14.0&#x02013;48.2%), illicit amphetamine use (51.7%, 32.1&#x02013;71.3%), and cocaine use (26.5%, 11.9&#x02013;41.1%).</p></sec><sec id="S13"><label>3.3.</label><title>Prescription medication use</title><p id="P18">Overall, 8.8% (8.2&#x02013;9.4%) of respondents reported taking prescription opioids, benzodiazepines, stimulants, or MOUD for any reason during pregnancy (data not shown). Among persons who took MOUD, 62.1% (51.7&#x02013;72.5%) also reported cigarette use, and 30.2% (20.1&#x02013;40.2%) reported cannabis use. Approximately one-third of individuals who reported cocaine use also reported taking MOUD or prescription opioids during pregnancy, and over two-thirds of respondents who reported heroin use during pregnancy also took MOUD (68.4%, 48.7&#x02013;88.1%). Among persons who took prescription stimulants during pregnancy, 10.0% (0.0&#x02013;23.3%) reported cocaine use and in the 11 jurisdictions that asked about alcohol, 31.0% (7.4&#x02013;54.6%) reported alcohol use. Additionally, one in four persons who took prescription stimulants also took prescription opioids during pregnancy.</p></sec><sec id="S14"><label>3.4.</label><title>Substance use screening</title><p id="P19">Approximately 95% of respondents reported that a clinician asked about alcohol or cigarette use during pregnancy (alcohol: 94.5%, 94.0&#x02013;95.0%; cigarette: 95.1%, 94.7&#x02013;95.6%) (data not shown). Approximately 82% (82.1%, 81.3&#x02013;82.9%) of respondents reported that a clinician asked about use of cannabis or other drugs during pregnancy. Prevalence of respondent-reported clinician screening for cannabis or other drugs was 73.3% (59.4&#x02013;87.3%) for individuals who reported prescription stimulant use compared with 96.8% (92.2&#x02013;100.0%) for persons who reported cocaine use (<xref rid="F1" ref-type="fig">Fig. 1</xref>).</p></sec></sec><sec id="S15"><label>4.</label><title>Discussion</title><p id="P20">In this large, population-based sample of individuals with a recent live birth, approximately one in five persons reporting substance use during pregnancy engaged in polysubstance use. Cigarettes, alcohol, and cannabis were the most frequently reported single substances used during pregnancy, and polysubstance use varied by substance type. These findings underscore the importance of messaging about the risks of polysubstance use during pregnancy and screening for multiple substances, followed by linkage to substance use treatment and harm reduction services.</p><p id="P21">Our results are consistent with other reports (<xref rid="R34" ref-type="bibr">Kar et al., 2021</xref>; <xref rid="R47" ref-type="bibr">Qato et al., 2020</xref>). However, our findings fill important gaps by examining any use of prescription medications with other substances and multiple combinations of polysubstance use across legal and illicit substances during pregnancy. Use of other substances with alcohol during pregnancy was higher in the National Survey on Drug Use and Health (NSDUH; 38.2% (<xref rid="R22" ref-type="bibr">England et al., 2020</xref>)) than in our study; however, NSDUH captures alcohol use at any time during pregnancy, while PRAMS only captures use in the last three months of pregnancy.</p><p id="P22">Our findings of demographic differences in substance use can inform clinical practice, not by singling out specific subpopulations to receive greater scrutiny, but by illustrating the importance of considering social determinants of health, community environment, and the broader social and systemic context (including the impact of structural racism and bias) in providing holistic and comprehensive care to all persons who are pregnant. In this analysis, people aged 20&#x02013;24 years more frequently reported cigarette and cannabis use during pregnancy, while people aged 35 years or older more frequently reported alcohol use during the last three months of pregnancy. Such age-related differences have been described in other studies (<xref rid="R12" ref-type="bibr">Cheng et al., 2011</xref>; <xref rid="R28" ref-type="bibr">Houston-Ludlam et al., 2019</xref>; <xref rid="R39" ref-type="bibr">Luke et al., 2019</xref>; <xref rid="R56" ref-type="bibr">Thompson et al., 2021</xref>). We also observed differences in substance use by race/ethnicity, with American Indian/Alaska Native persons reporting the highest prevalence of substance use during pregnancy, followed by non-Hispanic White persons. The higher prevalence of substance use among American Indian/Alaska Native persons must be understood within the context of historical and contemporary trauma, colonization, discrimination, and oppression (<xref rid="R25" ref-type="bibr">Gonzales et al., 2021</xref>; <xref rid="R44" ref-type="bibr">Morgan and Freeman, 2009</xref>; <xref rid="R55" ref-type="bibr">Szlemko et al., 2006</xref>), as well as barriers to culturally competent, inclusive, and non-stigmatizing treatment and prevention services (<xref rid="R25" ref-type="bibr">Gonzales et al., 2021</xref>; <xref rid="R55" ref-type="bibr">Szlemko et al., 2006</xref>). Additionally, factors such as systemic racism and bias may lead to inequities for persons from racial and ethnic minority groups compared to persons who are White, including higher rates of child services reporting and removal among prenatally substance-exposed infants (<xref rid="R11" ref-type="bibr">Chasnoff et al., 1990</xref>; <xref rid="R49" ref-type="bibr">Roberts and Nuru-Jeter, 2011</xref>).</p><p id="P23">We also observed higher prevalence of substance use among individuals who reported having depression or taking antidepressants during pregnancy. Co-occurrence of mental health conditions and substance use during pregnancy, particularly depression, has been identified in prior research (<xref rid="R4" ref-type="bibr">Brown et al., 2019</xref>). In addition, the use of medications such as opioids and benzodiazepines may be more common among people with certain mental health conditions (e.g. mood disorders, anxiety) (<xref rid="R54" ref-type="bibr">Sujan et al., 2021</xref>) and the convergence of mental health concerns, prescription medication use, and substance use may play a role in adverse pregnancy and birth outcomes (<xref rid="R59" ref-type="bibr">Viswanathan et al., 2021</xref>; <xref rid="R65" ref-type="bibr">Zhao et al., 2017</xref>). These findings indicate the need for screening for depression and substance use throughout pregnancy, and providing resources and treatment as indicated.</p><p id="P24">Although the overall prevalence of illicit opioid and stimulant use was low, use of illicit stimulants frequently occurred among respondents who reported use of illicit opioids, and vice versa. As illicit opioid and stimulant co-use has increased in the general population, so have overdoses involving both substances (<xref rid="R14" ref-type="bibr">Ciccarone, 2021</xref>; <xref rid="R46" ref-type="bibr">O&#x02019;Donnell et al., 2020</xref>). Many of these deaths are attributable to the presence of synthetic opioids such as illicitly manufactured fentanyl, due to both substances being in the same product or co-use (<xref rid="R7" ref-type="bibr">Centers for Disease Control and Prevention, 2020</xref>; <xref rid="R14" ref-type="bibr">Ciccarone, 2021</xref>). Drug overdose is a leading cause of pregnancy-associated death, and overdose mortality among people who are pregnant has been increasing in recent years (<xref rid="R6" ref-type="bibr">Campbell et al., 2021</xref>; <xref rid="R5" ref-type="bibr">Bruzelius and Martins, 2022</xref>).</p><p id="P25">It is recommended that people who are pregnant with opioid use disorder (OUD) be initiated on MOUD (<xref rid="R2" ref-type="bibr">American College of Obstetricians and Gynecologists, 2017</xref>). MOUD during pregnancy is associated with improved pregnancy, birth, and infant outcomes, including reductions in overdose, preterm birth, and low birthweight compared with people with OUD who did not take MOUD during pregnancy (<xref rid="R32" ref-type="bibr">Krans et al., 2021</xref>). Over two-thirds of respondents who reported heroin use also took MOUD during pregnancy, indicating potentially successful linkages to treatment, though the available data cannot confirm that they were receiving MOUD from a clinician or using it as provided, or whether they stopped misusing opioids with a transition to MOUD. Patients receiving MOUD may also have frequent touchpoints with the healthcare system, which can improve health outcomes and provide additional opportunities to address substance use. Nevertheless, there are multiple barriers to accessing MOUD and other forms of substance use treatment during pregnancy, including transportation and geographic barriers, childcare barriers, stigma, and fear of child welfare services involvement (<xref rid="R13" ref-type="bibr">Choi et al., 2022</xref>; <xref rid="R24" ref-type="bibr">Frazer et al., 2019</xref>); as a result, many persons who are pregnant with a SUD are unable to access treatment (<xref rid="R41" ref-type="bibr">Martin et al., 2020</xref>). Integrated behavioral health, SUD treatment, and prenatal and postpartum care services paired with social service support, harm reduction, and care coordination may reduce these barriers and improve overall health and wellbeing (<xref rid="R21" ref-type="bibr">Ellis et al., 2023</xref>).</p><p id="P26">The U.S. Preventive Services Task Force recommends screening all adults aged 18 and older, including people who are pregnant, for alcohol, tobacco, and unhealthy drug use (<xref rid="R18" ref-type="bibr">Curry et al., 2018</xref>; <xref rid="R37" ref-type="bibr">Krist et al., 2020</xref>, <xref rid="R38" ref-type="bibr">2021</xref>). Patterns of demographic differences in alcohol use in this analysis differed from those observed for other substances, particularly with regard to age, race/ethnicity, and educational status, which reinforces the need for universal screening for alcohol and other substances during pregnancy. However, the prevalence of respondent-reported clinician screening for cannabis and other drug use during pregnancy was lower than the prevalence of asking about alcohol and cigarette use. Barriers to clinician screening for substance use during pregnancy may include time constraints, competing priorities, and concerns about mandatory reporting laws, and persons who use substances during pregnancy may face additional trauma, stress, and stigma that interferes with prenatal care access and screening (<xref rid="R20" ref-type="bibr">Dozet et al., 2023</xref>). Adopting a trauma-informed and culturally sensitive therapeutic approach may help to reduce barriers to screening and treatment during pregnancy (<xref rid="R48" ref-type="bibr">Raffi et al., 2021</xref>). Additionally, findings of prescription medication use among persons who reported substance use during pregnancy, and vice versa, highlight the importance of ongoing discussions throughout pregnancy regarding potentially harmful interaction effects between medications and substances, such as co-use of alcohol with benzodiazepines or opioids.</p><p id="P27">Some people may not be able to achieve abstinence from substance use during pregnancy, or may not want to (<xref rid="R1" ref-type="bibr">Academy of Perinatal Harm Reduction, 2020</xref>). For these individuals, harm reduction strategies can be employed to reduce risks associated with substance use and promote health, worth, and dignity. Specific strategies include reducing the frequency of substance use, switching to a safer form of use (e.g. oral vs. injection), planning ways to stay safe before using substances (including not using alone, access to naloxone, and use of fentanyl test strips), and reducing the risk of skin bacterial and bloodborne infections if injecting substances by using sterilized injection supplies (<xref rid="R1" ref-type="bibr">Academy of Perinatal Harm Reduction, 2020</xref>). One pilot harm reduction program found that 96% of participants who were pregnant had negative urine toxicology screens at delivery and their infants had similar rates of prematurity as the state and national population, despite experiencing additional risk factors for prematurity (<xref rid="R63" ref-type="bibr">Wright et al., 2012</xref>).</p><p id="P28">Many factors can impact pregnancy and birth outcomes in addition to substance use (<xref rid="R1" ref-type="bibr">Academy of Perinatal Harm Reduction, 2020</xref>; <xref rid="R61" ref-type="bibr">Viteri et al., 2015</xref>), and it can be challenging to identify the causes of adverse health impacts, particularly when use of multiple substances are involved. Furthermore, there are no standardized definitions of polysubstance use during pregnancy. Previous studies have defined polysubstance use as co-occurring use or substance use disorders (<xref rid="R29" ref-type="bibr">Jarlenski et al., 2020</xref>; <xref rid="R27" ref-type="bibr">Hardt et al., 2013</xref>), use within the past 30 days (<xref rid="R16" ref-type="bibr">Coleman-Cowger et al., 2017</xref>; <xref rid="R30" ref-type="bibr">Jarlenski et al., 2017</xref>), or use at any time point during pregnancy (<xref rid="R45" ref-type="bibr">Nguyen et al., 2023</xref>). Additionally, polysubstance use in some studies may include nonmedical prescription medication use (<xref rid="R30" ref-type="bibr">Jarlenski et al., 2017</xref>) or any prescription medication use (<xref rid="R45" ref-type="bibr">Nguyen et al., 2023</xref>). This lack of consistency in defining and reporting on polysubstance use during pregnancy may inhibit conversations with patients around the potential health impacts of substance use individually, in combination, and in conjunction with prescription medications.</p><p id="P29">This analysis was subject to several limitations. First, findings may not be generalizable beyond the included jurisdictions. Additionally, only 11 of the 25 jurisdictions asked respondents about alcohol use, limiting our sample size for reporting on use of alcohol with other substances. Individuals&#x02019; responses to the survey may have been impacted by recall and social desirability biases. Sample sizes were small for some illicit substances and demographic groups and prevalence estimates with wide confidence intervals should be interpreted with caution. Due to question wording, we were only able to examine e-cigarette and alcohol use during the last three months of pregnancy. Due to high rates of reducing or quitting alcohol use once pregnancy is recognized (<xref rid="R22" ref-type="bibr">England et al., 2020</xref>; <xref rid="R26" ref-type="bibr">Handmaker et al., 2006</xref>), our results may under-estimate alcohol use prior to the last three months of pregnancy, and do not allow for distinction between the characteristics and behaviors of those who quit early and those who did not. PRAMS also does not collect information on substance use or medication frequency, duration, and dose. Additionally, our data were from the 2019 PRAMS survey and do not reflect changes in substance use that may have occurred during the COVID-19 pandemic. For example, one study found that cannabis use during pregnancy in Northern California increased by 25% during the pandemic compared to the 15 months prior (<xref rid="R64" ref-type="bibr">Young-Wolff et al., 2021</xref>). Finally, we only had information on selected prescription medications, and did not have complete information about patterns of use as prescribed or misuse, with the exception of prescription opioids.</p><p id="P30">Polysubstance use was common in this sample among individuals who reported use of any substance during pregnancy. The frequency of polysubstance use during pregnancy demonstrates the importance of understanding substance use epidemiology and implementing screening, prevention, patient and clinician education, and intervention efforts, including overdose prevention and harm reduction services, for people who use substances while pregnant. Furthering understanding around substance use while also taking prescription medications is also important. Additional data and research may help to better understand polysubstance use throughout pregnancy, including patterns of use by pregnancy trimester and other factors that may be associated with polysubstance use, including social determinants of health. Importantly, clinicians and public health practitioners can engage people who are pregnant in conversations around the risks of substance use during pregnancy, especially polysubstance use, emphasizing respect and dignity through the lens of a harm reduction approach.
